Evening. Like the narrator, we
take a brief moment to reflect on
our past accomplishments, yet

EdgewoodVillageisaneighborhood
inthecityofAkron,Ohio–near
theHelenArnoldCommunity
LearningCenterandtheAkron
UrbanLeague–wherehealthcare
isevolving.Agrass-rootsinitiative
knownasABetterCommunity
(ABC)beganwhenSummaHealth
System’sresidencyprogramin
familymedicineaddedacritical
componenttoitscurriculum:to
reach,interactandservepatients
notonlyinhospitalsandclinics,but
alsointhecommunitieswherethey
liveandwork.
Whatisuniqueaboutthisprogramis
itsapproach.Ratherthanhealthcare
professionalsdeterminingthe
healthcareneedsofthecommunity,
thecommunityisdetermining
itsownhealthpriorities.Care,
educationandsupportareprovided
bythemedicalstaffandresidents
ofSummaHealthSystem,along
withpartnersthatincludetheAkron
MetropolitanHousingAuthority,
theUniversityofAkron
CollegeofNursingand
TheOhioStateUniversity
extensionprogram.
Theinitiativebeganwhen
communitymembersand
healthcareprovidersmetto
discussdisparitiesinhealth
andhealthcare.Inorderto
createaconstructivedialogueabout
thesocialdeterminatesofhealth,
medicalstafffromSummaHealth
SystemaskedresidentsofEdgewood
Villagetoparticipateinanexercise
called“photovoice.”Community
membersweregivendisposable
camerasandaskedtotakephotos
thatshowedpositiveandnegative
aspectsoftheirneighborhood.
Positiveimagesincludedwalking
pathsandparkbenches,while
negativeimagesincludedvacant
lotsand“fooddeserts”–city
blockscontainingretailandfast
foodrestaurants,butnomarketsor
storeswhere
individuals
canpurchase
freshproduce.
Whenthe
photoswere
assembled
intoa
montage,
communitymembersmetagain
toreviewanddiscuss“whatthe
picturestoldthem”abouttheir
communityandhealthconcerns.
Theythenprioritizedtheirhealth
issuesandchosetofocusonweight
managementandopportunitiesfor
physicalactivity.
Whiletheprogramisinitsinfancy,
muchofthegroundworkhasbeen
laid.Forexample,arepresentative

markersonthecommunity’s
walkingpathsoindividualscould
measuretheirexerciseregimen.
Discussionshavetakenplaceabout
healthyeating.Tryinganewslanton
communitygardenswassuggested
byaneighbor:Ratherthanallocating
aspecificspaceforonegardenfor
theentirecommunity,
wouldthegroupconsider
smallcontainergardens
residentscankeepoutside
theirownhomes?The
premiseisindividualsare
morelikelytotakeadvantage
oftheavailabilityoffruitsand
vegetablesgrownathomeversus
acommunitygardenthatisnot
withinwalkingdistance.
Whilethecommunitydetermines
itsprioritiesandprograms,medical
students,theirprofessorsand
mentorsstudytheimpactofthe
worktodetermineoutcomes.
Theprojectisidealistic;however,
measuringresultswillhelpboth
community
membersand
theirsupporting
partnersdevelop
themost
effectivemodels
ofcarethatwill
helpthemreach
theirobjectivesanddeterminethe
sustainabilityoftheinitiative.
Meanwhile,familymedicine
residentswillgainexperiencein
caringforpatientsintheirown
communities.Thistranslates
intoenhancedempathyand
understanding,awareness
andperhapsmodificationsof
recommendationsbasedona
patient’sabilitytoadhereto
thatpractice.
“Intheend,it’saboutbeingamore
sensitiveandunderstandingphysician,”
saidSharonVanNostran,D.O.,
aphysicianwithSummaHealth
System.“Wewanttoteachour
residentshowtobecome
partofthegeneral
communitysowhenthey
graduate,theyknowhow
apatient’senvironment
contributessignificantly
tohis/herhealthstatus
andtheirrespective

willingnessto
acceptacertain
amountof
accountabilityfor
theirwell-being
–iftherighttools
areinplace.”
Oneoftheprogramsthatwillbe
offeredtotheresidentsofEdgewood
VillageistheStanfordModelof
ChronicDiseaseSelfManagement*.
Whenimplemented,laypeoplefrom
theneighborhoodwillbetrained
andcertifiedinspecificareasof
healtheducation.Thentheycan
supportthoseinthecommunities
whohavespecificchronicdiseases,
suchasdiabetes.Additionally,they
canconducteducationclasses,offer
supportandprovidefeedback.The
communitywilldetermineifitwill
choosetheStanfordModel;however,
thehopeistheyfindvalueinworking
witheachotherastheysharesimilar
circumstancesand
perhaps,thesame
chronicconditions.
Shouldthecommunity
choosetoacceptthe
StanfordModel,
emergingcommunity
leaderswillbetrained
andcertified.Again,theemphasis
isonmembersofthecommunityas
theydeterminehowtheywilladdress
theirhealthcareconcerns.
Onefactoriscritical,however.
Whateverprogram(s)ultimatelyis
chosen,itmustbeoutcomesdriven.
Thehealthofthepopulationmust
improveortheprogram(s)will
notbereplicated.
“Onethingpeopleneedtoknow,
itisextremelyhardworkand
itdemandstime,energyand
commitmenttodevelopaprogram
suchasthis.Itrequiresthesame
dedicationtosustainwhileit
evolves,”saidDr.VanNostran.
“Thehopeforthisprojectisnotjust
thatitworksfortheneighborhood
ofEdgewoodVillage.Thehope
istostartinthisneighborhood,
havemeasureablesuccess,then
havethiscommunitysharethose
learningsandexperienceswith
otherneighborhoods.”
Inmanyways,thisprojectisamodel
ofsuccessforthecommunity,the
residentsinthefamilymedicine
residencyprogram,thepartnerships
amongtheentitiesmentioned
previouslyandforSummaHealth
System.WiththesupportofSumma
Foundation,thehealthsystem
consistentlyhonorsitspromiseto
meetthesocialandmedicalneeds
ofthecommunitiesitserves.

Tenyearsago,MarcPenn,M.D.,
Ph.D.,FACC,renownedcardiologist,
directorofresearchatSumma
CardiovascularInstituteandprofessor
ofmedicineandintegrativemedical
sciencesatNortheastOhioMedical
University(NEOMED)andhisteam
ofresearchershypothesizedthehuman
bodytriestorepairitselfwithstem
cellswhenit’sinjured.Thechallenge
isthatitisclinicallyinefficient.Many
inthemedicalcommunityarguedthe
inefficiencywasduetoalackofstem
cells.Dr.Pennargueditwasclinically
inefficientbecausethebodylackedthe
signalsthattriggerstemcellstostart
thehealingprocess–thustheanalogy
ofanorchestra(thestemcells)withno
conductor(thetrigger).
Throughclinicaltrialsandextensive
research,Dr.Pennandhisassociatesset
outtofindthatconductorandadecade
later,theyidentifiedoneknownas
SDF1andcompletedthe
firstclinicaltrialstudying
itseffectsinpatientswith
heartfailure.Heandhis
teamconductedfurther
researchtodeterminehow
thatconductorcanrepair
thedamagecausedby
myocardialinfarctionandminimizeor
eradicatethelong-termeffectsofheart
failureforpatientswhomaylivewith
thisconditionformanyyears.
ThetypeofresearchDr.Pennand
histeamareconductingisknownas
regenerativemedicine,broadlydefined
asmedicaltherapiesthatenablethe
bodytorepair,replaceand/orrestore
damagedordiseasedcells,tissuesand
organs.Heandhiscolleaguesare
undertakinganumberofstudiesto
determineifand/orhowtheheartcan
healafteramajoracuteincidentsuch
asamyocardialinfarction.Theyalso
areaddingtothebodyofworkthat
addressestheissuesofchronicdisease
management
forcardiac
conditions.
“Medicine
hasbecome
verygoodat
treatingacute
diseases,”said
Dr.Penn.
“The30-daymortalityrateforan
acuteheartattackdroppedfromabout
15percent20yearsagotounderfive
percent.Thegoodnewsispeoplelive.
Thechallengeistheylivetohavemore
heartattacksandchronicheartfailure.
Medicinehastransitionedfromacute
illnesstochronicdiseasemanagement.
Heartfailureisburgeoningbecause
patientssurvivethoseacuteincidents
andnowhaveweakordamagedhearts
fortherestoftheirlives.Thegoalof

Dr.PennjoinedSummaHealth
Systemin2011afterworkingfor
majormedicalinstitutionsformany
years.Hechosetocontinuehis
researchandpracticeatSummaHealth
Systembecauseitallowshim–and
otherlike-mindedresearchers–to
collaborateinawaythatisunique
inourregionandrareacrossthe
country.SummaHealthSystemhas
astellarreputationforcollaboration
withotherorganizationssuchasthe
AustenBioInnovationInstitutein
Akron,NEOMED,theUniversity
ofAkron,KentStateUniversityand
otherinstitutionsofhigherlearning
aswellascompaniesthathold
themselvestothehigheststandards
ofexcellenceintheirrespective
disciplines.Itisthiscollaborationand
commitmentthatdrawsprofessionals
suchasDr.PenntoSumma.
“Inmydiscussionswiththeleadership
atSumma,andparticularlywith
thedepartmentofcardiology,I
sawthedeepcommitmentofthese
professionalsandtheirdesireto
bringopportunitiestotheirpatients.
Theywanttoparticipateatthislevel
andthat’swheretheopportunity
oftranslatingsciencefrom‘bench
tobedside’existsforsomeone
likeme,”hesaid.
Asaresearcherandpracticing
cardiologist,heisequallypassionate
abouttheroleclinicaltrialsplayin
helpingmanypatientsinourown
communities.Clinicaltrialscan
transformthedeliveryofhealthcare.
SummaHealthSystemhasmultiple
clinicaltrialsinprocessconcurrently,
particularlyin
thedisciplines
ofcardiology
andoncology.
“Thinkofitthis
way,”saidDr.Penn.
“Considerapatientwhohasjustbeen
broughtintothehospital24hoursago
withaheartattack.She/heistakento
thecatheterizationlab.Thatindividual
isstentedtostoptheheartattack
andgivenuptofiveprescriptionsand
receivesexceptionalcare.Thereare,
conservatively,hundredsofpatients
whoparticipatedinclinicaltrialssowe
could‘figureout’howweshouldtreat
thatpatient,optimizethechanceof
survivingthatheartattackandreturn
toasnormalalifeaspossiblewith
decreasedriskoffutureevents.Thus,
anyonecurrentlyinahospitalbed
hasbenefitedfromthewillingness
ofotherswhocamebeforeher/him
toparticipate.”

healthcareorganizationmakea
majorfinancialcapitalinvestment
inbuilding,expanding,renovating
andupgradingtheemergency
departments,outpatientfacilities,
urgentcarecentersandclinics
withinitssystem?Inessence–
whykeepbuilding?
Becausethefutureofhealthcare
requiresustoviewtheindustry
differently.Whilethepracticeof
providingcarefor“one
patientatatime”ina
hospitalsettingremains,an
evolutionistakingplace.
Amajorcomponentof
thatevolutionincludes
managingthehealthof
populations,improving
thehealthofthatpopulation,
preventingorminimizingdiseases
andprovidingcareefficientlyandat
areasonablecost.Giventhediversity
ofanydefinedpopulation,itisto
beexpectedindividualswillhave
differenthealthcareneedsatdifferent
times.Whiletheneedsmayvary,the
criteriaareconsistent.People
wantcarethatisaccessible,
convenientandaffordable.

emergencydepartmentshadtobe

Thistremendousvolumeimpacts

individualsrequiresmorethan
physicalspace(beds,examination

requiresnew,moreeffectiveand
efficienttreatmentprotocols,the
mostadvancedmedicalequipment
availableandtrained,experienced,
compassionate,caringhealthcare
professionals–thelatterisa
hallmarkcharacteristicof

individualsreceive

Embeddedin

beflexibleandadapttomeetthe
currentneedofapopulationunder
differentscenarios.Aspartofits
overallstrategy,theorganization
hasthecapabilityto“right-size”
quicklyandefficiently.Itsabilityto
respondtochangesinthehealthcare
environmenthasbeentestedand
SummaHealthSystemhasresponded
successfullytimeandagain.Summa
HealthSystemrecognizesboth
