Welcome
Onbehalfoftheadministration,teachers,andstaffofPhalenLeadershipAcademy(“PLANetwork”),wewelcomeyoutoournetworkofschools.AtthePLANetwork,webelievethateducationisapartnershipthatthrivesonlywiththesupportofeverystaffmember,theinvolvementofeveryparent,andthecommitmentofeveryscholar.
Wealsounderstandthatcommunicationisofutmostimportanceinestablishingasuccessfulhome-schoolpartnership.Inanefforttoinformandeducateourschoolcommunity,wehavecreatedthishandbooktoinformyouonimportantpoliciesandpracticesofthePLANetwork.Pleasetaketimetoreviewandfamiliarizeyourselfwiththeinformationcontainedwithinthehandbook,asitwillserveasavaluablereferenceinseekinganswerstocommonlyaskedquestions.Shouldyouhaveaquestionorconcernthatisnotaddressedinthishandbook,pleasecontacttheschoolPrincipal.

B.GeneralInformation
Enrollment
AllscholarsareeligibletoenrollinaPLANetworkschool,aslongasthereareseatsavailableperourcharterwiththeIndianaCharterSchoolBoard(“ICSB”),ortheyresideintheattendanceareaofourinnovationschool.WewelcomescholarswhowillbeenteringgradesK-10.Allinterestedscholarsmustsubmitanapplicationform.Anadultwhoisanon-custodialparentordoesnothavealegalguardianshipandwishestoenrollascholarinaPLANetworkschoolmustcontacttheSchoolEnrollmentCoordinatorordesigneeforassistancetocompletetheenrollmentprocess.
Parentsmustprovidethefollowingdocumentstotheschoolduringtheenrollmentprocess:
• Birthcertificate,• Courtdocumentsspecifyingparentalrights,responsibilitiesorcustody(if
appropriate),• ProofofIndianaresidency,• Proofofimmunizations,and• FerpaStatement
Note:TheMcKinney-VentoAct
The McKinney-Vento Act protects scholars from discrimination based on their lack of a permanent residence. Homeless Scholars are eligible to receive a variety of services including: transportation services, free meals under the school nutrition programs, limited English proficiency class, gifted and talented programs, and special education services based on a scholar’s eligibility. Homeless Scholars will not be denied enrollment based on lack of proof of residency. Please consult the McKinney-Vento Act Liaison if you need assistance.

ScholarAttendance
ItisimperativethatScholarsbeinattendanceeachschoolday.Learningisaresultofactiveparticipationinclassroomandotherschoolactivitiesthatcannotbereplacedbyindividualstudy.Scholarattendancehoursare7:15a.m.to2:15p.m.Scholarsarrivingafter7:15a.m.,leavingearly,orbeingsignedoutforaportionoftheinstructionaldaythatislessthan1⁄2day,willbemarkedastardy.ScholartruancymaytriggeraProjectAttendreferralinasfewas2instances.ChecktheschoolwebsiteorcontactthePrincipaltogettheschool’sprotocolforreportingabsencesandcheckinginlate.
PLAstrivesfora95%attendancerate.Tomeetthisexpectation,noscholarshouldexceed9absencesperyear.Unlesspreviouslynotifiedofextenuatingcircumstancesnecessitatingthefrequentabsences,theschoolmaycontacttheparentonceascholarreachesthemidwaypointof5absences.Thiscommunicationwillbebyphoneorletter.Ifyourchildhasachronicillness,weencourageyoutocontacttheschoolnursetoavoidanyattendanceissues.
• MissedSchoolWorkDueToAbsenceFollowingareturnfromabsence,Scholarswillhavetimeequaltothelengthoftheirabsencetocompletetheirmissedwork.Pleasedonotrequesthomeworkinanticipationofaplannedabsence.Iftherequestforhomeworkisduetoanillness,therequestmustbesubmittedby10:00a.m.ParentsshouldcallthePrincipal’sofficetorequesthomeworkandtopickupbooksandmaterialsattheendoftheday.
Tardiness
Scholarsareexpectedtobeontimeforallscheduledclasses.Scholarswillbeconsideredtardyifnotintheclassroomattheassignedstarttimeforthatclass.Ascholarwhoenterstheclassafterthebellhasrungwithoutanexcusedpasswillreceiveanunexcusedtardy.Ifascholaraccumulates5tardies,scholarwillserveasuspensionthefollowingday.Ifthesamescholaraccumulates5additionaltardies,they’llservea2-daysuspensionandwillfollowthispattern(5more,3-daysuspension,5more4-daysuspension).Oncescholarhits10suspensiondays,thescholarwillbeexpelledfromschool.
IntenttoReturn/ContinuingScholars
Eachspring,parents/guardiansofcurrentscholarsmustcomplete‘intenttoreturn’paperworkand/orprocedurestoregistertheirscholarforfollowingschoolyear.Itis

believedthatfamilieswhodonotsubmitintenttoreturnmaterialswillbeconsideredtohavewithdrawnfromthePhalenLeadershipAcademies.
WithdrawingfromSchool
ParentsmustcontacttheFrontOfficeAdministrator,iftheychoosetowithdrawtheirchildfromschool.Ifastudentiswithdrawingtohomeschool,form1005mustbecompletedattheschool.ForInformationorquestionsregardingtheHomeSchoolEnrollmentProcess,pleasecontacttheIndianaDepartmentofEducation'sHomeSchoolLiaisonat317-232-9111.
ClassroomAssignments
Theprincipalassignsscholarstohisorherclassroomandprogram.Anyquestionsorconcernsabouttheassignmentshouldbediscussedwiththeprincipal.ChangestoanassignmentofascholarwithanIndividualEducationProgram(IEP/SpecialEducation)canonlyoccurthroughacaseconferencecommittee.
DressCode-SeepolicyinAppendix.
EarlyDismissal
Noscholarwillbeallowedtoleaveschoolpriortodismissaltimewithoutparentalpermission.Noscholarwillbereleasedtoapersonotherthanacustodialparentwithoutapermissionnotesignedbythecustodialparent(s)orotherlegalauthorization.Thepersonpickingupthescholarshouldhavephotoidentificationandpresentittotheschool.
EligibilityForExtracurricularActivities
MostactivitiesconductedatPLANetworkschoolsareinlargepartcurricular.Theseactivitiesareconnectedtothecurriculumeitheracademicallyorinthedevelopmentofsocialorphysicalskills.Thus,itimportanttothescholar’stotaldevelopmentthathe/sheparticipatesintheseofferedactivities.Ifintherareinstanceascholarpresentsadisciplineproblemordisruptsanactivitytothedetrimentofotherscholars,theprincipalwithinputfromtheteachers,willdecidewhethertodenythatscholartheopportunitytoparticipateincertainschoolactivities.Somesportsingrades7-10willrequireasportsphysical.
PhysicalEducation
PhysicalEducationclasseshavespecialdressrequirements.ScholardressforPEclassmustallowsafeperformanceofallphysicaleducationactivities.ScholarswillbeallowedtowearP.E.clothes.Scholarswillbepermittedtotaketheirshirtsoruniformshomeonaregularbasistobewashed.Everyscholarmustwearflat-soledathleticshoesthatarenotslip-ons.Adoctor’sstatementwillberequiredforthreeconsecutiveabsencesfromphysical

educationclassinonegradingperiod.Anotefromaparentwillexcuseasingleweek’sabsencefromparticipationinphysicaleducationclass.
ScholarFundraising
ThePrincipalwillapproveallfundraisingactivitiesfortheschool.Scholarsarenotpermittedtosellitemsforpersonalgainorfororganizationsthatarenotaffiliatedwiththeschoolonschoolproperty.
EmergencyClosingsandDelays
Intheeventofsevereorinclementweatherormechanicalbreakdown,PLAmaybeclosedorstartatalatertime.Informationregardingschoolclosing,delayedstartingtime,orearlydismissalwillbeprovidedviaourSchoolMessengersystem(phoneandtextalerts).Additionally,thePLAwebsitewilllistdelaysandclosings(www.phalenacademies.org).Ifweatherclosestheschool,thenallextracurricularactivitiesincludingathleticpracticesandgameswillbecanceled.TheTVwillalsopostcancellationsviaABC,CBS,andNBC.

AllscholarsmustcontactPLAifheorshechangeshisorheraddress.Parentsand/orscholarsmustobtainchangeofaddressinformationfromtheprincipal’sofficeortheycanobtainthedocumentsonlineatwww.phalenacademies.org.
BoardMeetings
ABoardofDirectorsgovernsPLAcharterschools.TheseboardmeetingsaresubjecttoIndiana’sOpen-DoorLaw.InformationregardingBoardmeetingswillbepostedontheschoolwebsiteandinspecifiedlocationsthroughouttheschool,within48hoursofthemeeting.TheBoardofDirectorsapprovesandauthorizesallitemsintheschoolhandbook.
ThePledgeOfAllegiance
Eachday,scholarscouldbeprovidedwithanopportunitytorecitethePledgeofAllegiance.AscholarisexemptfromparticipationinthePledgeofAllegianceandmaynotberequiredtoparticipateinthePledgeifthescholarchoosesnottoparticipateorifthescholar’sparentchoosestohavethescholarnotparticipate.
Visitors&VisitorTags
Parentsorguardiansareencouragedtovisittheclassroomaslongasitisnotdisruptivetothelearningenvironment.Parentsshouldnotseektoconferwithateacherduringtheschoolday(e.g.,betweenclasses,duringclass,duringafter-schooltutoring,etc.).Whilewewelcomeeveryfamily’sinsight,perspectiveandparticipation,teachersmustfocusoninstructionduringtheschoolday(andimmediatelyaftertheschoolday).Werespectfully

askthatyouscheduleaparent/teacherconferencethroughthemainofficeortheadministrator’soffice.
•ArrangingAConferenceParentteacherconferencesareheldinthefall.Theseconferencesarescheduledbyclassroomteachers/teams.Parentswishingtoscheduleanadditionalconferencewithateacher/teamoranadministratorshouldcontacttheindividual(s)involvedtomakearrangements.
Guestswhocannotrespectthispolicywillbebarredfromenteringtheacademicareasofthebuildingandconfinetheirbusinesstothefrontofficeortheywillberemovedfromourcampus.Inrarecaseswherevisitorbehaviorcausesamajordisruption,visitorswillbeaskedtoprearrangeallvisitsthroughschoolofficialsandsecurity.
Allvisitorsareexpectedtocheckinattheofficetoobtainavisitor’stag.Pleasedonotbeoffendedifyouarestoppedifyoudonothaveavisiblevisitor’stagdisplayed,asourScholars’safetyandsecurityareoftheutmostconcernforallofus.Please consultthePLANetworkVisitor’sPolicyregardingthestepsforschedulingand/orobtainingpermissiontovisittheschoolandclassrooms.Parents/visitorscanbedeniedaccessbeyondthemainofficeforanyreasonduringschoolhours.ThepolicyislocatedintheAppendixofthishandbook.
Loitering
PLAstaffwillsupervisescholarsthroughouttheday.Scholarsmaynotloiteronschoolcampus.Scholarsmustleaveschoolgroundswithin15-20minutesofdismissal,unlessstayingoncampusforanapprovedandsupervisedeventorpurpose.Scholarsstayingafterschoolforanextracurricularactivitymustbeunderthesponsor’ssupervisionatalltimes.
Confidentiality
TheFamilyEducationalRightsandPrivacyAct(FERPA)prohibitsthePLANetworkfromdisclosingpersonallyidentifiableinformation(“PII”)fromscholars’educationrecordswithouttheconsentofaparentoreligiblescholar.Aneligiblescholarisascholarwhohasreachedtheageofeighteen(18).
Parentsoreligiblescholarshavetherighttoinspectandreviewthescholar’srecordsandrequestthatPLAcorrectrecordswhichtheybelievetobeinaccurate.PLAmustobtainwrittenpermissionfromtheparentoreligiblescholarinordertoreleaseanyinformationfromthescholar’srecord.PLAmaydiscloserecordswithoutconsentunderthefollowingconditions.
• Schoolofficialswithlegitimateeducationalinterest;• Otherschoolstowhichastudentistransferring;• Specifiedofficialsforauditorevaluationpurposes;• Appropriatepartiesinconnectionwithfinancialaidtoastudent;

• Organizationsconductingcertainstudiesfororonbehalfoftheschool;• Accreditingorganizations;• Tocomplywithajudicialorderorlawfullyissuedsubpoena;• Appropriateofficialsincasesofhealthandsafetyemergencies;and• Stateandlocalauthorities,withinajuvenilejusticesystem,pursuanttospecific
Statelaw.
PLAmayalsodisclose,withoutconsent,informationdesignatedas“directoryinformation”,whichincludesastudent'sname,address,telephonenumber,dateandplaceofbirth,honors,awards,datesofattendance,scholarphotos,andscholarworkdisplayedatthediscretionoftheteacher.PleasereadandsigntheNotificationofRightsunderFERPAfoundintheAppendixandreturntothePrincipal.
SchoolFeeWaivers
TheparentofascholaroranemancipatedminorwhoisenrolledatPLA,andwhomeetsthefinancialeligibilitystandardestablishedbyIC20-33-5-2maynotberequiredtopaythefeesforcurricularmaterials,supplies,orotherrequiredclassfees.IfPLApaysthefees,itmayseekreimbursementfromtheDepartmentofEducation.Ifthereimbursementamountislessthanthefeeassessedforthecurricularmaterial,PLAmayrequestthattheparentoremancipatedminorpaythebalanceoftheamount.
Cafeteria/FoodService
Everyfulldayofschool,breakfastandlunchwillbeavailableforscholarstopurchaseinthecafeteria.Scholarswillbeallowedtoenterthecafeteriaat7:40a.m.forbreakfast.Forscholarswithpaidorreducedlunches,thescholarprepaysonhis/heraccountandwillbechargedaccordingly.Scholarswhoqualifyforfreelunchwillusehis/heraccountnumbertoassistinrecordkeepingfortheFederalSchoolNutritionProgram.Scholarswithspecialdietaryneedsmustprovideamedicalstatementtosubstantiatetheneedforafoodsubstitution.Themedicalstatementmustincludeanidentificationofthemedicalorotherspecialdietaryconditionthatrestrictsthescholar’sdiet,thefoodorfoodstobeomittedfromthescholar’sdiet;andthefoodorchoiceoffoodstobesubstituted.Medicalstatementsfordietrestrictionsmustberenewedeveryyeartoreflectthecurrentdietaryneedsofthescholar.PLAbarsscholarsfrombringingfastfoodforschoollunchorevents.
Fire,Tornado,andSafetyDrills
PhalenLeadershipAcademiescomplieswithallfiresafetylawsandwillconductfiredrillsinaccordancewithstatelaw.Specificinstructionsonhowtoproceedwillbeprovidedtoscholarsbytheirteacherswhowillberesponsibleforsafe,prompt,andorderlyevacuationofthebuilding.Tornadodrillswillbeconductedduringthetornadoseasonusingtheproceduresprescribedbythestate.Thealarmsystemfortornadoesisdifferentfromthealarmsystemforfires,andscholarswillbeinformedofthedifference.Safetydrillswillbeconductedonceperschoolyear.Teacherswillprovidespecificinstructiononthe

appropriateproceduretofollowinsituationswherescholarsmustbesecuredinthebuildingratherthanevacuated.Thesesituationscanincludeaterroristthreat,apersoninpossessionofadeadlyweapononschoolproperty,orotheractsofviolence.
ParentTeacherOrganization(PTO)
PTOeventsareamajorpartofthe“life”ofourschools.Theyprovideavarietyofspecialopportunitiesforourscholarsandyourparticipationisextremelyimportant.Volunteersareneededindifferentcapacitiesthroughouttheyear,manyofwhichoccuroutsidetheschoolday.PleaserefertoPLANetwork’swebsiteormainofficeforinformationregardingthePTO,meetingstimes,andrelatedactivities.
ScholarCouncil
TheScholarCouncilprovidesservicetotheschool,increasesschoolspirit,andpromotesbetterrelationshipsandunderstandingbetweenadministration,faculty,andscholars.TheScholarCouncilalsointroducesnewideasforpoliciesandactivitieswiththeschoolanddiscussesproblemsarisingwithintheschoolcommunity.Scholarcouncilmembersrepresenttheirclassesbybringingideas,suggestion,comments,andquestionstotheCouncilandtakinginformation,ideas,andplansbacktotheirclassmates.TheScholarCouncilrepresentsthescholarbodyasawholebypresentingideas,suggestions,andcommentstotheschoolfacultyandadministration.
ChildAbuse/Neglect
PLANetworkschoolpersonnelaremandatedreportersofsuspectedabuseand/orneglect.Mandatedreportersarerequiredtoreportsuspectedchildabuseorneglectimmediatelywhentheyhavereasonablecausetobelievethatachildwhois17yearsofageoryoungerandknowntotheminaprofessionalorofficialcapacityhasbeenharmedorisindangerofbeingharmed(physically,sexually,orthroughneglect)andthatacaregivereithercommittedtheharmorshouldhavetakenstepstopreventthechildfromharm.
Animals
LiveanimalsarepermittedonschoolpropertyonlywithadvancepermissionoftheclassroomteacherandPrincipal.Forreasonsofsafety,animalsmustbecagedandmaynotbetransportedonaschoolbus.Forhealthreasonsofindividualscholarsandteachers,someclassroomsmaynotbeabletohaveanimalvisitors.
FlowersandBalloons
Pleasedonothaveflowersorballoonsdeliveredtoschoolforbirthdaysorspecialholidayswithoutprearrangedapproval.
LostandFound

ScholarswhofindlostarticlesshouldturnthemintothePrincipal’soffice.ScholarswhohavelostitemsshouldcheckwiththePrincipal’sofficetoseeiftheirmissingitemsareinthePrincipal’soffice.
Library/MediaCenter
Thelibraryhasbooksandmagazinesforstudyandrecreationalreading.Itisopenforuseduringtheentireschoolday.Scholarsmustreturnbooksandotheritemscheckedoutfromthemediacenterontime.Ifascholarlosesabook,he/shemustpayforit.Ifabookisdamagedwhilecheckedouttoascholar,he/shemustpayforitsrepairorreplacement.
CellPhonePolicyPLAScholars(AllGradeLevels)Scholarsarepermittedtohavecellphones/headphonesinschool,buttheymustbeturnedoffandplacedinlockerorinpocket(notvisible).Ifascholariscaughtwithphone,wewillfollowthisprocess:
1stOffense:Phonewillbetakenbytheadult,scholarwillbeplacedinISSfortheday,andaparent/guardianMUSTretrievethephone.2ndOffense:Phonewillbetakenbytheadult,scholarwillbesuspendedforoneday,andaparent/guardianMUSTretrievethephone.3rdOffense:Follow2ndOffenseprotocolbutaddasuspensiondayforeachadditionaloffense.Scholarsmayutilizetheircellphonesduringtheirlunchperiod,IFtheyaregrantedspecialpermission.Additionally,ifANYscholarusesaphonetorecordafight,recordotherscholars,orrecordstaffmembers,phonewillbeconfiscated,andscholarwillserveautomaticsuspension.*Filminganythingduringschoolhoursandpostingtosocialmediacouldresultinsuspensionand/orpossibleexpulsion.
Dances
Scholarsarepermittedtoattenddancesunlessnotifiedbyadministrationforbehavior/attendanceissues.Parents,guardians,andfamilymemberssuchassiblingsorfriends,whodonotattendPLAwillnotbepermittedtoattenddances.

C.Transportation
PLANetworkhasavarietyoftransportationoptions.Notalloptionsareavailableateachschool.Checkwithyourschooladministratorregardingwhichoptionsareavailableatyourschool.
ParentalTransportationorWalkingToSchool
Scholarswhoaredroppedofforarewalkingtoschoolshouldarrivebetween7:35-7:45a.m.Scholarsareexpectedtorespectthepropertyofotherswhenwalkingtoandfromschoolkeepinginmindthatschoolrulesapply.
Bicycles/Scooters/RollerBlades/Skateboards/Heelys
Helmetshavebeenshowntoreduceheadinjuries.Therefore,asapartofitsoverallsafetyprogram,PLArequiresscholarswhoridebikesorscooterstowearahelmet.Scholarswhodonothaveonahelmetwhileridingbikesorscootersonschoolpropertymayhavetheirbikeorscooterprivilegessuspendedforaperiodoftimetobedeterminedbytheprincipal.Bicycles/scootersshallbeparkedinthebikerackandlocked.Skateboardsmaynotbebroughtonschoolproperty.Heelysorotherwheeledshoesarenotpermittedonschoolproperty.
SchoolBusConductandSafetyExpectations
TheIndianapolisPublicSchoolCorporation(IPS)providesbustransportationforsomeaffiliatednetworkschools.RidersmustresidewithintheschoolattendanceboundarytorideIPSbuses.AllridersmustcomplywithIPSpoliciesandproceduresforridingtheschoolbus.AnyviolationofIPStransportationpoliciesormisconductthatjeopardizesthesafeoperationoftheschoolbusorthesafetyofthescholarsridingthebusmayresultinthescholarbeingdeniedtheprivilegeofridingtheschoolbus.Whenthishappens,schoolattendanceisstillrequired,andparentsmustmakeotherarrangementsfortheirchildrentogettoschool.BusDisciplinewillfollowthisprocess:Warning,1dayoff,3daysoff,5daysoff,and10daysoff.Wereservetherightatanytimetoremoveascholarforbehavioralconcerns.
FieldTrips
PLAmayusebustransportationforfieldtripsandduringsuchtripsscholarsmustcomplywiththefollowingbussafetyandconductrules:
• Goimmediatelytoassignedseatandremainintheseatfortheentirebustrip. • Remainseatedandfacingforwardwhilethebusisinmotion. • Talkinaquietreasonabletoneofvoice.Loud,boisterous,profanelanguageor

Adult Transport

SchoolClinic
Ascholarwillbepermittedtolaydownintheclinicandaparentwillbecontactedifthechilddoesnotfeelbetterinareasonableamountoftime.
Thegeneralguidelinesforsendingillscholarshomeare:temperatureabove100degrees,vomiting,diarrhea,and/orseriousinjuryorextremepain.Ascholarmustbefever-freewithouttheuseoffever-reducingmedicationfor24hoursinordertoreturntoschool.Ascholarmustalsobefreeofvomitinganddiarrheafor24hoursinordertoreturntoschool.Parentswillbecontactedwheneverthescholar’sinjuryorillnessissuchthatcontinuedattendanceatschoolisnotpossible.Theschoolnursemustreleaseallscholarswholeaveschool,duringschoolhours,duetoillness.
Intheeventthatascholardoesneedtobesenthomeduetoillness,ifaparentcannotbereached,officepersonnelwillcallthepersonslistedasemergencycontactsandrequestthattheypickupthechild.Noscholarwillbereleasedfromschoolwithoutproperparentalpermission.
Parentsareencouragedtokeeptheschoolinformedofthestatusofthescholar’shealthbycommunicatingwiththeschool.Itiscrucialthatanyscholarallergiestomedicationsandvariousfoodsarecommunicatedbytheparenttotheschoolnurse.Foodallergiesrequireadoctors’statementforspecialdietaryneedsthroughthecafeteriaservice.Ifaclinicor

ImmunizationRequirements

InjuryandIllness
AllinjuriesmustbereportedtoateacherorthePrincipal.Ifaminorinjury,thescholarwillbetreatedbytheschoolnurseandreturnedtoclass.Ifadditionalmedicalattentionisrequired,theschoolwillfollowtheemergencyprocedures,andcontactthescholar’sparents.Anaccidentreportwillbecompletedforsignificantheadinjuriesandotherseriousinjuriesthatoccurintheschoolbuilding,ontheschoolgrounds,atpracticesessions,oranyothereventssponsoredbytheschool.
Theseincidentsmustbereportedimmediatelytothepersoninchargefollowedbycommunicationtotheprincipal'sofficefor24-hourfollow-upoftheinjury.Parentswillbenotifiedoftheseinjuries.Acopyofthereportwillbekeptintheschoolclinicfile.Anyscholarreturningtoschoolwithcrutchesmustobtainwrittenpermissionfromaphysicianstatingthatthescholarisallowedtousecrutchesinschool.ThenoteshouldindicatehowlongthescholarwillneedthecrutchesandwhetherheorsheshouldbeexcusedfromPhysicalEducationclass.
Theparentsshouldcontacttheschoolofficeifthechildwillbeattendingfieldtripsorridesthebusregularlyfortemporarydisabilityneeds.Thescholarshouldpresentthenotetotheschoolnurseuponarrivalatschool.Theschoolnursewillissueapasstoleaveclassearlyandforscholarassistanceincludingpermissiontousetheelevator.Thepasswillremainineffectforthedurationoftheuseofthecrutchesand/ormedicaltreatment.Specificguidelinesforuseofequipmentonschoolcampuswillbediscusseduponthescholar’sreturn.Wheelchairsareavailableintheclinicandareonlytobeusedtotransportscholarsorstaffmemberswhoareillorinjured.Intheeventyourchildneedscrutchesorawheelchairtoattendschool,theparentmustmakearrangementswithyourhealthcareprovider.
PrescriptionMedicationPolicy
Ifascholarneedsprescribedmedicationduringtheschoolhours,aparentmaycometoschoolandgivethemedicine.Scholarsarenotallowedtoleavethebuildingtogohometo

takemedicationandthenreturntoschool.Aschooladministratorordesigneewilladminister/dispensesomemedicationwiththeproperforms.
Allmedicinemustbeinitsoriginalcontainer.Therewillbenoexceptionstothispolicy.Scholarsmaynothavemedicationintheirpockets,lunchboxes,lockers,etc.Allmedicationbroughttoschoolmustbeturnedintoaschooladministrator.Thispolicyincludesallfieldtripsandextracurricularactivities.Scholarsmaynotpossessoruseprescriptionmedicationsatschoolindependentlyfromaschooladministrator.Noncompliancemayresultindisciplinaryactionandpossibleexpulsionfromschool.Scholarswithanacuteorchronicdiseaseormedicalconditionareexemptfromthisprohibition.Thesescholarsareallowedtopossessandself-administermedicationonanemergencybasis.Parentsofchildren,whoself-administermedication,mustsubmittheirwrittenauthorization,onanannualbasis.Theauthorizationstatementmustincludeastatementfromthescholar’sphysicianindicatinghisorherapprovalforthescholartoself-administerthemedication.
Over-The-CounterMedicine(OTC)
Aparent/guardianmustsignanOTCmedicationcardtopermitschoolpersonneltoadministerOTCmedicationatschool.Themedicationhastobeprovidedbytheparent/guardianandwillbegivenaccordingtothedirectionsonthebottleandthedosageneedsthatareappropriatefortheageandsizeofthescholar.OTCmedicationmustbeintheoriginalcontainerandonlyonemedicationperbottle.OTCmedicationmustbekeptintheschoolclinic.AnOTCpermissioncardmustberenewedatthestartofeveryschoolyear.ForOTCmedicationssuchascoughdrops,coldmedicine,TylenolorIbuprofen,aparentshouldlistthemedicationontheOTCcardtoindicateascholarmaytakethismedicationatschoolorsendanotewiththemedicationforthenursetoattachtotheOTCcardifithasalreadybeensubmitted.HerbalandHomeopathicmedicationswillnotbeadministeredatschool.
TransportingMedications
Scholarsmaybringmedicationstoschoolonthebusaslongasthescholarturnsthemedicationovertotheschoolnurseorofficeupontheirarrivaltoschool.ScholarswillnotbepermittedtotransportmedicationshomeduringtheschoolyearorattheendoftheschoolyearunlessaMedicationTransportReleaseformhasbeensignedbytheparent/guardianandreturnedtoschool.However,PLAwillnotpermitscholarstotransportcontrolledsubstances,whichincludesnarcoticpainmedication,Sudafed,andsomeADD/ADHDmedications,evenwithaMedicationTransportReleaseformonfile,thesemedicationsmustbetransportedbyaparent.Attheendoftheschoolyear,medicationswillbedisposedofthedayafterthelastdayofscholarattendanceunlesstheauthorizationformhasbeenreceived.
HearingAndVisionScreening

TheStateofIndianamandateshearingandvisionscreeningforstudents.Ifascholardoesnotpassascreening,theexaminerisrequiredtoreferthechildforfurthertesting.HearingscreeningsshouldbeperformedinKindergartenorgrade1standgrades4th,7th,and10th.Thespeechtherapistfortheschoolcorporationperformsthisscreening.VisionscreeningsshouldbeperformedinKindergarten,andgrades1st,3rd,5thand8th.Theschoolnurseperformsthisscreening.Itisimportanttorememberthatthesetestsareonlyscreeningtools.
Pediculosis(HeadLice)
Theschoolnurseordesigneewillberesponsibleforallliceidentification.Allchecksforheadlicewillbedoneinaconfidentialmanner,torespectthescholar’srighttoprivacyandtoavoidembarrassment.Theparent/guardianwillbenotifiedbytelephone,iflicearedetectedorthattherewasexposuretosomeoneinfectedwithlice.Iftheparent/guardianisunavailable,theemergencycontactswillbenotified.Aninformationalchecklistwillbegiventoparents/guardiansofScholarsidentifiedwithliveheadlice.Educationalinformationontreatmentandpreventionwillalsobedistributed.Intheeventofdiscoveryduringafieldtrip,PLAwillprovidetransportationbacktotheschool.Foraspecialneedsscholarwhoistransportedbyschoolbusperthescholar’sIEP,theschoolwillcontinuetoprovidetransportation.
BedBugs
Theschoolnurseordesigneewillberesponsibleforbedbugsidentification.Allchecksforbedbugswillbedoneinaconfidentialmanner.Ifthenurseordesigneesuspectsbedbugbites,thenurseordesigneewillcontacttheparent/guardiantodiscussthepossibility.Ifalivebugisfoundonthescholar,thescholarshouldhaveafreshchangeofclothingtofinishtheschooldayandoriginalclothesshouldbebaggedandsenthome.Ifalivebugisfoundonascholar,bookbag,etc,thebugwillbecollectedanddoublebagged.Itwillbetakenforpositiveidentificationfromanexterminator.
Losttimeintheclassroominterfereswithscholarlearning,thus,classroomcheckswillnotbeconducted.Theassociatesofscholarsattheelementary,middleschoolandhighschoollevelswillbecheckedatthenurse’sdiscretion.AllsiblingsoftheinfestedchildwillbecheckediftheyareenrolledinaPLANetworkschool.Checksfortheinfestedchildandhis/hersibling(s)willbeconductedonMondaymorningforfourweeksafterinitialdiscoverytoaidfamilieswithdetectionandtreatment.

PLAplacesahighpriorityonmaintaininganatmosphereofrespectthatisconducivetolearningandissafeforallscholars,staffandguests.Scholarswhoengageinillegal,dangerousordisruptivebehaviorwillbecalledupontocorrectthatbehavior.Eachscholarisauniqueindividualwithuniquepersonal,social,andeducationalneeds.Asaresult,everydisciplinarysituationbecomesuniqueinnature.Consequencesformisbehaviorprovidethebestlearningvaluewhenmatchedtotheuniquescholaranduniquesituation.Theoddsofchildrenlearningfromtheirmistakesincreasesdramaticallywhenchildrenseeareasonableconnectionbetweentheirbehaviorandtheresultingconsequence.AtPLAweutilizeaBehaviorManagementSystemthatfocusesonthatconnectionandindividualizesdisciplinarymeasures.Becausedisciplineisuniquelytailoredtoeachindividualscholar,pleasecontactyourscholar’sschool.Questionsregardingdisciplineatyourscholar’sschoolshouldbedirectedtothePrincipal.
Shootingdice/gamblingisprohibitedonschoolpremisesandifcaught,itwillresultindisciplinaryactions.
Allreportedincidentsofbullyingorharassmentwillbefullyinvestigatedwithin24hours.
Anyfistfightingwillresultina5-dayto10-daysuspension.Iffightingleadsto10-daysuspension,scholarmaybeexpelledfromschool.
Ifascholardemonstratesanytypeofphysicalaggressiontowardsastaffmemberand/orhasmadephysicalthreatstowardsastaffmember,thescholarwillbeimmediatelydismissed(expelled)fromthenetwork.
Ifascholarissuspendedorexpelled,theycannotattendanyextra-curricularevents(practices,games,dances,etc.).Scholarmustrefrainfromenteringschoolpropertyandiftheydoenterschoolproperty,lawenforcementwillbecontactedduetotrespassing.
Afterascholarreaches10totaldaysoutofschoolonsuspensiontheywillbeplacedupforexpulsion.Parentswillreceiveofficialpaperworkandbeaskedtoattendanexpulsionmeeting.
Anyscholarwhoisarrestedorinvolvedinunlawfulactivitycouldbeplacedupforexpulsioniftheirpresencejeopardizesthesafetyorlearningofotherscholarsincludingpossessionofcigarettes,vapes,drugs,alcohol,and/orweaponsofanysort.
Inadditiontothegroundsspecifiedinthishandbook,ascholarmaybesuspendedorexpelledforengaginginunlawfulactivityonoroffschoolgroundsif:
• the unlawful activity may reasonably be considered to be an interference with school purposes or an educational function; or
• the scholar’s removal is necessary to restore order to protect persons on school property;

including an unlawful activity during weekends, holidays, other school breaks, and the summer period when a scholar may not be attending classes other school functions.
**Any scholar in possession of an illegal substance will result in a suspension or expulsion.
**If a scholar comes to school under the influence of a controlled substance or narcotic, they will be sent home immediately. This could result in suspension or immediate dismissal.
** If a scholar is expelled or withdraws from the academy due to behavior reasons, the scholar MUST conference with their parent/guardian as well as building leadership to discuss return and expectations moving forward.
Social Media Expectations
In regards to social media, we still hold scholars to a high standard. Any unlawful activity, violent threats, bullying, or blatantly inappropriate behavior will be reprimanded and follow the student discipline code of conduct. Posting inappropriate videos, fighting, or exploiting others will result in disciplinary action (depending on severity, it may be a suspension or expulsion).

Therighttoinspectandreviewthestudent’seducationrecordwithin45daysofthedaytheSchoolCorporationreceivesarequestforaccess.Parentsoreligiblestudentsshouldsubmittotheschoolprincipal[orappropriateschoolofficial]awrittenrequestthatidentifiestherecord(s)theywishtoinspect.Theprincipalwillmakearrangementsforaccessandnotifytheparentoreligiblestudentofthetimeandplacewheretherecordsmaybeinspected.
Therighttorequesttheamendmentofthestudent’seducationrecordsthattheparentoreligiblestudentbelievesareinaccurateormisleading.ParentsoreligiblestudentsmayasktheSchoolCorporationtoamendarecordthatisbelievedtobeinaccurateormisleading.Parentsshouldwritetheschoolprincipal,clearlyidentifythepartoftherecordforwhichanamendmentisrequested,andspecifywhyitisinaccurateormisleading.IftheSchoolCorporationdecidesnottoamendtherecordasrequestedbytheparentoreligiblestudent,theSchoolCorporationwillnotifytheparentoreligiblestudentofthedecisionandadvisethemoftheirrighttoahearingregardingtherequestforamendment.Additionalinformationregardingthehearingprocedureswillbeprovidedtotheparentoreligiblestudentwhennotifiedoftherighttoahearing,
Therighttoconsenttodisclosuresofpersonallyidentifiableinformationcontainedinthestudent'seducationrecords,excepttotheextentthatFERPAauthorizesdisclosurewithoutconsent.
Oneexceptionthatpermitsdisclosurewithoutconsentisdisclosuretoschoolofficialswithlegitimateeducationalinterest.AschoolofficialisapersonemployedbytheSchoolCorporationasanadministrator,supervisor,instructor,orsupportstaffmember(includinghealthormedicalstaffandlawenforcementunitpersonnel);apersonservingontheSchoolBoard;apersonorcompanywithwhomtheSchoolCorporationhascontractedtoperformaspecialtask(suchasanattorney,auditor,medicalconsultant,ortherapist);oraparentorstudentservingonanofficialcommittee,suchasadisciplinaryorgrievancecommittee,orassistinganotherschoolofficialinperforminghisorhertasks.
Aschoolofficialhasalegitimateeducationalinterestiftheofficialneedstoreviewaneducationrecordinordertofulfillhisorherprofessionalresponsibility.
Uponrequest,theSchoolCorporationdiscloseseducationalrecordswithoutconsenttoofficialsofanotherschoolcorporationinwhichastudentseeksorintendsto

Wheredisclosureistoastateorlocaljuvenilejusticeagencyandrelatestotheabilityofsuchagencytoservebeforeadjudicationthestudentwhoserecordsarebeingreleasedandsuchagencyreceivingtheinformationcertifiesinwritingthattheagencyhasagreednottodiscloseittoathirdpartywithouttheconsentofthestudent’sparent,guardian,orcustodian.Suchinformationmaynotbeusedtoaidinthesupervisionofadelinquentchild.

SchoolVisitorPolicy
Purpose
ToaidPLAinmaintainingasafeandacademicallyeffectiveschoolenvironmentforscholars,staff,andthepublic,PLAhasestablisheduniformguidelinesforgrantingandrestrictingaccesstoschoolpropertyandschoolevents.
Scope
Thispolicyappliestoallvisitors,includingparents,maintenanceworkers,serviceproviders,reporters,etc.Emergencyservicepersonnelandlawenforcementpersonnelonthepremisesforanemergencyareexemptfromthispolicy.
Guidelines-General
PLAstrivestocreateanatmosphereofcamaraderieandcooperationbetweenschoolpersonnel,parentsanditscommunity.ThisgoalisbalancedagainstPLA’scommitmenttocreateanacademicenvironmentthatdoesnothinderascholar’seducationalopportunities.TothisendPLAhasimplementedthefollowingstepsforallvisitorstotheschool.
Step1.Allvisitorsmustreporttotheadministrativeoffice.Heorshemustsigninandstatethepurposeforhisorhervisit.Permissionwillbegrantedforentrytotheschoolunlessthereisalegalprohibitionagainstthevisitor’spresenceattheschool,thevisitordoesnothavealegitimatepurposeforbeingattheschool,orthevisitorcausesadisruptiontotheacademicprocess.
Step2.Thereceptionistwillgivethevisitoravisitor’sbadge,whichthevisitormustdisplayonhis/herclothing.Thebadgemustbevisibleatalltimes.Avisitorwithoutabadgewillbeaskedtoobtainapassortodisplaytheirpassinavisiblelocation.Ifthevisitorrefusestoobtainordisplaytheirpass,heorshewillbeaskedtoleavetheschool.
Step3.ThePLABoardhasdelegateditsauthoritytograntordenyentrytotheschooltothePrincipal.ThePrincipaloftheschoolisresponsibleforensuringthatscholarandschoolsafetyarenotcompromisedbythepresenceofavisitorintheschoolbuildingoronitspremises.Permission,oncegranted,mayberescindedifavisitordoesnotcomplywiththestatedpurposeofthevisitorviolatesanyschoolpolicy,administrativeregulation,andlawordisruptstheschoolenvironment.
Step4.Ifthebehaviorofavisitorisdisruptivetotheschooland/orunderminesthelearningenvironment,preventstheorderlyconductoftheactivities,administration,orclassesoftheschool,involvesillegalactivity,orposesarisktothesafetyofstudentsorstaff,thePrincipalwillinstructthevisitortorefrainfromthedisruptivebehavior.Ifthebehaviorcontinuesthevisitorwillbeaskedtoleave.Ifthevisitorrefusestoleaveafterrequestedtodoso,thestaffwillcallthepolicetoremovethevisitorfromthepremises.

Step5.ThePrincipalmusttellthevisitorthereasonforhisorherdecisiontodenythevisitorentry,orthereasonforthevisitor’sremovalfromtheschool.ThePrincipal’sdecisiontodenyorrestrictentryofavisitormaybeappealedtoMichelleReeves,DirectorofLegalServicesandHumanResources.Shemaybereachedatmreeves@phalenacademies.org.
Guidelines-ClassroomObservations
Thepresenceofadultobserversintheclassroomcansometimesdistractthescholarsanddisrupttheoverallclassroomprogram.Tolessenthiseffect,PLAhaslimitedtheobservationtimetoonehourperweek.Ifadditionalobservationtimeisneeded,thevisitormaycontactthePrincipalforapproval.Anyextensionsmustbeforthebenefitofthescholar,andmustnotunderminetheacademicenvironmentofotherscholarsintheclassroom.Step1.AllvisitorswishingtoobserveaclassroommustcontactthePrincipal,inadvanceofthevisit,toscheduleatime.Tominimizeclassroomdisruptions,morningvisitsshouldbescheduledduringthehoursof8:30a.m.-9:00a.m.10:30a.m.-11:00a.m.andafternoonvisitsshouldbescheduledduringthehoursof1:00p.m.-3:00p.m.ThePrincipalmayrescheduleordenytheobservationrequestduetoschooltestingorassessments.Step2.Uponarrivalforaclassroomobservation,thevisitormustsigninatthefrontdesk.Visitorsshouldarriveattheclassroomatthestartoftheirscheduledobservationtime.Step3.Allvisitorsmusttakeaseatintherearareaoftheclassroom,orinanareaoutofthedirectlineofsightofthescholars.Visitorsmaynotbringotherchildren,relativesorfriendswiththemfortheclassroomvisit.Step4.Visitorsmaynotengageinanybehaviorthatdetractsfromthelearningenvironment(e.g.talkingonthecellularphone,playinggames,callingorengagingtheirchildinotheractivitiesduringclass).

DRESS CODE CONSEQUENCES
Scholars are required to be in the JRPLA dress code and comply with all dress code expectations on a daily basis. If a scholar is not in compliance with expectations, administration will follow this process:
1. Scholar will be given a verbal warning. 2. Scholar will receive one day in ISS and parent/guardian will be notified. 3. Scholar will receive a one-day suspension and parent/guardian will be notified. 4. Follow step 3 as multiple times as necessary.
