A close correlation was found between the two ratings, except for patients who reported very severe pain. This suggests that in most cases a physician can, by interpreting the patient's reported level of pain in light of the medical history and physical examination, accurately assess the patient's level of functional activity. Disability and pain have always been somewhat subjective and dif- ficult to measure. In particular, it remains uncertain whether there is any correlation between the physi- cian's clinical estimates of a pa- tient's degree of pain and disability and what the patient is actually experiencing. With the wide avail- ability of medical and rehabilita- tive treatment, and with the depen- dence of insurance payers on phy- sician's assessments, it becomes im- portant to measure whether any such correlation exists. Today, disability is usually inter- preted in one of two ways: (1) in terms of ability to be gainfully em- ployed; or (2) as a comparison be- tween the condition of the disabled person and that of a healthy person of the same age. Problems abound in both interpretations. First, in terms of work capacity, a person unfit for employment in one job may be fit for another if the work conditions (e.g., environment, type of work, hours) are altered by consent between the disabled worker and the employer. The en- tire definition of "work" changes in light of these concessions. Second, a person's disability cannot be judged by only one set of standards. The level of disability is in many ways controlled by the individual's ability to adapt to changing conditions. In short, a person is well if he or she can carry on usual daily activities; a person is disabled to the extent that he or she depends on help from others. Several studies in Great Britain 1.2 have been aimed at the identifica- tion of the disabled population that deserves public medical and social support. Recent research in the United States has largely focused on measuring levels of disability of patients who are suffering from various diseases. 3 Patients In an attempt to clarify the rela- tionship between the identification of disability by a physician and the individual's perception of his or her disability, we undertook a study of rheumatoid arthritis patients. cripples the most. More than 5'12million people, or about 10% of thepopulation of England, consult aphysician annually with arthriticcomplaints.4

Our study was based on 119 pa­tients between 20 and 66 years ofage who were admitted to one offour hospitals for the nonsurgicaltreatment of rheumatoid arthritis

in and around Manchester, Eng­land. The four different types offacilities are described in a separatepaper,' which also analyzes thecomparative costs and effects of themedical management of these samepatients.

Methods

Four hospital-based consultantphysicians who specialize in thetreatment of arthritis and rheuma­tism agreed to participate in thestudy. Each consultant was locatedat a separate treatment center. Fol­lowing the completion of a shortquestionnaire by the physician, pa­tients were admitted to one of thefour facilities for the correctivetreatment of rheumatoid arthritis.The consultant rheumatologist incharge determined the level of dis­ability of his own patients.

Physician assessment

The Steinbrocker FunctionalIndexh was used to assess each pa­tient medically. It has four ratingsof function: incapacitated (4); lim­ited (3); adequate (2); and com­plete (I). A low score of I indicatesthat the patient is not incapaci-

tated; a score of 4 represents com­plete disability.

Patient assessment

A standard verbal descriptivescale,? pioneered by Keele in 1948and replicated in numerous studies,was modified for use to assess apatient's level of pain. Originally,the scale contained five categoriesor states of pain: (I) none; (2)slight; (3) moderate; (4) severe; and(5) agonizing. For this study, onlyfour categories were used; the cate­gory "none" was eliminated andthe category "agonizing" was re­placed by "very severe." Reducingthe verbal index categories to fourmade it possible to correlate themdirectly with the categories on theSteinbrocker Index. Since there arepersistent research problems in try­ing to quantify pain, we simplyasked each patient to assess his painsubjectively and assign it a score ofI to 4.8

Results

Results of Chi-square tests showedthat patients in all facilities weredemographically equivalent andthere were no intergroup dif­ferences in number of patients,mean age, sex, social class, age atonset of disease, or duration of dis­ease. Other variables inspected(such as number of joints involvedand joint causing most difficulty)did not bias the results of this study.

The only statistically significantdemographic variable among thepatient groups was the equalnumber of men and women amongthe small patient population at oneof the four treatment facilities. Inthe other three facilities, there werethree times as many women asmen. Most patients (63%) were be­tween 40 and 59 years of age; andon the whole, patients had the dis-

ease an average of 9.5 years. Morethan 50% were middle class, withan equal proportion of lower- andupper-class patients among thefour facilities.

Pain-scale scores

As noted above, each patient, uponadmission to the study, rated him­self or herself on a four-point ver­bal descriptiv.e pain scale. The re­sults are shown in Figure I. Some12 patients rated their pain asslight; 46 patients complaine9 ofmoderate pain; 39 others statedtheir pain was severe; and 22 com­plained of very severe pain.

Physician's Steinbrocker Index

Figure 2 illustrates the ratings fromthe consultants' Steinbrocker Indexof 119 study patients. This indexwas scored independently of thepatient's verbal index after theconsqltant rheumatologist con­ducted a complete history andphysical examinatlon on each pa­tient. Fifteen patients were ratedslight; 63 patients were considered

moderately disabled; 35, severelydisabled; and only six, incapaci­tated or very severely disabled.

Statistical significance

The Chi-square test (with Yate'scorrection) was used to determinethe probability associated with thehypothesis that the ratings from theSteinbrocker Index were not signi­ficantly different from those of thepatients themselves. Each categoryof the Steinbrocker ratings was

Where the physicians' ratingswere compared with the patients'ratings on the verbal descriptivepain Index, there were no signifi-

Correctdiagnosis may depend on combin­ing the patient's description of hislevel of pain with the rest of theclinical evidence.

As in many other chronic ill­nesses, pain is a m<ljor contributingfactor to the disease state in pa­tients with rheumatoid arthritis.The results of our study provideevidence that diagnosis of the stateof ill health in rheumatoid arthriticpatients does coincide with the painlevels reported by the patients.

As third-party insurance payersbecome more concerned about thecosts of patients who complain ofchronic disabling diseases, it is im­portant that physicians be able toestimate the level of functional ac­tivity a patient should be aQle tomaintain, given the presence ofpain. The rheumatologists in ~:>ur

study estimated a level of func­tional ability which, for the mostpart, coincided with the patient'sestimates of their pain. The onlysignificant discrepancy was forthose patients who recorded verysevere pain but whose history andphysical examinations did notalways bear out that assessment.

The following tentative conclu­sions can be drawn from this com­parison of the Steinbrocker Func­tional Index rating and the painself-scores of patients:• The estimates of physicians andpatients concerning p",in and func­tipnal ability were not significantlydifferent in over 95% of cases.III!I The degree of pain experiencedby a rheumatoid arthritis patientthus generally coincided with hislevel of functional disability as as-

The findings of this study implythat the degree of functional activ­ity. or capacity correlates directlywith pain as perceived by the indi­vidual patient and so, in most cases,rheumatologists can accurately es­timate a patient's functional capac­ity on the basis of the patient's ownestimate of the severity of hispain. 0
