I.0 GENERAL INFORMATION 1.1 System Overview
This PARDO/SARDO e-tracking tool, customized and enhanced by the Division ICT Coordinators of DepEd Region VI, is an electronic version of the tracking tool designed to improve the gathering of information that will identify pupils/students at risk of dropping out of school at all levels. This tool intends to provide relevant and accurate information to school heads, teacher advisers, school guidance counselors, and other stakeholders about pupils/students who are at risk of dropping out.
This e-tracking tool aims to achieve the following objectives:a. facilitate efficient and effective means of collecting data in monitoring learners’ information that contribute to school performance; b. consolidate learners’ information that will guide the school authority and stakeholders in determining the appropriate intervention relative to School Drop Out Reduction Program; andc. provide accurate data in determining the risk status of pupils/students in dropping out of school
The minimum requirements for this tool are:a. Operating System –Windows XP Professional b Microsoft Office Excel 2003C. Zoom in or out accordingly to properly display all the objects on the screen

I. Pupil Risk Profile TemplateWARNING!ALL ENTRIES MUST BE ENCODED MANUALLY. DO NOT COPY PASTE!
A. General Information1. Grade – choose from the drop down menu2. Section – type manually3. School Year – type manually4. Name of Teacher – type manually5. School ID- type manually (School Name, District, Division and Region will automatically display)
6. Date filed – type the current date (the date when the form was accomplished)7. LRN – Number will automatically display as soon as LRN and Name of Pupils (typed manually in appropriate box). 8. Gender (chosen from the drop down menu or filled manually) are filled in appropriate box.8. Age will automatically display when date of birth of each pupil is entered manually in appropriate box. Age is the actual age of the pupil as of the encoding date. 9. Links are provided at the bottom of the page to lead the user to the different sheets/templates as indicated:
a. Family Backgroundb. First Grading Achievementc. Attendanced. Participation in Activitiese. Emotional/Personal Adjustmentsf. Health
10. Family Background Sheet consisting the data of LRN, Name of Pupils, and Gender has been automatically generated after completing the General Information Sheet. 11. Scoring Guide is provided for the entry of:a. Educational Attainment:
Only ONE (1) of the following entries is a valid entry:*father AND mother*father ONLY*mother ONLY*guardian ONLY b. Socio-economic statusAverages of the data encoded will be automatically generated when the data are encoded. 12. Quarterly Grading Achievement of the students refers to:a. the grades in the first quarter during the first administration of the tool. b. the grades during the second administration of the tool which is done after the release of the Form 138 in the second rating period A Scoring Guide is provided for this purpose. All learning areas must be filled up completely to generate the average. 13. Quarterly AttendanceQuarterly attendance refers to the number of days the learner is present in the school. Scoring Guide for attendance sheet which contains the quarterly attendance of learners must be filled up using the scale provided. 14. Participation/Interest in Class ActivitiesParticipation /Interest in Class Activities of pupils are based on the scale reflected in the scoring guide. 15. Emotional/personal adjustmentIndicators that are treated as contributing factors for the emotional / personal adjustments of the pupils will be rated based on the scoring guide provided. Note: Numerical values of 1 to 5 are negative statements that indicate negative behavior manifested by the learners. 16. Health Condition of the LearnersThe rating scale for the indicators of the health condition of the learners is reflected on the scoring guide. 17. The PARDO ProfileThe PARDO profile is the summary of the ratings in each category. It will automatically generate the data after the completion of Categories A-F. 18. PARDO SummaryThe PARDO Summary will show the graphical representation of the Risk Level of PARDO by gender.

This PARDO/SARDO e-tracking tool, customized and enhanced by the Division ICT Coordinators of DepEd Region VI, is an electronic version of the tracking tool designed to improve the gathering of information that will identify pupils/students at risk of dropping out of school at all levels. This tool intends to provide relevant and accurate information to school heads, teacher advisers, school guidance counselors, and other stakeholders about pupils/students who are at risk of dropping out.
This e-tracking tool aims to achieve the following objectives:a. facilitate efficient and effective means of collecting data in monitoring learners’ information that contribute to school performance; b. consolidate learners’ information that will guide the school authority and stakeholders in determining the appropriate intervention relative to School Drop Out Reduction Program; andc. provide accurate data in determining the risk status of pupils/students in dropping out of school
