The Path Links NHS Pathology Service provides pathology for Northern Lincolnshire and Goole NHS Foundation Trust (population 440,000) and United Lincolnshire Hospitals NHS Trust (population 757,000, one of the fastest growing populations in England). It is a countywide Pathology service run by a single management board and the main working base is at Lincoln County Hospital. The board is clinically led by a Director who is appointed by the Chief Executive of Northern Lincolnshire and Goole NHS Foundation Trust. The current Path Links Director is Dr Steven Griffin, Clinical Director - Clinical Support Services, Path Links and Digital Strategy. A more detailed description of the management arrangements is provided later and summarised in Appendix 1. PATH LINKS NHS PATHOLOGY SERVICE The Path Links NHS Pathology Service has developed over the last nineteen years to provide pathology services to hospital sites within the Northern Lincolnshire and Goole NHS Foundation Trust (Grimsby, Scunthorpe and Goole) and the United Lincolnshire Hospitals NHS Trust (Boston, Grantham, Lincoln and Louth) which together serve a population of 1.2 million. The development of Path Links is in line with the objectives of HSC 1999/170: Modernisation of Pathology Services. The service is recognised by Lord Carter’s Report on Pathology services to be successful in improving the quality of Pathology services and providing efficient, cost effective services to patients. From 1st April 2001 management of the service passed to the Northern Lincolnshire and Goole NHS Foundation Trust, the Lead Trust, thereby creating an integrated, single managed Pathology service. Path Links provides services for six acute hospital sites, over 180 primary care GP practices and a number of NHS Community Trusts and private healthcare providers. Cellular Pathology has been consolidated onto the Lincoln site, providing a centralised Surgical Pathology service for the entire county of Lincolnshire with a single laboratory information system linking all of the laboratory sites. Autopsies continue to be performed on site in Grimsby and Lincoln. Clinico-pathological and multidisciplinary team meetings are provided on most sites. Telemedicine video-conferencing facilities are used to facilitate participation in these meetings. DIRECTORATE OF CELLULAR PATHOLOGY The Cellular Pathology Directorate is currently established for 16 consultants. The Path Links strategy was formulated to allow the development of sub-specialisation in line with Cancer Peer Review Measures, provide cross-cover and facilitate audit, teaching and research. Path Links Cellular Pathology Directorate has never been afraid of progress and change. As a result, we are one of the first District General Hospital based cellular pathology departments to adopt whole slide digital imaging (WSI) for clinical reporting, utilising innovative technology. This is an exciting development with broad ranging capabilities for new and flexible ways of working. The laboratory is approved by the IBMS as a training laboratory for pre-registration and specialist training in Cellular Pathology. The Department is HTA compliant. SERVICE WORKLOAD The Directorate receives a wide range of surgical and biopsy specimens from all major specialities including General Surgery, Breast Surgery, Urology, Orthopaedics, Gynaecology, ENT including Head and Neck Surgery, Ophthalmology, Dermatology, Oral Surgery, Genito-urinary Medicine, Gastroenterology and General Medicine. A service to the NHS Breast Screening Programme is provided with reporting of assessment

specimens. NHS Bowel Cancer Screening Programme specimens are reported from Humber & Southbank as well as Lincolnshire screening centres. Bowelscope (second phase) specimens are received from two sites in Lincolnshire and roll out to other sites is underway. Non-Gynaecological cytology specimens include FNAC of breast, thyroid, lymph nodes, etc. Urine, bronchial washings and brushings, peritoneal and pleural fluids make up the bulk of diagnostic cytology and negative samples can be reported in Lincoln by a senior BMS with IBMS Diploma of Expert Practice in Non-Gynaecological Cytology. The department operates a ‘pull’ system of workload allocation to consultants, catering to their particular specialist and more general interests. Approximately 25% of the surgical workload is derived from primary care. The approximate workload for the year 2017 was as follows:

*Grimsby and Lincoln mortuaries Biomedical Scientists (BMS) perform dissection of RCPath specimen types A, B and C. Advanced Practitioners are progressing with dissection of specimen categories D and E. Currently, approximately 90% of specimen dissection is undertaken by BMS staff. There are established multidisciplinary team meetings for Breast, Colorectal, Lung, Skin, GI, Upper GI, Gynaecological, Head and Neck, Lymphoma, Thyroid & Endocrine, Urology, Cancers of Unknown Primary and for colposcopy, which are held on one or more sites at varying intervals. The new appointee will be expected to act as a lead/deputy lead pathologist within the multidisciplinary team environment, depending upon their preferred interests. Lymphoma cases are all referred for a second specialist opinion in line with National Cancer standards to the Haematological Malignancy Diagnostic Services in Leeds or Nottingham. Sarcoma cases are all referred for a second specialist opinion in Nottingham or Sheffield THE CELLULAR PATHOLOGY LABORATORIES 1. Lincoln County Hospital, Lincoln Lincoln County Hospital is situated close to the centre of Lincoln. It is a large and modern complex, catering for most of the major specialties. The Lincoln Cellular Pathology Department is accommodated on the ground floor and part of the first floor of a purpose built block in Lincoln County Hospital. Each consultant has an office which is equipped with a reporting microscope and computer terminal and will additionally include a reporting workstation as digital reporting is expanded. Most consultants use voice recognition technology for reporting. As an alternative to voice recognition, digital dictation facilities are utilised for efficient transfer of work to the secretarial staff. Digital photographic equipment is available within the department. Photomicrography and a fluorescence microscope are available.

The department is currently implementing WSI digital microscopy. The vision is to deliver our entire histology service via this technology allowing efficient primary reporting, remote site reporting and multidisciplinary review. This will also facilitate internal and external referral of cases. There is a seminar room equipped with a microscope video projection system for multi-disciplinary team meetings, and video-conferencing facilities. There is a nine station multi-headed teaching microscope with digital camera, a five station multi-headed teaching microscope and two wall mounted high definition screens plus a new PC in the library together with a selection of up to date textbooks. The Surgical Pathology laboratory has been redeveloped with specimen reception, cut-up suites and an enlarged microtomy, processing and staining area. Fully incorporating Lean design and working practices, the department has received national and international recognition as an exemplar model of an efficient Lean laboratory service. Immunohistochemistry is provided by two Leica Bond Immunostainers with a broad panel of antibodies and is performed continuously with a rapid (<24 hour) turnaround. An Artisan staining platform provides a broad repertoire of special stains. A cryostat is available to provide a frozen section service to the Lincoln hospital site as well as direct immunofluorescence. A transportable cryostat is used to provide frozen sections at any of the peripheral hospital sites when required. Hospital and coronial autopsies are carried out on this site. The mortuary is adjacent to the Histology laboratory and has three post mortem tables and storage facilities for up to 76 bodies. 2. Diana, Princess of Wales Hospital, Grimsby At the Diana, Princess of Wales Hospital there is a mortuary facility with three post-mortem tables, one in a high-risk room, and an observation gallery. Hospital and coronial autopsies are carried out on this site. Within the laboratory accommodation there is a consultant office with a microscope and computer. A seminar room equipped with a microscope video projection system and video-conferencing facilities is provided in the radiology department for multidisciplinary team meetings. 3. Scunthorpe Hospital There is a consultant office with microscope, computer and reporting workstation. A seminar room equipped with a microscope video projection system and video-conferencing facilities is provided in the radiology department for multidisciplinary team meetings. The mortuary has storage for 33 bodies. Post mortems are no longer performed on this site and cases are transferred to Lincoln County Hospital. 4. Pilgrim Hospital, Boston Post-mortems are no longer performed on this site. The mortuary has storage for up to 60 bodies and storage for three bariatric cases. A video-conference facility with microscope is provided in the hospital multidisciplinary meeting room. 5. Grantham Hospital At the Grantham site there is an office equipped with two microscopes, two computers and a reporting workstation. Office facilities have been upgraded to facilitate reporting for consultants living in the south of the county, hence reducing travel requirements.

ABOUT THE POST The majority of the department’s general Cellular Pathology work is performed on the Lincoln site, however sessions (typically in a whole day block) are undertaken at Grimsby, Grantham or Scunthorpe Hospitals by the existing consultant staff to provide an autopsy service (Grimsby only) and/or to participate in MDT working. All Consultants will be expected to contribute to the following:

- Shared workload of histology and cytology as appropriate for their speciality interests - Specimen reporting and authorisation with clinical interpretation of reports - Provision of advice regarding specimen type/collection and investigations - Day to day supervision of diagnostic methods and examination of specimens - Occasional requirement to work or attend meetings at another hospital site - Provision of cover for planned leave of colleagues - Supervision, training and management of doctors in training in the department - Involvement in policy and SOP preparation - Responsibility for quality assurance performance within the department, including taking

part in all relevant National External Quality Assurance (NEQAS) programmes - Responsibility for achieving and maintaining full accreditation with CPA UKAS - Business planning, including the assessment and introduction of new methods, and forward

planning for staff and equipment requirements. Detailed job-plans and subspecialist interests will be discussed and agreed between the existing consultants and the successful candidate on appointment. Medico-legal autopsies are not part of the NHS contract but it is expected that suitably qualified individuals will be invited by HM Coroner to participate equally in this area of work. Participation in the autopsy service would be desirable, but is not mandatory. The Consultants clinical workload is biased towards their main interests but they also report general high volume work outside of their main areas of interest. There is no necessity to report cervical cytology. Workload will be tailored to experience and expertise. At times of high workload or vacancies, the outsourcing of work is utilised. There is an independent private hospital, St Hugh’s, in Grimsby and a private wing in Boston. The Cellular Pathology directorate provides a diagnostic service to these facilities and fees are equitably shared. The appointee is expected to have the FRCPath in Histopathology/Cytopathology or show evidence of equivalent training and experience. There is no on call commitment. This post attracts 10 programmed activities per week, however further PAs may be available for direct clinical care. The indicative PAs are outlined below. Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.

The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide and will be finalised following individual agreement with successful candidates. The

initial SPA of 1.5 is sufficient for revalidation purposes only. Further discussion on supporting professional activities will depend on the Trust's requirements and the individual's particular expertise. Obviously, study leave and professional leave are taken over and above the SPA allocation. In line with the terms and conditions, the final job plan is subject to the agreement of the Trust through the Clinical Director and the appointee and reviewed after three months. Six weeks’ notice of all periods of planned leave (annual leave, study leave and professional Leave) is required at all times. MANAGEMENT The Trust recognises that Clinical engagement is key to ensuring the delivery of the Trust’s Organisational Development, leadership and communication plans and objectives. The position of this post within the Cellular Pathology Management structure is detailed in Appendix 1. All Consultant staff and Senior BMS are members of the Directorate Management Team which meets regularly, currently monthly, to discuss discipline specific issues and strategy, making recommendations to the Path Links Board. The Cellular Pathology Directorate is responsible for the delivery of a co-ordinated, high quality, cost-effective service across all Path Links sites and for determining the strategic direction of the service for issues such as senior appointments, Clinical Governance, development of specialist services, purchasing and CPA/UKAS accreditation. The Cellular Pathology Directorate Operational manager (Mr Fraser Sim) works closely with the Clinical Director and both report to the Path Links Management Board. In conjunction with the Laboratory Manager, you will be expected to manage all aspects of laboratory work on a day-to-day basis including supervision of specimen examination and reporting. To continually assess methods used and to maintain CPA UKAS standards, ensuring continuation of full accreditation of the service. The appointee will also work closely with other service providers such as the Biomedical Scientists and administration staff in the day to day provision of an efficient and effective Cellular Pathology Service, with an expectation to support business planning, including the introduction of new methods to support the Cellular Pathology Operational Manager in budgetary management and assist with Policy and SOP preparation. The Path Links Director, currently Mr Steve Griffin, leads the Path Links Management Board. It meets bi-monthly and takes overall responsibility for strategic and policy decisions. The Board is

responsible through the Director to the Lead Trust Chief Executive, Mr Peter Reading. Each Path Links site has day to day site management arrangements co-ordinated by a designated Site Manager (at Lincoln, Mr Fraser Sim) supported by the Path Links General Manager and accountable to the Path Links Management Board. All Consultants undertaking work for a Path Links Site have the right to attend the Management Group and Medical Advisory Committee (or equivalent) on that site. CLINICAL AUDIT AND QUALITY ASSURANCE The Directorate holds regular audit meetings and the appointee will be expected to participate in these. The department participates in the following external quality assurance schemes: Regional General Histopathology EQA Schemes – East Midlands and Yorkshire Specialist Histopathology National EQA schemes including urological/prostate, pulmonary, gastrointestinal, gynaecological, liver, dermatological. UK NEQAS for Cellular Pathology Technique – General histology modules NHSBSP Histopathology EQA Scheme NHSBCSP Histopathology EQA Scheme UK NEQAS Immunohistochemistry – General, steroid receptors and GIST modules TEACHING AND RESEARCH There are regular clinical meetings and case presentations with clinical consultants specifically for the benefit of junior medical staff on all hospital sites. It is expected that the post holder will participate in teaching and clinico-pathological meetings. It is also expected that there will be a commitment to undergraduate and postgraduate teaching as required. Medical students are seconded from the Universities of Nottingham and Leicester to Lincoln and Boston. A new Medical School is due to open in Lincoln, in September 2019, in association with Nottingham University. Regular tutorials are offered for junior medical staff who are preparing for their postgraduate examinations. A medical school at Hull and York has a regular rotation of clinical students to Scunthorpe and Grimsby. Cellular Pathology Speciality Trainees rotate from Nottingham and all consultants within the department are expected play an active role in training. There is now a possibility of developing digital slide teaching sets. It is anticipated that the appointee will play an active role in progressing this. The department has a commitment to support research and clinical trials throughout the Trust. The size of the population served by Path Links and a common IT system across all sites will facilitate research projects. If desired, the development of research and or teaching links with neighbouring universities (e.g. Lincoln, Hull, York, Nottingham) would be supported. CONTINUOUS PROFESSIONAL DEVELOPMENT The appointee will be expected to fulfill the educational requirements set by the GMC and the Royal College of Pathologists. The Trust places a high value on supporting effective continuous professional development and has

developed and agreed a Trust wide Policy and best practice guidelines on CPD. Adequate resources and time will be provided for these activities (currently study leave granted is 10 days per year with a budget of £1,100 per consultant per year). Each consultant will be required to participate in regular reviews of their development with their Clinical Director to identify and agree their priority development needs. Sufficient time will be provided for preparation and undergoing appraisal and revalidation. THE EMPLOYER The employer for this post is the Northern Lincolnshire and Goole NHS Foundation Trust. The post holder will be required to comply with the local rules governing standards of behaviour pertaining at each of the hospital sites at which they may be working within Path Links. As the employer Northern Lincolnshire and Goole NHS Foundation Trust has developed a number of general policies and procedures that applies to the employment of consultants. The successful applicant will be required to familiarise themselves with these to ensure that they understand and adhere to them. ADDITIONAL INFORMATION PERTINENT TO ALL STAFF Health and safety - Healthcare associated infection Healthcare workers have an overriding duty of care to patients and are expected to comply fully with best practice standards. You have a responsibility to comply with Trust policies for personal and patient safety and for prevention of healthcare-associated infection (HCAI); this includes a requirement for rigorous and consistent compliance with Trust policies for hand hygiene including the ‘bare below the elbows’ approach, use of personal protective equipment and safe disposal of sharps. Knowledge, skills and behaviour in the workplace should reflect this; at annual appraisal you will be asked about the application of practical measures known to be effective in reducing HCAI. The Trust has the responsibility of ensuring that adequate resources are available for you to discharge your responsibilities. Safeguarding The Trust has in place both a Safeguarding Children Policy and a Safeguarding Adults Policy in line with national legislation. The Safeguarding Policies place a duty upon every employee who has contact with children, families and adults in their everyday work to safeguard and promote their welfare. In the event that you have concerns about possible harm to any child or adult you should seek advice and support from the Trust Safeguarding team or in their absence contact your line manager or your Assistant Divisional Director. GENERAL INFORMATION ABOUT THE HOSPITAL AND LOCALE Lincoln County Hospital, the main base for this post, is situated near the city centre and Cathedral. It is a large modern complex that caters for most of the major specialities. The A&E Department is the main centre for a large part of the county. The Department of Clinical Oncology and Radiotherapy is based on the Lincoln County Hospital site unit with three linear accelerators. A new Regional Acute Cardiac Unit was opened in 2013. Lincoln is a pleasant Cathedral City with a population of 95,600. The City offers a wide range of social and cultural activities with excellent sports centres. Lincolnshire is a large, rural county that is geographically varied including the Lincoln escarpment on which the beautiful medieval Cathedral stands, the North Lincolnshire Wolds, a designated area of outstanding natural beauty, and extensive beaches along the coastal strip. Humberside, East Midlands and Robin Hood (Doncaster) Airports are all within easy reach and are of increasing importance with regular scheduled National and European flights. The port of Hull has car ferry services to the continent. The cities of Nottingham, Sheffield and Leicester are 60-90 minutes drive/train journey away. East coast main line rail services are nearby and provide rapid, frequent services to London. The district provides a wide choice of affordable housing in urban and rural areas. Educational facilities for children in Lincoln and the surrounding area of Lincolnshire are of a high standard with a number of excellent primary and secondary schools including grammar schools. FURTHER INFORMATION Visit and subsistence expenses: In addition to their attendance at the Appointments Advisory Committee, shortlisted candidates will be reimbursed for any one visit to the hospital. In these circumstances candidates will be granted travelling and appropriate subsistence expenses. Other visits will be at the candidate’s own expense unless in response to a specific request from a senior Trust officer.
