Training Purpose

To review the new Outcomes Reporting functionality within CYBER.

What is Outcomes Management Reporting?

A reporting system that gives treatment providers (CME and OOH

providers) who complete Assessments (Needs, Needs with BPS, Crisis, and Strength

and Needs) the ability to track an individual child’s progress, as well as

clinical outcomes, over a period of time.

Benefits

Outcomes Reports will include assessment data for a youth, up until the date selected, showing the progress made over time.– Will include up to 18 assessments (of the same type, starting

with present day and going back to September 2002).– Reports are updated with current assessment data

immediately after the assessment is submitted.

Outcomes Reports will assist in treatment planning and in determining the appropriate intensity of service that best suits the youth’s needs.

Four Assessments

Outcomes reporting is available for four assessments:– Strength and Needs– Crisis Assessment Tool– Needs Assessment– Needs Assessment with BioPsychoSocial

Generating a Report

Search for a youth in order to access their Assessments using the Your Caseload button (1), the Quick Search (2), or the Youth/Child Search (3).

Go to the youth’s Treatment Plans and Assessments screen.

Select an Assessment to generate the report by clicking on it once in the grid.

Report will include data points for past Assessments, including the Assessment selected, for up to 18 assessments (going back from September 2002 to present day); all other information will be specific to the selected assessment.

Click on the “Get Outcome Report” button.

Report will open on the View Report tab

Report is printable from this screen, and can also be exported to four formats – PDF, Excel, TIFF file, Rich

Text Format.

Each report will include:

Demographic information for the youth from the Demographics accordion of the selected assessment.– Including the Name of Assessor, Assessor Agency, contact

information, and current Case Management Entity.

Graphical displays of assessment data (including a totals graph which will plot the total score of up to 18 assessments, including the one selected, and individual graphs for each domain of the assessment).

Areas rated a 2 (needing action) or a 3 (needing immediate action) will be listed individually as Alert Items.

All comments included in the selected assessment.

Points to Remember

Data will be plotted on each graph using the following dates as guidelines:

– The Assessment Date will be used if entered.

– The Assessment Completion Date will be used if the Assessment Date is missing.

– The Submit to CSA Date will be used if both the Assessment Date and Assessment Completion Date are missing.

Points to Remember

Each domain of the assessment will be scored using the following calculation:

– Total score of the domain/number of areas in the domain x 10

– For example; A youth receives a total score of 14 in Life Domain

Functioning. 14 divided by 13 (total number of areas in the domain) equals 1.1 (rounded up). 1.1 times 10 equals 11. 11 is the score that will be plotted on the grid for this domain in this assessment.

Points to Remember

If an assessment is selected that was submitted to the CSA prior to September 2009, the historical data will include areas that were rated as a 2 or 3 in the Alert Items section of the report; comments in each domain will not be included.

DemographicsDemographic information populates from the Demographic module of the selected assessment.

Alert Items

Overview of items rated either a 2 (Areas Needing Action) or a 3 (Areas Needing Immediate Action), separated by

Domain, for the selected assessment.

Sample Totals Graph The Total Scores graph will

plot the total combined score of all domains for up to 18 assessments of the same type as the one selected by the user.

Each provider type will be indicated with a different color on the graph, as identified in the legend, and will appear in the informational table below the graph with the provider’s name as it appears in CYBER.

Points to Remember

Provider Type – “UNK” – Unknown Provider Type

– Caused by historical data that did not fully map into CYBER or a site-level provider that doesn’t have an abbreviated ID.

– Providers may need to refer to the youth’s record to research who completed previous assessments.

Points to Remember

Number of Assessments – this number may not match the number of data points in the Total Scores chart.

If more than one assessment was submitted on the same day for a youth, the last assessment will appear on the chart. Multiple submissions were possible in previous CYBER releases but functionality released in early 2011 does not allow this to occur.

Comments, when entered, for each domain for the selected assessment will appear on the Outcomes Report with the score associated with the section.

Medications, Diagnosis and Recommendations

Medications will include current and past medications, as indicated on selected assessment. (Medications are not included on the Crisis Assessment.)

Diagnosis information will populate from the selected assessment.

Recommendations will appear on Outcomes Reports for Needs Assessments, Needs with BPS and Crisis Assessments.
