QUOTE Proposal of (hereinafter
called "BIDDER"), organized and existing under the laws of the State of

to the Fallon County commission (hereinafter called "OWNER").
In compliance with your Advertisement for Quotes, BIDDER hereby proposes to
Furnish, Store, & Deliver a PRECAST DOUBLE 10’ X 6’ REINFORCED
CONCRETE BOX CULVERT; in strict accordance with the CONTRACT
DOCUMENTS, within the time set forth therein, and at the prices stated below.
By submission of this QUOTE, each BIDDER certifies and in the case of a joint
QUOTE, each party thereto certifies as to his own organization, that this QUOTE
has been arrived at independently, without consultation, communication, or
agreement as to any matter relating to this QUOTE with any other BIDDER or
with any competitor.
The Precast Reinforced Concrete Box Culvert shall be completed and available
for delivery by May 1, 2016.
BIDDER acknowledges receipt of the following ADDENDUM:

BIDDER agrees to perform all the work described in the CONTRACT
DOCUMENTS for the following unit prices or lump sum:

SPECIFICATIONS The Precast Reinforced Concrete Box Culvert shall conform to requirements as
specified in the Standard Specifications for Road and Bridge Construction of the
Montana Department of Transportation, 2014 Edition.
DESIGN MIX OF CONCRETE
1. The design minimum 28 day compressive strength of concrete shall be as per fabricator’s design, however shall not be less than 4,500 psi.
2. Type V Cement shall be used.
GENERAL NOTES
Design shall be in accordance with Section 554 of the Specifications, with the following criteria:
1. Box culvert and box culvert end section design shall conform to the AASHTO LRFD Bridge Designs Specifications, 2014 Edition with 2015 interims.
2. Design Live Load: HL-93.
3. The design of the barrel sections shall be based on a minimum fill height of 1 foot and include all subsequent fill heights up to and including the maximum fill height of 4 foot over the box culvert.
4. Minimum inside corner fillet shall be 6 inches.
5. Minimum precast barrel section length shall be 4 feet.
6. The Contractor shall stencil the structure with the date of construction.
7. Contractor shall provide a minimum of four (4) Galvanized Hidden Tie Bolts for between each end section and each full barrel section of the RCB Culvert.
8. Contractor shall provide Galvanized Sleeves, Type IV Epoxy, and #5 Epoxy Coated Rebar 16 inches in length in floor of box for cutoff walls. Galvanized Sleeves shall be cast into the upstream and downstream most end sections with a minimum of five (5) Sleeves required per barrel section at the upstream and downstream ends.
9. Contractor shall provide EZ-STIK Joint Sealant by Press-Seal for all joints.
10. Contractor shall provide Sealwrap Primer and Sealwrap by Mar Mac for all joints, lifting holes, and pulling holes.
11. Contractor shall provide Non-shrink Grout for all lifting and pulling holes.
12. Contractor shall provide Panel Hooks, Eye Bolts, and Concrete Inserts for the RCB Culvert as shown on the Fence Anchor Details.
13. Cutoff Walls shall be provided as shown on the General Layout Drawing and included with the quote price per each for “DBL 10’ X 6’ PRECAST RCBC END SECTION”.
DELIVERY AND STORAGE
1. All costs for storage, loading, transporting, and delivery to the project site shall be included in the quote for the Precast Concrete Box Culvert.
SHOP PLANS
1. The fabricator shall submit shop drawings in accordance with the specifications to Brosz Engineering Inc., PO Box 357, Bowman, ND 58623 ([email protected]) for review. Include design and check design with initial submittal. After review, corrections (if necessary), and approval by Brosz Engineering Inc., the Engineer will authorize fabrication.
