Please take your time in deciding if you would like toparticipate. You are encouraged to ask questions at any time during this study. Youmay wish to delay participation until your questions have been answered. You may also contact the supervising facultymember, Dr. David Vogel, at [email protected] or 294-1582.INTRODUCTIONThe purpose of this study is to examine perceptions of mental illness and therapy.You are invited to participate in this study because you are a student in thePsychology department at Iowa State University and you participated in thePsychology Departments Mass Testing in the Fall of 2010.DESCRIPTION OF PROCEDURESThis is a completely online study designed to better understand your perceptions ofmental illness. For the first part of the study, you will watch a brief online video aboutpeoples experiences with mental illness followed by an online survey about yourreactions to the video. This part of the study should take about 1.5 hours and is worthtwo research credits.One week later, you will be emailed with the opportunity to take a follow-up surveyfor more research credit. This part of the study should take 30-40 minutes and isworth one research credit.RISKSWe do not anticipate that these procedures will cause you any harm, but if youexperience discomfort you may talk to the investigators about your concerns. You arefree to skip any question that you do not wish to answer or that makes you feeluncomfortable. You are also free at any time to choose to end your participation.There will be no negative effects if you choose to skip a question or discontinue yourparticipation in the study. If you choose to end your participation all data collectedwill be erased.

BENEFITSIf you decide to participate in this study there will be no direct benefit to you otherthan learning about psychological research from a participants perspective. Yourparticipation in this project may help the researchers develop a better understandingof how people perceive those who have a mental illness.COSTS AND COMPENSATIONYou will not have any costs from participating in this study. In addition, there is nomonetary compensation for your participation. Rather, you will be compensated byreceiving research credit in your undergraduate psychology course for participating inthis study. If you agree to participate in this study, you can earn up to 3 researchcredits. It involves two online sessions distributed over 2 weeks. For the first part,you will watch a brief online video followed by an online survey (2 research credits).For the second part, you will be emailed two weeks later and asked to take anotheronline survey (1 research credit). If you choose not to participate, you may contact theCourse Information Office (515-294-8065) for alternative research options in order toearn research credit for your class.PARTICIPANT RIGHTSYour participation in this study is completely voluntary and you may refuse toparticipate or leave the study at any time. If you decide to not participate in the studyor leave the study early, it will not result in any penalty or loss of benefits to whichyou are otherwise entitled.CONFIDENTIALITYRecords identifying participants will be kept confidential to the extent permitted byapplicable laws and regulations and will not be made publicly available. However,auditing departments of Iowa State University, and the Institutional Review Board (acommittee that reviews and approves human subject research studies) may inspectand/or copy your records for quality assurance and data analysis. These records maycontain private information.To ensure confidentiality to the extent permitted by law, the following measures willbe taken to protect your privacy including: (a) assigning you a unique code numberthat will be used instead of your name; (b) combining your data with the datacollected from other participants so that no individual information will be identifiable.If the results are published, your identity will remain confidential
