How Much Does it Cost?
Yearly fee: $299 per person, $269 for each
additional family member.
Renewal: Membership expires 12 months after
joining, on the last day of that month (e.g., sign up
Jan. 10, 2016, membership expires Jan. 31, 2017).
Does it Save Me Money?
If you don’t have dental insurance now, you’ll save
money just on exams and cleanings alone. The plan
costs even less for additional family members!
What Are the Exclusions?
Our Dental Health Club is NOT a dental
insurance plan and can only be used in our
office.
Membership is non-refundable and
non-transferable.
Membership benefits can NOT be combined
with ANY dental or medical insurance plan
or workman’s comp.
If you have periodontal disease, the two
preventive cleanings covered in the plan
will commence after successful periodontal
treatment.
Other Membership Information
12-month membership fee is due in FULL upon
joining. Membership is effective on the day
payment is received.
It is your responsibility to maximize membership
benefits by scheduling all appointments within the
12 month membership period.
What Else Should I Know?
If appointments are not used, you will NOT be
entitled to a refund. Note: No-show/same day
cancellation for a preventive cleaning uses up one
of the 2 preventive cleanings covered by
membership benefits.
In order to receive the 15% courtesy discount on
additional dental treatment, payments are due in
FULL at time of treatment. If Care Credit is used,
you’ll still enjoy a 10% discount (because our
practice covers their lending fees).

Dental Insurance plans restrict your dental health
care. Our Dental Health Club discounts ALL your
dental care with no hassles. YOU will be in control
and deal directly with US!
For a flat annual fee members will
receive, for a full year:
2 preventive cleanings and periodontal disease
evaluation
2 dental exams by our Doctor, including oral
cancer screening
1 set of full mouth x-rays, if needed
2 fluoride treatments (as prescribed by our Doctor)
15% courtesy discount on ALL additional dental
treatment at our office. Includes ALL general
dentistry procedures (periodontal cleanings,
fillings, implants, extractions, crowns, and more)
No insurance? No problem!
