Purpose

This procedure outlines the process for optometry screening and the provision of eyeglasses or magnification reading spectacles to detainees at the Alexander Maconochie Centre (AMC), people admitted to the Dhulwa Mental Health Unit (DMHU) and young people at the Bimberi Youth Justice Centre (BYJC).

This procedure does not apply to the provision of emergency eye care.

This procedure applies to Justice Health Services (JHS) Primary Health staff working at adult correctional centres, the DMHU and the BYJC.

The procedure relates to all people in adult correctional centres and people admitted to the DMHU.

This procedure applies to people at adult correctional centres with a sentenced or remanded length of imprisonment that exceeds 12 months. People who are anticipated to be imprisoned for less than 12 months are not eligible for optometry services unless there is a co-morbid diagnosis impacting on sight e.g. diabetes.

This procedure applies to people who are admitted to the DMHU for a time that is likely to exceed 12 months unless there is a co-morbid diagnosis impacting on sight e.g. diabetes.

There are no time limits on young people at the BYJC to access optometry services.

A sentenced detainee at the AMC can request an optometry examination by submitting a Health Request Form. This form is obtained from either nursing or administration staff. The person will then be booked into a Nurse Clinic to discuss their eyesight concerns.

A person admitted to the DMHU can request an optometry examination by speaking with their doctor or allocated nurse. The allocated nurse will book the person in to see the JHS Population Health Nurse, who will visit DMHU to conduct the examination.

Young people at the BYJC can speak to the nurse on duty if they would like to see the Optometrist. For young people at the BYJC each request to see the optometrist will be reviewed on a case-by-case basis.

Once the nurse has assessed the person and performed a basic eye test, the nurse will make a recommendation on the Optometry Assessment and Recommendation form, available from Q:\MH\Justice Health Services\POLICIES, PUBLICATIONS & SOPS\JHS Policies and SOP's\Policies\JHS Optometry Services – 2017 and recommend an action or triage an appointment with the JHS Optometrist or medical officer as appropriate.

Referral must be made immediately to a medical officer if any of the following are of concern: Sudden change of vision; Unilateral tearing; Recalcitrant unilateral red eye; Nystagmus; Eyelid swelling; or Other changes in vision can be referred to the Optometrist.

People diagnosed with diabetes require a referral directly to the optometry clinic where the JHS Optometrist will complete an external referral for them. The person is not required to have a nurse assessment or to attend in person to see the JHS Optometrist at this time.

If after performing a basic eyesight check, the criteria to see an Optometrist are not met, a recommendation may be made for suitable strength for magnification reading spectacles. Magnification reading glasses can be purchased by the person. A magnification chart that can be utilised is at Attachment A.

All assessments and recommendations are to be recorded in the consumer’s electronic clinical record (ECR).

Section 2 – Optometry Services

All people receiving services from JHS are eligible for one optometry screen every two years unless clinically indicated, when a person will see an Optometrist sooner. This appointment will be provided by a qualified Optometrist selected by JHS. Optometry examinations conducted more frequently will be paid for by the person.

People in the care at DMHU and who are able to access leave and have this approved through the DMHU Leave Panel may be taken to a community Optometry provider where they will access services through their Medicare entitlement. Should glasses be recommended and the person has access to finances the person will be able to select frames and lenses based on their financial capacity. If the consumer does not have leave, the DMHU

Social Worker will work with the consumer to identify their preferred spectacle type and obtain the spectacles on their behalf.

Section 3 – Eligibility for Spectacles

JHS will provide one set of lenses-and-frames to eligible people every two years dated from the first issued pair.

Prescription glasses must be prescribed by an Optometrist contracted by JHS for the provision of providing optometry examinations.

Prescription glasses are to be dispensed only by Optometrists or by approved JHS’s optical dispensers.

Non-essential extras such as transitional lenses can be supplemented at the person’s own cost. Prescription glasses are provided as follows:o Basic plastic frames and acrylic scratch-resistant lenses only will be provided, unless

otherwise clinically indicated. The need for alternative frames and/or lenses is to be documented in the person’s ECR.

o One pair of prescription glasses will be provided with one of bifocal, trifocal or progressive power lenses, except in circumstances where one pair of reading glasses and one pair of distance spectacles would better meet the clinical needs of a person.

o Tinting and prescription sunglasses will not routinely be provided, unless clinically indicated. The need and specific medical condition, including diagnosis, will be documented in the client’s health record.

Contact lenses are not provided, unless otherwise clinically indicated. The specific condition requiring the issuance of contact lenses, including diagnosis, shall be documented in the person’s ECR.

People may provide their own frames (at their own cost, if approved by ACT Corrective Services for detainees) for the lenses to be inserted.

Dispensing services are provided by the selected provider chosen by JHS. Repairs may be provided by JHS if damage occurred during the time of incarceration or

admission. Replacement lenses will be provided when clinically indicated. Repairs or replacement frames and lenses will be provided where the spectacles are

beyond their useful life or have been lost or damaged. This shall be determined by the approved JHS Optometrist or by the Assistant Director of Nursing (ADON), JHS.

An initial supply of disinfection solution will be provided for people who are received into custody with contact lenses or for people received into the DMHU. Further supplies will need to be purchased by the person themselves. Young people at the BYJC will be provided with disinfection solution by ACT Health for the duration of their stay.

All people will to be provided with information about their responsibilities for care of prescription glasses.

People with diabetes will be given priority appointments to see the Optometrist.

The Provision of Optometry Services and Spectacles Procedure must be followed in all cases. Any deviations must only occur at the approval of the ADON, Primary Health or the Primary Health Clinical Director.
