April 16, 2015 Dear Colleague and Preceptor, Thank you for agreeing to participate as a preceptor in the final course Nursing 1072: Advanced Nursing Practice Scholarship for a Master of Nursing student at the University of Toronto. We truly appreciate the support and guidance you will be providing to the graduate student who will be completing a practicum with you between April 21, 2015 and June 23, 2015. We greatly appreciate the time you will spend with this student, your professional commitment, and your advanced nursing practice knowledge and skill. The student you will be preceptoring has come to you well-prepared but concerned about having a great learning experience during this final practicum course. You are a crucial person in making this a positive learning experience and facilitating the transition into the advanced nursing practice role. Attached you will find a course outline for Nursing 1072. Included in the course outline is detailed information about the requirements for the course, including assessment / evaluation strategies, preceptor role expectations, student role expectations, and so on. Nursing 1072 provides learners who are in the FINAL term of their MN program with opportunities to integrate previous learning and continue the analysis, synthesis and integration of advanced theoretical principles and concepts related to advanced nursing practice. The Canadian Nurses Association (CNA) has identified core competencies required of advanced practice nurses in the document 'Advanced Nursing Practice: A National Framework' (CNA, 2008). A copy of this framework is accessible at the following website: https://www.cna-aiic.ca/~/media/cna/page-content/pdf-en/anp_national_framework_e.pdf This course specifically provides students with opportunities to further develop and establish their knowledge, skills, and judgment related to CNA categories of competencies: clinical, leadership, research and consultation and collaboration. Which competencies the student focuses on in this course is dependent on their field of study in the MN program (administrative or clinical) and their future goals. Details of course expectations are described in the course outline. Generally, the expectations are that upon completion of the course, the learner will be able to:

1. Access, appraise, and judiciously apply research and a wide range of nursing and related theoretical knowledge to guide advanced nursing practice as outlined by the Canadian Nurses Association (2008).

2. Identify the scope of advanced nursing practice roles and understand the advanced nursing practice role within the context of interprofessional practice.

3. Systematically identify issues and factors that facilitate and constrain accomplishment of advanced nursing practice objectives and/or interventions to support client and healthcare system outcomes.

4. Articulate key challenges and strategies for advanced practice nurses to promote scholarship in health care, education, and/or policy settings.

5. Communicate professional contributions and scholarly work through formal presentations.

Learners MUST complete 170 practice hours, keep a log of these hours, and complete a brief description of their weekly practice activities. These logs are to be signed by you and returned to us at the mid-point of the course and at the end of the course. Please note that learners MUST be available during normal hours

of work for your role. They must have created a learning contract for the course experience that is approved by you and the faculty person leading the section where the student has been assigned. Students will be attending mandatory seminars at the university on the afternoons of April 21st, May 5th, May 19th, June 2nd, June 9th and the final poster presentation for the practicum experience on June 23rd. On the final day of the course, June 23rd, 2015, students will be hosting an Advanced Nursing Practice Symposium at the Lawrence S. Bloomberg Faculty of Nursing. At this symposium, each student will display and describe a poster outlining their major accomplishment(s) within their practicum experience. We hope you will be able to join us anytime between 1-3pm to view student posters and reflect with them on their practicum experience over their final term in the MN program.

You are asked to assess student performance twice during the course, at mid-term (week 5, May 19th) and at end of term (week 9, June 19th). The assessment form includes assessments ratings of student accomplishment of their achievement indicators related to each of their approved learning objectives, their communication competence, and their engagement in evidence based practice. There is also one overall assessment rating. We welcome you to also add some remarks about the student’s progress, accomplishments, and challenges. Although we ask you to assess student accomplishment, the final pass / fail grade is determined by me in consideration of your assessments and other required course evaluation components described in the course outline. A student MUST pass all five components of the course to receive a final grade of ‘pass’ in this course. We have implemented an online process for submission of our practicum evaluations in Nursing 1072. Students will be required to submit their mid-term and end of term self-assessments online. Preceptors are asked to also submit their assessments of students on-line through a system called Link Health Pro (LHP). LHP is a simple system to use for student assessment. Your email address will be used as your user name and you will be provided with a password to access both the online mid-term and end of term evaluation documents for the student. You will also be able to see your student’s draft and final self-assessments (mid-term and end of term) before you need to complete your assessment. Thank you very much for your support. While we do not routinely meet with preceptors, we are always available to discuss any concerns that arise related to this student throughout the practicum. Please do not hesitate to call or email me if we can be of assistance or if you have any questions or concerns about the program or the student. It would be wonderful to see you at the Advanced Nursing Practice Symposium poster session on June 23rd, 2015 (1-3pm), if you are able to make it. Sincerely,

Doris Howell and Kimberley Widger Doris Howell RN PhD and Kimberley Widger RN PhD CHPCN(C) Email: [email protected], [email protected],

MN Program Overview The Master of Nursing Program is designed to prepare advanced practice nurses to provide professional leadership in their chosen advanced practice field. The MN program has three separate fields: Nursing Administration, Clinical Nursing, and Nurse Practitioner [Adult or Child Stream]. Here we focus on the MN program in relation to two fields of study: Nursing Administration and Clinical Nursing. The MN program is a course-based program including completion of nine graduate courses. There are 4 foundation courses completed by all MN students: Nursing 1017 (History of ideas/theory) Nursing 1022 (Research utilization - quantitative) Nursing 1028 (Research utilization– qualitative) Nursing 1034 (Program planning & evaluation) Students take one course from a group of 5 courses related to relational / interpersonal issues (Nursing 1012/1021/1032/1043/1016: ethics/profession/group/interpersonal relations) Students also must complete 3 courses from a defined course list related to their field of study (administration or clinical). In the FINAL term of their program, students MUST complete Nursing 1072: Advanced Nursing Practice Scholarship, a final consolidation course.

Student Assessment through Link Health Pro Student assessments are completed online through our online evaluation system – Link Health Pro (LHP). LHP hosts a community of students and preceptors affiliated with the Lawrence S. Bloomberg Faculty of Nursing. As preceptor, you will need to complete a student assessment at mid-term and end of term. Students must first prepare and discuss with you their own self-assessments at both times. Students will also complete preceptor and practicum settings evaluations. Aggregate results will be provided to the organization annually once a sufficient number of evaluations have been received to assure privacy and confidentiality for the student and preceptor. If you do not already have an LHP account with us, we will set up your account with LHP and email you an access password. If you have used LHP previously, you may sign on with your current login name and password. Preceptors are invited to build their profile within the LHP system, and are encouraged to share a photograph, which helps build recognition as part of the Bloomberg Faculty. We ask that you follow the procedures outlined below for student assessment.

Student Assessment Procedure

LHP will be utilized by learners and preceptors to complete mid-term and practicum final assessments. There are two formal assessment periods in Nursing 1072: mid-term (week 5) and final assessment (end of week 9). The procedure for student self-assessment and preceptor assessment of students at both periods of time should be the following (at a minimum):

1. Student uses self-reflection and previous preceptor and other health team member feedback to draft a self-assessment in LHP related to accomplishment of practicum learning objectives, communication skills, and engaging in evidence based practice.

2. Student shares self-assessment with preceptor and meets with preceptor formally to discuss progress and self-assessment. (The draft self-assessment is easily printed from LHP by the student to guide these discussions.)

3. Student submits self-assessment through LHP. 4. Preceptor completes student assessment (same as did student) and submits this into LHP. 5. Faculty reviews student self-assessment and preceptor assessment and follows up, as needed, with

student and preceptor. To complete a student evaluation assessment: You cannot complete an evaluation until the student has completed theirs. Once this is done – you will receive an email prompt.

1. Sign in to Link Health Pro (www.linkhealthpro.com) 2. Select Evaluations from the left menu 3. Click on the Clinical Evaluation button 4. Select the evaluation form specific to the course the student is enrolled in

(M for Midterm and F for Final / End of term) 5. Click Start to proceed with the evaluation 6. You will be able to view the student’s learning objectives and achievement indicators related to

CNA (2008) advanced nursing practice competencies. You will also see a small section related to their communication competencies and their engagement in evidence-based practice. We ask that you assess the student on each of their achievement indicators within each section and in the final overall assessment indicator. Please add some additional comments in the comment section.

7. Once you are finished with the evaluation, click on the Submit button. 8. Note that there is a save function. You may save evaluations in progress. However, the assessment

is not submitted unless you click on Submit.

COURSE DESCRIPTION

Nursing 1072 provides learners who are in the FINAL term of their MN program with opportunities to

integrate previous learning and continue the analysis, synthesis and integration of advanced theoretical

principles and concepts related to advanced nursing practice. According to the Canadian Nurses

Association ( 2008, p. 9), “advanced nursing practice is an umbrella term describing an advanced level

of clinical nursing practice that maximizes the use of graduate educational preparation, in-depth nursing

knowledge and expertise in meeting the health needs of individuals, families, groups, communities and

populations”. The Canadian Nurses Association (CNA) has identified core competencies required of

advanced practice nurses in the document 'Advanced Nursing Practice: A National Framework'

(CNA, 2008). This course specifically provides you with opportunities to further develop and establish

your knowledge, skills, and judgment related to CNA categories of competencies: clinical, leadership,

research and consultation and collaboration. Which competencies you focus on is dependent on your

field of study in the MN program and your future goals. You MUST be very familiar with this

framework. Advanced nursing practice involves analysing and synthesizing knowledge; understanding,

interpreting and applying nursing theory and research; and developing and advancing nursing

knowledge and the profession as a whole” (CNA, 2008, p. 10). In this course, learners activate, build,

and refine the knowledge and skills that are consistent with the core competencies as outlined in the

framework including clinician, researcher, leader, and consultant/collaborator. Learners are expected to

integrate knowledge from previous course work as well as current readings.

Nursing takes place in a variety of settings and contexts, including, but not limited to, hospitals,

communities, clinics, and educational settings. Scholarship in advanced practice is not confined to any

one role or setting. Scholarship is an attribute of the advanced practice of nurse clinicians, educators,

administrators/ managers, and researchers. Thus, Nursing 1072 is completed in a variety of settings to

allow options and selection of a focus for scholarship in practice that builds on the foundation of

completed course work and professional experiences. These foci include clinical, education,

management / administration, and research, including knowledge translation. Learners have options to

pursue practicum placements in any of the following settings: acute care hospitals, the community, long-

term care, complex continuing care, or colleges / universities. The practicum placement should align

with the individual student‟s learning goals, completed course work, and future career goals. Course

participation consolidates learner development as an advanced practice nurse.

Throughout Nursing 1072, learners engage in the following:

a formal guiding relationship with an advanced practice nurse preceptor

demonstration of advanced nursing practice competencies (CNA, 2008)

actively participation within a community of learners to understand and integrate advanced

practice nurse competencies.

Course Objectives

Upon completion of the course, the learner will be able to:

1. Access, appraise, and judiciously apply research and a wide range of nursing and related

theoretical knowledge to guide advanced nursing practice as outlined by the Canadian Nurses

Association (2008).

Page 13: Preceptor Information Package Nursing 1072 Advanced Nursing …€¦ · Preceptor Information Package Nursing 1072 – Advanced Nursing Practice Scholarship Faculty: Doris Howell,

NUR 1072 Advanced Nursing Practice Scholarship – Preceptor Package (Summer 2015)

13

2. Identify the scope of advanced nursing practice roles and understand the advanced nursing

practice role within the context of interprofessional practice.

3. Systematically identify issues and factors that facilitate and constrain accomplishment of

advanced nursing practice objectives and/or interventions to support client and healthcare system

outcomes.

4. Articulate key challenges and strategies for advanced practice nurses to promote scholarship in

health care, education, and/or policy settings.

5. Communicate professional contributions and scholarly work through formal presentations.

The course objectives will be met through the following strategies:

development of a guiding / preceptor relationship with a masters (minimum) prepared nurse

engagement in advanced nursing practice

active participation within a community of advanced nursing practice learners during seminars

completion of assessment / evaluation requirements for the course.

Course Format

Nursing 1072 includes both a seminar and practicum component which provides students with

opportunities to integrate theory and research into their advanced nursing practice.

Seminar Component: Course members will interact regularly over 10 weeks: in class for five 3-hour

seminars plus a final 5-hour class on the last day of the course which includes a poster presentation and

completion of course assessments / evaluations. Attendance at all seminars is compulsory.

Practicum Component: The advanced nursing practice practicum is completed with support of a

graduate prepared preceptor for approximately 17- 19 hours per week (over a 9 to 10-week period)

for a minimum total of 170 hours (usually all on the practicum agency site or at other related planned

learning activities). Practicum hours should be completed by the end of course week 10, though the end

of week 9 is preferred. The practicum placement should be carefully chosen to support integration of

previous learning during the MN program and in consideration of future career goals. Securing a

supportive, advanced practice nurse preceptor who is committed to guiding the learner into the role of

advanced practice nurse is essential in this course. Learners must have their formal learning contract

approved by both the preceptor and the faculty course leader. Initial learning objectives will be

developed prior to the start of the practicum and these will be revised and then confirmed by the

preceptor and faculty course leader by the third week of the course.

EVALUATION / ASSESSMENT METHODS

There are five sources of assessment / evaluation in this course. To receive a pass credit for Nursing

1072, you must achieve a pass grade in all five course components identified below. The five course

assessment / evaluation components are graded individually as pass/fail and all components must be

successfully completed to achieve a pass in the course. The faculty member is responsible for assigning

the pass/fail grade for all five components of the course. Failure to meet expectations in any one of the

course assessment / evaluation components, including advanced nursing practice scholarship in the

practicum, results in a fail grade. These five components include:

1. Submission of two practice scholarship logs (submitted weeks 5 and 9) that demonstrate and

describe learner performance of advanced nursing practice related to accomplishment of

approved learning objectives for the practicum experience

2. Active participation in seminar discussions.

3. Completion of an APN – Practice paper (due week 5) about student application of knowledge

and skills related to CNA competencies of APNs.

4. Submission of an abstract in response to the call for abstracts that meets criteria specified in the

call (final abstract due week 8).

5. Successful presentation of a poster presentation during the Advanced Nursing Practice

Symposium to the class, preceptors, and invited guests (in week 10).

Demonstration of Advanced Nursing Practice Scholarship (Weeks 1-10)

Learners are expected to spend approximately 19 hours per week [over weeks 1 through 10, though by

the end of week 9 is preferred, for a total of 170 on-site practicum placement hours] in the practice

setting. Learners MUST submit practice scholarship logs that are signed by their preceptor (twice during

the course) as evidence of the completion of their practicum hours (due weeks 5 and 9). Successful

completion of an approved learning contract is required to complete the course. Learners complete a

self-assessment / evaluation of the learning experience, guided by the standards of practice for advanced

nursing practice (CNA, 2008). Preceptors will assess learners in relation to completion of the learning

contract objectives at mid-term and at end of term. The final pass/fail grade will be determined by the

teaching faculty member.

Preparation for Advanced Nursing Practice Scholarship within Practicum

Criteria for selecting a practice placement and the process involved are outlined on-line at

www.nursing.utoronto.ca/students/clinical. Learners may ONLY begin their practice hours once ALL

the following three conditions have been met:

after the introductory class (April 21, 2015)

after securing faculty review of the DRAFT learning contract (during class April 21, 2015)

after the Graduate Placement Coordinator (Kathy Trip) and Clinical Placement Office (Sharon

Lee) confirm that all documents have been received and has deemed the student to be eligible to

begin practice.

Your Learning Contract

Each learner is responsible for developing a learning contract with guidance from the preceptor and the

course faculty. A template for the learning contract is attached at the back of this course outline. The

learning contract must be discussed with the preceptor and approved by the course faculty by week 3.

Page 19: Preceptor Information Package Nursing 1072 Advanced Nursing …€¦ · Preceptor Information Package Nursing 1072 – Advanced Nursing Practice Scholarship Faculty: Doris Howell,

NUR 1072 Advanced Nursing Practice Scholarship – Preceptor Package (Summer 2015)

19

The learning contract will outline key learning objectives (usually 3-5 [maximum] objectives) guiding

their advanced nursing practice experience. The objectives are developed in conjunction with the

preceptor who will use these and the core course practice scholarship competencies to assess the student.

Each objective must be linked to one or more areas of advanced nursing practice competencies identified

in the framework for advanced nursing practice (CNA, 2008). The learning contract details activities

(including strategies and resources) for achieving objectives, achievement indicators, and timelines for

completion. The DRAFT contract MUST be brought to Week 1 class and a confirmed learning contract

MUST be submitted to the course faculty at the beginning of Week 3 class. There will be an opportunity

to discuss DRAFT learning plans in the first class. Please note that learners are expected to have

talked with their preceptor at least once prior to the first class of Nursing 1072 to discuss

expectations of their roles and to explore how the student may contribute to a project relevant to a

new or ongoing project in their clinical placement.

Advanced Nursing Practice Scholarship Placement Expectations

All learners are expected to meet regularly with their preceptors throughout the term during work hours

for advanced practice preceptors (Monday to Friday) and should expect to spend a majority of their time

in the practice setting (with their preceptors or in other planned practicum activities) to ensure that they

maximize learning about the advanced practice nurse role, expectations, and challenges. Learners

MUST complete a self-assessment of the learning experience in relation to the objectives outlined in

their learning contracts. Preceptors will assess learner‟s completion of the learning contract relative to

advanced nursing practice core competencies.

During the practicum, learners are expected to contribute to their practicum agency through a negotiated,

mutually satisfying, scholarly focus that will be articulated in poster format in week 10. The scholarly

focus must be completed within the placement experience of 170 hours and may take the form of a

negotiated project OR active planning and delivery in teaching / learning interventions for staff or

patients / families OR working with nursing staff and other expert health care team members to ensure

that excellent nursing care is planned, provided, and evaluated. All practice scholarship must

concentrate on some aspect of the advanced nursing practice role.

COURSE DUE DATES:

Assessment Using Link Health Pro (LHP)

LHP will be utilized by learners and preceptors to complete mid-term and final practicum assessments.

There are two formal assessment periods in Nursing 1072: mid-term (week 5) and final assessment

(week 9). The procedure for student self-assessment and preceptor assessment of students at both

periods of time should be the following (at a minimum):

Page 20: Preceptor Information Package Nursing 1072 Advanced Nursing …€¦ · Preceptor Information Package Nursing 1072 – Advanced Nursing Practice Scholarship Faculty: Doris Howell,

NUR 1072 Advanced Nursing Practice Scholarship – Preceptor Package (Summer 2015)

20

1. Student uses self-reflection and previous preceptor and other health team member feedback to

draft a self-assessment in LHP related to accomplishment of practicum learning objectives,

communication skills, and engaging in evidence based practice.

2. Student shares self-assessment with preceptor and meets with preceptor formally to discuss

progress and self-assessment. (The draft self-assessment is easily printed from LHP by the

student to guide these discussions).

3. Student submits self-assessment through LHP.

4. Preceptor completes and submits student assessment through LHP.

5. Faculty reviews student self-assessment and preceptor assessment and follows up, as needed,

with student and preceptor.

Student self and preceptor assessments of students must be submitted through LHP. If a preceptor is

unable to complete and submit student assessments through LHP, arrangements may be made to

complete paper versions of the student assessment.

Two additional assessments should be completed at the end of the term on LHP by Nursing 1072

students:

1. Practicum setting evaluation

2. Preceptor evaluation.

Both these assessments are very important to determine the appropriateness and effectiveness of

practicum placements and preceptors. These evaluations are NOT released directly to preceptors until

there is at least 5 assessments of the preceptor (to promote student anonymity).

Active Participation and Leadership in Seminars (Weeks 1, 3, 5, 7, 8, and 10)

Throughout the practicum experience, learners are expected to share aspects of the practicum in seminar

discussions. At the completion of the practicum, students will be expected to evaluate the degree to

which they were able to meet their learning objectives.

Attendance at all seminars is mandatory and is part of the professional behavioral of a learner in the

Master of Nursing program. Participation is defined by open dialogue regarding class issues/topics,

meeting all deadlines for class assignments, and facilitating equity of discussion among all class

members during a seminar class.

Advanced Practice Nurse - Practice Paper (Due week 5) (Pass/Fail)

This scholarly paper focuses on the student‟s application of knowledge and skills used in the practicum

experience in relation to the Canadian Nurses Association (2008) competencies of Advanced Practice

Nurses.

Students must describe the evidence / theory that has guided their advanced nursing practice in this

practicum (evidence / theory based practice). Students must also address how one other area of APN

competence (as per CNA, 2008) was applied in the practicum (e.g., leadership, collaboration and

communication, clinical).

The paper must include:

- A brief introduction to the practicum setting and a clear statement of your focus for the

practicum (your project) and the rationale based on needs of the setting

Page 21: Preceptor Information Package Nursing 1072 Advanced Nursing …€¦ · Preceptor Information Package Nursing 1072 – Advanced Nursing Practice Scholarship Faculty: Doris Howell,

NUR 1072 Advanced Nursing Practice Scholarship – Preceptor Package (Summer 2015)

21

- Critical analysis and synthesis of the key research and theoretical sources / references

(demonstration of your evidence based practice knowledge and skill- the research competency)

informing your project

- Description of how at least one other APN area of competence is/ will be applied to the project

- Summary of key learning and insights that will guide your project

The paper must be NO LONGER THAN 1750 words or 7 pages (including text, headings, and

reference citations within text). Title page and references are not included in the word count. Paper must

be formatted according to APA 6th

Edition, using 12 point font, and be double spaced. A hard copy

MUST be submitted in class on the due date and MUST be first submitted to turnitin.com. Learners are

welcome to write from a subjective perspective or use a more formal objective perspective as long as

there is clear evidence of critical analysis and integration of theory and research in the paper.

Abstract Submission (Due Thursday of the week 8, after receiving peer review in class) (Pass/Fail)

Learners will be provided with an Advanced Nursing Practice Symposium “Call for Abstracts” in class

Week 6 (and attached at back of this course outline) that will guide their development of an abstract

(maximum 300 words) for the poster presentation at the Advanced Practice Nursing Symposium in the

final class week. Learners will prepare and submit an abstract in response to the Call for Abstracts.

Abstracts will be reviewed by peers and faculty. Learners will receive a pass/fail grade and feedback that

will be useful when developing their poster. The Call for Abstracts will be presented in class in weeks 5

and 7. After peer review in class week 8, learners will have the opportunity to revise their abstract and e-

mail the abstract to the faculty member by the Thursday of Week 8 for inclusion in the abstract booklet

for the Advanced Practice Nursing Symposium.

Poster Presentation: Advanced Nursing Practice Symposium (Completed Week 10) (Pass/Fail)

Learners will prepare and present a poster during the final class – Advanced Nursing Practice

Symposium - that highlights their practice scholarship during their Nursing 1072 practicum. Judges will

be present during the poster presentation to pose scholarly questions to presenters and to provide

feedback. Posters may take the form of one large poster or they may be 6-8 printed power point slides.

Learners will be provided with clear instructions following acceptance of their abstracts. Preceptors,

peers, and other learners in the graduate program will be invited to attend the poster presentation.

Learners are reminded that they may not publish their findings or present their poster outside of

their practicum setting and the poster symposium, if it reports upon any data collected without

prior ethical review being completed at the University of Toronto and the practicum setting.
