FUNCTIONAL THUMB, ACTIVE LIFEMANAGEMENT OF OAAs OA is a chronic disorder, the therapy goal is to manage symptoms and retain the quality of life for the patient. Acute pain management and inflammation reduction is obtained through medication, such as NSAIDs.

The therapy is often further supported by the use of adequate immobilization devices to rest the CMC joint, such as splints and braces. This way inflam-mation can be reduced and provides long-term durable pain relief and functional improvement.1

In times of reduced pain, wearing braces during daily activities, such as gardening or doing the washing up, can protect the joint from additional stress. Another option is to only wear a brace at night-time for further rest. Using braces that allow for the best combination of functionality and immobilization will improve patient compliance and acceptance.

Surgery may be required when the impact upon the patients’ daily life has become so significant, that it is intolerable and patients no longer respond to other therapies. Most surgical options remove some or all of the arthritic bone and use different techniques to stabilize the joint. Alternatively the total joint is replaced or fused. The procedures are followed by an extended immobilization period with splints, casts or braces. The treatment is rounded off with functional training.

MANAGEMENT OF ACUTE UCL INJURIESThe main goal of managing UCL injuries is to restore the full functionality of the thumb.

The options are selected based on the grade and severity of the injury, as well as other patient related factors, such as physical health and age.

Partial tears can be managed by immobilization only. There is a wide range of devices used, like splints, casts or special thumb braces. The duration of therapy depends on the severity of the injury.

Complete tears are usually treated surgically. Various techniques are used to repair the torn ligament. Post-operative immobilization for up to six weeks is required to protect the surgical site.

After the immobilization period, targeted functional hand training is indicated to regain the mobility and strength of the thumb.
