• Completing questionnaires about your experiences• Completing interviews about your experiences (during

parts of these interviews, we will assess heart rate/respiration and skin conductance)

Monoclonal antibodies have played a significant role in the treatment of many autoimmune diseases-MS included. Monoclonal antibodies act by blocking lymphocytes (T and B cells) that are targeting normal tissues. In MS, the normal tissues attacked by lymphocytes are myelin and nerve axons in the brain and spinal cord. In most cases, MS can be controlled by disease modifying therapies such as interferons and Copaxone. Monoclonal antibodies should be used as a second choice if MS is still not under controlled by these therapies.

Monoclonal antibodies available at this time are Natalizumab (Tysabri) and Alemtuzumab (Lemtrada.) The FDA is currently in the process of looking at Ocrelizumab for Primary Progressive and Relapsing-Remitting MS. Under study currently for MS are Rituximab (Rituxin), Daclizumab (Zenapex), and Ofatumumab. Each of these Monoclonal antibodies are unique

in their dosing, side effects, and treatment protocols. For example, Lemtrada is dosed intravenously yearly for 2 years and Ofatumumab is given subcutaneously every 4 weeks. Considering if you are a candidate for a Monoclonal antibody, as well as which one is best in your situation, is an individual decision between you and your physician.

Oak Clinic is participating in the Relapsing-Remitting trial for Ofatumumab. There are very detailed criteria for qualifying for this study such as treatment history, MRI results, and age. Please discuss questions regarding Monoclonal antibodies with your doctor during your visits, or call and ask for Research.

It’s Cold and Flu Season! If you are not feeling well, or think you might be coming down with a cold or the flu, please cancel your infusion treatment and reschedule! You need to take care of yourself and not expose others to your illness. Here are a few tips to help you avoid getting the flu this season:• Wash your hands regularly and often or use an alcohol based hand sanitizer.• Avoid getting close to people that are sick; for example, don’t shake hands.• Live a healthy lifestyle-get exercise, plenty of rest, use good nutrition, etc.• Stay hydrated!• For most, it is best to get an influenza vaccine but check with your primary doctor if you have questions.

We say Good Bye to Jane and Hello to Mallory! In mid-September, Jane Waterman retired and we wish her all the best! The Infusion Clinic welcomes Mallory Neumann, RN and she will be working with Sue Shannon and Renee Luke to treat patients. Mallory graduated from Walsh University and worked as a nurse in neurology before joining the Oak Clinic Infusion Team. She and her husband Zachary welcomed their daughter, Lola, in June of this year.

In February 2015, the Robert O. and Annamae Orr Foundation funded a Bladder Clinic within The Oak Clinic. Since the opening of this clinic, the staff has become more aware of the incontinence issues experienced by MS patients. Here are the answers to questions that are often asked:

WHY DO MS PATIENTS EXPERIENCE BLADDER PROBLEMS?

Multiple sclerosis (MS) is a disease that affects the central nervous system. In a patient with MS, the immune system attacks the protective material that surrounds the nerve cells. This causes damage and results in symptoms that may include bladder dysfunction. Up to 90 percent of MS patients experience some issues with bladder function, according to the Cleveland Clinic.

Why do MS patients have bladder storage problems?Bladder storage dysfunction is a symptom of overactive bladder. This means that the nerve damage within your body has caused your bladder muscle to contract more often than it should. The spastic contractions make you feel as if you need to urinate more frequently than you actually need to.

Symptoms of bladder storage dysfunction include the following:• strong urge to urinate• frequent need to use the bathroom• the need to get up several times at night to urinate• the inability to control your urination (also referred to as incontinence)

How can a patient manage bladder issues?Simple lifestyle changes can often help you manage the symptoms of bladder emptying and storage problems caused by MS. Avoid bladder irritants, which include cigarettes, caffeine, artificial sweeteners, and alcohol.Drink plenty of fluids, but avoid sipping a drink slowly. Drink each glass all at once to avoid frequent urges.Stop drinking two hours before you go to bed. If you have trouble emptying your bladder completely, wait several minutes after each time you urinate, and then try again. Use pads for incontinence or times when you know that you’ll be unable to access a bathroom immediately.

When should an individual seek help?A bladder emptying dysfunction can lead to a urinary tract infection (UTI). When your bladder doesn’t fully empty, you run the risk of developing a UTI because the retained urine left over in the bladder allows bacteria to grow. UTIs associated with MS are likely to recur, especially if you don’t get treatment for the emptying dysfunction.

Symptoms of a UTI include:• urgent need to urinate• frequent urination• burning feeling when you urinate• pain in your lower back or lower abdomen• fever• dark urine with an unusual smell
