TELEOLOGICAL THEORIES
Teleology is derived from a Greek word "telos"
that means "ends". This theory is about the
consequences of our decisions that mean when we do
something we should know the end of any action.
For example : Building
One of the most common branches of this
theory is utilitarianism, which has discussed in the 19th
century by Jeremy Bentham and John Stuart Mill. In
this branch we consider an act right if it is usefulin bringing about the best consequences. This
theory can be use in clinical-decision-making.

DEONTOLOGICAL THEORY:
"Deonto" is a Greek word which means
"duty". This theory has discusses by Immanuel
Kant. In this theory we consider basic duties and
right of individuals and groups and actaccordance with the guidelines.

CASE:You have been asked to begin training on
28-years old woman who was brought to
hospital last night because of a car accident.
She underwent surgery last night and theburse told you that her 4-years old daughter
was killed in the accident. And the decision was
not to tell her the truth until she is stable in
her health. Now the patient asks you directly
how are her daughter?

For example, refer back to Case 7 in which you must decide whether to tell a patient thetruth or to lie. If you believe that truth-telling is a sacred duty that you hold dear, then youwould not be able to lie to the patient and tell her that her daughter was alive. If youdid tell her a lie, that would be using the patient as a means toyour desired outcome, every person has an inherent dignity and isentitled to respect. however honorable it might be. In this case you are trying toachieve a better medical outcome. In this theory you do not consider the ends or theoutcomes, but rather the means that you achieve them. In this case you might not be able
to treat the patient, or you could discuss with the physician your feelings prior to enteringthe patient's room to avoid having to go against your duty of truth-telling.
This case is a good example of how conflict can arise within the health care system. If thenurse was using a utilitarian form of decision-making, the decision would have been basedon achieving the best medical outcome for the patient, no matter what means are used,including not being truthful. If the physical therapist is making a decision based on aperceived duty to the patient to treat her with dignity and respect, then the decision wouldbe not to tell a lie. If the theoretical grounding of each person's decision-making approachcan be discussed, fewer conflicts may arise since we would have a better understanding ofthe basis of a person's actions. While conflicts will continue, there may be a greater
willingness to reach a compromise on behalf of the patient's welfare, and in most casesthese resolutions must be made in a brief period of time.
