User Name (user defined) Password (user defined – please follow guidelines – to protect yourself) Confirm Password E-mail address Security Question (user defined) Security Answer (user defined) If you would like to receive notifications click on (Receive Processed Claim Notification) This feature allows you to receive e-mails regarding claims paid and or denied, claim number only..

Then - Create User

Confirmation Page - Once you receive this page you can access your account information.

You are now a Returning User Log In User Name: Password: Click Log In

This will bring you to your home page.

If you want to view claims you can select from the pull down tab by Acct Type Acct Type All ~ When you select All this will yield a better result ~ Claim Type (This is specific to you: dental, vision etc.)

If you want to view Specific claims only you can select from the pull down tab Acct Type All ~ When you select All this will yield a better result ~ Claim Type (This is specific to you: dental, vision etc.) * Always REFRESH after selection *

If you have the Flex Plan you will view your account balance on this tab. This will give you current year, previous year and what is paid and left on account.
