Credit card holder agrees to pay FULL AMOUNT OF CONTRACT according to contract Terms to the venue. Contracts that are not

paid in full do not guarantee a reservation. All guests in the VIP areas must be 19 years of age or older. Only guests with the appropriate credentials will be permitted inside the VIP areas. All alcoholic beverages inside the VIP areas will be served to guests by

the designated hostesses/servers only. Any guest showing signs of intoxication will not be served and may be ejected from the premises. Venue Management & Security reserves the right to disallow or discontinue service. Pre-purchased bottles that are not

consumed or opened will not be refunded; alcohol is NOT permitted to leave the premises. The booking must be checked in by the specified time. If the client does not show in time the venue has the right to sell the table and a $250 penalty fee will apply.

IMPORTANT: ALL PAGES OF CREDIT CARD AUTHORIZATION FORM MUST BE INITIALED IN ORDER FOR

RESERVATION TO BE CONFIRMED. CUSTOMERS MUST INCLUDE A LEGIBLE COPY (FRONT & BACK) OF BOTH CREDIT CARD AND VALID PHOTO ID TO CONFIRM RESERVATION.
