Policy and Procedure for Affiliation Agreements

Affiliation Agreement forms and processes are under University revision.

-Dr. Dandeneau

17

Policy and Procedure for Affiliation Agreements

• What is an Affiliation Agreement? – It is a legal document signed by a site and IUP that outlines the

responsibilities/liabilities of all parties during an off-site placement

• You cannot complete Practicum at a site that does not have an Affiliation Agreement in place

• The affiliation agreement policy is available in the Practicum Planning Manual

18

Policy and Procedure for Affiliation Agreements

• Students fill out the Affiliation Agreement Request form, identifying the person at the school or agency that has the authority to sign the Affiliation Agreement. This form should be submitted to the Department of Counseling practicum/field Coordinator

• If it is determined that an Affiliation Agreement is needed, the Department of Counseling practicum/field coordinator will submit the information to the Associate Dean, who will send the new Affiliation Agreement to the site

• After the site returns the signed agreement, it will be processed at IUP. The Associate Dean will notify the Department of Counseling when the agreement is approved.

19

Policy and Procedure for Affiliation Agreements

Please note: Since IUP is in the process of updating all Affiliation Agreements, the procedure must be followed for ALL potential off-campus practicum sites.
