tell them about the treatment pro-gram he wanted to put together. Kline showed them some slides, then made an unusual request: “I’d like you to do-nate your medications for 500 children forever.”
“We’ve never done that before,” the startled executives exclaimed. “no-body does that.”
They came around to the idea.By the fall of 2001, the clinic was
open and 340 children were on the full cocktail of AIDS drugs. Deaths among Constanta’s HIV-infected children dropped from 12 percent to 1 percent. Kline was doing what no one had reported doing before — providing large-scale, effective HIV treatment in a setting with few resources.
Larger problemsromania’s situation was tragic, but
nothing compared with Africa’s.In romania, the scope of pediatric
HIV was limited, the result of a one-time disaster.
In Africa, the problem was stag-gering and showed no sign of abating. Mothers, unaware of their HIV status, were transmitting the infection to their babies at birth or through breastfeed-ing. Drugs to prevent mother-to-child transmission weren’t in wide use, again, because of the cost.
In 1999, the Bristol-Myers Squibb Foundation, which provided drugs for children in romania, announced a $100 million initiative for Africa, called Secure the Future. It asked Kline to start training African doctors and
nurses to treat children with HIV.Meanwhile, international health
experts remained locked in a debate over whether to focus on treatment or prevention, an argument that infuriated Kline. one u.S. official, whose com-ments later were denounced as racist, remarked in 2001 that Africans didn’t have watches and couldn’t tell time — and therefore couldn’t adhere to complicated drug regimens.
Slowly, hope emerged. By 2002, in the face of worldwide criticism, drug companies were dropping their prices.
In Swaziland, Kline bumped into Kenneth Weg, at that time vice chair-man of Bristol-Myers Squibb and one of the first pharmaceutical executives to pay attention to Africa. Weg, due for a meeting in Botswana, invited Kline to

¿Qué son las pruebas de ADN para determinar la paternidad?Pruebas de paternidad simplemente establecen paternidad.
Todos nacemos con un modelo genético único conocido como
ADN (ácido desoxirribonucleico). Como el ADN es transmitido de
la madre y el padre a su niño, la identificación de ADN ofrece una
manera conclusiva de determinar el parentesco biológico.
¿Qué tan exactos son los resultados de las pruebas de ADN?Las pruebas de ADN son un método muy exacto de establecer relaciones biológicas. De hecho, cuando se realizan correctamente, los resultados de dichas pruebas son 100% exactos. IDENTIGENE sigue estrictas normas y procedimientos de laboratorio para garantizar que proporcionemos a nuestros clientes resultados de alta calidad sin er-rores todo el tiempo.
¿Cuál es la probabilidad de paternidad? Los resultados de la prueba de ADN establecen una “probabilidad” de la existencia de una relación biológica entre las partes analizadas. Aplicando nuestra tecnología, si el ADN del supuesto padre y el del niño coinciden, la probabilidad de que él sea el padre biológico del niño típicamente es de más del 99,999%. Por otra parte, si hay discordancias entre el ADN de un supuesto padre y un niño, se excluye dicha paterni-dad en un 100%.
¿A qué edad se pueden realizar las pruebas?Pruebas de frotis bucal de mejilla se pueden hacer a cualquier edad, empezando con recién nacidos. Pruebas prenatales también se pueden ejecutar empezando en la novena semana de embarazo, usando procedimientos asistidos por un médico.
¿Quién debería ser evaluado?Idealmente, la madre, niño, y supuesto padre deben ser evaluados. Si bien recibirá resultados exactos sin la muestra de la madre, le reco-mendamos que la madre sea evaluada si es posible. Coordinamos la recolección de muestras, aún si las personas viven en diferentes ciudades. Pruebas involucrando individuos desaparecidos o que han fallecido también son posibles usando una variedad de muestras.

夫定口服液剂在室温下可稳定24小时，在冷藏条件下可稳定30天。对于家庭不具备冷藏条件的情况，不应使用液剂。1 对于无法获得15毫克胶囊的情况下，对体重为10-
16.9公斤的儿童可考虑使用20毫克的胶囊。尽管这样会使剂量超过所推荐的1毫克/公斤的剂量，但是临床试验曾使用过更高的剂量，且一般耐受良好。但对于体重10公
斤以下的儿童，不建议服用剂量超过15毫克的胶囊。
拉米夫定——片剂无刻痕，但可用药房的药丸分割器分为两等份。片剂可压碎，分散于水中或少量食物上并立即服用。对于年龄较小的儿童来说，服用准确剂量的片剂
并不实际，因此体重15公斤以下儿童应使用口服液剂。口服液剂在室温下稳定。2 当儿童体重进入此体重范围内时，片剂剂量由½片改为1片，拉米夫定几乎没有不良反
应，此剂量一般应耐受良好。
齐多夫定——胶囊可打开，分散于水中或少量食物上并立即服用。片剂可压碎，分散于水中或少量食物上并立即服用。对于年龄较小的儿童来说，服用准确剂量的片剂
并不实际，因此体重7公斤以下儿童应使用口服液剂。口服液剂在室温下稳定。根据各自的身高和体重来计算身体表面积值，然后根据身体表面积值来确定所需剂量。
地丹诺辛——为了提供足够抗酸剂来缓冲胃酸以便使药物得到吸收，每剂必须服用2片。给药前可将片剂分散于水中，或者咀嚼或吞服药片。服药之前必须至少在进食之
前空腹30分钟，或在进食之后至少2小时给药。口服混悬液需要加入抗酸剂和水，在室温下仅能稳定24小时，或冷藏条件下可稳定30天。对于家庭不具备冷藏条件的情
况，不应使用口服混悬液。如与印地那韦同时服用，两者必须间隔一小时。根据各自的身高和体重来计算身体表面积值，然后根据身体表面积值来确定所需剂量。
奈韦拉平——片剂为刻痕片，可分成等份。片剂可压碎，分散于水中或少量食物上并立即服用。口服液剂在室温下稳定。奈韦拉平的诱导期剂量在头14天为每日4毫
克/公斤。之后如未出现皮疹，则按以下维持剂量继续用药：8岁以下儿童，每日两次，每次7毫克/公斤；8岁及8岁以上儿童，每日两次，每次4毫克/公斤。3 对此体重范
围内的儿童，可考虑使用液剂药剂以使剂量更准确。4 如果对此体重范围的儿童使用片剂，本图表建议上午服用1片，下午服用½片，以便得到近似于液剂的剂量，奈韦
拉平的半衰期足够长，因此分段剂量引起的药物剂量波动被认为是临床可接受的。
依法韦仑——胶囊可打开，分散于水中或少量食物上并立即服用。口服液剂在室温下稳定。口服液剂剂量比胶囊或片剂大。口服液剂的剂量和药动学性质不如胶囊和片
剂完善。因此，尽管某些场合可获得液剂药剂，但在可能的情况下仍建议使用胶囊或片剂剂型。
洛匹那韦/利托那韦——以洛匹那韦成份计算剂量。胶囊不可打开或压碎，必须整片吞服，可用于能够吞服胶囊的儿童。胶囊或口服液剂应与食物一起服用。胶囊或液剂
在给药前必须冷藏。从冰箱取出后，胶囊和口服液剂在室温（不超过25°C）下仅能稳定60天。当温度可能超过25°C时，应考虑按较小剂量配药的可行性或更频繁地补
加药量（例如，每次配药不超过一个月的药量。）6个月以下儿童不推荐使用洛匹那韦/利托那韦。5 根据药厂建议，液剂剂量已四舍五入至½毫升以便于测量。6 对于17
– 19.9公斤体重范围的儿童，每日两次每次两粒胶囊的剂量会导致高于推荐剂量约40-60%，但仅每日两次每次一粒胶囊的剂量会低于推荐剂量约20-30%。该体重范围的
儿童可考虑使用液剂。
奈非那韦——片剂可压碎，分散于水中或少量食物上并立即服用。必须与食物一起服用以利于吸收。口服粉剂给药需要复杂的给药技术，这在资源贫乏的情况下是不实
际的。7 两岁以下儿童的剂量尚未确定。用于体重10公斤以下的儿童, 一些临床医生对幼童使用的所列剂量最高达到75毫克/公斤/剂，每日两次的剂量。
印地那韦——给药的不方便，毒性及耐受力差造成印地那韦的使用局限性。每8小时的剂量使药物的持续性发生困难。但是也有少量信息关于儿童在印地那韦与利托那
韦一起服用的同时，有每日两次的给药剂量。建议儿童去尝试整片吞服胶囊，因为打开的胶囊里面的粉末会有很重的苦味。必须空腹服用（餐前1小时或餐后2小时）。
如果还需服用地丹诺辛，两者的服用时间必须间隔一小时。在服用印地那韦期间，患者必须大量饮水以防发生肾脏病。根据各自的身高和体重来计算身体表面积值，然
后根据身体表面积值来确定所需剂量。
甲氧苄啶/磺胺甲噁唑——用于人类免疫缺陷病毒感染儿童预防机会性感染的推荐剂量为：5毫克/公斤，每日两次，每周连续服用三天。考虑到甲氧苄啶/ 磺胺甲噁唑悬浮
液剂型的药剂浓度，并为促进坚持用药，对儿童每周每日一次给药可为一种更简便的选择。4毫克/公斤的剂量很容易从儿童的体重换算成悬浮液的毫升数，因为甲氧苄
啶/ 磺胺甲噁唑悬浮液的剂量浓度含量为8毫克/毫升，可按照每公斤体重需用½毫升悬浮液计算剂量。用于治疗细菌性和原虫性感染时采用较高剂量，其它病因应咨询医
师。

GoalThis program will provide a comprehensive review of cardiovascular complications related to cancer therapy. There has been an increased awareness of the interrelation of these disciplines in recent years and the need for cooperation between cardiologists and oncologists is becoming paramount. Additionally, there is an increased awareness in the literature about the need for such collaboration.
Educational ObjectivesAt the conclusion of this educational activity, the participant should be able to
• identify cardiovascular complications related to cancer therapy (chemotherapy, radiation therapy) and common cardiovascular problems associated with cancer;
• describe the pathophysiology of chemotherapy-induced cardiomyopathy and techniques for management;
• discuss management of cardiovascular complications in patients undergoing cancer-related treatment (myocardial infarction, atrial fibrillation, pulmonary embolism, heart failure);
• provide/develop a treatment plan for cardiac complications in patients with cancer and in cancer survivors.
