Online Services Release - Patient Summary (England)
Overview 26/03//2015
This Vision Online Services release for England delivers new functionality which
allows patients to access a Patient Summary view of their clinical data via their
Online Services account and allows you, as a practice, to control this. This is
currently being rolled out in England to practices on DLM 470 and above who meet
the required prerequisites (see Prerequisites (page 6)).
In order to implement Patient Summary access for your patients, please read What
do I need to do? (page 7) which details the Read codes required for patients to
access their summary when they login to Online Services and how you can monitor
this. The Read codes can be added at any time, whether you are enabled for Patient
Summary/Online Services or not. We have also created a set of Clinical Audits to
help you monitor patients with or without access to their summary (see Patient
Summary Clinical Audit (page 9)).

Initially, the following Patient Summary data is available for patients to view:
Allergy Details - Drug and non-drug. See Allergies Tab (page 12)
Prescriptions Issued - Acute, repeats (including discontinued repeats). See Prescription History Tab (page 12)

Prerequisites The following technical prerequisites apply before patients can benefit from using
the Online Services Patient Summary:
Patient Summary Enabled - Patient Summary is a free of charge
service and this will be automatically enabled at your practice by INPS.
You must already have Online Services enabled for this to happen. If
you want to have Patient Summary disabled, please contact the
helpline (patients can only view their summary if Read code 93440
Electronic record notes summary verified is recorded).
Vision Data Hub must be installed at your practice prior to patients
being able to access their summary. The rollout of the Vision Data Hub to all practices in England is currently being managed by INPS.
Vision Release DLM 470 - You must have Vision version DLM 470 or
above. You can check which version of Vision you are currently running from Help - About Vision on the Vision front screen.
Regardless of the above prerequisites, practices can start to record and monitor
Read codes for patients to access Patient Summary at any point. See What do I
need to do? (page 7)

What do I need to do? Before patients can access their Patient Summary, practices must ensure the
following:
Record Patient Identification Seen when Patient Registers for Online Services
When registering patients for Online Services, you must ensure that you have seen
patient identification and add the Read code 91B.00 Patient registration data
verified with the details of the form of ID seen to the patient record. This is an
NHS requirement for new and existing Online Services patients and ensures that
the patient accessing Online Services is indeed your registered patient. See Add
Patient Verification Details Seen

If you have not been recording this Read code when registering patients for Online
Services, they will not be able to access their Summary. You can use the Online
Services Clinical Audit to add reminders or send recalls to existing online services
users asking them to bring a form of identification when they next visit the surgery
and make sure this is done for newly registered patients (see Patient Summary
Clinical Audit (page 9)).
Note - You do not have to be enabled for Patient Summary Access
to implement any of the following.
Note - If you are presented with a new form of identification,
please add a new entry of the Read code 91B.00 Patient
registration data verified in the patient's journal.
Note - You can now add 91B.00 Patient registration data verified
if you can vouch for a patient who does not have documentation. You
should record that the patient has been vouched for in the free text
notes. See NHS England Guidance http://www.,

Record Read Code for Access to Patient Summary
In order for your patients to view their Patient Summary data when the
functionality is enabled at your practice, you must add the following Read code to
each relevant patient record:
93440 Electronic record notes summary verified
It may take up to 24 hours from adding this code until the patient can see their summary tab.
Before going live with Patient Summary, you should check for patients
who already have 93440 Electronic record notes summary verified on
their record which has been added in relation to something other than
the Online Services Patient Summary. For such patients, you can edit
the record and change the Read code to another appropriate Read code
(see Patient Summary Clinical Audit (page 9)).
You can start to record and monitor Read codes for patients to access
Patient Summary at any point (you do not need to be enabled to do this).
You may wish to review patient records before allowing patients to
access their summary. You can track patients with or without this Read
code using the Online Services Clinical Audit (see Patient Summary
Clinical Audit (page 9)). Remember at the moment they can only view
allergies (page 12) and prescription (page 12) information but this will be expanded in the future.
You must only record 93440 Electronic record notes summary verified
if the patient already has 91B..00 Patient Registration Data Verified
recorded. This is an NHS Requirement and is the practice's responsibility to manage.
Only patients with the exact Read code 93440 Electronic record notes
summary verified (and NOT any others within the Read
category/hierarchy) will be able to see their Patient Summary data.

Monitoring Online Services Patient Summary Access
You can use the Online Services Clinical Audit to monitor and target patients with or
without the above necessary Read codes. You will need to monitor:
Before going live, patients who already have 93440 Electronic record
notes summary verified on their record which relates to something
other than the Online Services Patient Summary.
Patients registered in the last 3 months who have 93440 Electronic record notes summary verified on their record via a GP2GP transfer.
Online Services registered patients without identification verified
recorded.
Online Services registered patients with access to their summary but with no identification seen recorded.
Patients whose identification has been recorded but don't have access
to their summary.
Patients who have access to their summary.
How to Exclude Patients from Accessing their Patient Summary
For future versions of this software you can add the following Read code to the
patient record which will exclude them from accessing their patient record in Online
Services (this does not apply to the current release):
9lX..00 Access to Patient Facing Service denied
Training Tip - You could use this code to identify patients who may
need their careers to be registered to access their Patient Summary.

Patient Summary Clinical Audit To help with tracking Patient Summary status, we have created a Clinical Audit
which is published as part of the Admin for SCR, Pt Online Services and
Care.Data audits. Using this audit, you can identify and address patients with or
without the necessary Read codes to view their summary.
The Admin for SCR, Pt Online Services and Care.Data audits can be downloaded

Patients Viewing their Summary When logged into their Online Services account, providing your practice has met all
the prerequisites (page 6), patients with the Read code 93440 Electronic Notes
Summary Verified will see a new Summary tab which currently contains Allergy
(page 12) and Prescription History (page 12) tabs.
Note - Patients can only view their Summary details - they cannot
add or edit information.
Allergies Tab
The Allergies tab displays any drug and non-drug allergies:

The following information is displayed about each allergy listed:
Date - Date allergy recorded.
Allergy - Non-drug Allergy name eg Peanut allergy and or Drug Allergy
name eg Penicillin (note that form and strength are not displayed).
Reaction - Either Intolerance, Allergy or Adverse Reaction.
Note - For practices in England using GP2GP, any degraded drug
allergies which have not been processed do not display in the Allergy
list for the patient to see. These should be processed by the practice as soon as possible.

Prescription History Tab
The Prescription History tab displays three panels:
Acute Prescriptions - In the last 12 months
Repeat Prescriptions - In the last 12 months
Unavailable Repeat Prescriptions - Repeat prescriptions that have
been inactivated in Vision and have been issued in the last 6 months.
Some items are displayed in the Unavailable Repeat Prescriptions list
as well as the Repeat Prescriptions list which may cause confusion.
These are likely to be old inactive repeat masters which have been
updated/replaced for the next repeat cycle.
All panels display the last issued date of the prescription, the drug name, quantity
and dosage instructions added by the prescriber (this includes any text added in the
dosage box when adding a prescription).
Prescriptions History Tab
Please note the following:
Any prescriptions that have been issued outside the practice within the specific timeframes, eg hospital prescriptions, are also displayed.
Patients cannot re-order repeat prescriptions from this screen. They
can do this from the Prescriptions Ordering tab.
