    I. ACKNOWLEDGMENT

First of all, I would like to thank the AFPMC for introducing and

referring us to the PNPGH, because of them, we find our summer

OJT and we get a great experience in PNPGH. Having a great ex-perience in PNPGH is not enough without our supervisors.

I would like to acknowledge first Police Senior Inspector June

Christy Manga for allowing us to have our OJT in their office. And for

guiding us while Im in my work.

And to Police Officer 3 Edison Balaan for some knowledge that

we learn without handling a book, and he is so very talkative abouthis experience in his job, family, childhood, college life, and, and

    many more.

    II. INTRODUCTION

Before we came at PNPGH, there are struggles that we en-

countered. We are expecting to have our OJT at AFPMC but ourluck is not there, they say that we need an affiliation letter before

we conduct an OJT to them, but our question is, HOW?

Affiliation letter is a four months process before your OJT, and

were working on it with Professor Morales, and we intend to have

our third year OJT to them.

In Philippine National Police General Hospital, I started to workApril 22, 2013 until May 31, 2013. Ive learn many things while Im tak-

ing my OJT inside the hospital,

    IV. OBSERVATIONS IN THE COMPANY

What I have observed at PNP General Hospital are the employees (NUP

    - Non-Uniformed Personnel and Police) assigned in the Out Patient Depart-

ment, they have a long patience in random patients attitude, most of all,

the police patients are so bossy and they are angry at all time. And thats

what weve not just observed, we also adopt and learn how to have a longpatience too.

The strength and challenges of this company is that they have more

patients everyday and most of the time, they are all angry, and we need to

sustain and reserve our patience in patients

I think the best practice of this company is to get more employees, and

that is for them also, to be more fast and reliable work force.

My over-all observation in Out Patient Department is the transaction of

the charts are very slow to send in different departments of the hospital, and

another, we did not receive the charts on time, so, we hard to find thecharts of the patient, most of them if they are a follow up check up.

    V. PERIODIC REPORTS (daily, weekly, monthly)

What we do daily are the following:

    Assisting patients about their charts

    Making a Hospital IDs

    Encoding and Indexing

What we do weekly are the following:

Every Friday, we are close at 9am to 1pm, because the all doc-

tors have a conference weekly, so we do not entertain patients at 9am and

    we will resume at 1pm

What we do monthly are the following:

Every two months we have a conference about Medical Re-

cord Section, Out Patient Department and Admitting department concerns

    and problems to talk about.

During my whole training in Philippine National police General Hospital,

I think the problems that I have encountered is the unallocated charts, mis-

place charts, loose sheets, when patient bring their own chart at home,

when doctor holds the patients Chart, wrong terminal number upon up-

date, wrong stapled chart and many more...

    VII. SELF-ASSESSMENT

    A. Skills and current technology learned/enforced:

I learned how to interact with different people, with a different per-

sonalities and attitudes. In our office, I can identify whos lying, like for ex-

ample, when they say that they have no hospital number (Terminal Num-ber) at all, we check it to the computer and when I found it, I jot it down

on a piece of paper, and I will say to the patient that you all ready have a

hospital number, then I will give to him/her the piece of paper with a hos-

pital number and I will ask him/her to keep it because that is a life time

hospital number in this hospital only.

    B. Equipment, tools, testing apparatus handled (software's used included)

Nothings new, Microsoft Office 2007, Word, Excel, Power Point and

    Publisher.

    C. Strong points versus weak points

My Strong points when I can find all the charts of the patients, I canmove fast, so I can find more patients charts but my weak points is, then

the chart of the patient cannot find at the right place, the process be-

came slow, because I will Search it in to the late submitted charts that is

not arrange by terminal number, so I will use ONE BY ONE searching to find

    the patients chart.

Another, my weak points is, when Im logging in the charts on the log

book, why? Because Im too slow to write, most especially if the word is un-

familiar. For instance, I will write his or her name upon update, weve all

know that in our new era, there is more types of name. but I can distributefast the charts in different Clinic after I log it in, and thats my strong points

    VII. SELF-ASSESSMENT

    D. Best experiences on the job:

On my last OJT at Convergys, its like Im in a high class company, we

are not require to rush. One time I did this, if we need to photocopy the

paper, we can, if I mistake 500 times, I can do it all over again to make aperfect copy. were inviting to join in there Movie Day, Team Building, they

are giving us a jacket of the company, mugs, pins, ID holder, ID lace, com-

pany T-Shirt, Etcetera. You will feel like youre a part of the company and I

also have my OJT allowance, But in Philippine National Police General Hos-

pital, they have no freebees at all, and when youre in work or office

hours, youre require to move fast, because there are more patients to

come and you need more patience...

    E. Evidence of Background preparation:

Faults committed (its causes and suggestion given by the supervisor)
