t was with great regret that recently I had to inform our organization about the passing of Dr. Lanny Goluboff on June 27th.

This is a big loss for Humber River Hospital. We were very fortunate to have Dr. Goluboff as a part of our family for so long. He was a leader and a mentor, dedicated to his patients and our community. The only people to whom he was more devoted to were his wife Shelley, his children and grandchildren.

I first met Lanny when I was a fourth year medical student doing an elective rotation at Northwestern General Hospital. By that time Lanny had been at Northwestern since January of 1972. He’d graduated from the University of Saskatchewan in 1966, and joined the Northwestern medical staff immediately after completing his senior residency at Toronto General Hospital and a fellowship at the University of Michigan Medical Centre.

An Internist with a sub-specialty in Endocrinology and Nuclear Medicine, he was determined to make a difference. As the years passed he did just that, making a positive impact on the lives of

countless patients, while earning the esteem of staff and medical colleagues, Boards of Directors and senior hospital administrators.

This continued to be the case throughout his entire career, resulting in a succession of leadership roles. These included President of the Medical Staff Association, Chief of Department, Chief of Medicine and ultimately Chief of Staff at Northwestern from 1989 until the merger in 1997. At that time, Dr. Lanny Goluboff became the first Chief of Staff at the new Humber River Hospital.

I enjoyed working with him; he was always professional and was able to make everyone around him feel at ease no matter the situation. He took pride in everything he did and always put his patients first. In fact, one time while undergoing medical treatment, Lanny unhooked himself from the machines so he could read patient scans!

Lanny had the unique ability to be pragmatic and visionary at the same time, a skill that served him well as a leader of staff and physicians. A little known fact about Lanny is that in the late 1980s, as Chief of Staff at Northwestern, he

first proposed the consolidation of local hospital services into a new, acute care hospital facility. I am happy that he knew his dream would come true, and saw the construction for himself.

Our hospital is built on our past. We have had the fortune of having great builders. Now as we move forward to a new chapter in our history, we continue to keep their vision alive through our work to provide excellent, expert, respectful care for our community. We have had role models like Dr. Lanny Goluboff to guide us on that path.

Lanny, thank you for all that you did for our patients and our hospital. We will miss you.

Leadership and Organizational Development Consortium

Congratulations to the Humber River directors, managers and supervisors who completed the Leadership Development program in May after a year of extensive training. A total of 15 HRH staff participated in the program.

The program was created through the Leadership and Organizational Development Consortium (LODC), a group of eight hospitals that partnered to find a pan-hospital approach to leadership and organizational development.

“By using new approaches to leadership, we saw a definite improvement in patient satisfaction scores over the year. Patient satisfaction means staff are more satisfied because they are happier at work,” says Diana Avgerinos, Manager, Finch Emergency Department.

The LODC worked with Hay Group to focus on strengths and gaps in the healthcare system. The program leverages the Ontario Hospital Association’s leadership competencies along with proven leadership concepts for effecting change and enhancing personal and organizational performance.

“I came to understand my leadership style and was able to hone my skills while finding areas to improve, which helped my team be more successful,” notes Dina Longo, Director, Diagnostic Imaging.

A key aspect of the program is the ‘cohort’ approach whereby participants progress through the program with a group of their peers from participating hospitals. This approach creates a shared learning experience and broadens peer networks, driving collaboration across hospitals.

“It was invaluable to work with peers at other hospitals, coaching each other on challenges we face and sharing experiences both during and after the program,” explains Helen Mazurkewich, Supervisor, Records Management.

The LODC was initiated by Humber River to embrace leading practices, share expertise and resources and build leadership capacity in the most impactful, efficient and cost-effective manner.

“The program taught simple, small, practical changes that made a significant shift in the performance of my team,” says Darlene Ginsberg, Manager, Mental Health.

The LODC is a great example of how hospitals can work together innovatively, creatively and efficiently to achieve great results. Since the launch of this program, there have been 162 graduates across the member hospitals, including 43 from HRH.

Humber River’s Infection Prevention and Control (IPAC) team has been instrumental in working with program areas to implement innovative and effective strategies to improve patient safety.

Joan Osbourne-Townsend and Comella Levers from our IPAC team recently showcased these strategies and the resulting successes at IPAC Canada’s National Conference in Nova Scotia and at the American Association for Professionals in Infection Control’s Annual Conference in California.

Their presentations highlighted new procedures, communications tools, and employee engagement and recognition programs that have reduced Healthcare-Associated Infections (HAI)

and increased Hand Hygiene Adherence (HHA) rates across the hospital.

According to Joan, “staff recognition is important to the success of a program. Teams that achieved greater than 85% compliance for Hand Hygiene were recognized by our President and CEO, Rueben Devlin, resulting in 100% of our units meeting this target.”

The success of these strategies can be seen across the hospital: HAI rates in the Finch ICU reached zero due to the team’s commitment to patient safety. HHA rates in the Emergency Department (ED) drastically increased thanks to the local leadership’s implementation of ED-specific education. Compliance with the mobile

equipment disinfection and ‘Bag & Tag’ programs in all medical units resulted in a 50% decrease in MRSA colonization. Joan and Comella also profiled HRH’s strategy to engage family and visitors in patient safety by providing education about Additional Precautions and personal protective equipment to Infection Prevention professionals from across Canada and around the world. Comella recognizes that “safety is enhanced when everyone works together to put patients first.”

We are proud of the success of all HRH teams in implementing these innovative strategies across our hospital and their commitment to patient and family-centred care.

Simplified Sign-On allows physicians and staff to use their ID badge to quickly and easily access patient files on computers throughout the hospital and on integrated bedside terminals in patient rooms.

So how does it work? Simply tap your ID badge against a badge reader and the system does an initial logon, then type in your password and you are logged into the Meditech system within 5 seconds. As you leave a patient room, simply tap the badge reader again to logout and the system will return to the patient information screen. For security and privacy, the system will automatically log you out after a defined period of time or inactivity.

scans barcodes on medicine and patient ID wristbands.

Humber River has begun the next phase in the implementation of our Electronic Health Record (EHR) with the launch of our new medication verification and administration system.

In mid-June, after extensive training and planning, our new Barcode Medication Verification (BMV) and electronic Medication Administration Record (eMAR) systems went live at the Finch site.

With BMV, barcodes on both the medication and the patient ID wristbands must be scanned into the system at the bedside before the medication is administered to the patient. The patient’s eMAR, which forms part of their EHR, is automatically updated, available for immediate access by all members of the patient’s care team.

BMV and eMAR enhance both quality of care and patient safety in a

number of ways. It helps reduce the number of medication errors through positive patient identification and real time documentation. The system provides immediate access to comprehensive medicine profiles, patient allergy issues and incompatible medication alerts. The BMV/eMAR system will also be integrated with the Meditech infrastructure to automate auditing processes.

This system takes us one step closer to our goal of a complete EHR and a Computerized Provider Order Entry (CPOE) system that will fully integrate patient medication administration with clinical decision making and care team and pharmacy workflows.

BMV and eMAR implementation is scheduled for the Church and Keele sites this Fall.

Key ComponentsOperational Readiness – The Cross Program Integration Team is working to ensure all our operational and clinical activities are aligned with our patient-centred care model and our vision of ‘lean, green and digital’. As well, these activities need to be integrated with Information, Communication and Technology (ICAT) and Managed Equipment Services (MES) requirements and follow our Service Delivery Model guidelines.

Program areas are working together to integrate their services to deliver a comprehensive patient care model and to adjust program plans to incorporate the enhanced capabilities of our new facility. Patient needs often cross various program areas and by working together we can provide enhanced quality, patient safety and patient-centred care.

One of the most important components of this move is training. Nurses and staff are already participating in extensive training to ensure patient safety as we introduce new technologies and processes.

Wilson Site Facility Activation (FA) – To ensure a smooth transition from a construction project to a fully operational hospital, a comprehensive strategy is being developed. The strategy will incorporate all planning elements and support operational start-up procedures, task execution, and orientation and training requirements. It will also integrate operations at the legacy hospitals, the Move Plan and Sequence and the Patient Transfer Plan.

There are seven planning elements for the FA including: Security & Access Plan, Space Assignments for beds, new equipment and I.T. hardware, Environmental Services, Materials Management, I.T. Hardware Deployment Plan, Hospital Commissioning and Operational Performance Testing, and a Facility Activation Operation Centre.

The FA Operation Centre will follow an ‘Incident Command Centre’ model and will include the development of Wilson Site FA Guidelines. The Guidelines will establish procedures to support HRH staff, physicians and volunteers during the activation period from May 11th to October 18th, 2015.

Move Plan and Sequence for each Legacy Hospital - The Health Care Relocations (HCR) team is our partner for the move to our new hospital. Together, we are committed to ensuring a smooth relocation of our patients, physicians, staff and designated hospital contents to the new hospital. Using the guiding principles of maintaining patient-centred care, minimizing downtime and mitigating risk, they focus on move coordination between all departments. They are also creating strategies to troubleshoot challenges while maintaining urgent and emergent services throughout the entire move period.

HCR is working with all departments to create a Move Calendar and Sequence Plan detailing the phases in which departments will move. They are also creating a Service Level Reduction Plan for each legacy site which will align with the Move Calendar to maintain safe patient care throughout the move. Departmental Move plans are being developed including ‘do not move’ and ‘move and install’ lists for equipment. To help everyone prepare for the move, HCR has begun developing the HRH Move Guide.

Patient Transfer Plan – Three key teams will be established to maintain legacy site urgent and emergent services on moving day including an Incident Command Centre Team, a Patient Transport Team, and a Cleaning Station and Clean Team.

Child and Adolescent Mental Health Clinical Day

Humber River’s Child & Adolescent Mental Health Program hosted the 6th Annual Clinical Day in May as a contribution to our community, for professional partners and caregivers. The program included presentations by national and international speakers who reviewed the latest developments in the management of children’s mental health challenges.

The event brought over 100 mental health professionals from across Ontario to gain new and practical knowledge in providing enhanced levels of care in children’s mental health. Participants included social, child and youth workers, teachers, psychologists, psychiatrists, and family physicians.

From l-r: Dr. Rajeevan Rasasingham, University of Toronto; Dr. Katharina

Manassis, The Hospital for Sick Children; Dr. Daniel Gorman, Hospital for Sick

Children; Dr. Antonio Pignatiello, Hospital for Sick Children; Dr. Maged Kodsi,

HRH; Dr. Ivan Perusco, HRH; Dr. Sonu Gaind, HRH; Paula Villafana, HRH.

RBC pledges $1.25 million to the

Child & Adolescent Mental Health program

The Child & Adolescent Mental Health Inpatient Unit at our new hospital will be named in recognition of RBC Foundation’s generous gift of $1.25 million. RBC’s support enables us to care for more patients and further enhance the great care we provide by giving patients and their families a therapeutic environment, which includes single patient rooms, program rooms with lots of natural light and an outdoor rooftop garden patio.

"We understand the toll mental illness can take on children and families, along with our communities. That's why we're proud to support Humber River Hospital as they help individuals in need gain the confidence and connections needed to live happy healthy lives,” says Susan Stauth, Regional Vice President, Toronto West Market, RBC.

“We are extremely grateful for RBC’s generous investment in the new Humber

River Hospital and for supporting our country’s future – children and adolescents.

We thank RBC for their leadership, and for their support of Humber River’s new

model of patient-centred care.”

Scholarship Awards 2014 1,000 Hats for Newborns

Baby Emma is the 1,000th baby to receive a newborn hat hand-made by Betty Kennedy. For 12 years, Betty has been knitting hats for newborns at Humber River as she “thinks about the little babies like Emma who will be wearing the hats.”

Humber River holds a special place in Betty’s heart because three of her children were born at the Church site, now at the age of 91, she is happy to volunteer her time doing something she loves, knitting.

We would like to thank Betty for her hours of knitting these cute little hats!

Thank you to the Scholarship Award Committee including HRH staff, Bob Parke and Rakesh Shukla and Association Volunteers, Ron Boise, Ann Coon, Kristina Ioudine, Pauline Kishimoto and Maria Percewicz for their time in reviewing all the applications.

Congratulations to the recipients of Humber River Hospital Volunteer Association’s Scholarships. Students were selected based on their volunteer activities, extra-curricular activities at school and their commitment to community programs. The Staff Student recipient Amanda Balmer is the daughter of Sandra Balmer in Human Resources. Amanda will attend York University’s Faculty of Arts this September.

The Volunteer Student Award recipients are Eniola Alola, who

Erane has raised a total of $235,000 which has helped purchase many pieces of equipment such as: ECG machines, ventilators, defibrillators and Isolettes, and supported renovations to various clinics throughout the hospital.

Thank you to the thousands of people for their donations and Erane for sorting and counting all that change!

Twenty years ago Erane Bocking started collecting change to raise funds for new equipment at Humber River. Instead of using simple tin cans to collect spare change, she happened upon an old Isolette that wasn’t being used so she dressed up a baby doll to put inside and has decorated it with each passing season and holiday celebration.

There are now two fundraising Isolettes at Finch and one at Church and twenty years later,

Family Campaign Celebration

Temple Sinai Golf Tournament

Humber staff, physicians and volunteers who supported the new hospital campaign were honoured by the Foundation at the Family Campaign Donor Appreciation Event on June 26th. The event recognized the generosity of our Humber family, who together successfully raised $5.5 million.

Guests included Hospital and Foundation Board Members, Campaign Cabinet Members, members of the Volunteer Association, and Humber staff and physicians.

Special thanks to the members of the Family Campaign Advisory Committee, led by co-chairs Mary Latter and Louisa Ceci, for their time and effort with our internal fundraising.

Our success would not have been possible without our Humber family ambassadors, whose enthusiasm and tremendous support helped engage all members of the Humber River family. We are greatful for their commitment.

The first joint golf tournament with Temple Sinai and the Humber River Hospital Foundation was held at Sleepy Hollow Golf Course on June 23rd. Over 80 golfers and 30 dinner guests participated in live and silent auctions, raising funds to support the youth programs at Temple Sinai and the new Humber River Hospital.

During the evening portion of the event, Rueben Devlin, Humber River’s President and CEO, spoke about the importance of these community partnerships and thanked the Temple for all their support. The Foundation looks forward to a continued successful partnership with the Temple Sinai Congregation.
