Welcome To Express Training Services Welcome to the Express Training Services’ Nursing Program. Our caring staff, faculty and administration are dedicated to fulfilling our mission, which is to prepare students for employment in a positive learning environment. Here learning takes place in a climate of excellence, which fosters inquiry, responsibility, and accountability on the way to becoming a Licensed Registered Nurse, or Licensed Practical Nurse. This handbook has been prepared to help you in your orientation to the school and to the healthcare industry which to some of you may be an exciting new world. Whether you are a young person continuing in your schooling or a mature adult coming back for a new career, you will find here a team of caring professionals made up of licensed nursing instructors and experienced support staff ready to serve you in our facility. You are starting on a program of studies that will result in your being an important professional in the health delivery field. You will be learning the most up-to-date procedures as you work toward your RN/LPN license. Our hope is that you succeed and find the work rewarding, pleasurable and profitable. Congratulations on your choice of training in the Nursing program at Express Training Services.

Introduction to the Express Training Services School of Nursing Express Training Services is given direction and approved by two agencies; The Florida Commission for Independent Education and the Florida State Board of Nursing. Persons having successfully completed the Nursing Program are eligible to make application with the Florida State Board of Nursing to take the licensing examination. After successful completion of the examination, they will be known as either Registered Licensed Nurses or Licensed Practical Nurses and may engage in the practice of Nursing in the state of Florida. The instructors are qualified professional persons who are Registered Nurses licensed by the Florida Board of Nursing as holding a BSN, MSN or PhD. Clinical experiences are scheduled at various health care agencies during the program. Clinical experiences are a planned part of the curriculum and necessary for completion. Objective The objective of this handbook is to provide written policies concerning admission of students, student health and welfare, attendance, vacations, holidays and student progress. Each student must understand these policies and refer to this policy handbook to clarify information. Faculty members are available for conference or to answer questions. This handbook is designed for individuals who have been admitted to one of the following programs in the School of Nursing: Licensed Practical Nurse, Nursing (LPN) is a 12 month, 1446 hour program. There are 722 classroom and lab hours and 724 clinical hours. You are expected to make every single clinical session. You can miss an occasional classroom session, but must make it up. You cannot miss ANY clinical hours. Registered Nurse, Nursing (ASN), is a 12 month, 1,324 hour program. There are 662 classroom and lab hours and 662 clinical hours. You are expected to make every single clinical session. You can miss an occasional classroom session, but must make it up. You cannot miss ANY clinical hours. Purpose and Mission The purpose of Express Training Services is to become the most effective licensed health care professional training provider in the State of Florida. Through continuous assessment of the health care industry’s needs and desires and the self-assessment of our course offerings, various classes will be offered to suit the variety of needs of our students. Express Training Services will assist all students in a successful transition from the classroom to the workplace.

Our mission is to provide the highest quality training, which enables our students to become highly productive and successful in their careers. The aim of the Express Training Services School of Nursing is to guide selected men and women in their personal and professional development to become safe health care practitioners and to help meet the health care needs of the community. This service is available to all individuals who meet entrance standards without regard to sex, race, color, creed, religion, national origin, age, or handicap. Congruent with Express Training Services mission statement and student learning objectives (below), the philosophy of nursing education is one of commitment to lifelong learning and the provision of safe, compassionate, quality nursing care to individuals, families and communities. The Nursing student will be trained to work under the supervision of a Registered Professional Nurse. The Nursing student is committed to: the promotion and restoration of health, the prevention of illness in individuals, families, groups and communities, and support for a dignified death. Nurse’s value, respect diversity, and provide culturally sensitive and competent care. Philosophy of the Faculty We believe that each student is a potential learner with individual differences to be recognized and respected. Education is a continuous learning process, dependent upon motivation, ability and application of the learner under the guidance and direction of the nursing instructor. Learning results in a change in the behavior of the learner. The nursing instructor sets the stage by planning a logical sequence of meaningful educational experiences centered on clearly defined instructor and student objectives designed to develop the student's potential. In an era of “discovery and change,” scientific principles in the physical, biological and social sciences are utilized as a basis for these learning experiences. The student has the responsibility to actively participate in the learning process. Provision of health care is an art based upon science, which involves the whole patient; mind, body and spirit. Our graduates help to meet the health needs of society by giving direct assistance to persons in situations of altered personal health according to their needs. As such, this care requires from the practitioner an attitude of compassion, caring and helpfulness and the exhibited traits of honesty, dependability, and accuracy. A planned guidance and counseling program is essential in assisting students in their personal, educational, and professional development. Continuous evaluation is necessary in measuring student and instructor effectiveness and is a tool for motivating students toward maximum self-growth. The faculty believes in the interrelatedness of the client in the family, community, and culture and looks at nursing as humanistic combined with scientific knowledge. Jean

Watson’s Theory of Transpersonal Caring is utilized to support these beliefs. Her ‘caring science framework’ provides ‘a foundation for research, practice, and education in nursing. Watson’s theory contains ten primary carative factors and seven assumptions. Two of her carative factors involve the use of the scientific problem solving method for decision making and the promotion of interpersonal teaching-learning while one of her assumptions is that the practice of caring is central to nursing. Student Performance Objectives The curriculum of the Express Training Services School of Nursing is designed to assist the student in developing the necessary skills for safe practice as a professional/practical nurse, and to prepare the individual to successfully complete the licensing requirements of the State of Florida. To do this you must:

• Complete all assigned activities as directed by the nursing instructors.

• Maintain a continuous grade average of 80% or above for all classroom evaluations.

• Perform math calculations for Pharmacology with a satisfactory grade as

identified in the Pharmacology course outline and semester policies.

• Complete all laboratory procedures for the instructors with satisfactory performance.

• Perform all nursing skills using standard precautions and correct bio-hazardous

waste disposal.

• Complete all required hours of clinical experience as assigned with satisfactory performance.

• Comply with the policies of the Express Training Services Nursing Program.

• Demonstrate a "caring professional attitude' by meeting affective objectives

identified in the Student Handbook.

• Demonstrate Employability Skills by complying with the attendance policy of the program.

When these objectives have been reached, you will be ready for graduation and to apply to take the licensing examination.

Terminal Objectives Upon successful completion of Express Training Services nursing program, the student is expected to participate in the nursing process within the RN / LPN scope of practice in accordance with the Florida Board of Nursing as follows:

Practical Nursing Student:

Data Collection 1. Collect, record, and report data regarding the status of assigned patients to the

appropriate RN in a timely manner 2. Identify changes in health status and patient responses to care and assist with

modifying the plan of care as needed 3. Recognize when patient’s needs are beyond the LPN scope of practice and seek

assistance from appropriate health care provider. (RN, ARNP, Physician)

Planning 1. Assist with the development of Nursing Diagnoses and participate in the

development of Plan of care by assisting with problem prioritizing, setting of measurable and realistic goals, and identifying nursing interventions.

2. Adhere to and assist with the development of employer policies.

Implementation 1. Follow an established Care Plan and provide appropriate, safe nursing care in

relatively stable or predictable situations under the supervision of RN or other Health Care provider recognized by the Florida Board of Nursing.

2. Assist with providing care to patients whose conditions are critical and/or fluctuating under direct supervision of RN.

3. Document and report effects of care and changes in patient’s condition. 4. Provide health teaching under the direction of RN utilizing the established plane of

care. 5. Establish effective interpersonal relationships with the patient, family members of

the patient, the health care team, and the public. 6. Use established lines of authority and communication within the work setting to

accomplish goals related to the delivery of patient care. 7. Demonstrates the ability to utilize resources in an efficient and cost-effective

manner.

Evaluation 1. Document and communicate evaluation information to appropriate members of the

health care team. 2. Assist with the modification of the plan of care on the basis of the evaluation. 3. Evaluate the responses of the patients to nursing interventions.

Registered Nursing Student:

The nursing process provides an operational framework for the nurse to insure safe, effective care. The nurse utilizes critical thinking to effectively implement the nursing process and provide the basis for appropriate clinical decision making. Critical thinking is the use of reasoning in the analysis, synthesis and evaluation of client, patient/resident data and other information. Clinical decisions are judgments made resulting in therapeutic nursing interventions to increase the effectiveness of care and mobilize resources to attain positive client outcomes. The common thread uniting different types of nurses who work in varied areas is the nursing process—the essential core of practice for the registered nurse to deliver holistic, patient-focused care. Assessment

An RN uses a systematic, dynamic way to collect and analyze data about a client, the first step in delivering nursing care. Assessment includes not only physiological data, but also psychological, sociocultural, spiritual, economic, and life-style factors as well. For example, a nurse’s assessment of a hospitalized patient in pain includes not only the physical causes and manifestations of pain, but the patient’s response – an inability to get out of bed, refusal to eat, and withdrawal from family members, anger directed at hospital staff, fear, or request for more pain mediation Diagnosis

The nursing diagnosis is the nurse’s clinical judgment about the client’s response to actual or potential health conditions or needs. The diagnosis reflects not only that the patient is in pain, but that the pain has caused other problems such as anxiety, poor nutrition, and conflict within the family, or has the potential to cause complications—for example, respiratory infection is a potential hazard to an immobilized patient. The diagnosis is the basis for the nurse’s care plan.

Outcomes/Planning Based on the assessment and diagnosis, the nurse sets measurable and achievable short- and long-range goals for this patient that might include moving from bed to chair at least

three times per day; maintaining adequate nutrition by eating smaller, more frequent meals; resolving conflict through counseling, or managing pain through adequate medication. Assessment data, diagnosis, and goals are written in the patient’s care plan so that nurses as well as other health professionals caring for the patient have access to it

Implementation Nursing care is implemented according to the care plan, so continuity of care for the patient during hospitalization and in preparation for discharge needs to be assured. Care is documented in the patient’s record.

Evaluation Both the patient’s status and the effectiveness of the nursing care must be continuously evaluated, and the care plan modified as needed.

RN Students will be introduced to Nursing Research, which provides the undergraduate student with a basic understanding of the research process and its application to nursing and nursing practice.

Admission of Students Application Procedure Age-Applicants must reach age 18 prior to completion of program. Students under 18 years of age must have enrollment agreement signed by parent/guardian Previous education-an official transcript showing evidence of High School graduation in the United States or its equivalent and/or G.E.D. scores is required by Florida Statute. Entrance exams-Applicant must take the T.A.B.E. test and score 11.0 or better on each of the Math, Reading, Language and Spelling sections. If applicant has associates’ or bachelor’s degree from an accredited United States University, and can provide proof, this requirement is waived. Additionally, applicant must take and pass Kaplan entrance exam. After completed application is submitted, each applicant will be called to schedule the free exam. Applicants must score at or above the 62nd percentile on the critical thinking portion of the exam and at or above the 69th percentile on the reading portion References -3 (three) are required; 1 (one) should be a work reference, and 2 (two) should be character references; (in lieu of a work reference, a teacher or another professional person - a doctor, nurse, dentist or a minister, rabbi, or priest may be acceptable.) A recent High School Graduate should have one from the principal, counselor, or a teacher. Essay- A double-spaced, typed essay must be submitted. The essay should include, at a minimum, why applicant wants to become an RN/LPN, and what qualities applicant believes are needed to be a successful nurse. Application fee-An application fee of $25 is required before consideration of the application. This application fee is non-refundable. Interview- Applicant must successfully complete a personal interview with an administrative representative of the program. IPN-Any applicant to the RN and/or Practical Nursing program at Express Training Services who has a recent history of chemical abuse or an arrest record indicating the same may be required to contact the Intervention Project for Nurses (IPN) prior to acceptance into the program. Based on the evaluation process of IPN the applicant may be asked to enter into a contractual agreement with IPN. Failure to enter into this contract and/or to comply with the requirements of the contract will prohibit admission into the program.

During the year that the student is enrolled, it is the responsibility of the faculty to observe for any indication of chemical use or abuse. At any time a student’s performance is suspicious, the school reserves the right to require the student to be tested for chemical, or alcohol use/abuse. If positive, the student will be withdrawn from the program and instructed to contact the Intervention Project for Nurses. At the recommendation of IPN, the student may be required to enter into a contractual agreement with IPN. The student may request reentry into the program which will be allowed as long as the student remains in compliance with the IPN Contract. Failure to comply with this policy or with the agreements of the contract will result in the student's withdrawal from the program. The Florida Board of Nursing is the agency responsible for giving permission to graduates of this program to take the licensing exam. Without compliance to the above a graduate may be denied admission to the examination and thus status as a Registered Nurse and/or Licensed Practical Nurse. Drug screening-Applicant must complete a drug screening. Background check-Applicants must complete a Level II background screening. Professional Liability Insurance-Student must obtain professional liability insurance Essential functions and physical requirements-The following is a list of essential functions and physical requirements of the Nursing program and of the clinical affiliates. Eligibility for admission will be based on physical ability to perform the essential functions of the program. Reasonable accommodation will be made for each applicant. However, where reasonable accommodation does not overcome the effects of the applicant's disability or limiting condition, a recommendation to pursue an alternate program will be made:

a. Sufficient visual acuity such as needed in the preparation and administration of medications and for the observation necessary for patient safety and care. Clarity of vision is required for distances of 3 inches to 20 feet. Ability to distinguish color is required.

b. Sufficient auditory perception to receive verbal communication from patients and

members of the health team and to assess health needs of people through the use of monitoring devices such as cardiac monitor, stethoscope, infusion pumps, doptone, fire alarms, patient call lights and safety alarms.

c. Sufficient gross and fine motor coordination to respond promptly and to

implement the skills, including the manipulation of patients and equipment required in meeting health needs.

d. Sufficient communication skills (speech, reading, writing) to interact with individuals and to communicate their needs promptly and effectively as may be necessary in the individual's interest.

e. Sufficient intellectual and emotional functions to plan and implement care in the

decision-making process. Adequate emotional stability to react appropriately in an emergency and in situations of high stress

f. Sufficient musculoskeletal strength and range of motion necessary to re-position

patients in bed, lift supplies, and perform cardiopulmonary resuscitation. The ability to reach above the head and to push/pull/lift up to 50 lbs. occasionally and 25 lbs. frequently. Flexibility necessary to squat and to bend at the knees in order to see chest-tube drainage and urine collection devices.

Should any of these essential functions be impossible for you to perform, it will be necessary for you to consider another program. In the event that equipment could compensate for your non-compliance with the essential functions, it is your responsibility to purchase this equipment (e.g. electronically enhanced stethoscopes, hearing aids or other adaptive devices). Health Requirements Prior to entrance, the following shall be completed:

1. Proof of good mental and physical health as attested by a licensed M.D., D.O. or A.R.N.P.

2. Record of immunizations as follows:

a. MMR (Measles, Mumps, Rubella)-(Rubella is required). Must have written

verification of immunization after 1968 or a positive titer to show immunity to rubella.

b.PPD-Every student will be required to have a 2 step mantoux TB skin test - PPD skin tests for Tuberculosis. Both must be completed and read as negative prior to any clinical experience. The second PPD must be completed within 7 to 21 days after the first test. On admission, PPD results cannot be more than 12 months old. During enrollment as a student, the PPD will need to be repeated annually. If the PPD serum cannot be taken, a copy of a chest x-ray report that is no more than one year old must be added to the physical exam report. The chest x-ray must be repeated annually or a written statement from a M.D., D.O., or A.R.N.P. indicating clear respiratory status.

c. Hepatitis B Vaccination-the school highly recommends you be immunized against Hepatitis B. If, upon admission to the program you choose not to be

immunized, you must sign a letter of declination. If you have been immunized, please provide written documentation of such for your student file.

Standards for Satisfactory Academic Progress Satisfactory Academic Progress Policy All students are required to meet the standards of academic performance that are outlined in the sections below and they are evaluated regularly to determine that the standards are met. These standards have multiple components: a minimum cumulative grade point average requirement (CGPA) and a maximum time frame requirement to successfully complete all required courses/clinical rotations for the program. As described below, each student must achieve the minimum CGPA within the maximum time frame established. Failure to meet these standards may result in dismissal from the nursing program. Maximum Time Frame (MTF) The MTF, which is 1.5 times the normal completion time of a program is computed from the very first semester in which the student enrolled and originally began his or her studies at Express Training Services. Satisfactory Academic Progress (SAP) Evaluation 1. Students are evaluated after the completion of every course and at the end of each

academic semester. 2. If a student fails a course before the semester ends, the student is immediately placed on academic probation.

A. The student will remain on academic probation until: i. The student retakes the failed course when it is next offered and

passes it on the next attempt; or, ii. The student retakes the failed course and fails it again; or, iii. The student takes another course (before retaking the first course)

and fails it. B. If the student takes the course a second time and passes it, the student is removed from academic probation. C. If the student fails the course for a second time, the student is academically dismissed from the college. D. If the student takes another course (before retaking the first course) and fails it, the student is academically dismissed.

3. At the end of a semester, any student whose cumulative grade point average (CGPA) is below 2.0 will be placed on academic probation.

A. The student will have one semester to raise his or her CGPA to 2.0 or higher. B. Any student who fails to earn the minimum CGPA will be academically dismissed from the college.

Purpose Health care workers must be reliable and dependable employees. We believe that it is essential within the nursing curriculum to teach employability skills as an integral part of the educational program, and to practice these skills throughout the program. The patient's welfare is dependent on the worker being in the right place at the right time. The Nursing program has an established number of absence hours allowed for progression in the program. Terminology

1. Regular attendance - expected performance. Being in the classroom or at the assigned clinical area at the right time and remaining in attendance for the full class day constitutes one day of attendance.

2. Tardy- not being in the classroom or at the assigned clinical area by the scheduled

time without 30 minutes notice will constitute a tardy. 3. Absence-excused -Not being in attendance in the classroom or the clinical area on

time, for the full day, or for leaving early, having been excused by the instructor for an acceptable reason and for timely notification.

4. Unexcused absence-being absent for a reason that is not acceptable. Unexcused

absence requires disciplinary action. Three unexcused absences constitute a failing grade for the course.

Policy 1. Attendance - regular attendance is necessary for the student to take full advantage of

available educational opportunities. All students shall be counted in attendance when they are: • Present in assigned class • Participating in a school-sponsored function on or off campus.

2. Excused absence - absence shall be excused for the following reasons:

• Illness of the student (medical evidence may be requested). • Death in the immediate family. • Emergency in the immediate family. • Religious holidays of the specific faith of the student. • Subpoena by any law enforcement agency. • Other justifiable reasons approved by the principal or designee upon prior request

If you have been absent or tardy, you must report to your instructor before returning to class!

4.Unexcused absences - absences not included in the above shall be considered

unexcused. Absenteeism or tardiness because of repeated transportation problems is not excused. Students may sometimes be served by buses that service all areas of the county. Three tardy arrivals are considered one unexcused absence and are deducted as 8 clinical hours. This is considered unacceptable performance, poor attitude, and lack of aptitude. This will not be tolerated. If you have given notification of an expected late arrival at least thirty minutes before the scheduled time of arrival, this won’t be called an absence. However, frequent late arrivals will establish doubt of the student's ability to be a dependable employee. Each Instructor reserves the right to consider frequent late arrivals, even with proper notification, a tardy. Prior to doing so a conference will be held between the student and the instructor to establish these criteria. A student will be withdrawn from the program if 9 or more tardies are accrued in a 3-month period.

5.Hours - During your nursing program we expect you to be in attendance daily. However, we understand that occasional illnesses do occur. To be eligible for graduation, and to make application to take the licensing exam, you must attend 90% of your classroom hours. If you miss classroom hours, you must make the work up before advancing in the program. You also must attend 100% of your clinical hours. There will be several different chances to make up clinical hours missed. Make up of missed clinical hours is entirely at the instructor’s discretion.

Arrival to a clinical assignment 5 minutes late without prior instructor approval will result in your being sent home as your patient assignment will have been assumed by staff. Students must attend 90% of their class. Classroom attendance on a daily basis is mandatory as well. Student’s Responsibility • A student shall be in program attendance every day and on time in every class

unless excused. • A student is to notify the instructor by telephone at least 1 hour before class if

(s)he will be unable to report. The phone number for you to leave a message is 352-338-1193. If the student will be late or absent from the clinical assignment, the instructor must be notified by cell phone at least 30 minutes prior to the scheduled start time for that experience. This must be done for every day of absence. Failure to do so will result in a tardy.

• A student shall be responsible for obtaining and completing all assignments

missed during an excused absence. • A student shall bring appropriate materials to all classes and clinical experiences

and be prepared to work. Failure to do so may result in the student being asked to leave the clinical site.

School’s Responsibility • Attendance shall be defined as the span of time a student is under the jurisdiction

of any one instructor.

• All assigned work missed during unexcused absence(s) shall receive a zero (0) mark. Upon written request by the student, the instructor may allow work to be made up (for a grade), if there are special circumstances which justify the exception.

Student Health A health record of each student will be kept on file during the entire course. This will include the record of a recent physical examination by a local Licensed Medical or Osteopathic Physician or A.R.N.P., and record of immunizations. Immunization records must show evidence of Rubella, and 2 current negative PPD's or a recent negative chest x-ray report. All health records must be kept current to be eligible to participate in clinical experiences. Immunization against Hepatitis B must be recorded in the student file or the student must sign a declination statement regarding choice of this immunization. Activity Restriction-If at anytime you are placed on activity restriction, this information must be given to the instructor with details of the specific restriction, and to how long this restriction will last. Pregnancy- if there is any possibility of a pregnancy, the instructor must be notified. A doctor's written permission must be provided to participate in clinicals. It is the student's responsibility to take the necessary precautions in the clinical area in regards to x-rays, radiation, chemicals, viral exposure, etc. Illness-the instructor reserves the right to withdraw a student from a clinical assignment based upon evidence of illness. This would be considered an excused absence. Drug testing- It is the intention of Express Training Services to test students in an unbiased and impartial manner. Testing may be done for any or all of the following reasons:

The instructor reserves the right to require random drug testing based upon reasonable suspicion. Reasonable suspicion means a reasonable and definable belief that a student possesses or uses drugs or alcohol, is intoxicated or impaired by drugs or alcohol, or is diverting drugs or alcohol based on reasonable inferences drawn from facts. Among other things, such facts and inferences may be based upon:

• Observable indications of potential drug or alcohol use while in the classroom or clinical assignment, such as direct observation of drug use or of the physical symptoms or behaviors of being under the influence of alcohol or a drug.

• Abnormal conduct or erratic behavior while in the classroom or clinical assignment, or a significant deterioration or reduction in performance.

• A report of suspected drug or alcohol use, provided by another student, a member of the Express Training Services faculty or staff, a clinical employee or an otherwise reliable source.

Drug and/or alcohol testing will be done at the expense of the student. The student will be allowed to return to class and/or clinical only after a negative drug screen has been provided to the instructor. Fees

1. Tuition is payable prior to the first day of each semester.

2. Those students entering the program must make a deposit during pre-registration. The balance is to be paid on or before the first day of the first class, unless special fee arrangement is requested and approved.

3. Each student will be given a receipt upon payment of monies for fees. Be sure to

retain your registration receipt.

4. Those students who have applied for a PELL grant may have tuition and lab fee waived until distribution of the grant has been made.

5. If a student drops out of the program for any reason, the entire tuition must be paid again before re-entry.

Refund Policy Tuition Should a student be terminated or cancel for any reason, all refunds will be made according to the following refund schedule:

1. Cancellation must be made in person or by certified mail. 2. All monies will be refunded if the school does not accept the applicant or if the

student cancels within three (3) business days after signing the Enrollment Agreement and making initial payment.

3. Cancellation after the third (3rd) business day, but before the first class, will result in a refund of all monies paid, with the exception of the registration fee.

4. Cancellation after attendance has begun, but prior to 40% completion of the program, will result in a pro rata refund computed on the number of hours completed to the total program hours.

5. Cancellation after completing 40% of the program will result in no refund. 6. Termination Date: The termination date for refund computation purposes is the last

date of actual attendance by the student unless earlier written notice is received. 7. Refunds will be made within 30 days of termination or receipt of Cancellation

Notice. Percentage of refund = Number of hours remaining divided by total number of hours in course times 100. If this number is greater than or equal to 40%, no refund will be issued. Refund Amount = Percentage of refund times tuition fee Textbooks, Uniforms No refunds will be given on a textbook that has been written in, marked, or otherwise damaged. If the textbook is in good condition, a refund will be made according to the tuition refund rules. Once a uniform is worn, the fee cannot be refunded. Class Cancellation A full refund of all fees paid will be made for classes that are cancelled regardless of length. The refunds will be made to the appropriate agency or student as prescribed above. Textbooks and Supplies

• Laboratory supplies are provided as needed, and care plans can be printed by the student, from the website as directed by the instructor.

• Course syllabuses are to be printed by the student from the website as directed by the instructor for each course.

Computer Requirements It is mandatory that each student of nursing own a portable computer that can be used by the student during class activities/assignments/testing. Be sure you are using a reliable computer and have two reliable and compatible browsers installed on your computer (Internet explorer, Google chrome, and/or Mozilla Firefox). Create a Plan-B in case your computer or internet browser fails. Have Microsoft Office or equivalent for class projects/assignments (Word, PowerPoint). You will also need to create a Gmail email account to give to your instructor. School information is exchanged on Schoology, which is a cloud-based learning management system and configurable social network for education Uniforms • It is the responsibility of the student to purchase uniforms as described during pre-

registration. • A nametag will be issued to each student with the student's picture, full name, and

program title. The name pin must be worn at all times on the front of the upper torso while at school or in the clinical agency. If the nametag is lost, it is the student's responsibility to purchase a replacement for $5.

Code of Ethics

THE CODE FOR LICENSE PRACTICAL/VOCATIONAL NURSES The Code, adopted by NFLPN in 1961 and revised in 1979, provides a motivation for establishing, maintaining and elevating professional standards. Each LP/VN, upon entering the profession, inherits the responsibility to adhere to the standards of ethical practice and conduct as set forth in this Code.

1. Know the scope of maximum utilization of the LP/VN as specified by the nursing practice act and function within this scope.

2. Safeguard the confidential information acquired from any source about the patient.

3. Provide health care to all patients regardless of race, creed, cultural background, disease, or lifestyle.

4. Uphold the highest standards in personal appearance, language, dress, and demeanor.

5. Stay informed about issues affecting the practice of nursing and delivery of health care and, where appropriate, participate in government and policy decisions.

6. Accept the responsibility for safe nursing by keeping oneself mentally and physically fit and educationally prepared to practice.

Health Information Portability & Accountability Act (HIPAA)

Confidential care and treatment is the right of all patients in all clinical arenas

used for clinical experiences. The diagnosis, treatment and all other information concerning patients is confidential and may not be released to anyone, including family members, without the written consent of the patient following facility protocol. Even the presence or identity of a patient in an institution or their ability to pay their bill is considered confidential information.

The student will not at any time during or after clinical experiences, disclose or discuss confidential patient health information (PHI) or any part of the experiences which is of a confidential nature to anyone who does not need the information to perform their duties. A student cannot seek or obtain information regarding confidential matters, which is not necessary to fulfill responsibilities as a student, including information of friends or families.

Disclosure of confidential information may cause irreparable injury to an individual, ETS and/or the clinical agencies, and may result in a civil lawsuit. Violation of this agreement or disclosure of any confidential information in an unauthorized manner can result in a student’s immediate dismissal from the Nursing Program. If in any situation the student is uncertain or unclear of their responsibilities in protecting patient/client/resident confidential health information, the student will seek the guidance of instructors or appropriate supervising personnel.

knives, 12. The possession of a concealed weapon permit does not allow students to possess a

firearm in the classroom, in their automobile or clinical sites during class sessions,

13. Conduct that interferes with the learning process of any other student, instructor sessions, or progress of the class,

14. Instigating and/or participating in rebellious activities against the school, its students or teachers, and any behavior deemed disruptive,

15. Solicitation, 16. Profanity, 17. Vandalizing school property or that of any student, school faculty, instructors,

clinical sites and staff, 18. Theft

Plagiarism/Cheating

All assignments and/or assessments are intended to determine the skills, abilities, understanding, and knowledge of each of the individual students who completed this course. Cheating is defined as obtaining an unfair academic advantage and any student found using any form of cheating, attempting to cheat, or assisting someone else to cheat, including the use of and/or purchase of test banks. Plagiarism is defined as an act or instance of using or closely imitating the language and thoughts of another author without authorization and the representation of that author’s work as one’s own, as by not crediting the original author. We take these issues very seriously and they will be subject to disciplinary action in accordance with our policy; these acts may result in expulsion from the program. If you are having difficulty with your work, it is important to seek help from your mentor/tutor/instructor rather than be tempted to use unfair means to gain marks. Smoking/Breaks Those who smoke MUST leave the building since this is a NO SMOKING building. There are designated smoking areas. Students are allowed to purchase sodas or snacks only during break or lunch time. It is the student's responsibility to return to the classroom on time after breaks and lunch. Failure to do so will result in a “tardy". Use of Telephone/Recording Devices/PDAs/Pagers/Computers • School telephones are to be used by students by permission of the instructor, and

only in an emergency and for only 1 to 2 minutes per call. • The presence of cell phones and/or pagers in the classrooms or hallways is not

allowed. Cell phones may be used outside of campus buildings during breaks, lunch, before and after class. However, cell phones must be off, or on vibrate only when in the building. Students are not allowed to leave class to answer phones or place calls except in extreme emergencies. Cell phones and pagers are not allowed at the clinical sites. Telephones are available for students’ use in an emergency and must be used only during break time and away from the patient unit.

• Students will not be allowed to use recording devices in the classroom. Recording videos or copying computer software is prohibited. Cell phones/headsets/ear buds/cameras are prohibited in any patient setting. PDAs, cell phones, calculators or similar devices will not be allowed in any testing area (or at the discretion of the instructor). It is mandatory that each student of nursing own a portable computer that can be used by the student during class activities/assignments/testing.

Vacations/Holidays/Hurricane Days • Vacations - Vacation days are planned during each program. The total number of

vacation days will vary with each class. • Holidays- A tentative schedule will be announced at the beginning of the school

year. • Hurricane Day- In the event of school being closed due to a hurricane, the day will

Clinical Experience This is an integral part of your curriculum and is set up to give you learning experiences that relate to classroom instruction. Instructors carry cell phones and their use will be explained during orientation. As applicable, when in a clinical agency, you may reach your instructor by calling him/her. Your instructor is always present in the clinical facility when you are there. Do NOT go on duty before meeting your Instructor. Do NOT leave until dismissed by your Instructor. Do NOT use the overhead paging system to reach your instructor, unless specifically instructed to do so by the instructor. If you arrive late to the clinical facility, you must call your instructor on his/her cell phone, before going to your assigned area. School hours can vary at different times during the year due to assignment scheduling. Clinical hours will vary with the agency and assignment. Your instructor will give you a monthly clinical rotation schedule so you may plan your activities in advance. Hours may include evenings, and/or weekends and may begin as early as 6:30AM. Students will be required to change Lab/Clinical groups throughout clinical rotations. This will be done in order to maintain fairness among shift times (nights/weekends, etc.).

Student Progress Grading System A 90%-100% B 80%-89% C 70%-79% (below 80% is unsatisfactory) Examinations

Exams will constitute the major portion of your theory grade. Exams may be objective or essay. Your course syllabus will inform you of the weight exam grades will carry in determining your final grade. You will be given both unit and final exams and, on occasion, content quizzes. A late exam may be given only with permission of your Instructor and on an individual basis. There are NO scheduled make-up exams for failing grades. A student may petition the faculty and administration for special consideration for a failing course grade. 80% is the passing grade in theory. All laboratory practice must be satisfactory. In order to complete the course, not only passing grades, but also standards as described in this

handbook, with regard to attitude, judgment, concern for patient welfare, and grooming must be satisfactory. Field trips, conferences, workshops or special service assignments may require a written report. Clinical Performance Progress in the clinical areas will be evaluated as satisfactory or unsatisfactory, with frequent counseling in areas which need strengthening. At the end of each clinical rotation the instructor will complete a performance evaluation and discuss this with the student. (See evaluation criteria in clinical competency instructions.) The clinical competency checklist in the Student Training Record identifies the duties and competencies of each clinical assignment. Each instructor will complete a daily student evaluation using the Student Training Record. Termination by Due Process 1. Evaluation- a student who is having difficulty with a course in theory and/or laboratory practice will be given a deficiency notice. All students will receive a progress report midway through each course. 2. Recommendation and re-evaluation- suggested activities will be recommended by the instructor and a time limit for identification of improvement and re-evaluation will be set. 3. Probation- if there is no improvement, it may be necessary to discuss the problem with the Director and to place the student on probation for a period of from two to four weeks. During that time, satisfactory performance must be maintained (i.e., no absence, tardiness, test failure or inappropriate behavior will be accepted). 4. Termination-

If at the end of the period of probation there is little or no improvement, the student will be allowed to resign by "due process” or remain in attendance without credit. The school reserves the right to ask a student to withdraw from the program for any of the following reasons:

• Unsatisfactory academic or clinical performance

• Unsatisfactory demonstration of the expected affective behaviors.

• Ethical and/or social misdemeanor

• Violation of the attendance policy

Library Library books are available and may be checked out by students from ETS staff. You are responsible for books signed out in your name. If not returned, it is the student's responsibility to purchase a replacement. The length of time you can keep the book will be assigned when you check it out. No course grades will be assigned nor refunds given until library books are returned or replaced. You will not advance to the next course until library books are returned or replaced. This is a comprehensive online research center featuring complete articles-full text and graphics. You will be able to access it at home, school or work.

Part-Time Work Part-time working is discouraged. If it is necessary, however, a student should work only 16 hours a week. An instructor may advise you to reduce the number of hours worked in the event of academic underachievement. You may not be allowed to participate in class activity if you are unprepared. Unprepared students are a distraction to other members of the class and thus violate others rights to make expected progress for the day.

Student Conduct

• Students are expected to have the necessary student materials and to always be on time.

• When participating in class, only one student should talk at a time. Usually, the

speaker is the student so recognized by the instructor.

• Method of address - Ethical practice in the hospital requires that students address staff members by Mr., Mrs., or Ms. when hospital policy approves another mode of address, you may use that mode in that facility after receiving instructor permission to do so.

• Students will address patients as Mr., Mrs., or Ms, never as " Granny", "Dearie”,

or other terms of endearment. The school encourages friendliness, but never familiarity .

• In the lab there will be no playing with equipment, no rearranging of the lab

furniture, and no lying or sitting on the beds, unless it is part of the lab exercise.

• There will be no eating, drinking or gum chewing in the classroom/lab area or in the clinical facility while on patient units.

• Confidentiality-you must refrain from discussing patients or hospital situations at

home, in the cafeteria, elevators or anywhere you can be overheard. Patient information, including chart forms, and lab/x-ray results must never be photocopied for any reason. Doing so is a breach of confidentiality and will not be tolerated. This is grounds for dismissal from the program.

• You must have all equipment when on a clinical assignment.

• Students are encouraged not to leave the clinical facility for lunch or break. If you

must leave the clinical facility, the instructor must be notified and permission to leave granted by the instructor.

Personal Appearance and Grooming The well-groomed health care worker symbolizes concern for the patient and attention to detail. To the public, you are the image of efficient helpfulness in time of need. A well-groomed health worker means:

• Uniform- must be loose enough to put necessary items in your pocket such as a small note book, stethoscope, pen, penlight, etc., and without wrinkles. (Dresses not shorter than the center of knee.)

• Hair- Must be restrained off the face, shoulders, and pulled back. • Nails-manicured, moderate length; only clear nail polish may be worn. (the nails

should not extend beyond the fingertip.) Artificial nails of any kind are not allowed.

• Cosmetics- Makeup should look natural. Perfume, cologne, or scented lotion are

not allowed when in uniform and only light scent in classrooms. Cologne scents should not carry more than three feet.

• Jewelry – Small post earrings for pierced ears only. A chain may only be worn if it

remains under the uniform at all times.

• Full uniform-must be worn at all times while in the hospital, during any clinical experience, and in the classroom as assigned by the instructors. You must change from your uniform to street clothes if other than a direct route home is taken. It is inconceivable that you would expose the public to contaminants you acquired on your uniform while “on duty". A long sleeved white tee shirt may be worn under your scrub top or a long sleeved scrub jacket may be worn over the uniform for warmth (no sweaters/jackets/hoodies).

• Shoes should be of a safe, professional style, closed toed; no slippers, flip-flops,

thongs, etc. For student safety, no shoes with open toes or heels will be allowed while practicing in the lab or in the clinical sites.

Student uniforms While you are a student in our program you will be required to be in proper student uniform at all times when in a clinical facility and on all days assigned by your instructor. You will need to have at least 2 student uniforms for this year. The required uniform is: Women: royal blue skirt or pants with royal blue top Men: royal blue uniform pants with royal blue top. A patch identifying you as a student must be worn on the left hand sleeve of the uniform. You will be given two patches at the beginning of the clinical rotation. Additional patches may be purchased for $5.00 apiece. All students

You will also need a watch with a second hand, a pen light, black ink ballpoint pen, and stethoscope. Adherence to these policies reflects your attitude and adjustment to the program. Instructors reserve the right to not admit you to class if improperly attired. This is especially true in the clinical area. Student Management Procedures All students are required to participate actively in student management procedures. Teamwork and cooperativeness in the classroom are indications of your expected clinical performance. Classroom management- each student is responsible for keeping his/her own area tidy. Your instructor may assign a student(s) daily to tidy the classroom. Dismissal- the class is dismissed by the instructor when the day's objectives have been met and the classroom is appropriately arranged. Do not leave until dismissed. Laboratories- participation in laboratory activities is an integral part of your program. You will be assigned time in the laboratory for practice and return demonstrations. When in the laboratory: Dress in complete uniform with equipment and supplies as if going to the clinical area. Conduct yourself as you would in the clinical agency. This will enhance your performance when you go to the agency for clinical experience. Replace all supplies and equipment to the appropriate place. Everything in the lab has a place so put everything in its place. Lab demonstrations will constitute a portion of your grade and must be graded satisfactory as stated in the course objectives. It is your responsibility to be certain the lab is left in proper order. Affective Objectives For successful completion of a health occupations course, you must demonstrate the attributes of a concerned individual who respects the needs and values of the client. You will have met this objective when you have demonstrated:

• A caring attitude by maintaining a safe and medically aseptic environment for the patient. This attribute is exhibited by maintaining the personal appearance of a well-groomed healthcare worker as described in the Student Handbook and by completing all environmental safety objectives of the program.

• Dependability and reliability by adherence to the attendance policy of the program

as stated in the Student Handbook

• The use of realistic judgment necessary to perform health care duties in a safe, ethical and legal manner. This will be evidenced by your ability to observe a client’s need and respond to it in an appropriate and professional acceptable way, as evaluated by your instructor and the clinical affiliate representatives.

• Loyalty to the school and health care facilities by using correct “chain of

command" procedures, keeping peer confidences, giving respect to authority end maintaining hospital confidentiality.

• The ability to follow directions and accept responsibility by completing assigned

work on time and in the manner directed in the learning activities
