Patch testing with TROLAB allergens

Patch testing is the most important diagnosticand investigative method currently available forstudying allergic contact dermatitis in clinicalpractice. The method includes the epicutaneousapplication of a specific allergen at a definedconcentration and in a defined vehicle which willinduce a cutaneous inflammatory reaction in asensitised person, but will cause no reaction in anon-sensitised person.

TROLAB patch test allergens have alwaysbeen developed, revised and produced followingthe recommendations of the International ContactDermatitis Research Group (ICDRG) and theEuropean Environmental and Contact DermatitisResearch Group (EECDRG). Their many yearsexperience helped us to select the test sub-stances and determine test concentrations andtest vehicles, so that generally the test allergensdo not trigger toxic reactions whilst allergic onescan be induced. Today we add new test allergensto the TROLAB programme only, if test concentra-tions and vehicles have been proven andconfirmed in clinical trials.

TROLAB patch test allergens are dispersed inpetrolatum (if no other vehicle is indicated) andfilled in 5 ml syringes. Some test allergens containadditives like e.g. sorbitan sesquioleate or solidparaffin. These additives are indicated on thesyringe lable.

Almirall’s experience as a pharmaceuticalcompany specialising in the production of topicaldermatological products also guarantees that thepatch test allergens are produced in a high gale-nic quality.

TROLAB patch test allergens are produced inaccordance with the WHO guidelines of GoodManufacturing Practice. Raw materials are selec-ted carefully. The allergens are homogenouslyincorporated into the ointment in the form of extre-mely small particles. The stability of the final pro-duct is checked by chemical analysis.

AII TROLAB Patch Test Allergens are registe-red as MEDICINES in Germany with the PaulEhrlich Institute, Langen.

Our overall objective is to offer our customershigh quality standardised patch test allergens.However, it should be noted that the interpretationof the test results requires much experience onthe part of the testing physician, i.e. careful anam-nesis and examination of the clinical findings forthe final assessment of the relevance of the testresults.

Application of test substances

TROLAB patch test allergens can be used withall types of patch test plaster. Very well acceptedare Finn Chambers on Scanpor�*. It is important toconsider that all test concentrations were definedby using this test plaster.The following descriptionis for using Finn Chambers on Scanpor� (for othertest plasters please follow instructions of manufac-turers).

When using Finn Chambers on Scanpor�, theserial numbers of the substances to be tested mustbe clearly marked on the upper side of the adhesi-ve tape (Fig. 1), for example: S1 - S10 (Standard,substances 1-10). The small tape sections carryingthe numbers will later remain on the skin and allowto perform the reading with the practical ReadingPlate*.

Place the test tape in front of you, as shown inFig. 3. Now you can fill the chambers downwardstarting on the top right row. The test ointments areapplied directly into the aluminium chambers (Fig.3). The chambers must never be filled completelybut only slightly more than half-full (5 mm length).No substance should be brought on the rim of thechamber!

When using test solutions, a filter paper dischas to be placed into the chamber first (Fig. 4 a).The filter paper has to be saturated with one drop

of the test solution (Fig. 4 b). Please avoid excess.On the other hand, the filter paper disc must notdry up, as in that case false-negative test resultswould have to be expected. We recommend to pre-pare the plaster just before applying it to the pati-ent’s skin in order to achieve relevant test results.

The test tapes that have been prepared asdescribed are applied to the back in normal uprightposition, with the lower margin being fastened first.Thereafter they are pressed against the skin with

the palm, gradually from bottom to top, and thencarefully stroked down. The air can thereby escapefrom the chambers and a good fixation is achieved.Subsequently, the test chambers are again pressedagainst the skin two to three times, exerting slightpressure with the fingers in order to spread the testsubstances evenly on the skin (Fig. 5 a + 5 b). Theadhesive tapes should be applied only to the cleanand dry skin (non-greasy, no ointment residuesetc.). You might use diluted alcohol to clean theskin.

The Reading Plate is a simple aid to recognizethe test reactions without having to mark the skin ofthe patient with ink. As a preparation for the rea-ding, before the test chambers are filled, the num-bers of the substances to be applied have alreadybeen noted on the upper ends of the adhesivetapes (see Fig. 1). Fig. 6 demonstrates how to usethe Reading Plate when the adhesive tape is remo-ved (usually after 48 hours). The cutting edge of theplate is slightly pressed against the test tape at anangle of about 45 degrees. The broader side of theplate should thereby be held on the left. Now,grasp the test tape and tear it off in a brisk upwardmovement, thereby leaving the strip (about 1 cmwide) where the numbers are marked (Fig. 7). Afterthe removal of the Finn Chambers on Scanpor�, cir-cular imprints on the skin reveal whether the testprovided adequate occlusion. If these imprints arenot visible, the test has to be repeated (it is knownfrom experience that this rarely happens if theadhesive test tapes were properly applied).

Fig. 7 demonstrates how the Reading Plate isused during the first as well as during the subse-quent readings when the circular imprints of thetest chambers on the skin have already disappea-red. Place the left corner of the Reading Plate onthe left corner of the remaining strip of the adhesi-ve tape. The holes in the plate now indicate thelocations of the test sites.

Please note:

The first reading should be performed about20 to 30 minutes after the removal of the tape, assome skin sensitive persons will react with rednessafter tape removal. Excessive test ointments haveto be carefully removed with an individual swab forevery substance prior to the first reading. Makesure not to wipe the excessive ointments into theadjacent test areas.

If no Reading Plate is available, the locations ofthe individual test chambers have to be marked onthe skin with a suitable marking pen before remov-ing the test tape, so that positive test results can beclearly identified during further readings after 72and 96 hours.

Interpretation of test results

When recording the results in the patient’s testsheet, it is important to score according to the reac-tion actually seen and not according to the subjec-tive interpretation placed on the reaction by the read-er. Irritant reactions should be recorded as positiveirritant (IR) and not as negative.

To recognize irritant reactions it may be helpfulto know of the irritability of the skin at the test sideand the time of testing. Almirall is offering Sodium-laurylsulfate (SLS) 0.25 % in water as a control irri-tant for patch testing. Information on the irritancycontrol patch test with SLS 0.25 % in water can befound on the following pages.

Whether or not the positive test reaction demon-strated by the patch testing is relevant to the patient’sdermatitis, either as the primary cause or as anaggravating factor, must be determined by the criti-cal judgement of the physician. If the clinical and his-torical facts are consistent with the patch test result,one can be reasonably certain of its significance.

In doubtful cases, a repeated open applicationtest (ROAT) in the antecubital space or on the outeraspect of the upper arm may be helpful.

Information on the irritancy control patchtest with Sodiumlaurylsulfate (SLS) 0.25% inwater

Why to do an irritancy control when patchtesting with contact allergens.

Every contact allergen produces an irritancyeffect on the skin to some degree. It is thought thatthis irritancy is important for a substance to be ableto act as a contact allergen. Due to this particularproperty, irritant or false positive test reactions mayoccur in diagnostic patch testing with contact aller-gens. The frequency and extent of such unwantedreactions differ not only between allergens but alsobetween individuals, depending on their particularsensitivity to irritants. This irritability may vary bet-ween different skin regions and over the course oftime. Therefore, the German Contact DermatitisResearch Group (Deutsche Kontaktallergie-Grup-pe ; DKG) recommends to perform an irritancy con-trol patch test when patch testing with contact all-ergens, in order to assess the individual sensitivityto irritants at the time of patch testing in the testarea.

Which irritant is suitable?

There are different kinds of irritants, e.g. weakbases or acids, or detergents, which disturb theepidermal barrier. For the purpose of an irritancycontrol patch test, Sodiumlaurylsulfate (SLS) hasproved useful. SLS is not an allergen and, tested ata low concentration it causes visible reactions onlyin very sensitive individuals. DKG studies haveshown that patients, in whom SLS 0.25 % in waterelicited an irritant reaction, had questionable andweak positive patch test reactions to contact aller-gens more frequently than others. In most cases,these patch test reactions, too, are irritant, and notallergic, reactions. Thus, patch testing with the irri-tant SLS 0.25 % in water as a marker for sensitive

skin facilitates the assessment of erythematous andweak positive patch test reactions to contact aller-gens.

How to perform and read the SLS patch testand how to interpret the results.

The irritancy control patch test with SLS 0.25 %in water is performed in exactly the same manneras the classical contact allergen patch test. Forreading and assessment of the test reactions, theDKG recommends the following categories (modi-fied from Tupker et al. 1997):

When patch testing with SLS 0.25 % in water,only very rarely reactions of grade sls3 or sls4occur. In the DKG studies, less than 30 % of thepatients tested had a reaction; most of the reac-tions were classified as sls1 or sls2. In patientshaving a visible reaction of this kind, an increasedirritability of the skin is probable.

Significance and limitations of the irritancy con-trol patch test.

An irritant reaction (sls1 or stronger) to SLS0.25% in water does not automatically mean thatevery weak patch test reaction to an allergen isalso an irritant; however, the probability that an irri-tant erythema or a weak false positive reactionoccurs in this patient is markedly higher. This isparticularly true for test allergens known to have anincreased irritant potential. The irritancy controlpatch test facilitates the assessment of this kind oftest reactions; however, it does not replace theneed for a thorough interpretation of the test reac-tions (allergic versus irritant) by the physician andfor the assessment of clinical relevance.

As mentioned above, the irritability of the skinmay vary between body regions and over the cour-se of time. Thus, the irritancy control patch test withSLS only gives information about the irritability ofthe test region (upper back) at the time of patchtesting. It can be considered a regionally limitedsnapshot, which does not allow any generalisationconcerning the patients’ skin irritability.

1. Nickel Sulphate is in the North American Standard Series 2,5 % as the waterfree salt;in Europe the NiS04 · 6H2O salt 5 % is approved by the authorities. The molecularcontent of nickel ions of both test allergens is almost equal.

Testing NiS04 · 6H2O at 2,5 % as proposed by the ICDRG means testing only 50 % ofthe Nickel content that is being tested in Europe and North America.
