This tutorial will help you learn how to:

REQUEST ACCOUNT ACCESS

If you do not have a MyChart account, you can request an access code.

Complete the form and select the “Submit”

button at the bottom.

RECEIPT OF ACTIVATION CODE

Fill out the form and select the “Next” button at the bottom.

If you received your activation code, select the button shown here to enter the code.

LOGIN TO MYCHART

Enter your username and password and click “Sign In”

• Request proxy for:• Children age 14–18• Other Adults

• Print, sign and mail paperwork to Meriter

PROXY ACCESS

Proxy Access is requested for

Dependent child or adult

Form is mailed to member for signature

Form is signed and returned to Meriter HIM

department

HIM determines through verification process if

should approve or needs additional information.

Member is contacted that Proxy access has

been given/rejected by letter

PROXY ACCESS

Once you receive verification that proxy access is approved, you will see a new tab(s) on the right side of the home page with the dependent’s name.
