The analysis of matrix spike/matrix spike duplicate (MS/MSD) quality control samples for each of the aqueous extractable analyses requires the collection of additional sample volume. Please contact the lab.
Wastewater containing residual chlorine needs to be preserved with sodium thiosulfate for the analysis of pesticides by EPA 608, SVOC by EPA 625, and VOC by EPA 624. Prior to sampling, add 80 mg of sodium thiosulfate to each liter amber and 10 mg to each 40 ml vial.
Per the method, a trip blank is required to be analyzed with samples if there are dectections. Please let your project manager know when you order bottleware that you will be analyzing drinking water samples for 524.2 so trip blanks can be provided.

Expedited TAT Pricing Schedule
All pricing in this service brochure is based on a standard turnaround time of 5 business days. Rush turnaround times can be accommodated and will be priced according to the schedule below.
Normal 5-day turnaround analyses
3-day…………………………………………………………..125%
2-day…………………………………………………………..150%
1-day…………………………………………………………..200%
Emergency/Weekend/Holiday………………………………..300%
Toxicity Characteristic Leaching Procedure (TCLP) analyses
3-day…………………………………………………………..150%
2-day……………………………………………………….….200%*
1-day…………………………………………………………..300%*
*Due to the lengthy amount of time involved in the TCLP herbicide extraction, it may be difficult to provide results for this analysis on turnaround times of less than 2 days. Please call the lab ahead of time to discuss rush turnaround times for TCLP analyses.
Sample Cancellation Charge
Due to our rapid standard turnaround time, analyses may be started shortly after samples arrive at the laboratory. In some cases, within a matter of hours. In the event that it is necessary to cancel the analysis of a sample, it is crucial to notify the laboratory as soon as possible to avoid being charged. If a sample is canceled after analysis has begun, you will be charged for the work completed at the time of cancellation.
Sampling Kits and Coolers
Sampling kits and coolers are provided and shipped (via non-priority status) to our clients at no additional charge. These kits include all containers, preservatives, chain of custody forms, labels, and coolers to collect your samples and have them shipped to the lab. Overnight and priority shipments can be accommodated with the difference in cost being conveyed to the client.
Please keep us abreast of pending projects with as much notice as possible so we may assemble your sample kit and have it delivered in time for sampling to begin. Sample kit and container requests can be made by phone, fax, or via our internet site at www.phaseonline.com. We do request that all coolers and any unused sample containers be returned to us within 30 days.

Sample Submission Requirements
All samples submitted for analysis must be accompanied by a completed chain of custody form or letter indicating the required analyses. For your convenience, chain of custody forms can be obtained from the lab or downloaded from our website at www.phaseonline.com. Below are instructions and an example of a properly completed chain of custody form.
Section 1 CLIENT: Name of company submitting samples. OFFICE LOC: If the company submitting the samples has multiple locations, indicate which office is submitting the samples. PROJECT MANAGER: Person PSS should contact if additional information is required. This is also the person the report will be sent to unless specified differently in the “Special Instructions” box in the lower right hand corner of the form. PHONE NO: Phone number PSS should call if questions arise about sample or analysis. EMAIL: E-mail address the report should be sent to. FAX NO: Phone number that the report should be faxed to. PROJECT NAME: Provide name of project as it is to appear on the analytical test report. PROJECT NUMBER: Provide the number of the project (if applicable) as you want it to appear on the analytical test report. SITE LOCATION: Provide the site location where the samples were taken (if applicable) as it is to appear on the analytical test report.

General Information
Sample Submission Requirements (Continued)
Section 1 (Continued) P.O. NUMBER: Provide purchase number (if applicable) that should appear on invoice. SAMPLERS: Provide the name(s) of the person(s) who took the samples. DW CERT NO.: If the sample is for drinking water compliance in Maryland, please list the sampler certification ID. Section 2 LAB NO.: For laboratory use only. SAMPLE IDENTIFICATION: Identify sample as it should appear the analytical test report. DATE: The date the sample was collected. TIME: The time the sample was collected. MATRIX: The physical nature of the sample i.e. Water, Soil, Oil, Liquid or Solid. (See matrix codes listed on COC for matrix abbreviations.) No. CONTAINERS: Number of containers provided for each sample. SAMPLE TYPE: Specify C for composite sample or G for grab sample (if applicable to project).
Section 3 Preservatives Used: Specify preservative used for each analysis requested, leave blank if none were used. Analysis/Method Required: Specify the analysis requested for each sample by listing the analyte (or list of analytes) and test method on the diagonal line. Put an X in the box below the parameter that corresponds to the sample that requires that analyte. REMARKS: Additional information about a particular sample, i.e. “hold for further instruction” or “expect high levels”. Section 4 # of Coolers/Custody Seal/Ice Present/Temp/Shipping Carrier: For laboratory use only. Data Deliverables Required: Specify level of data required with report. Level I is PSS’s standard report. Contact lab for information on Level II and Level III data packages (please note that a surcharge usually applies to Level II and Level III data packages). Requested Turnaround Time and Special Instructions: Select requested turnaround (TAT) time for samples. PSS standard TAT is 5-days. If requesting multiple TATs for samples, please fill out a separate COC for each TAT to ensure samples are reported correctly. If TAT something other than the options listed, please indicate this in the “Special Instructions” box. Section 5 “Relinquished By:” This is one of the most important parts of this form. Phase Separation Science offers a no cost “all day” courier service as a courtesy to clients within our service area. The Routine Courier Service Area map below indicates the normal area of operation for this service. This range can be expanded but may incur an additional cost. The no cost pickup service requires that samples be ready for pickup at your office or project site by 9 AM. The samples will be picked up sometime between 9AM and 5PM and delivered to the laboratory the day of the pickup. Sample pickups can be accommodated at other times or transported to the lab on an expedited basis, but these services normally incur an additional cost. Please contact the lab for the costs associated with rush pick-up service. Please note that if the samples are not ready upon the courier’s arrival, additional charges for wait time may also be incurred.
To schedule a no cost “all day” pickup, call our office at 410-747-8770 or send an email to [email protected]. For sample deliveries outside of our normal business hours, please call a client services representative to discuss special arrangements. Samples can be shipped to the lab from anywhere via UPS and FedEx.
All couriers are trained in proper chain-of-custody procedures for your convenience, but do not carry ice or coolers, so samples must be properly iced in a cooler for transport to the laboratory.
