Test name Date

Student name Class

DirectionsUsing the Answer Key for the appropriate Progress Monitoring Test, mark each correct

answer by circling the item number. Mark each incorrect answer by drawing an X through

the item number. Add the number of correct answers in each part. List skills that were not

maintained for further review and practice. To fi nd the percent score, divide the number

correct by the total number of items—in each subtest and for the total test. (For example, a

student who answers 8 of 10 items correctly in Sight Words and Spelling has a score of 8/10;

8 ÷ 10 = 0.8, or 80%.)

After scoring a student’s test, you may want to go back and look at the test items he or she

answered incorrectly. Make note of the skill tested in each item answered incorrectly, and

look for any trends or instructional needs that may be apparent. For example, a student who

misses all of the items testing vowels may need additional instruction in this area.
