SENIOR SOLUTIONS JOB DESCRIPTION

POSITION: CASE MANAGER

Classification: Hourly/Non-exempt

Supervisors: Case Management Program Director or Case Management Supervisor as assigned.

QUALIFICATIONS:

Education/Experience:

Bachelors Degree, preferably with a human service or counseling orientation, and at least 3 years of social service or related experience working for a human service agency that provides assessment and direct services to elders. Masters degree preferred.

Case management experience working with elders and younger disabled highly desirable.

Job Skills:

Excellent organizational skills and ability to work with elders and their families

Offering motivational interviewing and options counseling.

Interviewing and assessment skills.

Ability to communicate effectively, both orally and in writing.

Mediation and negotiating skills.

Ability to be assertive and to use sound judgment in dealing with difficult and stressful situations.

Ability to work independently in the field and as part of a larger team of professionals.

Ability to establish positive working relationships with service providers.

Experience and comfort level with electronic record keeping and database/government reporting systems.

Ability to work with complex written regulations.

Ability to understand the need for privacy and to maintain client confidentiality.

GENERAL DESCRIPTION:

Senior Solutions Case Manager (under the mandates of the Older American Act) assists people 60 years and older in obtaining the services needed in order to live as independently as possible consistent with client's capacity and/or preferences for care, quality of life and agency resources. Case Manager will work with clients and their caregivers to develop individualized long range future planning options. Case Manager acts as an agent for the client, but is also accountable for seeing that services provided do not exceed program budgetary constraints. Overall, the goal is to assess the needs of each client and with them implement a planning process which will empower client (and caregiver, where applicable) to use program services and available resources most appropriate to meet those needs and create future sustainability for independent living. There should be special emphasis on targeting those most in need and at risk of losing independence. Services may be offered to a few younger adults with disabilities under special circumstances.

Responsibilities:

Works in collaboration with other service providers to create a care plan to enhance an elder’s independence and assist an elder in making choices related to their needs and wants.

Monitors and reassesses program service plans as required.

Has working knowledge of services and programs available to elders and younger persons with disabilities.

Regardless of income level, offers options counseling to elders to help them plan for ongoing independence and safety.

Assists people, when necessary, to gain access to existing services such as public Benefits, housing, legal services, etc.

Ensures that all provided services do not exceed budgeted amounts and are consistent with government guidelines.

Participates in case consultation with multi-agency teams on behalf of clients when appropriate.

Acts as a supervisor for Senior Solutions’ Senior Companions within their geographic area.

Adheres to policies, procedures and standards of performance for case management as established by the State of Vermont.

Provides public education and information to clients and caregivers.

Maintains complete and accurate written and electronic documentation of all cases and reports on these activities as required.

Attends meetings/trainings as required.

Has a valid driver's license and access to reliable transportation.

Performs other duties as assigned by Supervisor.
