It is our pleasure to welcome you to the 2015 Pediatric Sedation Outside of the Operating Room weekend conference, Friday Fundamentals of Sedation: Reviewing the Basics, Bridging the Specialties Symposium, Friday Pediatric Sedation Simulation Workshop, and Saturday Pediatric Dental Sedation Simulation Workshop. This year we are presenting a unique, enriching, and exciting multidisciplinary conference.

We are thrilled to host the conference in a new and international destina-tion this year: Cancun, Mexico. This will be the first international offering since the conference started 10 years ago, in 2005. As always, we have a diverse group of internationally and nationally recognized speakers and attendees from a variety of disciplines. The disciplines represented include: anesthesiology, emergency medicine, dental medicine, hospital medicine, intensive care medicine, pediatrics, nursing, oncology, cardiol-ogy, radiology, and hospital administration. Attendees travel from coun-tries not limited to but including Italy, Netherlands, Greece, Israel, Can-ada, Australia, Hong Kong, New Zealand, Japan, Brazil, Indonesia, South Africa, and Singapore. The specialized tracks, round table luncheons, and open forums enable us to share our experiences, ideas, and challenges.

Dr. Pablo Sepúlveda will be the Symposium Coordinator of a brand-new one day Pediatric Sedation Symposium that will feature lectures and a TCI Workshop entirely in Spanish. Harvard Medical School faculty and sedation experts from Latin America will join together to present this special offering.

GAIL A. VAN NORMAN, MDKeynote Address: Suffering, Transcendence and the Oblitera-tion of Consciousness: Deep Conscious Sedation in Children at the End of Life

Gail Van Norman MD is board certified in both Internal Medicine and Anesthesiol-ogy, and specialty trained in Cardiothoracic Anesthesiology. With experience in both private practice and academic anesthesiology, she developed an early inter-est in the ethics of clinical practice and trained in clinical ethics under Al Jonson in the department of Bioethics at the University of Washington. She became a founding member of the Committee on Ethics for the American Society of Anes-thesiologists, and served as its Chair from 2008 to 2011. After serving as Vice-President for a large private-practice group of anesthesiologists in Washington State, she returned to academic medicine to lead the PreAneshthesia Clinic at the University of Washington Medical Center, where she currently practices anesthe-siology and preoperative medicine. She is a Professor of Anesthesiology and Pain Medicine, and Adjunct Professor of Biomedical Ethics at the University of Wash-ington. She is the Editor-in-Chief of the the book, Clinical Ethics in Anesthesiology: a Case-Based Textbook, and has authored numerous ethics articles for medical journals and chapters on ethics for clinical textbooks.

TRAVIS ROY Keynote Address: Overcoming the Challenges that Choose Us

From an early age, Travis found his passion in the sport of hockey. Travis dreamed of one day playing Division 1 college hockey, and even having his shot at the NHL and the Olympics. In the end, Travis chose to play for the defending national champion Boston University Terriers. On October 20, 1995, with his family and friends in the stands, he took to the ice for

his first shift. Eleven seconds after the puck dropped, Travis found himself lying face down on the ice, unable to move or feel a thing. Within seconds he not only knew he was para-lyzed, but that life as he knew it was over. The prognosis was that he would be a quadriplegic for the rest of his life.

In the 19 years since Travis’ accident he has overcome many challenges. Ten months after his accident Travis returned to Boston University, and four years later he graduated with a degree in Communications. He wrote the book, Eleven Sec-onds, A Story of Tragedy, Courage, and Triumph, and also founded the Travis Roy Foundation, which has raised over $3 million to improve the lives of spinal cord injury survivors and fund medical research.

COURSE DESCRIPTIONBACK BY POPULAR DEMAND! A Comprehensive Review of the Essentials of Pediatric SedationThis one-day course is a comprehensive introduction and review of the fundamentals of pediatric sedation. This offers a unique opportunity to update your knowledge in the format of didactic sessions, open forum question-and-answers and clinical case reviews. An optional hands-on workshop will provide personalized one-on-one faculty attention to gain hands-on experience using manikins to practice basic airway resuscitation skills.

This course is designed to provide a review of topics related to the safe and effective delivery of minimal, moderate and sedation by inhalation, enteral, and parenteral routes of administration. This course is intended to be an introduction and review of the up-to-date policies and guidelines governing sedation. The topics will include physiological monitoring, airway evaluation, the etiology and management of central and obstructive apnea, patient assessment, discharge and recovery guidelines, risk factors for adverse events, emergency preparedness, non-pharmacologic techniques and the basic pharmacology of commonly used sedatives. Clinical case presentations will be presented in an interactive format to maximize discussion, collaborative sharing of ideas and consideration of all options. Case studies will include challenging and complex situations. Question and answer sessions will provide participants with forums to ask questions and discuss specific issues with course faculty.

This course will feature recognized leaders and faculty from different specialties, all leaders in the field of sedation. The attendee will have the opportunity to gain hands-on experience learning airway techniques to rescue patients from deep sedation, including manikin practice with face masks and ambu bag along with the insertion of nasal and oral airways in a personalized setting with manikins and real equipment.

LEARNING OBJECTIVESUpon completion of this activity, participants will be able to:

• Review current rules and regulations regarding minimal, moderate and deep sedation.

• Evaluate the pharmacology and clinical applica-tion of commonly utilized sedatives and weigh the advantages, disadvantages, efficacy and adverse events associated with each option.

• Evaluate complex and challenging sedation scenarios and formulate a plan.

• Provide a comprehensive review of patient evalu-ation including airway assessment, relevant medi-cal history and vital signs necessary to evaluate patients as appropriate sedation candidates.

• Apply the discharge criteria and available recovery guidelines for discharge home following sedation.

• Determine the role and application of capnography.• Prepare for unanticipated emergency situations.

SYMPOSIUM SCHEDULE

This full-day Introduction to Sedation Outside the OR symposium provides an overview of sedation and is directed toward a multidisciplinary audience that includes physi-cians, dentists, nurses and trainees. The morning session “sets the stage” for successful sedation and challenges the participants to consider their goals for sedation and how best to achieve them. Morning presentations address pediatric-specific anatomic and physiologic considerations, presedation assessment, appropriate monitoring and personnel as well as the Pediatric Airway Workshop; all of which are directed toward both medical and dental participants. The remainder of the session targets medical- or dental-specific groups to reflect particular interests and practice. Case scenarios will be presented with the intent to foster vigorous discussion of options for sedation and best practice models. The day will reach a conclusion as the medical and dental groups reconvene for interactive Simulated Medical and Dental Sedation Demonstra-tions. These interactive video simulations are designed to provide medical and dental participants the opportunity to directly observe the application of sedation outside the OR and raise questions regarding best practice. The simulations will emphasize the principles and skills presented during the symposium and allow participants the opportunity to reinforce their knowledge and understanding of the day’s curriculum.

PEDIATRIC SEDATION SIMULATION WORKSHOP OBJECTIVESThis course is intended for practitioners providing IV moderate sedation and deep sedation and offers an educational high fidelity simulation experience to provide skills for successful recognition and management of sedation-related complica-tions. This unique and challenging program provides a clinical simulation experience that addresses the proper assess-ment and management of complications associated with moderate sedation, deep sedation. Following a brief introduction explaining essential principles, participants will demonstrate appropriate patient management on human patient simula-tors. (Laerdal SimMan). This full-day simulation workshop will provide each registrant with six clinical scenarios intended to improve individual skills as well as to impart an understanding of a team approach to crisis resource management. Each registrant will become part of a small team which will will rotate together through each of the six scenarios over the course of the day. The scenarios are intended to improve individual skills as well as the team’s reactions and responses. Effective post-simulation debriefings will integrate critical performance feedback, promote reflective practice, and aug-ment future performance. The full-day of high fidelity simulation scenarios are taught by national and internationally rec-ognized faculty of simulation experts and aims to provide participants with knowledge and skills to recognize and manage sedation-related emergencies.

Course registration is limited to provide each registrant with an optimal, educational hands -on experience. There will be an optional “working” lunch which offers the opportunity to have a hands-on experience with personalized faculty atten-tion using manikins to develop and improve airway skills. The Basic Airway Workshop is intended to solidify skills at deliv-ering positive pressure ventilation using a face mask and ambu bag along with the insertion of naso and oral airways.

Medical simulation is a highly adaptable tool for evaluating individual knowledge and team performance. Medical simula-tion can improve patient safety by raising an individual’s awareness of their practice and by enhancing team cohesion and performance. Simulation provides a safe arena for learning and evaluation, trains with actual equipment used in the clini-cal realm, and provides for repetition and reflection. Medical simulation should be educational,reflective and fun.

Upon completion of this activity, participants will be able to:• Explain the importance of human-factors leading to and in the management of crisis and prepare for the management

of rare events.• Summarize the components of a simulation based learning encounter and determine how to incorporate simulation

into the process of training sedation care providers.• Establish and demonstrate appropriate components of an adequate primary assessment.• Develop an algorithm for the recognition and management of various anatomical and pathological causes of airway

obstruction.• Create a differential diagnosis for the etiology of respiratory depression and understand the different management

techniques.• Demonstrate appropriate management of cardiovascular compromise.• Demonstrate proficiency in directing a team during a crisis.

Specifically, the healthcare professionals have represented the specialties of anesthesiology, radiology, emergency medicine, hospital medicine, pediatrics, dental medicine, dental anes thesiology, intensive care medicine and gastroenterology.

COURSE DESCRIPTIONThis annual conference entitled, Pediatric Sedation Outside the Operating Room, is taught by multi-specialists, renowned in pediatric sedation, from both the United States and internationally. This course offers both didactic lectures and panel discussions. Optional hands-on basic airway workshops will also be offered. Small round table discussions with the faculty and afternoon concurrent tracks provides the attendee with an opportunity for discourse and interactive discussions.

The field of sedation continues to evolve and policies, recommendations and Center for Medicaid and Medicare Service guidelines change in response to the health care environment. Currently, sedation guidelines and recommendations differ between specialty societies: For example, the American Society of Anesthesiologists (ASA) guidelines on deep sedation are not concurrent with those of the American Society of Gastroenterology nor American College of Emergency Physicians. Sedative agents, delivery methods and routes of delivery are changing as new techniques and outcomes continue to be explored and presented. This course is designed to fill the practice gaps created by this combination of evolving practice guidelines, differing practice standards among specialty societies, and continuing advances in sedation medicine and tech-nology by updating and reviewing state-of-the-art techniques for safe and effective delivery of sedation.

This conference is intended to improve learner competence and performance by reviewing not only the basics of sedation but also updating the registrant to current, evolving and state-of-the art practice (i.e. knowledge). The hands-on work-shops on airway management and patient simulation using mannequins will allow the learner to apply this competence to their everyday practice (i.e. performance) and prepare for the rare, unanticipated emergencies. Being prepared for emer-gencies as well as for the more common adverse events, the learner will be able to be better equipped to improve patient outcome. This confer ence is to increase the participant’s breadth of knowledge of pediatric sedation along with elaborate on ways to improve and track patient outcomes.

LEARNING OBJECTIVESUpon completion of this activity, participants will be able to:• Analyze recent changes in guidelines, regulations, policies and future developments in sedation systems and their

impact on patient care.• Complete an in-depth review of physiologic monitoring and develop an appreciation of the role and interpretation of
