HELPFUL INFORMATION WHEN USING THIS PRACTITIONER DIRECTORY

1. PRIMARY CARE PRACTITIONERS (PCP): The Health Plan practitioners are listed in alphabetical order under the correct specialties. They are also listed based on their county. You must choose a PCP from the list in the Primary Care section of this directory. You may change your PCP at any time by calling or writing The Health Plan.

Non-Physician Practitioners can be Listed as a PCP .Certified.Nurse.Practitioners West Virginia Physician Assistants

2. SPECIALTY CARE PRACTITIONERS OR SERVICES: If you need a specialist or specialty services, your PCP will refer you to a practitioner who is a part of the Plan. You need approval from your PCP before getting care from a specialist. If you listed the OB/GYN with The Health Plan, you don’t need any referral.

Non-Physician Practitioner can be listed as your Secondary Care Provider .Certified.Nurse.Midwife

3. EMERGENCY/URGENT CARE SERVICES: Unless it is urgent or an emergency, The Health Plan will not pay for care planned or given by practitioners who are not part of The Health Plan. If you need urgent care, call your PCP.first.for.advice..If.you.have.an.emergency.and go to any emergency room, call your PCP as soon as you can. If you are admitted to the hospital, call your PCP to help manage your care. All follow-up care after emergency or urgent treatment must be done in the network practitioner’s.office.to.be.paid..If.you.are.told.to.see.a.specialist,.be.sure.to.check.this.book.first..You can also call Customer Service at the phone number in this book. If you received medical care from a nonparticipating practitioner, do not pay the bill. Please send any bills to the Plan to review. You can also call Customer Service at the phone number in this book.

4. DEFINITIONS:

a. “Emergency services” means covered services that are:1. supplied by a practitioner and2. needed.to.figure.out.and.treat.an.

emergency condition. Emergency services are paid for in and out of The Plan.

b. “Emergency medical condition” is a medical condition that has severe, harsh warning signs. It can include severe pain. A prudent person might think the following could happen without immediate care:• severe risk to the health of the person

or unborn child,• severe harm to bodily functions,• severe harm to organs.

c. “Urgently needed services” means covered services generally given when a member is away from the Plan service area for a short time; but care is required right away. For example, there is sudden bad health, injury or condition. It is not reasonable to get the service through the Plan network. This is mostly for services that are outside the Plan’s service area. There might be unusual conditions. An example is brief trouble with access to the Plan’s network. Reasons are a stroke or a short-term barrier to traveling to Plan practitioners who are on hand.

d. Clinic pharmacy is within a medical clinic.e. Dispensing prescriber is a practitioner

who is able to sell medicines.f. Home infusion pharmacies specialize in

the preparation of medicine that is given by IV or by injection.

g. Long-term care (LTC) pharmacies provide medicine to residents of LTC facilities (nursing homes, residential care facilities, mental health facilities, hospices, and long-term care or skilled nursing care beds in hospitals).

h. Retail pharmacies sell medicines to patients.

i. Specialty pharmacies coordinate many aspects of patient care; improve outcomes for patients with complex conditions; provide patient education.

j. VA pharmacy can distribute medicines to veterans.

5. PODIATRIC OR CHIROPRACTIC SERVICES: If you need a chiropractor, a referral is needed only for children under 18 and/or out-of network care. It must be approved by the Plan’s Clinical Services Department before your visit. Chiropractors are limited to certain services. It is based on their contract with the Plan. Podiatric services require pre-authorization for surgical.procedures.other.than.in-office..The.Health Plan reviews these claims. Podiatrists who do procedures are limited by the Plan rules. This is based on experience, training, and.certification..This.does.not.include.routine.foot care.

6. ANCILLARY/OUTPATIENT SERVICES: Providers such as ambulance, outpatient surgery centers, medical tools and goods, physical therapy, lab, X-ray, etc. are listed in the back of this book. You can see any practitioner in this part of the book. Some services need a referral from your practitioner. Be.sure.you.get.the.referral.first..Please.read.your.Member.Handbook.to.get.details.about.this process.

7. REFERRALS AND PRECERTIFICATIONS: If you want to know if a service needs a referral, please call our Customer Service Department. If you want to check the status of a referral, they can also help you.
