We are in the process of updating your accounts and realized we need some more information for <POA>. She is currently not a customer of KANZA Bank and in order to add her to our system we need some information on her. We need two forms of ID which include: We need a copy of one of these items: State Drivers License (DO NOT ACCEPT IF EXPIRED OVER 30 DAYS) Passport (DO NOT ACCEPT IF EXPIRED) Military ID Government ID Employee ID Card And a copy of one of these items as well: Social Security Card Voter Registration Birth Certificate Credit Cards Bank Cards A copy of two items from the first list would be acceptable as well. If you could have <POA> make the appropriate copies and have her fill out the Customer Account Application that is enclosed we would appreciate it and then we will be able to update your accounts with her listed as your power of attorney. Please return in the envelope provided.
