POST: SENIOR HOUSE OFFICER IN DENTAL SURGERY (GENERAL DUTIES)

BASE: UNIVERSITY DENTAL HOSPITAL

THE TRUST

Background

The Cardiff and Vale NHS Trust was launched on 1 April 2000. All the hospitals in the Trust portfolio are situated in and around the City of Cardiff, the Capital of Wales. The Trust is regarded as a 'centre of excellence' for many clinical services. There are close links with the University of Cardiff Dental School, each have a major teaching, research and development role both nationally and internationally. The Trust serves a population of approximately five hundred thousand residents with Bro Taf Health Authority being the main commissioner. In addition it provides a number of Regional specialties and acts as a tertiary referral centre for specialist services for a population of up to 1.5 million.

1. THE HOSPITAL(S)

The University Dental Hospital consists of a purpose built independent building on the Heath Park site. It is the only unit in Wales that provides services for both the specialist dental treatment of patients and education of dental students.

The hospital treats over 92,120 outpatients and 1,352 day cases annually. It supports the delivery of education to approximately 220 dental undergraduate students as well as providing support for postgraduate dental education and research. It has a budget of £6.8 million and, with the Community Dental Service Directorate, forms the Dental Service Group of the Trust.

It has a complex number of specialties including:

2. THE WORK OF THE DEPARTMENT

Whilst appointed under the description ‘General Duties’, the Senior House Officer is normally allocated to one of the Clinical Departments (Dental Health and Development or Adult Dental Health) and may rotate after 6 months.

4. DUTIES

Clinical

Whilst appointed under the description ‘General Duties’, the Senior House Officer is allocated to an agreed rota (see attached). There are five Senior House Officer posts which will be allocated and will normally rotate to at least one different post.

The Senior House Officer appointments are for one year, and will normally rotate between a minimum of two positions on a six monthly basis.

This post offers experience in all aspects of Restorative Dentistry with an emphasis on Endodontic and Fixed Prosthodontic Dentistry.

Conservation

Two sessions per week is taken by assisting with referrals clinics whilst the other three are personal treatment sessions. The patients accepted for treatment by SHO’s will require endodontics and/or fixed prosthodontic care. In the latter, there is an emphasis on occlusal management. There will also be experience in the management of tooth surface loss and limited experience in the treatment planning of implants.

Prosthetics

A range of treatments is provided for patients including complete and removable prostheses, maxillofacial prostheses as well as implant supported restorations.

The TMJ clinic provides an opportunity to assess, diagnose and treat patients with temporomandibular joint disorders.

Combined Oral Surgery/Prosthetic Clinics are held several times a month.

Examination, Emergency and Radiology

The department is the centre for the initial relief of oral/dental pain in adult patients. It provides emergency treatment and also screens and diagnoses oral and dental disease. In addition, it provides primary dental care to patients who attend on a casual basis, together with those referred with particular diagnostic or acute problems. This department offers experience in a wide range of oral conditions including the dental management of medically compromised patients.

The department encompasses the speciality of dental and maxillofacial radiology. It provides specialist advice and reports on dental and facial radiographs when requested.

This post offers a wide experience in the restorative treatment of patients with special needs. Primarily the patients have medical problems affecting the delivery of dental care, and also includes patients with head injuries, learning disabilities and physical disabilities.

Special Needs

Four sessions are devoted to the treatment of medically compromised clinics within the Periodontology Clinic

PHDS

Four sessions are spent in the PHDS and offers experience of providing:-

an emergency dental service to all in-patients an assessment and treatment service to in-patients in rehabilitation and continuing

care.

This is an outreach service to 9 hospitals and provides a broad experience of adults requiring special oral health care and the provision of domiciliary care.

Three sessions are spent in the Consultant Oral Medicine Clinic, which sees a mixture of new and review patients. One of these sessions is a Joint Clinic with Oral and Maxillofacial Surgery and consists of the review of dysplasia and oral cancer patients.

These posts offer experience and training in all aspects of Paediatric Dentistry. The specialty of Paediatric Dentistry provides comprehensive and therapeutic oral health care for children from birth to adolescence including care for children who demonstrate intellectual, medical, physical, psychological and/or emotional problems. It encompasses a variety of disciplines, techniques and skills that share a common basis with other dental specialties. However, as with the medical specialty of paediatrics, paediatric dentistry is age- rather than technique-, disease-, or region of the body- specific.

There are four programmed personal treatment sessions each week and the postholders staff the Paediatric Dentistry Casualty Service. There is opportunity to work with the supervising consultants on diagnostic and treatment planning clinics. The postholders will work with patients with a wide range of problems, and will be given the opportunity to treat children who are sedated with Nitrous Oxide and Oxygen. There is a close working relationship with orthodontics. Postholders may be able to carry out orthodontic treatment under consultant supervision, for some of their paediatric dentistry patients.

These sessions involve the assessment and treatment of patients requiring extractions and minor oral surgery.

OM2 Oral Medicine and Pathology and Radiology Supervising Consultants Professor M A O Lewis

This post will offer experience in all aspects of Oral Medicine and diagnostic Oral Pathology. Three sessions are spent in the Consultant Oral Medicine Clinic, which sees a mixture of new and review patients. One of these sessions is a Joint Clinic with Oral and Maxillofacial Surgery and consists of review of dysplasia and oral cancer patients. Each afternoon is spent in the Oral Pathology laboratory taking in specimens and reporting in conjunction with the Consultant Oral Pathologist. One morning per week is spent in the Radiology department undertaking radiography including sialography. Some experience in ultrasound will be gained.

5. TEACHING

The Senior House Officer is expected to take part in organising and teaching Medical Dental and Nursing Students who are posted in the Trust regularly. They are also expected to participate at postgraduate meetings held in the Department and the Hospital. All junior dentists are expected to attend protected half-day study sessions, where applicable.

6. RESEARCH

Excellent opportunities are available to pursue research, both those currently being undertaken in the Department and those of interest to the individual.

7. ADMINISTRATIVE

The SHO may have some input in organising the weekly clinical meetings and the programme for undergraduate medical/dental students.

8. STUDY AND TRAINING

Postgraduate Training

All posts are recognised for training by the Royal College of Surgeons, Faculty of Dental Surgery. There are postgraduate training courses held at the University of Cardiff Dental School.

Facilities for Study Leave

Study Leave may be granted for educational purposes to attend courses at the discretion of the Clinical Director and Postgraduate Organiser, provided the clinical service is covered and must be planned well in advance. At least six weeks notice is required. Study Leave Approval Form and Study Leave Passports must be completed and authorised by Consultant and Postgraduate Dental Tutor.

9. MAIN CONDITIONS OF SERVICE

a) The post is covered by the Terms and Conditions of Service for Hospital Medical and Dental Staff (England and Wales) and the General Whitley Council Terms and Conditions of Service as amended in negotiation with the Trust Medical and Dental Staff Negotiating Committee. Details of these may be obtained from the Dental Personnel Department.

b) The post is whole-time. You will be advised of the working pattern for this post prior to taking up your appointment.

c) The appointment is designated as non-resident but you are required to be resident during your rostered working hours.

d) The junior doctor accepts that he will also perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant, in consultation with his colleagues both junior and senior.

e) Applications for annual leave must be submitted in writing to the Personnel Department at least six weeks before leave is taken. It must also be requested having first taken into account your colleagues leave so as to enable adequate clinical cover to be maintained.

f) The Trust is contractually obliged to monitor junior doctors’ New Deal compliance and the application of the banding system, through robust local monitoring arrangements supported by National Guidance. You are contractually obliged to co-operate with those monitoring arrangements.

11. OTHER FACILITIES

Cardiff, the City and the Capital of Wales, has a typical air of a cosmopolitan city, being the administrative, business, cultural and education centre for Wales. Cardiff has much to offer, modern shopping centres, Edwardian arcades with exclusive boutiques and a large indoor market.

Cardiff Castle sits in the middle of the city along with impressive buildings such as the City Hall and the National Museum of Wales. For entertainment, Cardiff has many venues - St David's Hall for world class concerts, the Millenium Centre, New Theatre and Sherman Theatre, large cinema complexes as well as Chapter Arts Centre.

The city also contains the Welsh National Ice Rink and the National Sports Centre and the new Millennium Rugby Stadium. The development of some 2,700 acres of Cardiff Bay has created a 500 acre lake, 8 miles of waterfront and a new commercial and leisure environment. The Vale of Glamorgan stretches to the coast and is dotted with small country villages. Situated in the Vale are the picturesque beaches of Southerndown and Llantwit Major. Within an hour from Cardiff is varied countryside - the Brecon Beacons and the Wye Valley.

Night Life

Restaurants in Cardiff are excellent and there is a wide choice of nightclubs, discos, a jazz centre and bars.

Clubs - All staff within the Trust are eligible for membership of the University of Wales Hospital Sports and Social Club which includes facilities for squash, badminton, swimming etc, a sauna and jacuzzi together with a large social club.

Car Parking - There are parking facilities at all the hospitals in the Trust.

Shopping - Adequate shopping facilities within a convenient distance of all Hospitals.

Transport - All Hospitals are on regular bus routes with rail stations nearby.

12. CONDITIONS OF APPOINTMENT

a) The completion of a satisfactory health declaration questionnaire and screening is a condition of the appointment. The postholder must comply with the UK Health Department guidance on "Protecting Health Care Workers and Patients from Hepatitis B" (PSM(93)12) as implemented by the Trust.

b) Because of the nature of the work of this post it is exempt from the provision of Section 4 (2) of the Rehabilitation of the Offenders Act 1974 (Exemption Order 1975). Applicants are therefore not entitled to withhold information about convictions of the Act. In the event of employment, failure to disclose such convictions could result in dismissal or disciplinary action. Any information given will be completely confidential and will be, considered only in relation to an application to which the order applies.'

c) You must hold current registration with the General Dental Council throughout the duration of this contract.

d) You are normally covered by the NHS Hospital and Community Health Services indemnity against claims of medical negligence. However, in certain circumstances (e.g. in service for which you receive a separate fee or in an emergency situation outside of work) you may not be covered by the indemnity. The Health Departments, therefore, strongly advise that you maintain membership of your medical defence organisation.

e) The Ionising Radiation (Medical Exposure) Regulations 2000

This post may involve the direction of medical exposure of ionising radiation.

The Ionising Radiation (Medical Exposure) Regulations 2000 impose a legal responsibility on Trusts for all staff who refer patients for medical radiation exposure such as diagnostic x-rays to supply sufficient data to enable those considering the request to decide whether the procedure is justified.

f) Cardiff and Vale NHS Trust operates a No Smoking Policy within the Trust.

g) Removal expenses are in accordance with the Trust policy.
