LOCATION: Anderson's Scuba 541 Oceana Blvd., Pacifica, CA 94044 www.andersonsscuba.com TIME: Monday – 6PM (sharp) to 10PM. First Classroom/Pool Session. Wednesday – 6PM (sharp) to 10:30PM. Second Pool session. YOU MUST BE ON TIME! Or you will be required to reschedule! PLEASE BRING THE FOLLOWING ITEMS WITH YOU TO PARTICIPATE IN THE POOL SESSION: o Original Registration Forms o Completed Knowledge Reviews o PADI Crewpack and DVD o Swimsuit/Towel Be prepared to do a 300 yard swim using mask, fins and snorkel, followed by a 10 minute survival float. OCEAN SESSION – Information and Checklist

In order to complete your scuba certification, please refer to the following information.

REFERRAL STUDENTS ONLY Your next instructor (not SF Scuba Schools) will require you to bring:

o A passport style photo (approximately 1 3/4” x 2 1/4”) o Your PADI Dive Logbook. �� o Your Referral Form, which you will receive after completion of the pool sessions and final

exam. IMPORTANT: If I don't get your exam, you won't get your referral form for your trip. ��MONTEREY (NON-REFERRAL) STUDENTS ONLY �� LOCATION: �� Breakwater Scuba – 225 Cannery Row, Monterey, CA 93940 ��www.breakwaterscuba.com�� TIME: ��8AM sharp. Please do not be late! �� PLEASE BRING THE FOLLOWING ITEMS WITH YOU TO PARTICIPATE IN THE OCEAN SESSION:

o Dive logbook* and pen or pencil o Cash or credit card for all day parking ($10/day) o Bathing suit, sunblock, water, snacks, and clothes for layering. o Bags, bin, or proper car for the wet dive gear. You will need to take your gear overnight!
