He is also a Tenured Associate Professor of Management in the H-E-B School of Business & Administration at the University of Incarnate Word in San Antonio, Texas, USA. So far, he has presented on three continents about the business of medical tourism and has also produced several articles, papers, and is currently writing a book on the economic issues and impact of medical tourism. He is also a consultant, author, speaker, and futurist with a focus on topics such as healthcare, technology, and training. Author of 7 books on travel related medicine and many scientific research articles. Editor in Chief British Global and Travel Health Association Journal and Travelwise. Lecturer on travel medicine and elderly care at Stirling and Glasgow Universities. Extensive world travel and medical work especially in developing countries.

Laszló PúczkóManaging Director, Xellum Kft., Hungary‘Therapy or surgery? The evidence based treatment options in EU cross-border healthcare’Laszlo Puczko is an economist (MSc), art & design manager (MA) and PhD (in tourism). Laszlo has been involved in wellness, thermal and medical tourism for over 15 years. He is the co-author of the first global overview of Health and Wellness Tourism. He is the Managing director of Xellum Ltd, a consulting and research firm. Global medical tourism is characterized by invasive treatments from plastic surgery, to dental interventions. Many countries, however, have assets and traditions that can, in many cases prevent or postpone invasive treatments. This is a great, but so far mainly missed opportunity for many destinations and service providers. Laszlo has been involved in wellness, thermal and medical tourism for over 15 years working for private companies and public sector organizations. Among others he advised the Hungarian National Tourist Office on health tourism marketing, the Ministry of Regional Development on the Tourism-specific Quality of Life Index preparation, prepared a Regional Health Tourism Development Strategy for Southern-Transdanubia, and now is advising the Hotel Association of Hungary on the service criteria for wellness and medical hotels. He has also worked for many private developers. His expertise was used from conceptualization to feasibility assessment of wellness tourism projects. In his latest project he is involved in the Global Spa Summit’s initiative on wellness tourism and spas. He was Board Member of Travel & Tourism Research Association Europe Chapter (2008-2010), and has been the President of Association of Tourism Consultants (Hungary) since 2006, Senior Professor at Budapest Corvinus University, and guest lecturer in the first European MA programme in Wellness and Spa Service Design and Management (Estonia).

Among the healer’s assets are honorary awards from the Assembly of Business Circles ‘Saint Sophia’, the Order of Saint Vladimir Ukrainian Orthodox Church and other regalia. In 2013 she was awarded the highest distinction of EBA - the International Socrates Prize for contribution to intellectual development of Modern Society. The office also distributes the shares of compulsory insurance of Jordanian vehicles tenders among insurance companies and settles down their accidents as well as settling down the accidents’ claims of Arabian vehicles insured with Orange Cards and Jordanian vehicles insured at unified insurance offices in the Arab countries. Joif has launched in 2013 his initiative The new Electronic Traffic Accidents Reporting System (ETARS) in Jordan in cooperation with Public Security Directorate which aims to providing data and photos for insurance companies to settle motor claims and stemming fraud in this branch. This step came after the success of issuing TPL policies online on behalf of insurance companies since 2005

through NEMIS ( Nationwide Electronic Motor Insurance System) which made clear difference in the insurance industry in Jordan in terms of speed, accurateness and online keeping companies updated with the new issued policies up to date. His adventure with sport, more important than learning at that time, began already in primary school. At the beginning of the second grade of high school, started his first serious achievements in cross-country and middle-distance runs. This is how his adventure with feats in sport began. In high school, he practiced middle-distance runs and won numerous champion titles of the province. In 1982, being at the top of his powers and certain to win the gold medal, he did not pass through eliminations during the Indoor Championship in Athletics of Poland in Zabrze.At that time, he understood that sport should be an extra activity rather than life purpose. It is sport that taught him to look for his opportunity over and over and forget about shortcomings. It is sport that shaped his spirit showing him the taste of victory and that it is worth every effort to feel it as often as possible. As intensive as his trainings were, as intensively he started to study, which was reflected in good results of his school leaving exams and entrance exams. As a result, in the same year, 1982, he began medical studies at Medical Academy in Białystok simultaneously continuing to practice running. Throughout his studies, he represented the academy during running competitions in Poland and abroad. In 1988, he graduated with high average grade and underwent internship in the Regional Hospital in Maków Mazowiecki, and from 1990 to 1998 he worked in the department of internal diseases in the Regional Hospital in Pułtusk where he acquired further specializations.In 1992, he passed a first level specialization in internal diseases exam with an excellent result. A year later, he defended his PhD dissertation titled ‘Activity of granulocytic alpha- amylase in certain inflammations’. PhD conferment procedure was conducted in the Medical Academy in Białystok. In the following year, after the Health Minister’s consent to shorten his internship, he passed a second level specialization in internal diseases exam with a very good grade. Shortening of the internship was issued by his specialization supervisor due to dr. Gajda’s substantial scientific and professional activity along with his good grades obtained in partial internships. In 1995, he commenced sub-specialization in cardiology and completed it two years later. In 1997, he opened his cardiologic clinic, which he runs presently. In 1998 he completed specialization of family medicine and in 2008 specialization of sports medicine, which is his fourth second level specialization. In 1998, he decided to provide a more stable future for his family and created the clinic Medical Center ‘Gajda-Med’. During the last years one of tourist branches caused by the process of globalization that gives the possibility to combine medical treatment and relaxing abroad became the branch called medical tourism. During their trips patients can receive professional medical help at different medical centers.What about investments in such branch of tourism they are constantly growing. The global market of medical service has already been created and now it is permanently developing.Creation of medical tourism and its development is being influenced with the infrastructure and a number of other factors of social and economic origin where the main point is the existence of the infrastructure of the world standard and highly qualified staff. At the same time creation of the infrastructure of the world standard as well as highly qualified staff demands investments. That is why it is logically explained that at first medical tourism appeared in highly developed countries. These countries are Germany, Switzerland, France and Austria. In the USA, Canada and Israel medicine has always been developed and continues to be on its highest level.Some Asian countries (Malaysia, Thailand, Singapore, Hong-Kong, Turkey, India, Southern Korea), some countries of Eastern Europe(Poland, Czech Republic,

Hungary), some ones of Southern and Central America (Costa-Rica, Mexico, Brazil, Cuba), Near East (United Arabic Emirates) are constantly approaching to the level of leading countries.Not high social and economic standards of the population of Tajikistan, their low purchasing capacities are one of the reasons why medical tourism in Tajik republic is only at its point of start.Meanwhile during the last several years some medical centers with the latest equipment (Avicenna, Shifo) where highly qualified specialists who rose their qualification in some leading clinics of Russia, Germany, Japan, Israel, Southern Korea Iran work have already been created. In cooperation with specialists of upper mentioned centers different complicated operations are being held. We should also mention that unfortunately because of low purchasing capacity a numerous number of local population can not afford themselves to be treated at these centers.What about the entrance tourism it is poorly developed. Such situation can be explained that both private and state medical establishments are not known abroad and do not play any significant role in earning currency.In contrast to other Asiatic republics of the former USSR Tajikistan possesses huge recreational recourses where the most important of them are waters, mud lakes and natural climatic conditions.In the 30s of the 20 century famous balneologists of the USSR investigated mineral springs such as Khoja-obi-Garm, Obi-Garm, Garm Chashma, Khavotag, Shambary and 50 other springs in various natural climatic zones of the republic

especially in the Pamir region and nowadays some holiday resorts work there.Highly mountainous resort Khoja-obi-Garm is located at the height of 1087 m.and has been working since 1939 being the largest balneological resort of the republic.(more then 500 beds).Siliceous, high termal sulfate-bicarbonate-sodium waters with total mineralization of 0,5-0,6 g are about 93 C.One of the main peculiarities of the resort is a radon steam and extra hot mineral waters.Balneological resort Shambary is located 28 km, west to the capital of Dushanbe at the height of 1003-1150 m.above the sea level. The water of Shambary is sulphate-chloride-sodium with mineralization of 2,9 - 9,4 and sometimes up to 17,4 g. with the temperature of 22,5 – 38 C.Thermal waters of Shambary are widely used for treatment of the digestive system, functional disorder of the nervous system as well as gynecological one. Balneological resort Obi-Garm is 100 km from Dushanbe at the height of 1200-1800 m.

The water there is chloride-sulphate-calcium-sodium with mineralization of 0,5-17 g and of 40-50C.It is used for treatment of such diseases as digestive system diseases, skin ones, of locomotors system ones and for drinking. Balneological resort Garm-Cashma is located in Pamir at the height of 2770m, 50 km from the centre of the autonomous region Khoroga. The water here is hydro-carbonate-sodium with mineralization of 3,3-3,5 g and of temperature from 36 to 68 C. Undeground waters contain 230-245 silicic acid and active microelements-iron, magnesium, strontium and others which give the water its balneological power.Combination of clarity and ionization of air, maximum amount of sunny days all year round and great concentration of ultraviolet rays effectively treats diseases of digestive system, of nervous one, of locomotors system and gynecological one too. In the north of Tajikistan there are balneological sanatoriums called Havatag, Zumrad and the unique climatic resort Istavtashan.Sanatorium Havatag is located 90 km from the town of Hojent at the height of 600m. The spring Havatag is of chloride-sodium type with mineralization of 4,4g. Weakly alkaline, silicic water is of temperature from 27 to 41 C.Besides its main elements the water contains bromine, iodine, iron, molybdenum, fluorine, lead, zine and others which make this water valuable for many groups of diseases.Sanatorium Zumrad is located at the height of 900 m, 3 km from the town of Isphara and is the one for 500 beds.Cardiological diseases, the ones of locomotors system, of periferic nervous system, metabolism are treated in it.The only in Tajikistan climatic resort Istavrashan for treatment of cardiological diseases is located 6 km from the town of Istavrashan and is 84 km from Khujand at the height of 1004 m.The resort is located on the northern slopes of Turkistan mountain range in favorable temperature and climatic conditions. Attractive nature, comfortable temperature, clean air, long amount of sunny days are good for heliotherapy, aero therapy on this resort where the main methods of treatment are good for cardiology.So, our brief review of modern medical tourism shows that despite a large amount of hot and cold springs, muds ,climatic resources, beautiful nature, comfortable temperature, clean air this branch of tourism only begins to develop.Dozens of places with hot and cold springs make a potential for creation and development of medical tourism and recreation. Unfortunately modern level of social and economic development of Tajikistan can not allow using completely all its recourses and needs investitions.In times of globalization foreign investors together with local ones could do this.

Professor Shabalin started his career in 1962 as a doctor of Omsukchansky regional hospital of the Magadan Region. From 1965 to 1975, he held various scientific posts of the Leningrad Scientific Research Institute of Hematology and Blood Transfusion. From 1975 to 1985 he was the director of the Institute. From 1985 to 1992 Prof. Shabalin was the Head of the Main scientific department of the RF Ministry of Public Health. From 1993 to 1997 he was the deputy minister of the RF Ministry of Public Health. From 1997 to 2013 he was the Director of the Russian Clinical Centre for Gerontology of the Russian Public Health Ministry. Since 2013 till present Professor Shabalin has been the Head of the Gerontology and Geriatry Department of the Russian National Research Medical University named after N.I. Pirogov.

Professor Shabalin is the author of 670 scientific works, 9 monographs, holds 72 patents for inventions, as an academic advisor he supervised 25 doctoral and 32 master’s theses.

Grachev was the Head of Staff of the Federation Council Committee on Science, Culture, Education, Health and Ecology. In 1997 – 2007 he was elected a deputy of the State Duma, the Chairman of the Committee on Ecology. Prof. Grachev is an honorary member of the PACE. Since 2011 he has been the President of the Vernadsky Nongovernmental Ecological Foundation.

Prof. Grachev is the author of 243 inventions, 625 published works, 29 monographs, 10 textbooks, 15 manuals.

The most important demographic phenomenon at the turn of the 20th and 21st centuries was a global increase in life expectancy and, as a consequence, an adequate increase in the proportion of older people in the general population of the world. The growth rate of the elderly population is far ahead of the growth rate of the total population. At the same time, the older the age group, the more intense its population is growing. Dramatic aging of the population

is a major social problem that has a signifi cant impact on the economic, industrial and social relations of the present-day world. Importance of demographic aging is so great that the UN puts it on a par with the problem of preserving peace in the world.

Throughout the human history, there are many examples of longevity. It is known for the fact that there were people being up to 120-122 years

old. Women live longer than men. Jeanne Louise Calment lived 122 years. This Frenchwoman lived the longest life of all people. The oldest man is 118 years old. Apparently, these are the limits of human life.

Important facts are not only isolated cases of longevity, but also the general increase in life expectancy (Figure 1.).

It is important to note as well that the important thing is not HOW MUCH, and HOW to live your life. There are many examples of active aging: the famous Russian actor V.M. Zeldin at his 99 continues to perform on stage. Well back in the early 30-ies of the last century he became famous for his roles in the movies (almost 85 years ago). Artist Boris Yefi mov lived 106 years, 103 years of which he regularly went to work. Shigechiyo Izumi of Japan lived longer than all men in the world - 120 years. His job seniority is 98 years.

There is a desire to dissociate themselves from the rest of the world. It is wrong. It is important not to be afraid of novelties, try at least to elementary understand it. Professor Shabalin started his career in 1962 as a doctor of Omsukchansky regional hospital of the Magadan Region. From 1965 to 1975, he held various scientific posts of the Leningrad Scientific Research Institute of Hematology and Blood Transfusion. From 1975 to 1985 he was the director of the Institute. From 1985 to 1992 Prof. Shabalin was the Head of the Main scientific department of the RF Ministry of Public Health. From 1993 to 1997 he was the deputy minister of the RF Ministry of Public Health. From 1997 to 2013 he was the Director of the Russian Clinical Centre for Gerontology of the Russian Public Health Ministry. Since 2013 till present Professor Shabalin has been the Head of the Gerontology and Geriatry Department of the Russian National Research Medical University named after N.I. Pirogov.

Professor Shabalin is the author of 670 scientific works, 9 monographs, holds 72 patents for inventions, as an academic advisor he supervised 25 doctoral and 32 master’s theses.

Since 2011 he has been the President of the Vernadsky Nongovernmental Ecological Foundation.

Prof. Grachev is the author of 243 inventions, 625 published works, 29 monographs, 10 textbooks, 15 manuals.

The most important demographic phenomenon at the turn of the 20th and 21st centuries was a global increase in life expectancy and, as a consequence, an adequate increase in the proportion of older people in the general population of the world. The growth rate of the elderly population is far ahead of the growth rate of the total population. At the same time, the older the age group, the more intense its population is growing. Dramatic aging of the population

is a major social problem that has a signifi cant impact on the economic, industrial and social relations of the present-day world. Importance of demographic aging is so great that the UN puts it on a par with the problem of preserving peace in the world.

Throughout the human history, there are many examples of longevity. It is known for the fact that there were people being up to 120-122 years

old. Women live longer than men. Jeanne Louise Calment lived 122 years. This Frenchwoman lived the longest life of all people. The oldest man is 118 years old. Apparently, these are the limits of human life.

Important facts are not only isolated cases of longevity, but also the general increase in life expectancy (Figure 1.).

It is important to note as well that the important thing is not HOW MUCH, and HOW to live your life. There are many examples of active aging: the famous Russian actor V.M. Zeldin at his 99 continues to perform on stage. Well back in the early 30-ies of the last century he became famous for his roles in the movies (almost 85 years ago). Artist Boris Yefi mov lived 106 years, 103 years of which he regularly went to work. Shigechiyo Izumi of Japan lived longer than all men in the world - 120 years. His job seniority is 98 years.

strengthens the brain, but also develops confidence. For a man who exceeded sixty, there is only one recipe for longevity - work. This is the basis of longevity system (Fig. 3).

Sense of happiness is the psychological basis of longevity. And happiness is more dependent on our internal systems than the external conditions. Centenarians are able to control their mood and even see the silver lining of the cloud. For them, the vessel is always half full, rather than half empty.

A human being is a favorite child of evolution, because he is the most developed. Our improvement and elevation have processed everything both flora, and fauna. And the evolutionary quest for the best, most perfect structure now led to the creation of a higher sample of matter - it is the human brain, intelligence. And because

humanity has become the most progressive branch of biological matter, it was the evolution that increased the population of the homo sapiens species, and allowed him to live long.

Brain is our most lazy organ. The hardest thing is to overcome mental laziness. Brain often suffers not from work rather from idleness. It does not wear at all from an active work, but only consolidates and develops. Just like muscles, the brain grows old without work. It is true for all age groups. The active work of the brain is especially important in the old age for preserving memory and mental clarity. If it is possible, it would be necessary to continue working professionally, helping young colleagues write articles, books, learning foreign languages, solving crossword puzzles, memorizing poems and verses. It prevents decrepitude brain.

However, we should not overstrain our brain because it could lead to stress, and there would be no benefits of such activity. An excessive stress is mentally and physically harmful. There are no professional athletes among centenarians, but there are many scientists (members of the Academy of Sciences live longer than businessmen).

Active longevity and psychological stress are incompatible. Among centenarians the share of laid back, friendly, "balanced" people actively involved in both mental and physical labor is the biggest (Fig. 4).

Personality development begins in childhood and never stops: it is necessary to purposefully think hard, act, perfecting the world and ourselves, accumulating knowledge and skills to pass them to the next generations. Education, professionalism and, to some extent, the success of each individual are the basis of the constant improvement. One should start to think about longevity already during his younger age. The correct way of life and self-development are the basis of longevity (Fig. 5).

Glaucoma is a chronic disease that causes vision loss through the degeneration of retinal ganglion cell (RGC) neurons and their axons in the optic nerve. It is a leading cause of irreversible blindness estimated to aff ect 79.6 million people worldwide by 2020 [1]. RGCs and the optic nerve, integral parts of the CNS, have demonstrated similar mechanisms of cell death in glaucoma to those of Alzheimer’s disease (AD), marking glaucoma as a neurodegenerative disease [2].

The intraocular pressure (IOP) is one of the main risk factor blamed for the pathology. The increased IOP in glaucoma may cause oxidative stress in retinal neurons. Although retinal circulation has an autoregulatory system to maintain a constant oxygen supply to the retina, it might be impaired under increased ocular pressure. A diminish in the retinal blood fl ow may lead to a kind of ischemic injury to retina, characterized by the formation of toxic reactive oxygen species (ROS) [3].

The ROS formation is a key process in many pathologic conditions. The relationship of glaucomatous optic excavation and retinal oxidation is still under investigation. Growing evidence supports that retinal ganglion cells in glaucomatous eyes are under oxidative stress that is associated with pathogenic mechanisms leading to glaucomatous neurodegeneration. Increased ocular pressure triggers molecular oxidation in the retina [4]. The reasons of increase in ROS in glaucoma were suggested to be due to diff erent mechanisms. The retina is especially susceptible to oxidative stress because of its high consumption of oxygen, its high proportion of polyunsaturated fatty acids, and its exposure to light.

The antioxidant enzymes act as free radical scavengers to cope with oxidation. Therefore, the antioxidative therapy might be of help in glaucomatous neuron degeneration.

Lowering IOP generally slows progression in glaucoma, but does not necessarily stop degeneration and there is a signifi cant

proportion of glaucoma patients who despite adequate IOP control continue to lose vision. In glaucoma, therefore, “neuroprotection” is restricted to mean therapy directed specifi cally at neurons in the whole visual pathway which is independent of its eff ects on IOP.

Intervention in the apoptotic cascades may provide therapeutic targets for drugs that could delay or ameliorate RGC loss in these chronic retinal neurodegenerative conditions. As the topical ocular hypotensive treatment is the main option in glaucoma nowadays, the direct neuroprotective eff ect of hypotensive eye drops is very important.

Evidence is mounting that topical ocular hypotensive exert direct neuroprotective eff ects in RGCs, both in vitro and in vivo.

The protective eff ect of beta-blocking medication on the visual fi elds of glaucoma patients has been studied, but with confl icting results [5]. In vivo experiments using topical and systemic application of β-blockers demonstrated the neuroprotective properties of betaxolol [6]. According to the Low Pressure Glaucoma Treatment Study2 (LoGTS), the α2 agonist brimonidine may have a benefi cial eff ect on visual function independent of IOP lowering [7]. Recent study demonstrated that topical brimonidine decreased MDA and increased GPx and catalase; hence it may act as an antioxidant [8].

Our recent research of antioxidant activity of carboanhydrase inhibitors for topical glaucoma treatment revealed that fi xed that they had the advantages over other eye drops, using for glaucoma, due to their high antioxidant activity [9].

Prostaglandin (PG) analogues have been a fi rst line drug for glaucoma treatment, because maximum intraocular pressure reduction is derived by once daily application without any systemic side eff ect in all kinds of glaucoma types. As well as the other ocular hypotensive drugs, PG analogues have been expected to have an additional eff ect to protect neurons independent of IOP reduction. Latanoprost showed neuroprotective eff ect on glutamate-induced

Summing up, we would like to recommend organizations to use active longevity of older workers. Our recommendations to scientists-gerontologists is to develop a program of active longevity, together with trade unions and political parties, in those countries that support the PACE resolution on this issue.

In everyday life in Russia we see, unfortunately, a very diff erent picture. Despite the fact that the Russian Constitution stipulates in the Paragraph 2 of the Article 19 the prohibition of any form of restrictions imposed on the citizens’ rights on social affi liation, in-laws of the Russian Federation have bans and restrictions, prohibiting occupation of certain positions on the age indicator, excluding the actual physical and mental condition of the individual.

An unreasonable prohibition on practicing research and teaching positions (rector, provost, etc.) seems unreasonable. Since the Article 332 of the Labour Code (as amended. Federal law dd. 30.06.2006 N 90 -FZ) prohibits from holding the provost position at the age of 65 and older. Many examples of highly effi cient work of scientists, including in management positions, suggest that this namely age is a heyday for scientists, as well as of CEOs. The legendary Russian Minister of Atomic Energy Ye.P. Slavsky took his position when he was 60 years old and

then led one of the most important industries in the Soviet Union for almost 30 years. The whole world practice confi rms this thesis. Just look at the age composition of the parliaments of the leading countries of the world. In the Japanese parliament 60 years old means young parliamentarians. Wisdom is more important than all other factors. Wisdom comes with experience and knowledge, and those who accumulate knowledge during all the life, analyze the experience; the one who has proven that he is socially useful and needed by the society as a wise vault of the world experience, has the right to work.

And indeed, we should fall into a muse "Is it ethical to deny a person his right to work, a creative work!». In our view it is immoral and impractical in terms of the interests of the society.

In conclusion, let us say a few words about the prospects for longevity. Everybody is trying to fi nd some secrets –but there are no secrets. Bases are given above. The perspective lies only with the scientifi c research. The science of the 20th – 21st centuries works wonders. Biological processes in living nature are very long-lasting (Fig. 6).

What are we worse than perch, sea urchin or parrot? "They remember Pushkin and will remember us." We are confi dent that it is real to extend people’s life up to 100 years old, and the records like 120 years old and even 150 years are quite beatable. In the nearest future science is likely to discover inexhaustible sources of energy and reveal the reserves of prolongation of the human life.

Wisdom comes with experience and knowledge, and those who accumulate knowledge during all the life, analyze the experience; the one who has proven that he is socially useful and needed by the society as a wise vault of the world experience, has the right to work.

And indeed, we should fall into a muse "Is it ethical to deny a person his right to work, a creative work!». In our view it is immoral and impractical in terms of the interests of the society.

In conclusion, let us say a few words about the prospects for longevity. Everybody is trying to fi nd some secrets –but there are no secrets. Bases are given above. The perspective lies only with the scientifi c research. The science of the 20th – 21st centuries works wonders. Biological processes in living nature are very long-lasting (Fig. 6).

What are we worse than perch, sea urchin or parrot? "They remember Pushkin and will remember us." We are confi dent that it is real to extend people’s life up to 100 years old, and the records like 120 years old and even 150 years are quite beatable. In the nearest future science is likely to discover inexhaustible sources of energy and reveal the reserves of prolongation of the human life.

Nowadays fi xed combinations are becoming more and more popular in the treatment of glaucoma as well, they are not inferior by their hypotensive activity comparing to PG analogues. However, there are no data in the literature about the antioxidant properties either of PG analogues, or of fi xed combinations.

Purpose: to compare the antioxidant activity (AOA) of the most widely used prostaglandins analogs and fi xed combinations for topical glaucoma treatment.

Methods: the model of oxidation-induced haemolysis was used to study AOA of travoprost, bimatoprost and latanoprost and fi xed combinations (dorzolamide/timolol (Cosopt), dorzolamide/timolol (Dorzopt Plus), brinzolamid/timolol, latanoprost/timolol, brimonidin/timolol, travoprost/timolol and bimatoprost/timolol).

Antioxidant activity (AOA) was measured by a method based on the induced erythrocytes hemolysis. When adding solutions of tretbutyl (TB) and clotrimazole (CT) to a suspension of red blood cells washed from the plasma, their hemolysis is observed. Compounds with antioxidant activity (AO) inhibit hemolysis caused by free radicals (TB radicals). Clotrimazole (CT) has the ability to amplify the intensity of hemolysis. According to the degree of inhibition of free hemolysis it is possible to judge about the AO

activity of the test compound. The degree of hemolysis was determined by the change in hemoglobin concentration in the incubation medium. This method in detail has been described by us previously [9].

In this paper we investigated the antioxidant activity of three PG analogues: latanoprost (latanoprost 0.005%, Pfi zer), travoprost (Travoprost 0.004%, Alkon) and bimatoprost (Bimatoprost ophthalmic solution 0.03%, Allergan), and the main fi xed combinations such as dorzolamid/timolol (Cosopt: Dorzolamide hydrochloride 2% + Timolol maleate 0,5%, MSD Pharmaceuticals), dorzolamid/timolol (Dorzopt Plus: Dorzolamide hydrochloride 2% + Timolol maleate 0,5%, S.C. Rompharm Company S.R.L.), brinzolamid/timolol (Brinzolamid 1% + Timolol maleate 0,5%, Alcon), latanoprost/timolol (Latanoprost 0,005% + Timolol maleate 0,5%, Pfi zer), travoprost/timolol (Travoprost 0.004% + Timolol maleate 0,5%, Alkon), brimonidin/timolol (0,2% Brimonidin tartrato + Timolol maleate 0,5%, Allergan) and bimatoprost-timolol (Bimatoprost 0,03%+ Timolol maleate 0,5%, Allergan).

Results have revealed the highest AOA in travoprost (17%). However, it decreased upon adding larger quantities of the drug into the model system. In contrast to this, bimatoprost showed an increase of its AOA in higher concentration. Its AOA peaked at 38% in 120 mcl of bimatoprost. AOA in latanoprost was the lowest. Upon reaching 120 mcl, latanoprost exhibited oxidative properties. The data is represented in Fig.1.

It is noticeable that at the minimum of the investigated drug’s volume the highest AOA was observed in travoprost; at the maximum volume – in bimatoprost. In volume up to 100 mcl of latanoprost and up to 110 mcl of travoprost these drugs lead to the increased erythrocyte hemolysis.

Figure 2 shows the comparative characteristics of AOA fi xed combinations in the study of diff erent volumes of drugs added to the model system. The highest antioxidant activity was recorded in the combination of dorzolamide/timolol (Cosopt). It exceeded the AOA of other fi xed combinations when analyzing all the investigated volumes. It was increasing with the increase of the amount of the drug added to the model system (from 30 mcl to 60 mcl to 90 mcl), and accounted for 40 %, 52 % and 75%, respectively. It is noteworthy that the AOA of Cosopt twice exceeded the AOA of its analog - Dorzop Plus (Figure 2).

According to its antioxidant characteristics another drug is approaching to those fi xed combinations, also containing a carbonic anhydrase inhibitor (brinzolamide/timolol), the AOA of which was 16%, 42% and 54%, respectively, when the volume of the test drug was 30, 60 and 90 mcl. Among the fi xed combinations with prostaglandins, latanoprost/timolol took the lead regarding its antioxidant properties (Fig. 1).

DiscussionIn this study we compared the main medications for glaucoma treatment for their novel properties, concerning their antioxidant activity. The results showed that among PG analogues the highest antioxidant activity at the minimum doses of the studied drugs was characteristic for travoprost. This activity increased and reached the highest value at a dose of 60 mcl of travoprost, and further increasing the added amount of the drug decreased its AOA. A diff erent pattern was observed with respect to latanoprost. This PG analogue showed the lowest antioxidant activity and behaved as an oxidant, increasing hemolysis, when was tested with 120 mcl of the drug Among all the tested PG analogues only bimatoprost showed direct dose-dependent antioxidant activity: AOA of the drug increased with its volume under the study.

Takano N., et al. have recently demonstrated protective eff ects of bimatoprost on RGS in vitro, using the model of oxidative stress. Interestingly, this eff ect was dose-dependent: the greater the concentration of bimatoprost was added to the model system, which reproduced the reaction of free radical oxidation, the greater the surviving retinal ganglion cells were observed [11]. These results are consistent with those that we found in the model system in vitro, where oxidative stress led to hemolysis, and this hemolysis was inhibited by bimatoprost in dose-dependent manner. It has been postulated that PG analogues may have a potential to exert

neuroprotective effect by local administration. In some of the recent studies PG analogues indicated IOP independent neuroprotective effect on glutamate- and hypoxia-induced RGC death at clinically available intracameral concentration [12].

Recent studies by N.Osborne have shown an antioxidant effect of latanoprost: it was found that the drug promoted the development of the endogenous antioxidant glutathione [13].

The neuroprotective effect of PG analogues may be derived from the other mechanisms unrelated to the FP receptor stimulation. Many papers suggest that prostanoid EP2 is neuroprotective [14, 15]. Additionally, cyclooxygenase 2 (Cox2), one of the key enzymes to produce intrinsic PG by FP receptor stimulation, was reportedly neuroprotective [16].

In the course of the present study, for the first time PG analogues were compared in terms of their antioxidant activity, which refers to the neuroprotective properties of drugs.

Antioxidant protection plays a key role in oxidative stress conditions. Probably, with different neurodegenerative diseases, the activity of antioxidant defense molecules, that normally counteract the harmful effects of ROS, reduced. For example, the antioxidant enzymes superoxide dismutase (SOD), catalase, glutathione peroxidase (GSHPx) - and glutathione reductase (GSHRd) have reduced activity in the affected areas of the brain in Alzheimer’s disease and in the retina in glaucoma. For the first time we showed a decrease of the antioxidant protection of aqueous humor in glaucoma in 1996[17].

Application in the present study a model system with erythrocytes, hemolysis of which was caused by an oxidative stress, let us with a certain probability judge about the antioxidant activity of the tested PG analogs and fixed combinations. The results showed that antiglaucoma fixed combinations exhibited the highest antioxidant activity, containing carbonic anhydrase inhibitors (ICA). At the same time, the highest antioxidant activity was found in combination of dorzopt/timolol (drug Cosopt). This coincided with the data of the high antioxidant activity of dorzolamide obtained by us earlier [9].

Recent studies have shown that dorzolamide and timolol were able to protect mitochondrial DNA trabecular endothelial cells from oxidative stress [18], and with the combination of these drugs the antioxidant effect was amplified, which prevented trabecular from the damaging effect of hydrogen peroxide [19]. It should be emphasized that the explicit antioxidant properties of dorzolamide were particularly evident in relation to intact mitochondria trabecular endothelium, when the trabecular tissue was at least partially functioning. This substantiates the early administration of the dorzolamid, which may be the most effective in the initial stage of glaucoma. Clinical observations by V.Zanon-Moreno et al. about the high antioxidant activity of dorzolamide confirm this conclusion [20]. Attention should be paid to the recent findings about the ability of a fixed combination of dorzolamide with timolol to enhance the perfusion of the optic disk and retina [21].

Conclusion. Antioxidant activity of hypotensive drugs for glaucoma treatment is their important new features that are associated with the direct neuroprotective effect. The most commonly used

drugs at the present day are PG analogues and fixed combinations. They demonstrate a different antioxidant activity. Dorzolamide/timolol fixed combination has potential advantages over all other studied drugs, including fixed combinations and PG analogues, due to its highest antioxidant activity. This data should be taken into consideration as a novel option in glaucoma treatment.

Recent studies have shown that dorzolamide and timolol were able to protect mitochondrial DNA trabecular endothelial cells from oxidative stress [18], and with the combination of these drugs the antioxidant effect was amplified, which prevented trabecular from the damaging effect of hydrogen peroxide [19]. It should be emphasized that the explicit antioxidant properties of dorzolamide were particularly evident in relation to intact mitochondria trabecular endothelium, when the trabecular tissue was at least partially functioning. This substantiates the early administration of the dorzolamid, which may be the most effective in the initial stage of glaucoma. Clinical observations by V.Zanon-Moreno et al. about the high antioxidant activity of dorzolamide confirm this conclusion [20]. Attention should be paid to the recent findings about the ability of a fixed combination of dorzolamide with timolol to enhance the perfusion of the optic disk and retina [21].

Conclusion. Antioxidant activity of hypotensive drugs for glaucoma treatment is their important new features that are associated with the direct neuroprotective effect. The most commonly used

drugs at the present day are PG analogues and fixed combinations. They demonstrate a different antioxidant activity. Dorzolamide/timolol fixed combination has potential advantages over all other studied drugs, including fixed combinations and PG analogues, due to its highest antioxidant activity. This data should be taken into consideration as a novel option in glaucoma treatment.
