Power of Purpose

WelcomeEvents

Hello LeadingAge Missouri Members and Supporters!

POWER OF PURPOSE – that’s the theme for our 48th annual gathering of Missouri senior living and health care providers, caregivers and supporters. We invite you to join us September 19-21 at the iconic Chase Park Plaza Hotel in St. Louis’ vibrant Central West End for education, inspiration, fellowship and fun! Within these pages, you will see how we intend to expand your Power of Purpose.

About those theme words – “purpose”, of course, is “the reason for which something exists”. LeadingAge members exist to accommodate, care for and supply seniors. Your association exists to advance member purposes. Context dictates whether “power” means “capability”, “authority”, “control”, “strength” or “to inspire”. But, I like the latter two synonyms coupled with purpose to describe LeadingAge members. The members I know manifest strength and gain inspiration from the care, compassion and service they deliver to Missouri seniors. That’s Power of Purpose!

To care for and serve others well, we must take stock in our passion for what we do—and actively work to improve how we do it. That’s the purpose of the Annual Meeting & Exhibition. Power of Purpose lessons will occur through an optimistic keynote, a poignant documentary, meaningful peer recognition, insightful education sessions, CEO perspectives and exposure to products and services while enjoying collegial fellowship. And that’s just Day 1!

Days 2 and 3 offer the witty teachings of Jack York, lots more education, an overview of important policy changes and advocacy efforts, exhibit hall prizes and drawings, a social event with a world class backdrop and timely topical deep dives. Your passion for and power of purpose will be ignited.

Whether you’re an executive of a multi-site organization, a case manager in a hospice, a CNA or a service coordinator at an affordable housing community, you’ll find solutions for challenges and discover new ways to improve operations and quality. Passionate commitment, personal growth and organizational excellence coalesce here—join us September 19-21 in St. Louis to reenergize your commitment, refresh your knowledge and renew your passion for ensuring the future care of our Missouri elder community. Come experience the Power of Purpose! You’ll be glad you did!

15 Helping Administrators Manage Stress Kimberly Brown, M.P.P.A, LNHA, Executive Director, Creve Coeur Assisted Living and Memory Care, A Spectrum Retirement CommunityLong-term care administration can be a stressful job due to the pressure to keep operations running smoothly. Long-term care facility managers (Administrators, Executive Directors) also work long hours beyond the 40-hour workweek and often feel that they are “on” 24-7, 7 days per week, 365 days per year. They also spend a considerable amount of time in the office with little patient interaction. Even though their people skills and presence on the floor could very easily curtail some employee apathy and poor resident care. The enormous tasks Administrators have to deal with in leading their staff, caring for residents and managing the facility, places them in stressful situations; almost on a daily basis. Once stress takes toll on the Administrators’ personal and mental life, this can greatly affect their work, the organization and the residents, therefore it is important for them to have effective methods of identifying and addressing stress.

Schedule of Events

3:00 p.m. – 4:00 p.m.24 Through the Looking Glass – A Lesson in Empathy

Leslie Pedtke, Educator for Quality Improvement, King Management CompanyWhat happens when real caregivers become “real” residents? How will their experiences change how they think about being a resident? How will the care they provide to residents change in the future? ‘Through the Looking Glass’ is a program developed at Aviston Countryside Manor to teach empathy to our caregivers. Staff members moved into the nursing home, lived the role of a real diagnosis and accepted daily ‘challenges’ our residents experience every day. From learning the importance of communicating more with residents to encouraging meaningful activities, the resulting in sights of these caregivers sensitized other staff members - new and veteran - to practical and effective ways that they were able to immediately improve daily care to residents. This program became the culture of Aviston Countryside Manor and grew into an Elder Shadowing program that requires all newly hired staff to live with a resident for 24 hours before they are able to begin their new job.

25 Getting Serious About Quality, Value and Effective Care Patricia L. Scheets, PT MHS DPT NCS, Director of Quality and Clinical Outcomes, Infinity RehabReal-time learning for continuous improvement in the quality, safety, and effectiveness of care is critical for increasing clinical decision-making that is supported by accurate, timely, and up-to-date clinical information. This session describes a company-wide quality charter that directs performance improvement initiatives focused on improving the value of rehabilitation services in the post-acute environment. The presenters will describe how they adopted the Model for Improvement as their scientific foundation for real-time learning and how they used multiple Plan-Do-Study-Act (PDSA) cycles to complete each step. The Model for Improvement is promoted by the Institute for Healthcare Improvement and described as a means for “dynamic learning” which capitalizes on processes and knowledge growth. The presenters will also illustrate the use of the Knowledge to Action cycle, which guides the “Do” component of PDSA and their efforts with knowledge translation. The session will include results of their efforts to implement a core set of clinical outcome measures for all patients in over 280 skilled nursing facilities, intervention standards of care targeted toward improving specific patient outcomes, their standardization of the assessment process through use of the language associated with the International Classification of Functioning, Disability and Health (ICF), and the development and implementation of a treatment taxonomy needed to study their clinical practice, and demonstration projects related to implementing evidence-based interventions.

26 Bundle and Alternate Payment Models - How Do Post-Acute Providers Get In, Stay In, and Survive! Ron Scharff, Assistant Vice President, Research, RehabCareBundle Payment Care Initiatives (BPCI) and Alternate Payment Models (APM) find post-acuteproviders in four situations: 1) contemplating joining new initiative, 2) beginning a Phase I initiative, 3) advancing into Phase II participation, or 4) struggling to join a model that started without them. This session will detail how to achieve data driven success in each situation based on hundreds of provider experiences from 2015 & 2016.
