Open enrollment is May 1 – 15, 2020 Annual period for making decisions on health plans and flexible
spending accounts (FSA) Changes are effective July 1, 2020 Nothing is required if you are:
Not changing your health plan or membership, Not enrolling in an FSA, and Not participating in Premium Rewards

EmployeeDirect and select Health Benefits Direct. Preferred submission Employee ID is the 7 digit number on your insurance card.

“Insulin and other diabetic supplies are expensive! A friend told me about a program through Anthem called ConditionCare which provides a free health and wellness program to support type 1 and type 2 diabetics. They also support other conditions. I made the call and was connected with a nurse care manager who guided me through the process. Now my son speaks with ConditionCare once a month and his diabetic supplies and endocrinologist checkup costs are covered by the program. It was easy and the money we are saving can go towards other things for him!”
