Packet 4 for K1 visa applicants

K1 visas are based on petitions made by U.S. citizens for their fiancée/fiancé. This type of visa

allows the beneficiary to travel to the United States to marry the petitioner and later file a change

of status from non-immigrant to immigrant status. This information packet contains 14 pages

(English and Spanish version); please read the entire packet carefully.

In order to process quickly and easily your visa application, please make a packet of the

following documents for each visa applicant:

Pictures and passport: You must bring two pictures 5x5 cm. The pictures must have been

recently taken. The photographs must show the applicant's entire face on a white background.

Applicants should not wear glasses or hats for the photographs. The passport must be valid for at

least 1 year.

Original Birth Certificate: If born in Ecuador and your birth was filed in the Civil Registry

Office, we will only accept the hand-written certificate with the official seal; if your birth was

filed in the Corporación Registro Civil de Guayaquil we can accept the printed version

certificate with the official seal (Please see document samples in page 11). We do not accept

computerized or typed forms issued by the Civil Registry Office.

Original Divorce or Death Certificates: If previous marriages were executed in Ecuador, you

must bring the marriage certificate that is a direct copy of the hand-written record book of the

Civil Registry and must have their official seal (Please see document sample in page 12). If

they were executed in another country, please bring the divorce decree with the court’s original

stamp. Death certificates (if applicable) must be issued by the Civil Registry Office and must

have their official seal (Please see document sample in page 13).

Police Records: Records must be obtained from all the countries where you have lived more

than 6 months since the age of 16, except for Ecuador and the United States.

Medical Examination: All the information pertaining to the medical examination can be found

in page 4.

Evidence of Financial Stability from the United States: You may bring any documents that

prove that the applicant will not become a public charge in the United States such as bank letters,

Affidavit of Support I-134 Form (with original signature), last income tax report (1040), W-2.

DS-156 Form: This form must be filled out on-line and then printed from our web site:

DS-156K Form: This form must be filled out only by the fiancée or fiancé of a U.S. citizen. It

is not needed for children. You may download this form from our web site:


Proof of Relationship with the Petitioner: The following types of documentation may help to

prove your relationship:

Photos from you and your fiancée/fiancé from your visits or trips together

Correspondence exchanged between you and your fiancée/fiancé (letters, e-mails)

Telephone bills with calls from the petitioner to the visa beneficiary

Copy of passport pages from any trips your fiancée/fiancé has taken to visit you, along

with the biographic page

Note: You are not required to bring all of these documents. Please bring any evidence

you feel convincingly shows that you are in an ongoing relationship.

U.S. visas previously issued: If you have obtained any type of U.S. non-immigrant visa (for

example, tourist visas, student visas, work visas, etc.) you must bring those with you to your

interview.

Fee: You must pay the Visa Application Fee of $240 (Two hundred and forty US dollars) prior

to scheduling an appointment. You have three options to complete the payment:

Option 1 – Online payment: You may pay the application fee at http://ecuador.usvisa-

info.com by using your credit card at the time you schedule your appointment. Only Visa

and MasterCard are accepted.

Option 2 – Bank payment: You may pay the application fee at any Servipagos or

Banco ProCredit branch. If you pay your application fee at Servipagos or Banco

ProCredit, you must initiate the appointment scheduling processing online at

http://ecuador.usvisa-info.com. Then you will print the deposit slip displayed on screen

showing the amount to pay at Servipagos or Banco ProCredit. Take your deposit slip to

any Servipagos or Banco ProCredit branch to complete the payment. After making

payment you will go back to the appointment service website at http://ecuador.usvisa-

info.com and proceed to schedule an appointment by providing the payment receipt

number.

Option 3 – Phone payment: If you choose to schedule your appointment by phone

through our Call Center at 04-259-6800 from Ecuador or at 1-703-439-2331 from the

United States, you may pay the application fee by using your credit card. Only Visa and

MasterCard are accepted.

Appointment: Once you have all the required documents, please visit http://ecuador.usvisa-

info.com to pay the Visa Application Fee, to schedule an appointment and to arrange for the

delivery of your passport. Please follow these steps:

STEP 1: Click on “Log In” in the upper right corner of this website.

STEP 2: Read the disclaimer, check the box and then click “Confirm”.

STEP 3: Input your surname, given name, passport number, date of birth and nationality to

register your account. Type the two words that will appear as a security confirmation.

STEP 4: Click on “Schedule Appointment”, select “Immigrant Visas”, and then select the visa

type you are applying for. Complete your personal information based on the information in your

passport and include your case number (e.g. GYQ2010123456).

STEP 5: Select the “DHL Branch” where you would like to retrieve your documents. Please

do this for ALL applicants that will be applying for a visa (including children).

STEP 6: Select a date and time for your interview in the calendar displayed, click on “Select”

and click on “Schedule Appointment” on the next screen.

STEP 7: Print your appointment confirmation sheet sent to the email address you provided and

bring it to your interview.

You may also schedule an appointment and arrange your passport delivery via telephone by

contacting our Call Center at 04-259-6800 from Ecuador or at 1-703-439-2331 from the United

States.

Document Delivery: All passports and supporting documents will be returned to the applicant

via DHL, at no additional cost. Go to http://ecuador.usvisa-info.com and follow the steps

mentioned above. Please do this for ALL applicants that will be applying for a visa (including

children). Failure to comply with this requirement BEFORE attending your interview will cause

a delay in the return of your passport.

IMPORTANT NOTE: Please do NOT make any travel arrangements, purchase airline tickets,

dispose of any property, or give up employment until you have received your immigrant visa.







Medical Examination

All immigrant visa applicants, regardless of age or gender, must have a medical exam.

All medical exam expenses are paid for by the applicant.

To get a medical exam, take your passport and two regulation photographs to one of the

doctors listed below. You should also take any vaccination certificates (record of

immunizations) you have. The doctor will notify the applicant if he/she is missing any

immunizations and what steps to take. The doctor will make the necessary arrangements

to perform a chest x-ray, blood test and general health exam.

After the examination, the doctor will provide a sealed envelope containing the medical

exams. You must present this sealed envelope to the Consular Section of the U.S.

Consulate General in Guayaquil at the date and time of your appointment.

The medical exams will take a minimum of 72 hours. Keep this in mind when planning

your medical exams in advance of your appointment at the U.S. Consulate.

Adults: The cost for the medical exam, including chest x-ray, blood tests and general

health exam, is $110 (One hundred and ten US dollars). This cost does not include

immunizations, if necessary.

Children: Children under the age of 15 do not need chest x-rays or blood tests unless

directed by the doctor. The cost for children's medical exams is $40 (Forty US dollars).

This cost does not include immunizations, if necessary.

Note: K1, K2, K3, K4 visa applicants does not need immunizations

Prohibited Items

The Consular Section is implementing new security procedures to speed interview processing.

In order to minimize the amount of time applicants have to wait in line before their interview; the

American Consulate General will prohibit applicants from entering the building with anything

other than the documents they need for their application.

Applicants should make arrangements before coming to the American Consulate General, so that

the only things they have with them when they arrive are their documents. Purses, bags,

suitcases, backpacks, cellular phones, cigarettes, lighters, matches, food or beverages,

photographic or video devices, radios, computers, nail trimmers, scissors and weapons will not

be permitted to be entered to this Consulate.

Limited baby supplies will still be admitted.

FAILURE TO COMPLY WITH THESE INSTRUCTIONS WILL DELAY YOUR VISA APPLICATION




Paquete 4 para aplicantes a visa K1

Las visas K1 están basadas en peticiones hechas por ciudadanos de los Estados Unidos para su

novio(a). Este tipo de visa le permite al beneficiario viajar a los Estados Unidos para casarse con

el peticionario y luego ajustar su estatus a inmigrante. Este paquete informativo contiene 14

páginas (versión en Español e Inglés); favor lea el paquete completo.

Para procesar su aplicación de visa de una manera ágil y rápida, favor haga un paquete con los

siguientes documentos para cada solicitante:

Fotografías y pasaporte: Debe traer dos fotografías a color, de 5x5 cm. Las fotografías deben

ser tomadas recientemente. Deben ser tomadas de frente y en fondo blanco. El pasaporte debe

estar válido por lo menos 1 año.

Inscripción de Nacimiento Original: Si nació en el Ecuador y su nacimiento fue registrado en

el Registro Civil, únicamente aceptamos una copia certificada del libro del Registro Civil, escrita

a mano y con el respectivo sello; si su nacimiento fue registrado en la Corporación Registro

Civil de Guayaquil podemos aceptar la versión computarizada con el respectivo sello (Favor

revise el documento en la página 11). No se aceptan formas tipiadas o computarizadas emitidas

por el Registro Civil.

Certificado de Divorcio o Defunción Original: Si tuviera un matrimonio anterior realizado en

el Ecuador, debe presentar la inscripción de matrimonio con la marginación de su divorcio.

Únicamente aceptamos una copia certificada del libro del Registro Civil, escrita a mano y con el

respectivo sello (Favor revise el documento en la página 12). Si el divorcio se realizó en otro

país, debe presentar el decreto con los sellos originales de la corte. El Certificado de Defunción

debe ser emitido por el Registro Civil y debe tener su respectivo sello (Favor revise el

documento en la página 13).

Record Policial: Debe traer los records policiales de todos los países en donde haya vivido por

más de 6 meses después de los 16 años, excepto del Ecuador y los Estados Unidos.

Examen Médico: Toda la información relacionada al examen médico la encuentra en la página

#9.

Documentos de Solvencia Económica de los Estados Unidos: Debe traer cualquier documento

que pruebe que el solicitante no se convertirá en una carga pública en los Estados Unidos, como:

cartas del Banco, Affidavit of Support Forma I-134 (firmado en original), declaración de

impuestos del último año (1040), W-2.




Formulario DS-156: Este formulario debe completarlo vía electrónica y luego imprimirlo de

Formulario DS-156K: Este formulario debe ser completado sólo por el novio/a de un

ciudadano americano. No es necesario para los hijos. Puede imprimirlo de nuestra página web:

Pruebas de Relación con el Peticionario: Los siguientes documentos pueden servir para

demostrar su relación:

Fotografías de usted y su novio/a en sus visitas o paseos realizados juntos

Correspondencia enviada entre usted y su novio/a (cartas, e-mails)

Planillas telefónicas que muestren sus llamadas

Copia de todas las páginas del pasaporte de su novio/a para verificar sus visitas. Traiga

también una copia de la página biográfica del pasaporte del peticionario

Nota: No es necesario que traiga todos estos documentos. Favor traiga cualquier tipo de

evidencia que usted considere pueda mostrar que tiene una relación continua con su

pareja.

Visas anteriores a los Estados Unidos : Si a usted se le ha emitido una visa de no inmigrante

para los Estados Unidos (por ejemplo, visa de turismo, visa de estudiante, visa de trabajo, etc.)

debe traerla el día de su entrevista.

Costo: Debe cancelar el costo de la aplicación de la visa de $240 (Dos cientos cuarenta dólares)

antes de programar la cita. Usted tiene tres opciones para realizar el pago:

Opción 1 – Pago en-línea: Usted puede cancelar el costo de la aplicación con tarjeta de

crédito en http://ecuador.usvisa-info.com al momento que programa su cita. Unicamente

se acepta Visa y MasterCard.

Opción 2 – Pago en Banco: Usted puede cancelar el costo de la aplicación en cualquier

agencia de Servipagos o Banco ProCredit. Si usted cancela el costo de la aplicación en

Servipagos o Banco ProCredit, usted debe iniciar la programación de su cita en-línea en

http://ecuador.usvisa-info.com. Usted deberá imprimir la papeleta de depósito que

aparecerá en pantalla, la cual indica el valor a cancelar en Servipagos o Banco ProCredit.

Lleve su papeleta de depósito a cualquier agencia de Servipagos o Banco ProCredit para

realizar el pago. Después de realizar el pago debe volver a ingresar a

http://ecuador.usvisa-info.com y continuar con la programación de su cita ingresando el

número de recibo de su pago.









Opción 3 – Pago telefónico: Si usted programa su cita vía telefónica con nuestro Call

Center al 04-259-6800 desde el Ecuador o al 1-703-439-2331 desde los Estados Unidos,

usted puede cancelar el costo de la aplicación utilizando su tarjeta de crédito.

Unicamente se acepta Visa y MasterCard.

Cita: Una vez que tenga listos los documentos mencionados anteriormente, ingrese a

http://ecuador.usvisa-info.com para cancelar el costo de la aplicación a la visa, programar una

cita y para registrar la entrega de su pasaporte. Favor siga los siguientes pasos:

PASO 1: Haga click en “Ingresar” en la parte superior derecha de esta página web.

PASO 2: Lea los términos del acuerdo, marque el casillero y luego haga click en “Confirmar”.

PASO 3: Ingrese sus apellidos, nombre, número de pasaporte, fecha de nacimiento y

nacionalidad para crear su cuenta. Escriba las dos palabras de seguridad que aparecerán.

PASO 4: Haga click en “Programar cita”, seleccione “Visas de Inmigrante” y seleccione el

tipo de visa al cual este aplicando. Complete su información personal usando la información de

su pasaporte e incluya su número de caso (por ejemplo GYQ2010123456).

PASO 5: Seleccione la “Sucursal de DHL” donde usted quisiera retirar sus documentos. Favor

hágalo por CADA persona que este aplicando a una visa (incluyendo los niños).

PASO 6: Seleccione la hora y fecha de su entrevista en el calendario que aparecerá, haga click

en “Programar cita” en la siguiente pantalla.

PASO 7: Imprima la hoja de confirmación de su cita que le fue enviada al correo electrónico

que usted registro y tráigala el día de su entrevista.

Usted puede también programar una cita y registrar la entrega de su pasaporte vía telefónica

contactándose con nuestro Call Center al 04-259-6800 desde el Ecuador o al 1-703-439-2331

desde los Estados Unidos.

Envío de Documentos: Todos los pasaportes y documentos serán devueltos al aplicante por

DHL, sin ningún costo adicional. Ingrese a http://ecuador.usvisa-info.com y siga los pasos

mencionados anteriormente. Favor hágalo por CADA persona que este aplicando a una visa

(incluyendo los niños). Si usted no cumple con este requisito ANTES de asistir a su entrevista

habrá retraso en la entrega de su pasaporte.

NOTA IMPORTANTE: Favor NO haga planes de viaje, compre tickets aéreos, venda sus

propiedades, o renuncie a su trabajo hasta que reciba su visa de inmigrante.






Examen Médico

Todos los solicitantes de visa de residencia, no importa la edad ni el sexo, deben pasar

por un examen médico. Todos los gastos de los exámenes médicos serán pagados por el

solicitante.

Para realizárselo, lleve su pasaporte y dos fotografías al consultorio de uno de los

médicos del listado adjunto. Lleve también su carnet de vacunación si lo tuviere. El

médico le hará saber si alguna vacuna le hace falta. El médico hará los arreglos

necesarios para realizarle una radiografía del tórax, un examen de sangre y un examen

general completo.

El médico le entregará los resultados del examen en un sobre cerrado que debe ser

entregado en la sección consular de este Consulado el día de su cita.

Este examen tomará un mínimo de 72 horas para obtener el resultado, por lo tanto haga

con tiempo su cita con el médico.

ADULTOS: incluye radiografía del tórax, examen de sangre y examen general completo.

El costo es $ 110 (Ciento diez dólares). Este costo no incluye vacunas.

NIÑOS: los niños menores de 15 años no necesitan radiografía del tórax, ni examen de

sangre a menos que el doctor lo crea necesario. El costo es $ 40 (Cuarenta dólares). Este

costo no incluye vacunas.

Nota: Aplicantes a Visa K1, K2, K3, K4 no necesitan vacunas

Artículos Prohibidos

La Sección Consular ha implementado nuevos procedimientos para agilizar el proceso de

entrevista. Para poder reducir el tiempo que lo solicitantes tienen que esperar en fila antes de ser

entrevistados, el Consulado Americano prohibirá el ingreso de aquellas personas que traigan

cualquier pertenencia, material u objeto adicional que no sean los documentos requeridos para

aplicar para la visa.

Los solicitantes deberán planificar antes de venir al Consulado Americano para traer únicamente

los documentos requeridos. No se permitirá el ingreso de carteras, bolsos, maletines, mochilas,

teléfonos celulares, cigarrillos, encendedores, fósforos, alimentos ni bebidas, cámaras

fotográficas o de video, radios, computadoras, cortaúñas, tijeras, ni armas.

Artículos para asistencia de bebés serán aceptados.

NOTE: If you are not able to get a direct copy from the Civil Registry because the books are

lost or have suffered any damage you must bring a letter from your local Civil Registry office

including the reasons why they couldn’t provide you a direct copy. You must also bring a

direct copy of your birth/divorce certificate from the National Files’ books from the Civil

Registry’s main office in Quito.

NOTA: Si no pudo conseguir una copia certificada del Registro Civil porque los libros están
