CLINICAL CASES

CASE #1 – Right anacusis with contralateral CI

• Age – 43

• Sex – Female

• History

- 2011: left SSHL (medical therapy & HBOT)

- HBTO: sudden-onset right SNHL (!)

- Right ET: RW perilymph fistula (repaired)

- HA since then, with further worsening hearing and no benefit since 2016

- 11/2016: left CI (GO),

- Good functional results: VI 100%, WI 90%, SI 94%, SC: 95%

- HL, tinnitus, mild episodic dizziness

• Otoscopy & hearing test

CASE #1 – Right anacusis with contralateral CI

THI: 92 (catastrophic handicap)

• Imaging - CT

• Planned surgery: Right-sided sequential CI

CASE #2 – Anacusis with CL cholesteatoma

• Age – 44 years-old

• Sex – Female

• History

- Right SNHL 3 yrs ago (no benefit from medical therapy)

- Left-sided progressive HL

- No benefit from HA

- Tinnitus

• Otoscopy & hearing test

CASE #2 – Anacusis with CL cholesteatoma

• Other:

- THI: 56 (moderate)

• Imaging – CT (negative MRI)

• Planned surgery: Right STP + CI

CASE #3 – Unilateral SNHL

• Age – 43 years-old

• Sex – Female

• History

- 2016: left SHL (medical therapy) with little recovery, tinnitus

- IT steroid: no benefit

- Trial of conventional HA, Ponto softband trial, CROS HA unsuccessful

- Current complaint: unilateral HL, episodic tinnitus

• Otoscopy & hearing test

CASE #3 – Unilateral SNHL

• Other:

- Normal tymp

- No reflexes left ear

• Imaging – MRI (pre-op CT scan)

• Planned surgery: Left CI

CASE #3 – Unilateral SNHL

CASE #4 – CHL in only-hearing ear

• Age – 66 years

• Sex – Male

• History

- Left CWD mastoidectomy at the age of 13

- 10/2016: TC removal of right PBC

supralabirynthine-apical with intradural extension

preop grade 6 HB FN palsy

- Current complaint: HL

• Otoscopy & Hearing test
