7hen incidents do occ$r h$%an factors s$ch as fail$re to i%)le%ent )roced$res )ro)erl are oftena root ca$se. These fail$res %a in t$rn e attri$tale to a lac' of training instr$ction or$nderstanding of either the )$r)ose or )ractical a))lication of the Per%it to 7or' CPT7D sste%.
