Do not submit a case with any missing data at the margins; this can compromise the fit. Scan at least 4 opposing teeth for the bite registration to ensure the bite is aligned properly.

Insufficient scan data. Full scan of all anteriors are recommended on anterior restoration cases to recreate the same shape and size of the existing teeth and for articulation purposes.

Gingival Retraction• Proper gingiva retraction enables good scanning of the

margin line.• Intra-Oral scanners can only scan what is visible to the eye.

If the margin is subgingival, the scanner will have a difficult time capturing the margin without excellent retraction techniques.

• The two-cord retraction technique is recommended for a visible margin line.

Moisture Control• Dry teeth using compressed air.• Be sure to dry the interproximal and gingival areas of the

arch.• Use cotton rolls, dry tips, and saliva ejectors to keep the

mouth dry during scanning.• Make sure the scanner wand is clean and free of moisture.

Scan Sprays (if required by scanner)• Teeth must be dry before applying scan spray. Saliva and

spray can mix and may cause false scan data.• More is NOT better. Over-spraying can cause 3D images to

be inaccurate.• Sprayed teeth must be completely dry before scanning.

Missing and distorted data on 3M scan. This is most likely caused by wet spray/excess moisture in the mouth.

Missing data on adjacent teeth and occlusion. This can cause issues when fabricating models and articulating the bite.

The lingual margin data is missing in the image on the left. The right image shows the scan after the margins are fixed by 3M. Notice how the fixed areas are not smooth. A new scan is recommended on this case.

Intra-Oral Scanning Tips

Sufficient scan of adjacent and opposing teeth to properly articulate models.

Proper retraction to accurately mark margin. Sufficient scan data without missing data or distortion.

A full arch was provided to aid in articulating the bite, since there is no posterior stop.

Small areas on the adjacent teeth or opposing will not affect the case.

Smooth scan of all teeth and margin line.

Clear view of margin and all teeth are complete.

Recommended Data to Capture When Scanning

Anterior Crowns• Scan all anterior teeth so the restoration can be adapted to

the patient’s existing dentition.• If possible, scan a pre-op model of existing pre-prepped teeth

or temps for diagnostic/esthetic purposes.

Posterior Crowns• Scan at least 2 adjacent teeth on the mesial & distal of the

restoration.

Restorations With No Posterior Stop(s)• Full arch maxillary and mandibular scan is needed to ensure

bite is correct in design software.

Virtually chip-proof, BruxZir is a full-contour zirconia crown, bridge, inlay or onlay with no porcelain overlay. Individual dies and model sections are scanned using laser technology. Margins are marked and path of insertion determined digitally.

BruxZir crowns and bridges are computer designed. Interproximal contact pressure is consistent and digitallymeasured in microns.

Occlusal morphology is designed using anatomy libraries. Premature occlusal contacts are eliminated in the software.

Fabricated crowns are sprued and positioned in pre-sintered, pre-shaded authentic BruxZir zirconia disks.

Crowns are then milled to the exact CAD specifications and fired to complete the crystallization, shading, and hardening process.

IPS e.max® is a lithium disilicate glass-ceramic, digitally fabricated. With pricing comparable to a non-precious PFM and the ability to conventionally cement in posterior or anterior regions, you simply cannot go wrong by prescribing this material. Please remember to take a stump shade in addition to the final crown shade. Lithium disilicate is translucent and unable to block out the two darkest stump shades.

Monolithic Synergy: IPS e.max® & BruxZir® Solid Zirconia

IPS e.max is the restoration of choice in the anterior region due to its translucency and esthetics. BruxZir Solid Zirconia was used in the posterior for fracture toughness and wear resistance.

Monolithic restorations such as IPS e.max and BruxZir are becoming extremely popular due to their reliability and esthetics. Unlike PFM’s or porcelain fused to zirconia restorations, monolithic restorations are one solid material throughout. Models are scanned using laser technology. Crowns, copings, and frameworks are designed simultaneously under full contour designs. Metal collars, occlusal rests, metal occlusals or any design variations are built into the design digitally. Porcelain and metal thickness are digitally measured for optimal porcelain support and esthetics.

Designs are placed into a virtual articulator for optimal centric occlusal contacts and proper guidance in protrusive and lateral excursive movements.

Designed PFM copings and full contour sections are positioned onto the perfactory printer plate prior to printing.

Perfactory printing technology rapid prototypes all the design files into wax resin parts ready for processing.

Printed copings are sprued, invested, and cast out of the FDA cleared alloy of your choosing.

Printed full contour shells are sealed to alloy copings, sprued, and pressed using high quality ceramic ingots in the prescribed shade.

TCS® Unbreakable is a flexible denture base resin which is ideal for partial dentures and unilateral restorations. It provides a fully functional partial that looks better, feels better and is more durable than conventional metal rigid partials. The resin is a biocompatible nylon thermoplastic that allows your patient’s natural tissue tone to appear through the material. The translucency of the TCS material makes it the ideal product for cases when patients do not choose a fixed restoration or implant, but still want a highly esthetic restoration. TCS uses the Retento-Grip® tissue-bearing technique for retention, so no tooth or tissue preparation is needed.

Case Planning for Flexible Partials Unlike Acrylic Dentures, there is no chemical bond between acrylic teeth and flexible partial material. The denture teeth must be surrounded by flexible partial material, thus teeth cannot be ridge lapped or abutted to the ridge. The teeth are drilled to create mechanical retention. It is necessary to have 5mm vertical space to achieve the proper mechanical retention of the denture teeth; any less and the teeth will fail. The flexible material is nearly unbreakable, but the teeth WILL fail from over reduction and lack of clearance.

When should DuraFlex & TCS Unbreakable be used? DuraFlex and TCS can be used to create flexible removable partials, unilateral partials, temporary appliances for healing implant patients, and saddle/clasp areas for cast metal partials. Duraflex and TCS are great replacements for patients that are not interested in having metal appliances. They have a more comfortable fit and will be lighter in the mouth, easier to eat with and have a “snap fit” giving confidence throughout the day. The retention will remain the same throughout the life of the partial. Patients who have compared conventional cast frame partials to our thermoplastic partials report that they feel more natural in the mouth.

Case limitations include:• Collapsed bites are contra–indicated.• Deep bites where upper anteriors cover lower anteriors will fail

without use of metal backing/facing.• Metal backing supports the teeth and protects them from

occlusal forces.• 5mm of vertical clearance is required unless using metal

backing/facing, especially on isolated anterior teeth.

Flexible partial denture bases typically flex towards the center of the mouth. This can create undue stress on abutments. To eliminate follow these guidelines:• Lower arches with few abutments (ie: 22, 27) and/or extremely

resorbed ridges should incorporate a metal substructure to keep the partial more rigid.

• Tall isolated teeth and teeth with large degrees of undercut to the buccal should be avoided as abutments when possible.

Flexible partials are tissue born and free movement stresses the abutments.

Impression TechniqueOne of the many benefits of thermoplastic partials is that little to no tooth preparation is needed. We require only a master and an opposing model. A custom tray is normally not necessary. For a precise fit and minimal adjustments we highly recommend an alginate impression be used. Alginate impression materials will record the tissue at rest instead of under load and result in a better fitting flexible partial. Please also send an opposing model and bite registration.

Insertion & Adjustment TechniquesPrior to the initial insertion, soak the partial in hot water (not boiling) for one minute to soften the material and ease the insertion process. The Hot Water Treatment is very important as it allows for a smooth initial insertion and a good adaptation with your patient’s natural tissues. When the partial cools to a tolerable temperature, gently insert it into the patient’s mouth.

Indications for DuraFlex® & TCS® Unbreakable • TCS material is less flexible than DuraFlex

and is therefore better suited to clasp-less types of flippers that only utilize lingual undercuts.

• TCS is better suited to patients with large edentulous spaces due to it’s increased opacity compared to DuraFlex. In large edentulous areas where the material is very thick translucency can cause shadowing.

• Duraflex is more translucent than TCS and therefore better suited to patients concerned about clasp visibility.

• Duraflex is better suited to teeth with severe buccal inclinations due to the

If the patient senses discomfort due to tightness of the clasp, the clasp may be loosened slightly by submerging in hot water for approximately 30 seconds, removing it and gently bending the clasp outward while holding it underneath running cold tap water. Following the same procedure but bending the clasp inward can also tighten the clasp. If there is a need for adjustment when trying the partial in the patient’s mouth, the thermoplastic must be handled differently than an acrylic material. It is best not to use traditional carbide burrs when adjusting flexible partials. We recommend fine stone and rubber points for most adjustments and a coral stone for areas that require more material to be removed. Set your hand piece on a low speed and use fast, light scraping back and forth movements to reduce area. Do not keep bur in one spot, be sure to keep it in constant motion. Do not use too much pressure as this may burn or distort the restoration. If there are any threads remaining, these can be removed with a sharp blade. Preparation for Adding a Tooth A pick-up impression technique must be used. It is essential to remove all pastes or adhesives from the tissue-surface of the partial before taking the impression. With the partial in place in the mouth, take an alginate impression. Remove the impression from the mouth with the partial in the impression. Be sure the partial is fully seated in the impression material. Pour the model promptly, DO NOT separate the partial from the stone model. Ship the materials to Oral Arts with instructions to add the missing tooth or teeth and retention (if necessary).

Preparation for a RebaseIn most cases where an overall rebase is needed, the case is jumped into a new base. A rubber base impression is taken under the partial in a closed-mouth condition. The patient closes his/her mouth gently to ensure a correct occlusion as the rubber base is setting. Once the rubber base material has set, an alginate impression is taken and the impression is removed from the mouth with the partial and the rubber base in place. Carefully examine the partial to assure that it is positioned correctly in the alginate material. Immediately pour the model with regular yellow stone. DO NOT separate the partial from the stone model. Forward to Oral Arts with instructions. It is also recommended that a new fully edentulous alginate impression be taken of the arch we are working on to faciliatate the rebase.

It is Easy for the Patient to Take Care of the Appliance We recommend that the patient soaks and rinses their partial daily to keep it clean. It is important to remember that abrasive toothpaste can be harmful to your partial. (Most toothpastes will indicate wether or not they are abrasive.) We recommend non-abrasive TCS® Concentrated Denture Cleaner to clean your patient’s partial.

Step-by-Step Restorative Protocol

Consider anterior-posterior spread before moving forward with the BruxZir Solid Zirconia Full-Arch Implant Prosthesis option. Although a closed-tray impression technique is described here, open-tray impressions are acceptable.

Take an implant-level impression, including the palate and vestibules for maxillary impressions.

a. Remove the healing abutments from the implants.

b. Seat the impression copings on the implants and tighten the screws (Fig. 1). Take a radiograph to verify complete seating. Check the impression tray for proper fit.

c. Take an implant-level impression of the edentulous arch (Fig. 2). After allowing the material to set, carefully remove the impression tray, loosen the screws and remove the impression copings.

d. Replace the healing abutments.

e. After attaching implant analogs to the impression copings, carefully replace the impression copings back into the impression.

f. Send in the case with a lab RX that identifies the system and diameter of the implants.

Note: If the implants are more than 2 mm subgingival, have screw access holes that are too far to the facial, or require an angle correction greater than 10 degrees, an abutment-level restoration is recommended.

Sectioned and numbered implant verification jig on initial model

To ensure a passive fit of your restoration, it is vital to obtain an accurate final impression. You will receive a custom tray and an implant verification jig (IVJ) on a working model. This procedure should be followed to ensure an accurate final impression.

Verification Jig Remove the healing abutments from the implants.

Seat each section of the jig onto the appropriate implant and tighten the guide pin (Fig.3 & 4).

a. The sections should not be in contact. If necessary, remove one section, minimally trim it with a disc, and reseat it. Each section should have a gap about the thickness of a credit card. Visually verify gaps before luting.

Lute the sections together with a suitable material (e.g., Triad® DuaLine® – DENTSPLY; Pattern Resin™ – GC America; Zapit® – Dental Ventures of America or ADDS-IT – American Diversified Dental Systems) (Fig. 5).

a. Allow the material to flow through and completely around the gaps (Fig. 5).

b. Ensure the material is completely cured (Fig.6).

c. If any section has a cylinder-implant interface that is subgingival, a periapical radiograph should be taken to verify complete seating.

d. If any cylinder is not completely seated, the jig must be sectioned in that area, reluted and rechecked until a passive fit is obtained.

e. Check for a passive fit of the Custom Tray (no contact with jig or cylinders) prior to taking the impression (Fig. 7).

Final Impression Using medium body PVS material, take the final impression with an open-tray technique.

a. Inject impression material under and around the jig to capture the ridge and all anatomical landmarks as for a full denture including full vestibular extensions and the complete palate (Fig. 8).

b. Seat the filled impression tray ensuring the heads of the guide pins are exposed through the tray (Fig. 9).

c. Once the material has set, remove guide pins and then remove the impression (Fig. 10).

Note: the verification jig is picked-up in the impression. Inspect the impression for the required detail.

Reseat and tighten the healing abutments.

Bite RimYou will receive a bite rim from Oral Arts.

With the patient sitting up, use conventional denture techniques to achieve accurate jaw relation records.

Take an impression of the opposing dentition and an impression of current denture for study model.

Select the shade and mold of the denture teeth. The study model for the patient’s existing denture can be used as a reference regarding the size and shape of the new teeth.

Return the case to Oral Arts with the master model, bite rim, bite registration, opposing impression and study model.

■ FOURTH Appointment Delivery of Final Prosthesis

You will receive the final prosthesis from Oral Arts.

Remove the healing abutments or the optional provisional CAD/CAM PMMA verification bridge.

Seat the final prosthesis on the implants.

Hand tighten the prosthetic screws, alternating from one side to the other.

Tighten the screws to the appropriate torque per manufacturer instructions. Wait approximately 5 minutes and retorque the screws (Fig. 14).

Confirm the occlusion (Fig. 15). Make adjustments as necessary.

Place a small amount of cotton in the screw access holes and fill with light cure composite or acrylic to prevent bacteria build-up (Fig. 16).

You will receive a wax setup from Oral Arts. The provisional CAD/CAM PMMA verification bridge will also be included if it was ordered.

Seat the wax setup (acrylic base with teeth in wax).

Evaluate the VDO, CR, esthetics, shade, tooth arrangement, occlusion, phonetics and midline (Fig. 12). If CR is incorrect, a new bite registration should be taken.

Send clinical photos with patient at rest and smiling.

Install the optional provisional CAD/CAM PMMA verification bridge according to Fourth Appointment instructions.

Send in entire case, including the wax setup, opposing model, and lab Rx with reset instructions (if necessary). If a reset is necessary, new bite registration should also be taken (Fig. 13).

Note: Tooth-colored composite or acrylic should be used for access holes in the teeth, while pink tissue or acrylic should be used for access holes in the prosthesis base

The Locator Bar Overdenture distributes occlusal loads more evenly across all implants and is indicated for patients with a heavy bite and when the implants have been placed in softer bone; the prosthesis is tissue and implant-borne.

Limited Warranty/Limitation of LiabilityOral Arts Laboratories (“the lab”) warrants that all dental devices (a “device”) are made according to your specification and approval in the belief that the device will be useful and makes no other warranties including, but not limited to, any implied warranty of merchantability or fitness for a particular purpose. This Limited Warranty and Limitation of Liability (“Agreement”) will be extended with the presentation of a verified proof-of-purchase, which includes an original invoice number/date, patient’s name and the original model. This limited warranty extends only to the original purchaser of a device and does not extend to patients or other individuals and/or entities to whom the device(s) is/are sold, implanted, inserted or delivered. Subject to the return of a device that is placed and then fails, the lab will repair or remake the device without charge for the cost of materials and workmanship or credit the original price paid, at the lab’s option, as follows: (1) porcelain to metal, all porcelain, all metal, single-unit inlay, onlay and crown composite resin final prosthetics (excluding mutually opposing implant- supported full arch bridges), milled implant bars, and screw-retained titanium or zirconia abutments (excluding abutments with angulations greater than 20 degrees), up to seven years; (2) composite resin bridges (excluding Maryland and inlay/onlay bridges) up to five years; (3) dentures and partials including screw-retained dentures but excluding immediate dentures and partials up to one year if the failure is due to defects in materials or workmanship; (4) thermoformed appliances and splints if the failure is due to defects in materials or workmanship, temporaries, composite resin Maryland and inlay/onlay bridges, up to six months; (5) immediate dentures and partials, flippers, retainers, surgical and radiographic guides, and all other dental devices up to thirty days if the failure is due to defects in materials or workmanship. You agree to pay all other costs of adjustment, repair and replacement of a device. Except where prohibited by law, the lab will not be liable for any loss or damages arising from the use of a device, whether direct, indirect, special, incidental or consequential, regardless of the theory asserted, including warranty, contract, negligence or strict liability and if such disclaimer is not permitted by law, the duration of any implied warranty is limited to 90 days from the date of delivery. The lab does not guarantee the performance of independent carriers. Oral Arts will only accept returns of implant parts on un-opened packages with the product in its original condition. The parties to this Agreement mutually agree to waive any and all class actions in favor of mandatory individual arbitration of all claims arising out of, or related to, this Agreement. Any controversy or claim arising out of, or relating to, this Agreement, or the breach thereof, shall be settled by arbitration administered by the American Arbitration Association. The place of the arbitration shall be Huntsville, Alabamaand Alabama state law shall apply.

Shipping PolicyOral Arts pays the cost of shipping your cases both ways! While many labs have instituted shipping charges at least one way we believe that getting your cases to and from the laboratory still remains our responsibility. You have several convenient options when shipping your cases. Choose from our local courier service in areas where applicable, FedEx, or UPS. We will provide you with complimentary airbills, packing materials, and boxes. This policy does not apply to overnight deliveries, weekend pickups, or insurance coverage.
