Title: Review of Standard Wording for Change Facility Undress Instructions

Issue Summary:WRPS-PER-20 12-153 1 identified that two alternate sets of instructions for doffing personnel protective equipment were foundposted in MO-5 12 (the Middle C Change Trailer). The instructions posted on the Southeast wall contained a different sequencingof steps than the instructions posted on the partition in the middle of the trailer.

The instructions on the Southeast wall state the following (Example A):"Doffing A Full Set of Protective Clothing.I . Remove rubber overshoes.2. Step onto first step off pad.3. Remove outer gloves.4. Open hood from bottom and remove from fr-ont to back, as applicable.5. Remove ALL exposed tape.6. Remove supplemental dosimetry and make sure coverall pockets are empty.7. Take down barrier Closure, as applicable.8. Remove coveralls, pulling down and turning them inside out.9. Remove shoe covers stepping onto step off pad as each shoe cover is removed.10. Replace barrier closure as applicable.11. Remove surgeons gloves (and liners as applicable).12. Perform exit survey per applicable RWP.13. Obtain release survey of supplemnental dosimetry and other materials, as applicable"

The instructions on the partition in the middle of the trailer state the following (Example B):"Sequence for Removing Protective Clothing1. Remove supplemental dosimetry2. Document No.: TFC-1301-FACT-0258Discussion and/or Requirements:HNF-5 183, Tank Farms Radiological Control Manual, Rev. 5e, Appendix 3C, "Contamni nation Control Practices", page 3-33,Removal of Protective Clothin ", states the following (Example C):

"Potentially contaminated protective clothing should be removed without spreading contamination and in particular withoutcontaminating the skin. Workers should be instructed not to touch the skin or place anything in the mouth during protectiveclothing removal. Instructions for protective clothing removal comparable to the sequence presented below should be postedadjacent to thle step-off pad in accordance with Article 325.6.

Sequence for Removing a Full Set of Protective Clothing at the Step-Off Pad

Before stepping out of the Contamni nation Area or- Airborne Radioactivity Area to the step-off pad, the worker should:L. Remove rubber overshoes.2. Remove outer gloves.3. Open hood from bottom and remove from front to rear.4. Remove respiratory protection, as applicable. When mandated by Industrial Safety concerns (e.g., exiting an Asbestos ReguilatedArea), follow the applicable egress instructions of Article 338.5. Remove all exposed tape.6. Remove supplemental dosimletry and make sure overall pockets are empty, as applicable.7. Take down barrier closure, as applicable.8. Pull down coveralls, turning them inside out.9. Remove shoe covers, placing shoes onto step-off pad.10. Replace barrier closure, as applicable.11. Remove surgeon's gloves and liners, as applicable.12. Obtain or perform a whole body survey.13. Obtained release survey of supplemental dosimetry and other materials.

This protective clothing removal sequence was developed by Hanford subcommittees on protective clothing and onl training andwas determined to be the most effective for conditions and activities conducted at Tank Farms."

Article 325.6 states: "Instructions for donning and removing protective clothing should be posted at the dress-out and step-off pad

areas."

This FACT addresses this issue by providing the following 2 options.

Proposed Outcome:The assigned action is to determine standard wording for Change Facility undress instructions. The TFRCM contains the currentlyapproved example of the doffing instructions. With the exception of the information in Step 4 pertaining to asbestos regulatedareas, which was added to incorporate a Technical Equivalency Determination, the TFRCM doffing instructions (Example C)match the doffing instructions posted at HAMMER (Attachment 1), which are also consistent with the instructional video providedat Rad Worker training.

The primary difference between the instructions of Example B and the current instructions in the TFRCM is thle step involvingremoval of supplemental dosimnetry. Example B (the preferred wording for the Middle C Change Trailer according to PER-20 12-153 1) specifies removal of supplemental dosimetry in Step 1. The Appendix 3C of thle TFRCM (Example C), and the postedtraining sequence for removing protective clothing (Attachment 1), both specify the removal of supplemental dosimnetry in Step 6(following the removal of rubber overshoes, outer gloves, hood, and all exposed tape).

The following Options are presented for Forum consideration:

Option #1: Keep the same wording in the TFRCM Appendix 3C (Example C). This sequence is consistent with the current training

undress sequence given to Rad Workers (Attachment 1).

Option #2: Modify thle TFRCM Appendix 3C wording to match that of Example B, or some other variation of undress sequencespecified by the Forum.

Discussion and/or Requirements:TFRCM Article 52 1. 1 states the following:

PART 2 Internal Dosimetry

521 Requirements

1. For the purpose of monitoring individual exrposures to internal radiation, internal dosimetry prograins (including routinebioassay programns) shall /835.402"c,)I be conducted for:

e. The following are example activities that have the potential to result in a committed effective dose of 0. 1 rem or morein a year:

- Work involves the use of a respiratory protection device and involves actually working with, or contact with the radioactivematerial. It is not intended to include occasional observation, unrelated work in the same room, or other activities involvingmuch less risk of contamnination.

* Work occurring in a High Contamination Area which involves contact with or disturbance of the contamination.

" Work with unenicapsulated radioactive material at or exceeding values listed in Tables 5-1 and 5-2. If the task does notinvolve working with the material, such as observing or supervising fiomn a distance or entering a room where the material isstored but not contacting the material itself, it is unlikely that a worker would exceed a 100 mrem unless workplacemonitoring indicates that a loss of control of the material occurred. (See criteria for follow-up bioassay, Article 522.5.)

Issue Summary:A MOP identified wide variances in how the project radiological control offices maintain their field logbooks. Facilities thatoperate 24 hours a day, 7 days a week (222-S and Base Operations) were found to be maintaining the logbooks closest to therequirements contained within TFC-ESHIQ-RP_-ADM-CD-03, "Radiological Control Logbooks", DOE Order 422. 1 "Conduct ofOperations". and DOE-STD-1035-93 "Guide to good practices for logkeeping'.

The PER generated from this MOP states: Additional gu-idance is also needed within TFC-ESHQ-RPADM-CD-03 to addresshow to properly documnent entries within the RadCon Field logbooks.

The Purpose and Scope of the Radiological Control Logbook procedure, TFC-ESI-IQ-RP__ADM-CD)-03 states:

'As written, the current HNF-5 183 requirement requires that Radiological Control personnel maintain logs without the qualifier ofthe Rad Con Standard which requires logs only during continuous or extended daily operations. Therefore, as written, the currentprocedure allowing management direction to allow logbooks to not be used is contrary with the current HNF-5 183 requirement.

Option #1. Eliminate requirement fll and 12 in HNF-5 183, Article 343 without replacement. Without these requiremnents theProjects Could continue to implement to different extents to use of' logbooks or choose not to use logbooks whatsoever representing-the current range of implementation.

Another option would be revise the current procedure to imiplemnent the current requiremient in HNF-5 183 where RadiologicalControl personnel Should m-aintain logs. In this option the purpose and scope of the procedure would be modified to reflect thlerequiremnent, and the body of the procedure changed to eliminlate unnecessary reqluiremfenlts. T'his option would require logbooksin all RadCon organizations. This is the optionl recomm1ended by the P"ER written on this issue.

Option #2: Revise TFC-ESHQ-RP _ADM-C[)-03, "Radijological Control Logbooks" to provide additional guidance andrequirements for the maintenance of RadCon field logbooks.

As a third option, both the requiremients of HN F-5 183, Article 343 and the Radiological Control Logbook procedure Could berevised, In this option, HNF-5 1 83 Article 343 requiremrents would be changed to align with the DOE RadCon Standard requiringlogs only in RadCon organizations with a routinely staffed shift beyond day shift. For these Situations, the Radiological ControlLogbook procedure would be written to set the req~irernents for the logs or logbooks. Use of logs or logbooks beyond these shiftscould be at the discretion of the Project RadCon Managers.

Option 113 Revise Article 343 (1 ) and (2) to reflect the wording of the DOE RadCon Standard. Revise TFC-ESHQ-RPADM-CD-03, "Radiological Control Log)books" to set mandatory mninimium requiremeints for Situations where these logs or logbooks areused.Proposed Outcome:If Option #1 is selected:
