DEAR LINGUAL ORthODONtIC fRIENDS,

It is a great honor and pleasure to welcome so many lingual orthodontic practitioners and speakers in Cernobbio, Lake Como, on the occasion of our 11th ESLO Congress.

I feel very proud to be “your” President! thank you for joining us here!

Lingual Orthodontics has made major impro-vements in various areas related to our spe-cialty such as CAD/CAM, customized brackets and archwires, self-ligating brackets, indi-rect bonding, mechanics, tADs, and multi-disciplinary treatments. this invisible form of treatment has significantly boosted adult and teenager orthodontics, thus reflecting the aesthetic needs of our contemporary society.

I would like to extend my warmest greetings to you all at this year’s Congress, the most important and exciting event in the calendar for our society. the ESLO Scientific and Orga-nizing Committees have made any effort to ensure you have a pleasant stay at Lake Como from every point of view: scientific, collegial, leisure and accommodation. I wish to thank very much both Committees for their great work!

the last Congress held in frankfurt showed fantastic and highly appreciated lectures and presentations.

this year the ESLO Scientific Committee is pleased to announce Germain Becker (Luxemburg), Robbie Lawson (UK), Esfandiar Modjahedpour (Germany), Roberto Stradi (Italy) as Chairing the Pre Congress course. furthermore, it has awarded the “Alain fon-tenelle Memorial Lecture” to Didier fillion. twenty one Key Note Speakers from all over the world will give us the last update in Lin-gual technique.

I would like to take this opportunity to thank many friends and colleagues for their incre-dible efforts and support which will make the event such a memorable success.

I am particularly indebted to my friend and Past President Germain Becker, for his devo-tion, tireless support and amazing help, wit-hout whose constant support the Congress would not be a successful event.

Many thanks to our Past President thomas Drechsler, the President Elect takis Kanare-lis, and the Secretary Guillaume Lecocq, for their constant support and advices. A tribute is also in order for our treasurer, Michel Steil, for his fantastic efforts in handling ESLO´s financial affairs.

Also, I must thank the entire Colloquium team for the perfect congress organization as well as the numerous industry partners who have sponsored our event with their generous donations.

I do hope that you will enjoy our time toge-ther in Cernobbio for a fantastic congress in a beautiful and modern location.

/ aim:How to optimize the use of the plaster set-up.

/ methods : the endings in lingual treatments can be jeopardized by the inflammation of the gum and to install a fix retainer is often impossible. In our practice we make a print of the plaster set-up to create an aligner which we put directly after the debonding of the lingual braces. the patients will wear the aligner until the gum is less swollen which will also give them time to a dental cleansing before the 3 to 3 in fiber.

/ resuLts : A perfect fitted aligner is put into the patient’s mouth who is amazed as no print has been made. Sometimes the aligner needs some adjustment.this system can also be used when patients can’t handle the braces any longer and the treatment is nearly finished ; the aligner will correct the one or two teeth which are not completely in the right position.

/ concLusion : this system works between 80 and 90% of the cases but it has its limits when there are a lot of strippings and extractions or when patients do not take care of their mouthes and there are many debonded brackets and too much tartar.

the newest and the latest bonding tray that is possible to order for Incognito brackets in Bad Essen, Germany is a 2-layer translucent tray that looks attractive and works with chemically- as well as light-cured raisins. A technique that is used in our office for buccal bonding for more than 10 years with great success. With the CPt the bracket placement on the malocclusion model is now also done digitally and the part of the manufacturing process where the brackets were placed manually is now eliminated.I shall demonstrate how to use this tray stress-free and what kind of advantages this new tray has. this lecture includes a video of a patient getting her upper and lower teeth bonded in real time.

Nowadays the aesthetic outcome is one of the major goal in orthodontics; as more and more adult patients seek a less visible therapy to ad-dress aesthetic issues, lingual orthodontics is the most efficient and hidden option available today. As Orthodontist we’ve always tried to obtain all the treatment objectives (function ,health, stability, aesthetics), but today as entrepreneur we have also to consider custo-mer expectation and perceived performance rating as one of the main goal as an appliance is efficient when it works, in the shortest time with patient’s satisfaction. In fact some patients don’t need and many of them don’t require a complete orthodontic treatment: in cases of post-retention relapses, minor spaces closure, mild crowding correction and limited correction of deep bite (known as Mi-nor tooth Movements therapies) easy lingual orthodontics with 2d lingual brackets can be performed successfully and therefore can be the most appropriate therapeutic tool. how it may complete the services of a contempo-rary dental office? Lingual orthodontics has always been asso-ciated with fear of debondings/rebondings, patient comfort, wire bending, better finishing and also the cost. In the quest to make lingual popular and acceptable treatment moda-lity customisation of appliances is the future .however they need to be more user friendly & cost effective. the titanium iLingual 3d is a step towards this effort.we will describe a system iLingual 3D tita-nium CAD CAM customized straight wire lin-gual appliance which offers 2 slot options a ribbon arch rectangular slot 0.022” x 0.016” or square slot 0.0175” x 0.0175”. and the first appliance to be produced in a 100% medically pure titanium alloy tilite. the process involves scanning with 3Shape R 700 high resolution scanner and virtual setup in Ortho analyser software.the doctor approved set up files are imported into the proprietory iLingual software for virtual placement of iLingual brackets. the numbered bracket files are sent to a Rapid prototyping machine which gene-rates resin prototypes of the braces which are then casted in tilite titanium based alloy and finished with precise slot to wire equation. the tilite titanium alloy is a 100% medically pure alloy with cast-ability as good as gold.the iLingual software also gives us the final straight/ plain archwire design on which wires are prebent and provided in sets of every case.

Patients may attend your office and highlight that they are not happy with their smile, even when you feel your treatment has been suc-cessful. When looking at the patient it may not be apparent to you what the issue is. So how can you see what your patients see? In this presentation we shall show how you can see your patients smile through their eyes. this may help identify possible concerns the patient has with their smile that may not be apparent when you observe the patient your-self and, help create satisfied patients.

/ MetHOD: We presented 10 Clinicians with printed 10 images of non symmetrical dental irregularities.

/ ResULts: We found that when the ‘problem’ was on the left the detection rate was higher than when on the right.

/ CONCLUsiON: It is apparent that patients of-ten see their smile different to their clinician. this has been proved by a simple experiment that illustrates how we read a smile. Abnor-malities are noticed more on the left than the right. Subsequently we suggest a simple pro-cedure so that you can see exactly what your patients are seeing when they observe their smile

/ aiM: the use of implant anchors for abso-lute anchorage has recently become an es-sential part of lingual treatment as it allows the movement of teeth and dental arches without intermaxillary elastics and minimizes damage to the periodontal tissues. Among dif-ferent types of temporary anchorage devices currently available, mini-screws are often used because of minimal patient discomfort, minimal surgical trauma, and relatively low cost. the purpose of this presentation is to introduce lingual mechanics using implant anchors through clinical cases.

/ MetHODs: Mini-screws, 1.3 mm in dia-meter and 6-9 mm in length, were placed in the maxillary palatal and mandibular buccal alveolar bones to provide absolute anchorage for tooth movement. Results: the following could be achieved with the use of anchor screws: 1) anteroposterior control – move-ment of the entire arch, molar distalization, and molar mesialization, 2) vertical control – intrusion of the entire arch, intrusion of upper and lower molars, asymmetric intru-sion of molars, and intrusion of the anterior segment, 3) transverse control – coordination of the width of the upper and lower arches, correction of upper and lower dental midlines, and correction of occlusal plane cant.

/ CONCLUsiON: the use of anchor screws enables us to correct malocclusions, improve incisor display with changes in occlusal plane cant, and produce facial changes without or-thognathic surgery.

Most adults suffer from the constriction on the maxillary arch. If the constriction and the crowding are limited, lingual orthodontics solves the problem by inserting a .018x.025 SE NItI or .0182x.0182 full-sized tMA wire in the maxilla to maximize lateral alveolar develop-ment. If the crowding is severe, orthodontists either extract teeth, or ask the surgeon to help them. Otherwise help is always required in the case of a narrow maxilla.

the expansion with the « Le fort 1 » should be limited to 4 mm if we want a stable result.

the Surgical Assisted Rapid Maxillary Ex-pansion (SARME) also gives us more expan-sion with better stability. Nevertheless, the posterior skeletal widening in this conventio-nal distraction is poor compared with anterior expansion.

Since Brown in 1938, only tooth-bone dis-tractors have been available. thanks to Mom-maert, the bone-bone versions have been available since 1999.

Why speak today about bone distraction in lin-gual orthodontics?

When it is not possible to treat transversal palatal discrepancy only with lingual brackets and full-size wires, usually orthodontists se-lect a tooth-bone distractor soldered on bands before prescribing LefORt 1 or a SARME sur-gery for their patient. In order to avoid doing so, we are going to present to you a new bone-bone distractor with minimal mucosal scar-ring and totally compatible with lingual brac-kets and complete Lefort 1 surgery.

On the other hand, some of our patients have complex symptoms of malocclusion, not only transversally but also sagittally and verti-cally. With conventional SARME, we can treat transversal palatal discrepancy by distraction

osteogenesis. however, this surgery is incom-patible with impaction and advancement of the maxilla, or with the advancement of the mandible at the same time if necessary. the-refore, two surgical procedures are needed, which makes it difficult for patients to accept the treatment.

to solve these problems, we developed in 2008, a new concept of low resistance distrac-tion ; two new tools were designed and crea-ted in collaboration with the Deltex company : first, our new bone-bone distractor that can be sagitally adjusted was produced. Although brackets positioning methods, which include treatment of set-up models as a labo-ratory phase, are laborious, they have several certain benefits. We have an opportunity to pass though every stage of orthodontic treat-ment before all manipulations with real denti-tion will be started. Virtual treatment with set-up models fixed in central relationship (CR) provides more predictable and stable results of further orthodontic therapy.Original dental casts mount down into an arti-culator with face bow and bite block in central occlusion (CO). By manipulation in an arti-culator with certain occlusion guides we can locate CR, which should be hold by a new bite block. Set-up models can now be mounting in an articulator with same face bow and CR occlusion register. During the virtual treat-ment we can create new functional occlusion which will match with good condylar position. By using the modified hiro method of brackets positioning, we can use templates (018x025 stainless steel) as a final arch wires with full bracket engagement in the last stage of treat-ment.Our approach to orthodontic treatment of Class II and dental asymmetry was radically changed by brackets positioning in CR. Class II dental compensation (camouflage with up-per bicuspids extraction) or asymmetric ex-traction are changed to non-extraction treat-ment or 4 bicuspid extraction when we deal with oversized teeth.

this communication shows several cases treated with a new lingual system.

this new evaluated concept of lingual treat-ment can be easily used in anterior 2D esthe-tic cases (social six) and with 3D concept control to treat severe malocclusions. It com-bines four different designs: anterior brackets with vertical slot, bicuspid brackets with hori-zontal slot, bicuspid tubes and tubes for mo-lars. Bicuspid brackets with open horizontal slot enable excellent tooth control in all mo-vements except for rotation. thus, in severely rotated teeth, we recommend using square tubes specifically designed for the bicuspids. With this alternative, the orthodontist has perfect control in all stages of the treatment. the bicuspid tube can also be used to hold the arch efficiently in esthetic anterior cases and allows low friction mechanics. In esthetic anterior cases and in 2D mechanics, we re-commend direct bonding or indirect bonding. In the case of 3D lingual mechanics, we use manual or digital set up.

A simple and effective new lingual system has been presented. It gives the orthodontist an easy way to treat his patients and has high rates of patient acceptance since it is comfor-table and brings excellent results.

Situations of severe anterior crowding gene-rally are indicative of premolars or lower incisor extraction. however, lingual ortho-dontics permits better control and there are evidences that stripping contributes signifi-cantly to conservative treatment. this case report describes an adult patient whose initial situation showed molar Class I, severe ante-rior crowding in lower arch and moderated in maxillary arch, constricted arches, modera-ted deep bite, straight profile.the goals were changing the arches shape with slight expan-sion, keep the Class I relationship and interca-nine distance, anterior crowding dissolution, establishing proper overbite and overjet.

the treatment option was lingual orthodontics using Prieto brackets, simplified indirect bonding system and straight wire. Initially, it was not possible bonding all brackets due to lack of space. So, were made stops and the archwire was bonded directally on lingual surfaces without brackets in some regions. In sequence, were used Niti coilsprings. the archwires shape has been worked carefully and was performed the interproximal strip-ping. the archwire sequence was: Niti 0.012; Niti 0.014; tMA 0.016, SS 0.016; tMA 0,016 x 0,022, SS 0,016 x0,022. It was indicated 2 mini implants positioned in palate to contribute for anchorage and torque, allowing treat-ment finalization within 18 months. Although the brackets favors the straight wire, it was necessary bends to reach suitable teeth posi-tioning.

the results were satisfactory contributing to functional occlusion, smile and face aesthe-tics. It was noted excellent torque control of mandibular incisors, improved dental arches shape, the intercanine distance was respected and periodontal status was favored.

Orthodontic mini implant (OMI) has opened a new horizon for orthodontists worldwide. It provides a maximum anchorage which expanded the range of tooth movement and gave a lot of potential for a variety of cases in current uses. Due to growing interests in orthodontic treatment and esthetics in post-adolescent population, lingual orthodontics have become a treatment of choice for many patients nowadays. Usage of OMIs is one of the many important aspects that must be considered also in lingual orthodontics.

Practitioners must be aware of anatomi-cal, mechanical and technical differences in applying OMIs in conventional and lingual orthodontics. In maxillary, there exists anato-mical difference between buccal and palatal alveolus, about bone, soft tissue and adjacent structure which makes another guideline for OMIs. And large area of hard bone in the mid-palatal suture area offers more options for treatment design that makes variable mecha-nism for variable cases. In mandible, howe-ver, employing OMIs in lingual area has many limitations due to anatomical considerations and patient compliance issues and buccally inserted OMIs in lingual orthodontics give another considerations for pertinent force mechanism.

A good understanding of where to insert OMIs provides the appropriate guidelines for selec-tion of OMIs, and a good understanding of a force mechanics for the treatment with those OMIs gives the variety of methods can be used.

We have obtained absolute anchorage using temporary anchorage devices (tADs) and have been able to achieve accurate and reliable treatment results. however, a lot of tADs were needed in some complicated and intrac-table cases. In addition, reinstallation of tADs or additional tADs were needed for cases in which the direction of teeth movement was changed.

We developed simple and easily managed tADs (i-station) that allow teeth to be moved freely using only two screws. In this system, the upper units can be changed freely, and teeth can be moved three dimensionally using a lever arm or wire. there are two types of i-station available; one is set on midpalate in the maxilla and the other is put on the external oblique ridge in the mandible. these devices have enabled treatment of intractable cases using orthodontic treatment alone, when conventionally such cases could have been treated only by surgical orthodontics.

these tADs could have the potential of broadening the range of orthodontic treat-ment.

/ aiM: Orthodontic treatment using fixed appliance takes a long time when compared to other dental treatment. thus, dentists are typically trained with wire bending exercises using the wax typodont system to visualize quick tooth movement. After applying ortho-dontic force to the typodont teeth, heat is typically applied to the wax as well as artifi-cial teeth to allow tooth movment. this type of conventional typodont heating system is good for wire bending exercises. however, it does not reflect realistic tooth movement when compared to a real patient. this is so because the heat softens the wax first before the teeth can be moved in the softened wax. thus, un-like tooth movements occur always.

/ MetHOD: to address this issue, I developed the heat Induction typodont System (hItS®) to apply heat to the metal teeth without any electric wires to the individual tooth.

/ ResULts: Unfortunately, there still exists no perfect model to simulate orthodontic tooth movement in vitro. Although hItS® cannot simulate orthodontic tooth movement as seen in a real patient, it could provide a more rea-listic simulation than any other conventional wax typodont system.

/ CONCLUsiONs : thus, I am confident that this novel hear induction typodont system can be a new tool to simulate tooth movement in orthodontics

/ aiM: Due to the scarcity of the actual sta-tistical reports based on the overall treat-ment result comparisons of the physiological variance regarding finite elemental analysis and biomechanics of labial and lingual ortho-dontics in the past, a comparative measure-ment of labial and lingual orthodontics was done to represent the unique qualities of lin-gual orthodontics incorporating the pre and post treatment of lateral cephalogram and dental casts. A further consideration of va-riance during retention period was weighed in order to take this variance to an advantage by considering the data beforehand of the treat-ment plan as well as in the set-up model.

/ MetHOD: 20 patients of various cases were examined. Comparative evidence of diffe-rence within the treatments of various cases, between lingual and labial orthodontics, ex-traction / non-extraction cases, respective to the change in the upper and lower incisal axis and the dental arch width were noted.

/ ResULts: Regarding the change in the upper and lower incisal axis and the dental arch width, there were significant differences between labial and lingual orthodontics. Even though the evidence was present and noted, the variance during the retention periods of either labial or lingual treatment were within clinically acceptable tolerance.

/ CONCLUsiON: the study resulted in a sta-tistical data that identified the variance. this variance can be taken advantage of by consi-dering the data beforehand of the treatment plan as well as in the set-up model.

Lingual treatment has to sustain its position in orthodontic treatment. trapped between the fast and popular labial treatment with self ligating brackets and the more comfortable Invisalign treatment the most substantial ar-guments for lingual treatments are invisibility combined with reliable results and probably a short treatment time as well. But how fast can lingual treatment be?

this presentation shows the results of an evaluation of more than 100 consecutively finished lingual cases using selfligating brac-kets and customized SureSmile wires. It is examined what kind of influence piezosurgery and finishing with positioner and aligner could have on lingual treatment time.

Depending on the malocclusion there are significant differences in treatment time between the several groups of the evaluation. Customized wires, a digital treatment simula-tion and a digital treatment plan as well sup-port the orthodontist in enhancing treatment time of lingual cases. In combination with pie-zosurgery and a final set of aligner and posi-tioner based on a digital tooth models, most of the treatments can be accomplished within 6-12 month.

the orthodontic norms described by Andrews, Roth or Ricketts serve both as a diagnostic aid and a guide for creating set-ups, that are widely used in Lingual Orthodontics. When achieving Angle’s Class I relationships, with full size arch wires inserted intrabuccally, orthodontists generally assume that an ideal functional occlusion is likely obtained. Is this statement true ?

Published studies about post-orthodontics functional occlusion suggest that major inter-ferences in protrusion or lateral excursion might be present despite of a ‘visually good’ occlusion (PubMed Central, Embase). A com-puterized analysis system (t-Scan III) was used on a sample of adult lingual patients, to evaluate the changes of occlusal force distri-bution after appliance removal over time, and the need for equilibration. Unbalance in Cen-ter of force was a common finding, especially in high angle cases, whereas second molars appeared to be responsible for most non-wor-king interferences.

the specificity of lingual orthodontics, with the attachment placed on the palatal aspect of upper incisors, makes the evaluation of actual occlusal contacts difficult because the bases might interfere with the closing pathway. Nevertheless, and prior to appliance removal, search for interferences should be performed, to allow corrective bends during the finishing phase.

the goal of our study was to investigate the ef-fect of occlusal pads, especially lingual brac-ket’s pads, on a human posture as patients often complain of pain in tMJ, head and neck after treatment with occlusal pads.

50 patients were examined before treatment, after placement of occlusal pads and after their correction. Electromyography and kine-siology testing were performed to confirm the effect of occlusal pads on a human posture. Imbalanced occlusal pads cause involvement of multiple musculo-fascial chains. It is ne-cessary to make a comprehensive analysis of all components of Low jaw movements, i.e. behavior of teeth, muscles, joints. In cases of placement of occlusal pads, their correction is necessary for dynamic and static balance.

the research as part of postdoctoral study has been conducted using P.I.C.O model of evidence based medicine: “ P”(Prevalence of condition )- survey of 275 Orthodontists in India offering Lingual Orthodontics was done and analysed. “I” (Intervention) -by doing a real time evaluation of Lingual Orthodon-tist’s in action using Kinect motion sensor. 3d SSPP program from University of Michi-gan, USA, was used for ergonomic assess-ment of task. “C” (comparison) – was done in subjects with ergonomic training with well designed work environment Vs no ergonomic training with poor designed work space) “ O”(outcome- was recorded)

/ ResULts: A large percentage of Lingual Orthodontist’s in India suffer from work rela-ted Musculoskeletal Disorders ,partly due to lack of ergonomic training and partly due to poor work space design. Real time evaluation of posture was done to know the abnormal angulations adopted by limbs during lingual treatment delivery. the data was evaluated using 3d Static Strength Prediction Program. Orthodontist’s given proper prior ergonomic training of posture fared better. to help Lin-gual Orthodontist an innovative aid-a hands-free intraoral mirror was designed, to perform the clinical procedure more ergonomically.

the increasing number of adult patients de-manding invisible braces has put a strain on Orthodontist as these procedures demand more chair side time and need ergonomic training to perform smartly. / 026:VERSAtILItY Of COMMON BASE fOR SAfE AND SOUND LINGUAL ORthODONtICSakiko Komori, Japan

Improvements in customized base design and bonding system led to develop a precise direct bonding system, named KommonBase. Kom-monBase is characterized by a large bonding base which can acquire the best fitting and a precise bracket position, and maximize bond strength. there is no need to use transfer trays because of its self-positioning shape. In addition, by using reference jig, brackets can be rebonded in case of bond failure and repo-sitioning. first generation of KommonBase has utilized flowable composite resin which was developed for restorative treatment, however, have potential risks for enamel frac-ture and discomfort on debonding. Kommon-Base require some features: more lower wear resistance, lower compression strength, and higher bond strength. Newly developed Kom-monBase resin supported above-mentioned features. Even in the case of occlusal interfe-rence, interfered resin can be ground automa-tically according to the function of occlusion. It is advisable to bond KommonBase with hybrid ionomer because the bond strength of hybrid ionomer is not affected by contamination of saliva and water. KommonBase also can be utilized unique appliance for the preparation of lingual orthodontics. In case of impacted upper canine, extruding canine followed by comprehensive treatment with only lingual brackets may be hard work. this appliance was enabling to control canine with three di-mensions, which may make complicated case to simple case. Details of this appliance will be introduced in the presentation. Kommon-Base resin and hybrid ionomer may lead to safe and sound lingual orthodontic treatment.

Self-ligation is an act of utilizing a perma-nent installed moveable component within a bracket to entrap the arch wire to a bracket. Self-ligating brackets by definition, these are brackets that have an in-built mechanism which can be opened or close to secure the arch wire. from the other hand, the connec-tion that is referred as ‘’ligation’’ , is the form of connection to a bracket that wires need to transmit a moving force, so the wire is in turn fixed to the tooth.

What are the advantages and disadvantages of those systems? What are the ideal requi-rements for each one? What about their ligate secure behavior and their full bracket enga-gement? their friction level, their ease and speed to be used? have they the same mainte-nance of optimal oral hygiene as the conven-tional brackets? What about the comfort of the patient and the cost of each category?

this topic is a try for some strong answers to all these questions and even more, to the difference between the two systems (conven-tional vs self-ligation). Over the years and un-der the given technological development, we believe that we have so much more to learn about the best use of a ligation, in the field of orthodontic systems and we all agree for the same conclusion: Ligate or not ligate, an orthodontic treatment should bring the best results for the human health and this is the only path we need to follow.

It has been suggested that alveolar cortico-tomy is a surgical aid that enables to reduce the orthodontic treatment time; to avoid periodontal complications; and to overcome the limits of conventional orthodontics. the current literature contains a variety of surgi-cal protocols related to corticotomy assisted orthodontic; however up to date there is no orthodontic consensus about corticotomies.

this work aim to introduce the bio-selective approach, which enables the surgeon to se-lect the location; the type of incision and the design of the surgery. this biomechanical vi-sion allows determining the nature, the timing and the intensity of the orthodontic forces. Different malocclusions will be presented. All the cases were treated in less than one year and the 2 years follow up shows stable results.

Bioselective approach is safe and effective alternative to accelerate lingual orthodontic tooth movement.

/ aiM: Since with any lingual method the distance between brackets is rather small, if your patient is predisposed to perio-dontium diseases, it might result in negative consequences. these patients need in altered treatment design, mechanics, and retention. the aim of the report is to present reasonable diagnostic and interdisciplinary treatment planning of periodontally compromesed pa-tients

/ MateRiaLs aND MetHODs: Subjects were 30 patients, with different periodontal condi-tion, divided into groups depend on using technique (Incognito 3M and Stb Ormco). the age ranged from 22 to 45. Periodontal status was examined clinically (PBI, gingival index) and Ct scan (interdental distance, buccal bone thickness and marginal bone levels).

/ ResULts: Significant decrease (two cases -greater than 0,5 mm) were found in the amount of recession.

Accurate-composed, successive treatment planning based on reasonable expectations helps us to achieve good result in difficult cases.

Since I started working with lingual ortho-dontics in 1999, the technique has gone through considerable changes, turning into the best choice for the occlusion and smile corrections. this type of treatment gives to the professional and the patient the opportunity to observe every modification of the teeth, wit-hout any appliance covering the result. Consi-dering that, this is an “appliance-free smile” making it possible for the orthodontist to plan, during the treatment, all the finishing details for a perfect smile. Nowadays all orthodontics cases can be treated with lingual appliances, from the simplest to the most complex cases. Biomechanics can be used in lingual to achieve very good results, comparing to the labial orthodontics cases.

/ MateRiaLs aND MetHODs: Different ap-pliances were used: Ormco 7th Generation, In ovation-L , In-Ovation MtM from GAC, Joy brackets from Adenta. All the brackets were bonded using the Creekmore tool. Arch wire sequence was modified depending on the necessity. Auxiliaries such as Micro-Implants were used when it was necessary.

/ ResULts: All the objectives, such as ex-cellent finishing, patient comfort and simple technique were obtained in these cases.

Several clinical cases will be presented (some of them, with more than 10 years post-treat-ment follow-up), so that we can discuss what is the best way to avoid problems in lingual or-thodontics, and how we can treat more com-plex cases with a simple technique. All the biomechanics demonstrated can be used in any lingual orthodontics technique, no matter what kind of brackets or bonding procedures are chosen by the orthodontist.

/ MetHOD: Adult orthodontics represent a challenging clinical situation especially in pa-tients who seek an invisible orthodontic treat-ment and are affected by periodontal disease. the risk of additional periodontal breakdown in the presence of poor oral hygiene might compromise the final result.

/ ResULts: In the present report the author will illustrate the biomechanical aspects and clinical management of the orthodontic treat-ment with the 2D Lingual System, showing the art needed in archwire bending, the clini-cal tricks and tips to improve the final esthetic outcome.

/ MetHOD: Different clincal cases and theore-tical findings will be discussed.

/ ResULts: there is a continuing improve-ment on the design and production of the Incognito™ appliances as a result of research, development and clinical feedbacks. for mi-nor corrections in the anterior the Incognito™ Lite appliance can lead to state of the art treat-ment results. Splints on the bicuspids provide anchorage during levelling and aligning and torque correction. Managing Class II treat-ment in lingual orthodontics is a frequent and sometimes challenging treatment objective. teeth loss in adults are frequent interfering significantly on occlusion, smile esthetics and soft tissue profile due molars mesialization, adjacent/ antagonist teeth migration, ope-ning diastema in anterior area, teeth rotations and presence of noticeable changes in intra/ inter arch relationships. this case report des-cribes a female adult patient multidisciplinary approach; whose the initial situation showed absence of first lower molars, canine and molar Class II relationship, deep bite, midline deviation and diastema between canines and lower lateral incisors. the proposed treat-ment goals were to verticalize lower second molars reaching space for rehabilitation with implants, to correct the sagital relationship of Class II and vertical relationship of deep bite, as well as the midline, and close anterior spaces in lower arch.

the choice for lingual technique contribu-ted for aesthetics and biomechanics, since brackets on lingual surfaces look like invi-sible beyond the advantage to be closer to the resistence center. It promotes easier teeth displacement, better torque and control. the brackets used were ORJ and the indirect bon-ding methodology was hiro System, based on conventional set up. the archwire sequence was: Niti 0,012; Niti 0,014; tMA 0,016; tMA 0,016x0,022; SS 0,016x0,022. the treatment time was 14 months reaching canine and molar Class I relationship, suitable overbite and overjet, midline coincidence, favoring rehabilitation with implants and dentistry pro-cedures.

the lingual orthodontic technique was effec-tive achieving excellence, and multidisciplina-ry approach counting with implants, dentistry procedures and teeth bleaching has valuable contribution to the restoration of masticatory function and aesthetics of smile and face.

Some problems in “horizontal bowing effect” control were observed during space closure phase in 12 Japanese cases with tooth extrac-tion treated with Incognito Appliance System. the cause of the problems was investigated and the countermeasures against them were discussed.

following values were measured in 12 Ja-panese cases with tooth extracion treated by Incognito Appliance System and compaired values of upper and lower arches of study mo-dels taken at finishing stage with ones of set-up models; 1. Inter-canine distance 2. Inter-2nd premolar distance 3. Inter-1st and 2nd molar distance the tendency of the values of Inter-2nd molar distance of the finished case is smaller than ones of the set-up model was observed.

In Incognito cases with tooth extraction, it is preferable to add “anti-bowing effect curve” to the archwires to avoid the occurrance of holi-zontal “bowing effect” in posterior dention. Also retracting anterior dentition derectly from the micro implant should not be done to avoid intrusion of lingual cuspids of molars.

Some problem of angulation control of ante-rior dentition were observed during space closure phase in Japanese cases with tooth extraction treated with Incognito Appliance system.the cause of the problem was investi-gated and the countermeasures against them were discussed.

tooth axis of anterior dentition were investiga-ted in intra oral photos from Japanese cases treated by Incognito Appliance System after space closure and set up models. the tenden-cy of distal-crown tipping was observed after space closure and also the treatment duration was relatively prolonged in those cases.

In Incognito cases with tooth extraction,it is preferable to add «anti-distal-tipping bend»to the arch wires to avoid distal tipping of ante-rior dentition during space closure phase. Adding 7-degree of anti-tipping bend to the arch wires at the lateral incisors and canine preferable tooth axises were maintained after space closure.

/ aiM: Aesthetic result of mandibular ad-vancement surgery depends widely on the orthondic preparation, the purpose being to produce an overjet equivalent to the skeletal discrepancy.

/ MetHODs: Most of times a decompensa-tion is necessary, which means a backward move of the lower arch. Use of mini-screws allows to achieve such a move without any effect upon the maxillary teeth. the tech-nique combines the use of a looped rod with the insertion of mini-screws in the retromolar triangles, practically without any post-surgi-cal effects.

/ ResULts: Setting up elastic tractions on the mini-screws allows smooth and controlled decompensation, with the advantage of avoi-ding the « non-return scheme « of bicuspids extractions .

/ CONCLUsiON: final result can be improved through the use of bite planes and vertical elastics to correct the cant of the occlusal plane.

/ aiM: to study the dentofacial changes of pro-truding cases by using lingual appliance and mini-screws.

/ MetHODs: to choose 20 protruding cases treated by lingual appliance and mini-screws in random. the mini-screws were implanted between the roots of the upper second premo-lar and the upper first molar in the palate. to observe the stability of mini-screws and mea-sure the cephalometrics in order to evaluate the effect of the lingual orthodontic treatment.

/ ResULts: the success rate of mini-screw in palate is 97.5%. the first upper molars are moving 0.23mm mesially in mean. the upper central incisors are moving 5.31mm distally in mean. the profile of the cases are improved obviously than before.

/ CONCLUsiON: We can control the anchorage of upper molar effectively by lingual appliance with mini-screws. Mini-screw is my best choice.

Many adult patients present a correct occlu-sion in posterior teeth or they have prosthesis of molars and bicuspids, but they also present malpositions of anterior teeth, and therefore, they demand esthetic orthodontic treatment. Many patients present spacing between ante-rior teeth produced by a vertical dimension loss or by a periodontal support loss in ante-rior teeth, and other patients present crow-ding and rotations which affect not only the incisors, but also the canines. Although the treatment of these malpositions with lingual brackets bonding in only incisors and canines is already in use, in many situations greater anchorage is necessary, and therefore, the brackets are also bonded in first bicuspids.

this lecture analyzes the biomechanical advantages of first bicuspids bonding in the treatments, and demonstrates them through case reports. the result was that the cases bonded from first bicuspid to first bicuspid had improved anchorage than cases bonded only from canine to canine.

In cases with spacing or crowding in anterior zone, in which greater anchorage is needed, it is better to bond from the first bicuspid to the first bicuspid..

the most prevalent difficulty found in lingual orthodontics involving extraction is a loss of torque control in anterior region. this pro-blem is almost inevitable in lingual ortho-dontics especially in maxillary anteriors due to its force mechanism causing malocclusion, severe overbite, severe overjet, misalignment of incisal edges and etc.

to resolve loss of torque problem in lingual orthodontics, orthodontists have employed a variety of methods such as placing exces-sive torque during set-up, applying additional torque while bracket bonding, utilizing seg-mental wire technique, loop mechanic, lever arm, and etc. Such methods were found to be technique sensitive, governed heavily by host factors, and difficult to control which may lead to unfavorable clinical results. Also, additional torque may not dissipate in timely manner which may act as another hindrance in achieving adequate occlusion, overbite and overjet. A novel method called twin archwire technique is hereby introduced which is less technique sensitive utilizing conventional multi-slot lingual bracket for proper torque control in maxillary anteriors

twin archwire technique is a less technique -sensitive and user-friendly method in alle-viating anterior torque control problem in lin-gual orthodontics which requires conventional ideal set-up with multi-slot lingual brackets without additional wire bending and set-up.

Skeletal Class III malocclusion in adults is one of the most challenging dentofacial deformi-ties to treat with orthodontics. Although cur-rent orthognathic procedures offer worthwhile changes for facial aesthetics in moderate to severe Class III patients, some refuse to undergo surgical operations for various rea-sons. Camouflaging the deformity by means of dentoalveolar compensation is the other treatment option for this group of patients. During the treatment, it is crutial to control the lower incisor inclination in which they are already retroclined in most Class III cases. from the mechanical point of view, in lingual orthodontics applying the forces behind the center of resistance of the lower incisors can prevent the uncontrolled round-tipping effect that naturally occurs with the standard labial orthodontics during the initial leveling and aligning phase via round archwires. Besides, predefined treatment goals with the help of the setup determine the required incisor incli-nations, neither more nor less, for compen-sating the skeletal deformity. the aim of this study is to present the treatment course and the results in a group of Class III patients who refused surgery.

Study group is comprised of four consecutively treated adult patients who had an ANB and IMPA angle ranged between -1 to -4 degrees and 65 to 83 degrees, respectively. In all of the patients successful results were achieved in correcting the malocclusion.

As a result, fully customized lingual appliance offered great mechanical advantages in the nonsurgical orthodontic treatment of challen-ging Class III cases.

Class II malocclusions are the most common reason that brings patients to visit an ortho-dontist in the European area.

A wide number of lingual orthodontic ap-pliances are normally used to treat these malocclusions, generally depending on the features and severity of them.

My lecture will be dealing with the Incognito (tM) Appliance System.

It is the most widely known and used totally customized lingual appliance, characterized by a very high level of accuracy in the manu-facturing process of each component.

Due to its precision and characteristics, Inco-gnito (tM) appliance has the potential to pro-vide excellent torque control, thus giving the orthodontist the chance to treat class II dis-crepancies by means of inter-maxillary trac-tion without the typical side effects normally observed.

Different biomechanical approaches to treat different kind of class II malocclusions will be shown in the lecture; additionally clinical cases will be deeply discussed, with thorough examination of Incognito peculiarities.

/ aiM: In this presentation, conventional lin-gual orthodontic treatment with single-slot brackets will be compared to the treatment method with multi-slotted brackets and tan-dem archwires. In additiuon, its effectiveness in 3-dimensional control of the tooth position will be discussed.

/ MateRiaLs aND MetHODs: Edgewise brackets are excellent orthodontic treatment tool that can control 3-dimensional tooth position. they are very effective in tipping and rotation control, but relatively less effective in torque control due to the short moment arm. Multi-slotted lingual bracket has a horizontal slot and a vertical slot. While conventional treatment mechanics were progressed with an archwire in the horizontal slots, we can add segmental archwires in the vertical slots.

/ ResULts: these tandem archwires in the two slots create additional moment arm for torque control, therefore we can express the prescription built into the brackets better than with single archwire in the single slot.

/ CONCLUsiON: Using multi-slotted lingual brackets and tandem archwires, we can control 3-dimensional tooth position more effectively and achieve desired treatment goal more efficiently.

Around 20 years ago, Kurz was the only ap-pliance that was marketed in lingual ortho-dontic practice.

It is still supported by many orthodontists around the world although it is big and bulky, providing speech problems and tongue irrita-tion.

As for myself, I have been using my original mini brackets, hirobrackets, since 1996 and obtained quite excellent results.

however, I am not fully satisfied with it.

Recently I had a chance to design new bracket system, “mienai lingual brackets”.

At the pre-congress course of WSLO in Paris, I have introduced 1st prototype of “mienai lingual brackets”, introducing 6 special cha-racteristics that other lingual brackets do not possess.

At the Congress, I would like to introduce the revised one, most recent “Mienai Lingual brackets”, very small and round shape.

Asian peoples hold the higher percentage of class III malocclusions.

We often find young patients with obvious class III tendency though they are still in the very early age. Some considerations should be taken into account before we utilize a non surgical way to treat class III malocclusions.

In this presentation, I will show how I use PARIS guidelines to evaluate young and adult class III patients and give some cases to illus-trate how I dealt with them clinically. I also use tADs as a useful tool to treat some class III malocclusions.

the development of the advanced active retai-ner (R2A) has led over the years to be a real full orthodontic technique.We started in 2005 with the alignment of the social six from 3 to 3,so we arrived to treat both the arches including bicuspids and molars by using mushrooms wires. two years ago we thinked about the substitution of mushrooms wires with straight wires. So ADA wire was born.

We use only nichel titanium round wires 0,14 and 0,16 , wax and fluid composite. the wire properly waxed is directly bonded on the lin-gual surface of the teeth without brackets. It slides into the fluid composite on lingual sur-faces from 3 to 3,and then goes inside the bite rising on the occlusal surfaces of bicuspids and molars. So the archform we use is the straight oval form. We have treated 58 cases of crowding in upper and lower arch , and we have observed for a time ranging from 3 to 12 mounths.the allignment by ADA wire is very effective and fast andvery good in finishing. During the treatments we have seen several open spaces between the teeth that we then properly closed by steel wires with loops.

ADA wire makes the alignment of the teeth easier because it is a very straight wire tech-nique without brackets ,without offset and without any loop on wire. ADA wire makes faster the alignment because slides inside the bite rising without occlusal interferences. ADA wire is a very suitable technique for any patient with high level of comfort , immediate good speaking, and it is very simply to clean. ADA wire is easy to bond and a very cheap technique for the orthodontist .

/ aiM: to reveal the differences in treatment outcome between conventional and fully cus-tomized lingual bracket appliances.

/ MetHODs: this study enrolled 33 female adults diagnosed with maxillary protrusion and treated with four bicuspid extractions. the subjects were divided into two groups: 23 patients treated with Kurz 7th and 10 patients treated with Incognito. the two groups were compared for cephalometric measurements before and after treatment with Wilcoxon sign rank test. We also calculated treatment accu-racy in terms of changes in the 25 measure-ments before and after treatment and ana-lyzed them with and Mann Whiteney U test.

/ ResULts: SNA and ANB were significantly reduced in both groups. the upper incisors were retracted by a large amount in the Kurz group, with lingual tipping in some cases, as indicated by U1 to NA in millimeter and de-gree. the variability of these numbers was smaller in the Incognito group, suggesting that proper torque was maintained as the up-per anterior teeth were retracted. In contrast to the Kurz group, Incognito Group showed a significant decrease in nasofacial angle and a significant increase in nasomental angle.

/ CONCLUsiON: Both groups showed favo-rable treatment results. however, this study suggests that the structural advantages of the Incognito appliance including the vertical slots for the anterior teeth and a high dimen-sional accuracy of the slot may permit more precise alignment of the teeth without exces-sive torque loss and thereby increase the ac-curacy of treatment. • finally, I will show the Clinical Case Documentations of Incognito.

the objective of this presentation is to give our experience on how Excellence in organization and Management can lead to Excellence in orthodontic results.the exact matching of set-up to treatment result in lingual orthodontics is a key perfor-mance indicator of orthodontic practice (objec-tively proven clinical indicators).

/ ResULts: this approach shows a higher level of performance in the treatment interim and final results, as measured by the indicators and reflected in the organisation’s aggregate management dashboards.the patient satisfaction also improves over time, as the treatment has been designed and explained with the results in mind. the EfQM process-orientation and PDCA me-thodology ensure full control of the treatment delivery cycle.

the author proves by 6 clinical cases the excel-lence of results in lingual orthodontics.

Buccal alveolar region and mid-palatal region are favored for intrusion of posterior teeth and retraction in labial orthodontics. the case of buccal alveolar region, it was chosen for a micro-implant, therefore clinicians are eli-gible to use a micro-implant instead. howe-ver, when the amount of intrusion or retrac-tion distance is farther, it would most likely to be fail. Because it is located in the inter-dental area. the micro-implant in mid-palatal region, on the other hand, can transmit force to the various places. teeth which take force from micro-implant in mid-palatal region are not allowed to the affect to micro-implant. In addition, this process will not be loosen and failed by teeth movement. the micro-im-plant need additional orthodontic appliance to connect by itself. So, we take a chance to implant micro-implant in mid-palatal region which can possibly provide the achievement of the intrusion and retraction in lingual ortho-dontic.

When a micro-implant is located in buccal alveolar region, it needs another labial ap-pliance to be in lingual orthodontics. Aestheti-cally, an advantage of lingual orthodontic will not be taken. In mid-palatal region, however, patients do not need to take discomfort from labial and buccal appliance. Accordingly, it is a comfort way to achieve treatment and also effective way to deliver forces, to implant at mid-palatal region for intrusion and retrac-tion in lingual orthodontics.

Use of micro-implant in mid-palatal region is effective to achieve intrusion and retraction for fujita lingual orthodontics bracket which have multi-slot.

this study is a theoretical approach, to a digital microscopic analysis, on which the simulation on mechanical properties, evaluates the way in which a retaining mechanism of the back of an orthodontic bracket, develops stresses that tend to deform or cause breakage during the process of debonding it from the surface of a tooth.When one body exerts a force on a second body, the second body simultaneously exerts a force equal in magnitude and opposite in direction to that of the first body. In other words, the New-ton’s third low of motion: for every action, there is an equal and opposite reaction. through Dassault Systems’ technology, a 3d detailed model drawing was designed, edited, visua-lized and simulated by using Solid works 2014 edition. three metal alloys took place in that evaluation: the 2205 stainless steel alloy (EN 1.4462), the Remanium star cl (Dentaurum) and the MP1 (EOS Gmbh). Appropriate design factors are based on several considerations, such as the accuracy of predictions on the imposed loads, strength, wear estimates, and the environmental effects to which the product will be exposed in service. What are all those considerations? Is a static nodal stress (Von Mises) analysis, or a static displacement ana-lysis, capable to specify standards in designing and constructing a perfect functional element?Using digital procedure and studying those pa-rameters, it might be explained and eliminated all the opposite effect caused by an inadequate construction.

a new metHoD oF Designing computer-aiDeD customizeD appliance on lingual ortHo-Dontics

the purpose of the study is to explore a new method of designing customized appliance on lingual orthodontics based on standard digital dental model.

We select the maxillary standard permanent dentition model manufactured by Japanese Nission dental products Inc. then the model was scanned by CBCt and all the collected geometric data were imported into Mimics for reconstructing the 3-dimensional digital model. In Solidworks, We drew the bodies of bracket according to the data of Stb brackets from Ormco company which measured by V-StARS (Video-Simultaneous triangulation and Resection System). then , we deepen the slot of the bracket of premolar. furthermore, a series of vertical slot brackets were added in. Based on the standard digital model, we designed customized lingual arch wire and brackets’ bases. then, the bodies of brac-ket, the bases of bracket and arch wire were assembled together. In the end , we integra-ted the base and body of bracket in Mimics. We design a series of brackets, so we can chose the suitable ones according to the case. through engineer software Solidworks and Mimics, we successfully designed customized lingual arch wire and brackets on a standard maxillary digital model.

It is feasible to design customized lingual arch wire and brackets with computer-aided design/computer-aided manufacturing software.

More and more adult orthodontic cases be treated with lingual orthodontic technique, it is most important pay attentation to consider the periodontal condition. Periodontics is the most common reason of loss teeth and one of the most common diseases (87% in China). Cur-rently, multidiscipline periodontal and lingual orthodontic therapy has been playing more and more important role in lingual orthodontics. It is the future of adult orthodontics.(1) Periodontics and lingual orthodontics Or-thodontic treatment for periodontal patients More and more orthodontist stared to treat adult orthodontic cases, however, diagnosis and treatment periodontics before orthodontic treatment are very important. first of all, bac-terial plaques and inflammation will be remo-ved or reduced. Second, orthodontist will pro-perly design the lingual orthodontic appliance. third, regular examination and teeth clean will be performed regularly. fourth, active perio-dontitis must be controlled prior linguchal orthodontic treatment. (2) Some periodontal surgeries will be very helpful for lingual ortho-dontic treatment. Such as frenectomy, fibero-tomy, gingivectomy, gingivaplasty, and PAOO. New periodontal regenerative methods have provided opportunities to gain new attachment, which improve the precondition for orthodontic movement of teeth into infrabony defects or for vertical movement with reduced bone support.With efficient interdisciplinary approach in combination of periodontal and lingual ortho-dontic treatment we are able to achieve the outcomes of the improve periodontal regene-ration, aesthetics, satisfied orthodontic results, and functional occlusion.

the aim of this study was to evaluate the knowledge of invisible orthodontics among the students of dental and medical schools and to find out if they can associate this term with lingual orthodontics

130 students was asked to complete a ques-tionnaire that contains 15 questions about orthodontics. 2 questions were about the ap-pearance of the orthodontic braces and if they know the presence of invisible orthodontics. Results showed that the apperance of vesti-bul braces was woted as “bad” by 56 per cent of all students, as “normal” by 29 per cent and as “good” by 14 per cent of all students. the question about the presence of invisible orthodontics was answered as “yes”by 68 per cent of the students, as “no” 16 per cent of them and as “have no idea” as 15 per cent of the students.

Lingual orthodontics is the excellent esthetic way to treat malocclusions. Appearance of the orthodontic braces have a negative effect on the people who seek for orthodontic treat-ment. Awareness of invisible orthodontics can be increased by first teaching the students of medical and dental schools.

One of factors which makes hard to treat is torque control in extraction cases of lingual orthodontic treatment. the character of lingual orthodontic appliance is difficult to apply labial moment to control torque at anterior teeth. So, torque loss is frequent. Orthodontists tried to solve the problem by using long lever-arm technic and anterior tandem arch-wire technic. But it can induce an inappropriate delivery of retraction force due to its “long” lever arm. And anterior tandem arch-wire technic can cause a general bowing of anterior segment, so it might not be free from the problem of torque control.I use twin arch-wire technic which used occlu-sal slot(vertically opened) and lingual slot fully inserted 0.018*0.018-inch stainless steal arch wire in the phase of space closure. Sufficiently full engaged two arch-wires present 3-dimen-sional location of teeth, and then they make little torque loss of anterior teeth and vertical bowing which are prevented by a arch wire in the occlusal slot and torque loss of anterior segment.Using the twin arch-wire technic, not only in anterior teeth but also in posterior teeth using two slots twin arch-wire technic in fujita lin-gual bracket which have multi-slot, in cases of retraction of premolar extraction samples. I will show cases that proved to be ample controlling of torque and good results with twin arch-wire technic.

In orthodontics the precision in bonding is essential for a right occlusion and esthetics. the problem is more evident in lingual ortho-dontics, due to the anatomical and clinical visibility problems that make more difficult the bonding.

/ aiM Of tHe wORK: In lingual orthodontics the bonding on the molars and bicuspids has a difficulty that is not present in labial: the bonding is on the lingual surface of the teeth, having as a landmark the lingual cusps, since, the goal should a position related to labial cusps, like happens in labial orthodontics. the aim of this work is to show a new jig, pro-jected by the authors, that allows: simplicity and precision in the bonding, both direct and indirect, of the 2D lingual braces.

/ MateRiaLs aND MetHODs: the jig is a sort of “L” made in plastic that fit perfectly the 2D braces letting to bond the braces at fixed dis-tances as regards to the labial cusps or to the incisal margins. the distances are: 2, 3, 4 and 5 mm. the special design allows an easy posi-tioning related to the long axis of the teeth. We have tested the jigs in direct and indirect bon-ding procedures.

/ ResULts: the bonding jig, proposed by the authors, allows with simplicity to relate the brace position to the labial cusps and, consequently to achieve a right lateral bon-ding plane. It proved itself effective in plan-ning the 2D braces position. Due to the pre-cision in bonding it is possible to reduce the need of a set-up or individualization of the lingual appliances, reducing the costs and the laboratory procedures in the most part of lin-gual therapies.

the proposed jig revealed itself of a easy use and assure the needed precision in bonding. for the moment exists only the jig for the me-dium 2D brace, but, seeing that the results the authors are going to design a similar jig for the other braces of the series.
