APPLICATION FOR PRE-QUALIFICTION OF CONTRACTORS FOR MAINTENANCE WORK AT AIIMS PATNA

(DETAILED NOTICE FOR PRE-QUALIFICATION)

AIIMS Patna on behalf of Director, AIIMS Patna invites application from class-1 contractor enlisted to CPWD, MES, Railway and State government PWD’s dealing with buildings and roads and of composite category (Civil & Electrical) for maintenance work of various buildings in the campus.

Scope of work: 1. Hostel Buildings 2. Medical College Buildings 3. Hospital Buildings 4. Miscellaneous Buildings 5. Residential Quarters

The contractors should meet the minimum qualifying criteria as mentioned in PQ document. The detailed PQ (Pre-Qualification) document can be downloaded from websites

www.aiimspatna.org. The contractors should according to their eligibility apply for the

maintenance works only. Application for pre-qualification supported by prescribed annexure should be submitted on sealed envelope duly super scribed with name of work and date of opening. The application will be received upto 14.00 hrs. on 10/08/2017 and will be opened by the committee formed by Director AIIMS Patna in the office of the Faculty-in-charge, Procurement Cell, AIIMS Patna, Medical College Building, Phulwarisharif, Patna-801507 in presence of intending applicants. The tender will be opened on 10/08/2017 at 15.30 hrs and Date & Time of Pre-bid Meeting is 21/07/2017 at 15.00 hrs. Application complete in all respects should reach to the Faculty-in-charge, Procurement Cell, AIIMS Patna, Medical College Building, Phulwarisharif, Patna-801507 along-with non-refundable processing fees of Rs.1,500/- (Rupees One Thousand Five hundred only) payable in the form of Demand Draft in favour of “AIIMS Patna” payable at Patna.

No Joint venture or Consortium of firms shall be allowed and the contractors should meet the above criteria by themselves.

The intended contractors should apply on the prescribed documents as per the PQ document along-with documentary evidence for the following:

1. Copy of Certificate of Registration in case of registration with CPWD/MES/Railways/

State PWD etc.

2. Copy of completion certificate of similar nature and magnitude’s project/works.

3. Copy of registration certificate for service tax, works contract tax, PF, ESI, labour

license, PAN no. and others related to construction works by Central/State Govt..

4. Copy of ITCC for last three years.

5. Audited Balance Sheet for the last three years i.e. 2014-15 2015-16 and 2016-17.

6. Completion Certificates of works from Clients / Owners,

7. Plant & Machinery holding,

8. Manpower details,

9. Details of pending litigation etc.

10. Valid Electrical License, if applicable

11. GST Regn. No. along with Valid Clearance Certificate.

12. Bank Solvency Certificate from a scheduled Bank for 40% value of Pre-qualification Limit issued not before March 2017.

13. The work shall be executed as per DSR 2016 Rates ( Civil, Electrical & Mechanical)

and non-schedule items rate shall be derived from existing market rate and the

decision of Director shall be final and binding.

Last date of receipt of pre-qualification application is 10/08/2017 upto 14.00 hrs. The delayed application shall not be considered. Three agency will be selected for maintenance work of AIIMS Patna. Approximate cost of maintenance for one year is approximately 2 crore and period of contract will be 36 months. The work shall be executed as per CPWD specification with updated correction slip and General Condition of Contract 2014 as amended up to date of CPWD shall be followed. The schedule DSR 2016 (with up to date correction slip) OR the items which are not covered in DSR 2016 the prevailing market rate shall apply. The institute shall invite quotations from among the enlisted vendors for any specific works/Maintenance works. However the rates quoted should not be more than the DSR rates 2016 (Civil, Electrical & Mechanical). The short-listed of pre-qualified contractors shall be valid for three year. Subject to yearly renewable on satisfactory performance, if the performance is not satisfactory the shortlisted contractor may be terminated. Corrigendum / addendums, if any to this PQ shall only be posted on AIIMS website and shall be bindings to the contractor. .

GENERAL TERMS & CONDITIONS RELATING TO WORK

1. The contractor shall furnish all details including their experience in the Application Format, (only and if the space provided is not sufficient, particulars shall be furnished in Annexure and such details shall be mentioned in the respective columns of the Application Format. The authorized person of the contractor shall sign in all the pages of the application with seal of the company/ firm.

2. Contractor should submit Application Format along with company profile/ Brochure

as mentioned in Advertisement.

3. No costs incurred by the applicant in applying, in providing necessary clarifications or attending discussions or site visits will be reimbursed.

4. The contractor shall furnish documentary proof with respect to the prequalification

criteria along with the application form.

5. Incomplete application without proper proofs for establishing their credentials will be liable for rejection and no correspondence will be entertained in this regard.

6. The evaluation shall be mainly based on the eligibility criteria, other factors like

timely completion, resources of the applicant, pre-qualification with other institutions/ works etc, manpower & logistical support of the applicant, their financial capabilities, quality consciousness, etc. will be considered as added qualifications. If found necessary, the contractor shall make arrangements to inspect any of the works undertaken by them.

7. Applications received after the due date and time shall be rejected.

8. AIIMS expects that contractor should furnish all the required documents to ensure a

transparent and genuine evaluation, so it is necessary to fill in the Pre-Qualification documents meticulously and sign & stamp each and every page.

9. Fill in the Pre-Qualification Documents in writing in ink or type.

10. Pre-qualification documents can be obtained from the Office of Procurement Cell

on any working day during office hours or may be downloaded from AIIMS website. Last date for submission of documents is 10/08/2017.

11. As the pre-qualification process is completed, it would be informed to all the

participants about the status of evaluation and afterwards tenders will be invited from pre-qualified firms / contractors.

12. Contractor will be given a rent free office and store space where his supervisor and

other staff will normally sit and place their tools & tackles and materials. However all furniture, stationery etc. required for carrying out his office works & the storage arrangement for storing his materials will be organized by the Contractor at his cost.

13. Contractor will keep at site at all times all essential tools and tackles required for

execution or routine maintenance jobs as attached with Annexure-III. In case any worker is found to be working without proper tools & tackles, Engineer-in-charge will be within his right to stop the work or levy some penalty.

14. Contractor will efficiently utilize his maintenance gang and cleaning gang and ensure that there is no idle period for occasional need of additional helpers required for a short duration, contractor will try to utilize his sweepers instead of bringing manpower from outside.

15. Each workman will have to attend his duty regularly. In case of absence of any

worker deduction will be made from the contractor’s bill at the rate of daily wages of the worker. If a workman is absent for more than two (2) days, contractor should immediately make alternative arrangement by bringing additional manpower.

16. Following Holidays will be applicable to the manpower under the contract:- a) Sundays; b) 3 National Holidays (26th Jan, 15th Aug, & 2nd Oct.)

In case of emergency you will have to attend as and when required by Engineer-in-

Charge.

The working time will be from 9.00 A.M. to 5.30 P.M.

17. All maintenance complaints should normally be attended within 2 days. In case of tedious/tricky complaints, time taken should not exceed 7 days. Complaints having some special problems, which can’t be done within 7 days, the complainant should be informed by giving a note indicting the date when the complaint is likely to be completed. In case of complaints/jobs where the occupant has to observe some precautions, he should be duly informed about this in writing after taking approval of Engineer-in-charge. In case of unreasonable delay the penalty clause will be apply.

18. All the materials brought by the contractor shall be ISI marked (wherever applicable) and of reputed & approved makes and their makes/brands./colour/finish will be approved by Engineer-in-charge before providing in the work. A list of suggested makes for materials will be provided during execution.

19. Contractor shall ensure that at all times sufficient quantity of various materials

required for routine maintenance are available in the store provided by client to contractor for the said purpose, so that all complaints can be speedily attended. Contractor should devise an efficient arrangement for procuring materials speedily from market as and when requirement arises.

20. Adequate signage work in progress, inconvenience caused in regretted or

diversion sign shall be put on the sites conspicuously visible even during night hours. The construction Malba at site shall be regularly removed. All field official and worker must be provided with safety helmets, safety shoes and safety belt.

22. DECISION OF THE EMPLOYER The Institute reserves the right to reject any prospective application without assigning any reason and to restrict the list of pre-qualified contractors to any number deemed suitable by it, if too many applications are received satisfying the basic PQ criteria. The decision of the Institute is final and binding. No interim inquiries/correspondence in this regard shall be entertained

23. PRE-QUALIFICATION EXPENSES All pre-qualification applications are received on the understanding that the Employer shall not entertain nor be held liable for any claims for expenses incurred by applicants in connection with the pre-qualification exercise.

24. If any information furnished by the applicant is found incorrect at a later stage, he shall be liable to be debarred from tendering/taking up work in the Institute.

ELIGIBILITY CRITERIA

1. The Firm/contractor may be an individual or Partnership Firm.

2. The reputed Firms preferably have valid registration in category Class-1, composite category (Civil & Electrical).

3. The contractor should have successfully completed similar works during the last five years ending March 2017.

4. The work means the same as shown on the cover sheet of this document which includes –

maintenance of building infrastructure including electrical work, plumbing & sanitary work, Carpenter/wood work, Drainage & sewerage System and all allied works etc. or having experience in construction of multi stored building not less than G+4

5. The contractor should have an average annual turnover (in the last three years ending

March 2017) enough to prove the approval / accepting of committee, and the capacity to successfully complete the work.

6. The contractor should have enough capacity in terms of personnel, labour force both skilled

and unskilled, equipments, plants and machinery, to satisfactorily complete the job in stipulated time.

7. The contractor shall furnish the list of key employees, employed by him with full details of

their previous employment.

8. The contractor shall submit copies of the Service Tax registration forms along with copy of Paid Tax.

9. Must not have ever been debarred by anybody from tendering for public/private project in India. The intending contractor shall submit a declaration & affidavit that he has not been debarred from tending by any authority during last five years.

10. Experience of having successfully completed works during the last 5 years ending last day of the month previous to the one in which applications are invited:-

Three similar completed works each of value not less than the amount equal to 40% of the

estimated cost put to bid

OR

Two similar completed works each of value not less than the amount equal to 60% of the

estimated cost put to bid

OR

One similar completed work of value not less than the amount equal to 80% of the

estimated cost put to bid.

The AIIMS reserves the right to verify the particulars furnished by the applicant independently. The AIIMS reserves the right to accept/reject any prospective application without assigning any reasons and no claims shall be entertained. The AIIMS reserves the right to decide on the number of pre – qualified contractors to participate in the bid among the number of eligible entrants.

SELECTION PROCESS

AIIMS shall evaluate the pre - qualification applications based on the eligibility criteria set forth and shall short list from the qualified applications. Only top three contractor shall be enlisted for the period of three year from the date of award of the letter, securing points out of maximum 100 points. All Applications, which meet the above requirements, will be evaluated under the following criteria by scoring method on the basis of details furnished by them.

AIIMS reserves the right in deciding the number of pre-qualified entrants, and right to accept/reject any or all the applications. The AIIMS reserves the right to modify any or all of the pre-qualification criteria. Total points 100 units and the details are as under:
