Member Value Proposition

Participants are reimbursed $20 every month in which they visit a participating fitness center at least 12 times

Members can choose from an expanding number of popular fitness chains, with approximately 10,000 fitness centers and Ys currently participating nationwide

Participating in Fitness Reimbursement can not only help members look and feel better, it actually pays them for visiting a fitness center.

Members select the state in which their employer is headquartered (the state from which their UnitedHealthcare benefit plan has been issued). Members may live across all 50 states.

After browsing the list of participating fitness facilities, members can click the link at the bottom to print a referral flyer that they can take to their fitness center. Note: This link is also found at the Fitness reimbursement website.

At the Fitness Reimbursement home page, users begin by searching for a participating fitness center, using either ZIP code or city/state.

They can also view an informational video.

Depending on the facility selected, members may:

• Enroll at the facility by printing an Activation Letter (at right) to bring to the facility OR….

– the Fitness ID provided by the facility

– banking information for the account that where the $20 reimbursement will be deposited

Once you have successfully enrolled, the next time you log into the Fitness Reimbursement site you will have access to your account information:

– Update your personal information

– View your current facility, change to a new facility

– View your account activity


United Healthcare Wellness Fitness Reimbursement: Reimbursement Process

Once successfully enrolled, members swipe their key card or fob at each visit to their facility.

Enrolled members who visit their selected fitness center 12 times or more per month are reimbursed $20 for that month. The $20 is applied as a credit to the member’s

bank account of record

Members do not need to submit any forms or take any additional action to get reimbursed

Reimbursements are credited at the beginning of the 2nd month after the month of activity (a 1 month lag). For example, members who visit a participating gym at least 12 times in January will receive the credit around March 1.



Facility Referral Members who did not find their fitness center can click on the banner (below) and print a referral flyer (at right).

Members give this flyer to the manager/owner of their fitness center or Y, who will contact the Fitness Reimbursement recruitment team to discuss participation.
