Older Persons Wellbeing Service
Background
New Service is designed to assist older people to maintain their independence within their communities - giving greater choices and control - Preventing loss of home or leading to
institutional forms of care
Service will be delivered around a hub and spoke model in 5 locality areas
Joint venture with the council to provide an in reach service for housing related support
Service will engage with hard to reach older people e.g. BME
Aims/Objectives
Maximise number of people supported to participate in social activities
Promote Healthy & Active Lifestyles
Increase the number of people supported across the city
Work alongside external agencies - Joint Partnership working
Reduce the number of people entering institutionalised care
Eligibility Criteria
Aged 65 and over
Must live in the City of Sunderland – All tenure eligible
Must have a support need to:Develop a stable lifestyle
Access other appropriate servicesAccess health servicesLive safely & securely
Become socially included with friends/family and/or their local community
Be willing to work with staff on an outcome based plan that defines future aspirations
