DEFINITION OF TERMS/PARTIES ExeGen – Executive Genesis Manpower Services, Inc, the EMPLOYER Client Company – The client of ExeGen where the contractual employee is assigned, detailed or posted Contractual Employees – All term personnel and project employees of ExeGen who are under contractual employment for certain Client Companies 2. GERNERAL PROVISION – this policies 3. APPEARANCE AND DECORUM - The employee is urged to dress accordingly and presentably in prudent business attire or in the proper uniform if required within the office premises, during work hours. Cordial and harmonious relationship with clients, other employees and officers of ExeGen and its Client Company is urged in order to promote a peaceful and healthy work environment. 4. HOURS OF WORK/OVERTIME PAY - The contractual employees shall be required to work for not more than eight (8) hours a day, for six (6) days a week, at the most, within the designated work premises. Overtime pay will be afforded the Contractual Employee, provided that the Overtime Slip was properly accomplished and approved by the Client Company’s authorized signatory 5. ATTENDANCE & TIME KEEPING a) Contractual employees are required to come to their designated workplace on time b) Absences must be avoided. Absenteeism, especially those that are beyond reason or those that are habitual, will result in the TERMINATION of employment. c) An employee is required to record his/her daily attendance on company’s logbook or bundy clock upon coming in for work and upon going out at the end of the employees work hours. Note the interpretation of entries in the time record for purposes of salary computation: No record of attendance in the logbook - ABSENT No record of coming IN but with record of time OUT- HALF DAY With record of time IN but no record of time OUT - HALF DAY d) Punctuality must be observed at all times. Tardiness will be dealt with accordingly. Three (3) tardiness in one month - written warning (memo) Four (4) to seven (7) tardiness in a month - suspension More than seven (7) tardiness in a month - termination Three (3) memos on tardiness - termination e) Keeping of accurate time record is necessary in the computation of hours worked and the payment of salary. 6. DTR SUBMISSION & RELEASE OF SALARY a) DTRs and time sheets must be submitted to ExeGen office on time. Cut-off periods for computation of

salary/wage are every 15th & 30th of the month. Deadline for the submission of DTR/Time sheets starts on the 1st & 16th day of month and ends on the 3rd and 18th of the month, respectively. DTRs submitted beyond the given deadlines shall be computed for the next cut-off period and the salary will be released on the next payday.

b) DTRs and time sheets must be duly signed and approved by an authorized signatoryc) Release of salaries is every 20th & 5th of the month thru ExeGen issued ATM cards.

In cases where the employee has not yet been issued an ATM card, one may either claim his/her salary personally at the ExeGen office or through a representative with the proper authorization letter

In case of loss of ATM card, inform ExeGen office at once and go to the nearest bank branch for proper reporting and documentation

d) DTR/time sheets should be signed & approved by an authorized signatory of the Client Companye) The following shall be deducted from the salary: (i) Witholding Tax, (ii) SSS, (iii) Philhealth and (iv)

Pag-Ibig Fund contributionsf) A performance deposit (bond) of PhP100 shall also be deducted from the monthly salary of the employee

to ensure job efficiency. The accumulated amount will be released to the employee, in its entirety, upon termination of his/her employment and the procurement of clearance from the Client Company.

g) NO ADVANCES TO THE PERFORMANCE DEPOSIT SHALL BE ENTERTAINED7. SALES REPORTS & OFFICE DAY MEETINGS - ExeGen shall have the option to hold the salary or sales

incentives of a personnel who fails to submit sales reports on time as well as those who fail to attend office day meetings

8. TERMINATION - Employment may be terminated for lawful and authorized causes under the Labor Code, including but not limited to (a) inability to learn and undertake the duties of the position that he/she is employed for, (b) recession in business of ExeGen or its Client, (c) inefficiency, (d) personnel reduction and (e) violation of ExeGen rules, regulations and policies, and (f) expiration of employment contract. In the event of termination, ExeGen undertakes to pay your compensation for the days actually worked and ExeGen shall neither be liable for the unexpired period of the contract nor for any separation pay.
