What is the EAP?
• EAP is a confidential counseling and problem-solving resource designed to help you identify personal issues and work to resolve their difficulties.

• If you contact Performance Plus, no one will know about it unless you tell them.
• Information will not be released to anyone unless you give written authorization, by court order, or as required by law.
• Participation in the EAP will not jeopardize job security or future promotional opportunities.

What Does It Cost?

Who Is Eligible?
• EAP services are available for employees and their dependent family members.
• Adult children who live with you are covered until age 21.

Our EAP Counselors
Performance Plus Counselors have a minimum of five years counseling experience following state licensure in their areas of specialty. These specialties include:– Social Workers– Licensed Professional Counselors– Psychologists– Marriage and Family Therapists

Counseling Sessions
• Sessions last approximately one hour.
• The counselor will work with you on your specific areas of concern.
• A plan of action will be outlined.
