Traditional healthcare data warehouses have been primarily designed to store post adjudicated claim information and limited clinical data. Lightbeam’s healthcare data model and warehouse were designed from the ground up to store more complete, real time clinical information. More complete clinical records enable dramatic improvements in quality measures management, risk stratification accuracy, and comprehensive care management, focusing your limited resources on the right patients at the right time.

Risk Stratification

Lightbeam calculates each patient’s risk acuity based on a combination of proven algorithms, protocols, and guidelines. Unlike other solutions, our focus is not simply based on a patient’s risk factors and historical cost. Rather, Lightbeam utilizes revolutionary predictive modeling algorithms to identify patients at risk of avoidable utilization, change in risk over time, and more. Integrated Analytics

Lightbeam moves “actionable data” to a reality. Combining clinical and claims data in our Enterprise Data Warehouse provides quality measure optimization for government and commercial incentive programs alike, managing measures for ACO, HEDIS, PCMH, and more.

Unique financial dashboards identify cost and utilization overruns, pinpointing the root cause where corrective action is needed. As opportunities for improvement are identified, Cohort Management pushes notifications directly into your care team workflow or into the physician’s portal for corrective action.

Care Management

Dramatically improve the efficiency of your care management resources by embedding real time clinical data and evidence based guidelines directly into their workflow and decision making.

By delivering community-wide clinical information from disparate systems to your care managers, you greatly reduce the arduous task of having to call physician offices to manually submit electronic and/or paper documentation. Automated evidence-based care plans suggest best practices for even the most complex patients. The result is less time spent manually piecing together data and more time spent actually managing patient care.

Patient Engagement

The relative health of a population is ultimately driven by the behavior of its patients. It is up to the patient whether they take their medications, exercise, eat properly, and visit their doctors when they should.

Because patients spend a very small percentage of their waking hours in the care of a physician, it is imperative that we have a window into the health of our patients while at home. GPRO Reporting

Lightbeam’s GPRO Module simplifies the process and enhances MSSP-ACO Quality Measure submission efforts. Our ability to extract data directly from your existing EHR(s) helps automate the completion of open care gaps and satisfy MSSP-GPRO quality measures. Load your Patient Ranking File to focus resources on patients specifically requested for GPRO reporting.

Lightbeam’s GPRO Reporting Solution displays a measure progress screen which allows users to view updates on all Quality Measures from a summary view, or in ranking order. Additionally, our solution provides the ability to measure progress by practice, provider, specialty and patient to identify performance results at each level. Once all measures are satisfied, Lightbeam creates the XML files in the GRPO format for submission to CMS.

HCC Coding

Over the next 3 years, up to 80% of your reimbursement will be value-based. Within your Medicare Advantage plans, your RAF score reflects the relative health of your patient population and drives top line rev-enue. Providers often do not use the most appropriate and specific HCC code, nor do they recertify all HCC codes annually. This leads to an inaccurate representation in your RAF scoring.

Lightbeam notifies the physician of HCC code status and specificity gaps at the point of care, ensuring accurate, up-to-date documentation, which will positively impact your RAF score.

Quality Measure Optimization

Cohort Builder

One of the biggest challenges in managing population health is the conversion and delivery of actionable information to appropriate resources. The Lightbeam Cohort Builder serves as the conduit between the Enterprise Data Warehouse and Tasking, Care Management, and Patient Engagement. Further, cohorts are automatically available as filters in all Analytics. You define and configure your business rules and filters through a user-friendly interface. Filters include:

- Patient Demographics- CCM Eligibility- Risk & ATI Scores- Care Opportunities- ADT Events- Social History

Once your filter(s) are configured, simply select the action(s) you would like to take as a result of the filter being satisfied. Action options include:

- Auto Assignment by Provider, Care Team, or Care Manager- Generation of a client-defined or automated evidence-based care plan- Create/Assign Task(s)- Create/Assign Care Gap(s)- Initiate Patient Engagement campaign(s)

Once a cohort is created, all members who meet the criteria can be tracked and managed throughout the cohort lifecycle, whether it is a static “snapshot” of patients or ongoing cohort enrollment..
