HIV WORKPLACE PROGRAMMEThe Tygerberg Hospital HIV Workplace Programme was launched on Thursday 30 April 2009 in the Disa Hall. The launch was attended by TBH Senior management, invited guests, hospital staff members and members of the local community.

Senior Hospital Manager, Dr Muller fulfilled the role of master of ceremonies and Mrs Basson delivered the welcoming address. In his presentation of the purpose of the HIV workplace programme, Dr Alfred Mukosi the Chairman of the committee made it very clear that organizations wishing to remain efficient and viable would have to address HIV in the workplace. “HIV is more than a health issue it has socio-economic implications with employers and employees either affected or infected by

HIV.”

Guest speaker Ms B Smuts from the Western Cape’s HIV and TB Directorate reported that nationally approximately 1500 people are infected daily with HIV. In the Western Cape, where HIV prevalence is the lowest, the provincial strategic plan is to reduce new HIV infections by 50%. She stressed that prevention programmes in the workplace are crucial in the fight against HIV as these increase staff awareness and access to information.

Representatives from the Medical Profession, Infection Control, Occupational Health and Safety, Clinical Psychology and Social Work presented their contributions to the TBH Workplace programme whilst the other disciplines engaged with staff members on a more informal basis at the respective information stalls.

The workplace programme for all staff members includes services provided by the Voluntary Counselling and Testing (VCT) programme (as highlighted in the March

edition of Onder Ons) as well as training and skills development by the Clinical Psychology Department.

EVENTSCleft lip And pAlAte Workshop

The Cleft Lip and Palate Unit at Tygerberg Hospital together with the Smile Foundation have been very active lately in providing care for children born with facial anomalies. On 15 May 2009 this service was extended by presenting a workshop to parents of children with cleft lip and palate.

Dr Alexander Zuhlke gave information to the parents on the medical aspects and showed in detail how the surgical procedures are carried out. Ms Hayley Elliot, speech therapist, taught parents how to develop the child’s speech and encourage the children to develop self esteem. Ms M Schulman Davies a psychologist from the Smile Foundation addressed the emotional reactions that parents may experience when their child is born with a cleft lip and palate, and the stress that goes with surgical treatment. Dr. Ina Steenkamp, Social Worker at Tygerberg hospital, addressed parenting skills and the care of the child.

The parents reported that many of their questions were answered and that they benefitted from the support of other parents in the same situation. They also enjoyed the refreshments!

On the 9th May 2009 it was “English only” in Disa Hall. The Nursing Club held a fundraising with invited guests and confectionary. The table hosts opened their sideboards and show cabinets to lay their tables with fine linen and show-piece fine china. The decorations and extras created a festive atmosphere. The guests were all dressed up in “Sunday’s best” and glitz to match the occasion. In real English tradition, ladies arrived with hats to match and gentlemen all collard and tied with top hat in fine style.

Ms June Saunderson with Mr Raymond VisagieSeveral sponsors joined the event by sponsoring a “tata ma chance” for a lucky draw. The main sponsors were Zone Fitness, Honey and Tupperware.

The singing by Miss K Sapto and dancing by the “Fountain Spiritual Dancers” lifted the spirits. His “Royal Highness” Dr. T

Carter and the honorable Mrs Carter were tabled with Miss A de Witt, the guest speaker. The message for the day was ‘Slow down and dance to the music of your heart’. Enjoy life in the moment.

HAEMOPHILIA EXHIBITIONWorld Haemophilia Day (17 April 2009) put the spotlight on this disease, strengthening the continuous drive to promote awareness and increase the rate of diagnosis. On 14 May an exhibition about Haemophilia was held in Tygerberg Hospital to increase awareness about Haemophilia amongst the public and personnel. Two mothers of affected boys also attended and talked to people about their experience of the condition.

Haemophilia A (factor VIII deficiency), Haemophilia B (factor IX deficiency) and Haemophilia C (factor XI deficiency) are inherited bleeding disorders; Haemophilia A being the most common by far (1 in 10 000 males) and Haemophilia C uncommon in SA. Haemophilia A and B are inherited in a recessive X-linked pattern, thus mainly boys are affected and girls have carrier status. It can also occur smultaneously. In South Africa, there are 1442 people with Haemophilia A and 264 with Haemophilia B registered on the National Registry.Disease severity is classified as mild, moderate or severe depending on the level

of the missing factor. Bleeding episodes are classified as minor or major. Interestingly, bleeding patterns differ and infants and young boys with haemophilia often only have mucosal and soft tissue bleeding (often nose bleeds) and develop the classical bleeding pattern of haemarthroses (bleeding into joints) and haematomas (bleeding into muscle) only when they are older. Treatment is to replace the missing factor by giving factor VIII or IX intravenously as soon as possible after the patient has developed a bleed. All patients are supplied with a Medical Alert disk to identify them. The acronym for treatment of bleeds is:P – Pain relief (avoid aspirin and NSAIMs)R – Rest (immobilization of joint)I – Ice application (5 minutes on, 10 minutes off)C – Factor Concentrate (replace deficient clotting factor immediately; 20u/ kg for minor bleeds and 40u/ kg for major bleeds)E- Elevation of the affected limb

Management of these patients should be at or under the supervision of a Haemophilia

Comprehensive Treatment Centre (Tygerberg Hospital, Red Cross Children’s Hospital and Groote Schuur Hospital). Serious complications can occur, i.e. development of antibodies (inhibitors) against the factor replacement which renders the usual treatment ineffective and chronic synovitis/haemophilic arthropathy. Patients must be carefully monitored for these. Head injury is also an emergency situation which necessitates immediate administration of high dose factor replacement. Treatment guidelines for Haemophilia in South Africa booklets are available from the Paediatric Oncology/Haematology Unit, ward G3.

Ward G8 opened on 15 September 2008 with 16 incubators. The increase in the number of babies led to the opening of more incubators and beds.

On 2 June 2009 the ward opened it’s first 24 hour Kangaroo Mother Care (KMC) room with 5 beds. The mothers were educated on how to take care of their babies in the KMC position.

The incubators increased to 30. The mothers practice intermittent KMC, while sitting next to the incubators. By the end of the day all incubators and beds were filled with patients. There are many advantages to both the mother and baby in practicing 24 hours KMC.

Some of the Advantages of KMCStay as warm as in an incubator•Are more psychologically stable•Bond better with mothers•

Fewer serious infections•Go home sooner•Breastfeeding better and longer•

PACS & RIS UPDATE: GOING FILMLESSNew equipmeNt

All of the new digital imaging equipment has been installed. This means all X-rays generated at Tygerberg Hospital are now digital. Both on 4th floor and 6th floor we have installed three new rooms. Please come and have a look at the new facilities.

ClosiNg of the last film proCessor

On Tuesday the 26th of May we turned off the last film processor. Dr Carter with Mrs. Hazel Dreyer (the longest serving radiographer in TBH) ceremonially turned off the last film processor. The CPUT Radiography School still maintains one film processor so that the students can receive training in both the film-based and the digital environments.

philips maNagemeNt team Visit

The Philips management team visited Tygerberg Hospital on the 14th of May. We discussed the progress made to date as well as the way forward. Philips has also requested that we become an international reference site for them with regards to the PACS & RIS implementation. The Philips team was also interested in how we aim to reach the rural areas and increase medical support to these areas.

isite usage statistiCs

During the month of May the hospital staff used iSite to view a total of 19,100 cases. This is remarkable if we think that only 3 months ago in February we only viewed 2,000 cases using iSite. Please refer to the attached diagram to see the dramatic increase in usage.

stop the press

There are still some areas that print film which will stop this within the next few weeks. It is important to realize that we will always retain the ability to print as a contingency measure and under special circumstances.

DistributioN to referriNg hospitals

We are in the process of establishing a procedure of distributing CDs to the referring hospitals that do not have access to our system yet. This is a short term measure until all the hospitals have access. At this stage we are waiting for feedback from the referring hospitals that they agree to this procedure.
