One operator hit a wooden fence with the rear of the truck, pulling out a few pickets whilst another operator ran over garden lights. 30/7/14 NSW Driver opened his freezer to get a tote out for a customer. It was deep into the freezer so the driver stepped up into the freezer to reach for the tote and his foot slipped. Driver then used his hook instead of stepping onto the icy floor 31/07/14 NSW Driver was loading his truck when he picked up a tote that was obviously overweight. It contained 54 cans of cat food at 400g each. The maximum weight set for these totes is 15kg. This has been reported to Woolworths with order number. 4/7/14 QLD Woolworths occasionally use larger totes that are being phased out due to their size and inability to stack safely. The Woolworths Capalaba store had a large amount of orders and had to use the larger totes. The driver picked up the larger tote and tried to put it on top of another totes - as the larger totes do not stack very well the driver slipped forward and could have injured himself. 10/7/14 QLD CSA walking down the stairs at the dock in a rush as he was running 10 minutes late and slipped on the stairs. He caught himself on the rail when falling. 29/7/14 NSW Driver drove to the top of a steep road, When the driver parked the truck on the side of the road he heard the totes fall over in the back of the pan. He got out of the truck to check and the totes had fallen over to the side due to the steepness of the road he was on. I Did you Know? MOTOR VEHICLE INCIDENTS 8/7/14 NSW Driver was sitting in traffic, Traffic moved forward the driver also moved forward, the 3rd party in front stopped and the driver went to hit the brake pedal but missed, the truck hit the 3rd party at a rolling pace.

23/7/14 NSW Driver was traveling city bound on Canterbury road when a 3rd party (taxi) stopped suddenly, Driver was unable to stop in time and rear ended the taxi at slow speed. 24/7/14 Vic Driver was departing a delivery, along Tills Drive, when his Thermos rolled onto the floor of the truck and became stuck under the brake. The driver was travelling at approx 20km/h when he was unable to stop and crashed into a telephone pole, resulting in a cable dislodging and falling onto the ground. The driver was shaken but not injured. 24/7/14 QLD CSA was slowly driving along Butterfield Street looking for the Customers address, CSA checked back to his mirror and noticed he was close to the parked cars. He stopped the vehicle and went and checked the parked vehicle. The CSA had already come in contact with the parked vehicle. CSA had damaged the Parked Car from the rear drivers’ side Mud guard to the Front door 29/7/14 NSW The driver was trying to enter side lane to deliver into a school following the customers’ instructions, while turning into the lane he had to squeeze between a brick fence and a pole while doing this he has collected the brick fence pulling the back corner of the truck up. .

26/7/14 NSW Driver was at his last delivery when he was organising the load for the last customer. As he placed the order towards the back of the pan, he placed his trolley onto the road. In doing so, he stepped onto the metal strip at the rear of the pan and whilst facing outside, slipped and fell. The driver suffered a fractured arm and an abrasion to the side of his head. The driver did not maintain three points of contact when exiting the rear of the pan, and he did not exit his truck correctly. You must exit the rear of the pan facing the rear of the truck whilst maintaining 3 points of contact, NOT facing away from the truck

Three points of contact is to be used when getting in and out of back of the truck. Always get out with your chest facing the back of the vehicle.

17/7/14 QLD Driver had almost completed his loading when he had picked up a single crate which had cans of soft drink and other items, as he moved to place it in the truck the handle side had come away from the crate and pulled his left arm down with it and then pulled him slightly off balance and he felt a pain in my back and muscles to the side of my spine. At the time the driver felt OK and did not mention this to anyone. As he finished loading and started driving his back cooled and he began to feel pain in his back muscles.

The total weight stacked in totes is not to exceed 15KG.

This is our only walk way onto the

docks. The door has been pushed

by pallets of goods on other side

and now its limited our walk way

making it dangerous to walk.

Loading dock at Croydon has had an

accumulation of leaves and rubbish

for several weeks. The leaves are

decomposing and the edge of the

dock where you walk is muddy and

slippery. Thank Paul.
