Verified as a Level I Trauma Center by the American College of Surgeons since 1993, Grant is the busiest adult Level I Trauma Center in Ohio and recognized nationally for trauma care, orthopedics, heart and vascular care, neuroscience, and robotic and minimally invasive surgeries. Grant Medical Center is an OhioHealth Stroke Network hub and MD Anderson Cancer Network® certified hospital. It is home to the Center for Blood Conservation (a bloodless surgery and medicine program), OhioHealth Mothers’ Milk Bank (a human donor milk bank for premature infants), and a certified registered nurse anesthetist program. Grant Medical Center is accredited by The Joint Commission and recognized as a Magnet hospital for nursing excellence by the American Nurses Credentialing Center Magnet Recognition Program®. Doctors Hospital is an OhioHealth member hospital located at 5100 West Broad Street, Columbus, Ohio 43228. Doctors Hospital is a 251-bed osteopathic teaching hospital and is the fifth-largest osteopathic teaching hospital in the country. Doctors Hospital has been named by IBM Watson Health™, formerly known as the Truven Health Analytics® 100 Top Hospitals, as one of the 100 Top Hospitals in the United States from 2010 to 2018. Joint Commission and renowned for its expertise in emergency care and heart and vascular care, and designated a Baby-Friendly® Hospital by Baby-Friendly USA. The hospital was recognized in 2017 as a “Most Wired” hospital by Hospitals and Health Networks® (Health Forum, 2017). In Fiscal Year 2017, Doctors Hospital had 8,362 admissions, 33,844 inpatient days, 1,644 inpatient surgeries, 162,984 outpatient visits, and 4,451 outpatient surgeries. Dublin Methodist Hospital is an OhioHealth member hospital located at 7500 Hospital Drive, Dublin, Ohio 43016. Serving patients since 2008, Dublin Methodist Hospital is a 147-bed facility and the first hospital in the nation to integrate totally digital, wireless technology with evidence-based design to provide a patient-centered model of care. Interior features include live trees, multi-story waterfalls and natural lighting, as well as high-tech networks and spacious, private rooms. The campus has 13 gardens, including courtyard and rooftop spaces. Dublin Methodist Hospital is a certified member of MD Anderson Cancer Network® and accredited by The Joint Commission. The hospital was recognized in 2017 as a “Most Wired” hospital by Hospitals and Health Networks® (Health Forum, 2017). In Fiscal Year 2017, Dublin Methodist Hospital had 6,439 admissions, 16,426 inpatient days, 2,254 inpatient surgeries, 72,708 outpatient visits, and 4,876 outpatient surgeries. OhioHealth Rehabilitation Hospital is an OhioHealth member hospital and joint venture with Select Medical located at 1087 Dennison Avenue, Columbus, Ohio 43201. Serving patients since 2013, OhioHealth Rehabilitation Hospital is a 74-bed, freestanding inpatient rehabilitation hospital and the largest in Ohio. Positioned to serve the inpatient rehabilitation needs of Franklin County and other central Ohio communities, the hospital is recognized locally and regionally as a leader in neurologic rehabilitation, treating patients referred from all over Ohio for specialized clinical programming in the areas of stroke, traumatic brain injury, spinal cord injury, orthopedic trauma and cardiopulmonary rehabilitation. OhioHealth Rehabilitation Hospital is accredited by The Joint Commission and the Commission on Accreditation of Rehabilitation Facilities (CARF). In Fiscal Year 2018, OhioHealth Rehabilitation Hospital had 1,740 admissions, 22,968 patient days and discharged 76.4 percent of its patients directly back to the community. OhioHealth is dedicated to serving communities through its community benefit. In Fiscal Year 2018, OhioHealth provided a total of $393.7 million in community benefit, including $90 million in charity care, $240.4 million in net cost of Medicaid programs, $54.1 million in net cost of medical education, $1.9 million in subsidized health services, $3.8 million toward community health services, $3 million in cash and in-kind contributions, and $0.5 million for research.

Implementation Strategy to Meet Community Health Needs

B. Purpose of Implementation Strategy This joint implementation strategy has been prepared to comply with federal tax law requirements set forth in Internal Revenue Code section 501(r) requiring hospital facilities owned and operated by an organization described in code section 501(c)(3) to conduct a community health needs assessment (CHNA) at least once every three years and adopt an implementation strategy to meet the community health needs identified through the community health needs assessment. This joint implementation strategy is intended to satisfy each of the applicable requirements set forth in proposed regulations released in December 2014 and describes the planned response by OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital to the needs identified in the 2019 community health needs assessment. The OhioHealth hospitals in Franklin County all define the community served to be the communities and residents of Franklin County. All OhioHealth hospitals in Franklin County, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital are dedicated to fulfilling the health system’s mission, “to improve the health of those we serve.” Each hospital has a community benefit section in its operating and strategic plan and has designated a community benefit budget to provide these services in Fiscal Years 2020-2022. The clinical and professional staff of these five hospitals will implement these programs and closely partner with other organizations in the community to address the identified health needs. Activities, outputs, outcomes and impacts of such programs will be reported regularly to the OhioHealth Community Health and Wellness Department. The joint implementation strategy for Riverside Methodist Hospital, Grant Medical Center, Doctors Hospital, Dublin Methodist Hospital and Rehabilitation Hospital was approved by the board of directors on June 6, 2019 and applies to tax years July 1, 2020-June 30, 2022. C. Community Health Needs Identified in CHNA Report OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist and OhioHealth Rehabilitation Hospital collaborated with the Central Ohio Hospital Council and various community stakeholders to conduct a community health needs assessment (CHNA) in Franklin County and determine the priority health needs that are being addressed in this implementation strategy. The Central Ohio Hospital Council organized the Franklin County Community Health Needs Assessment Steering Committee to participate in reviewing secondary data, identifying community health needs based on data and their experience in the community, and identifying priority health needs in Franklin County. On October 11, 2018, the Franklin County Community Health Needs Assessment Steering Committee members identified the significant health needs in Franklin County: (1) Mental health and addiction — includes significant number of Emergency Department admissions, lack of

mental health providers, increased rates of deaths from drug overdoses, especially from opiates, education and training about Naloxone (Narcan®) administration to avoid overdose deaths.

(2) Income/Poverty — includes lack of access to affordable housing and lack of access to healthy and nutritious

foods. (3) Maternal and infant health – includes health of women before pregnancy, preterm births, and infant mortality. The secondary health data for each of these priority health needs are available in the hospitals’ Community Health Needs Assessment (OhioHealth, 2015-2019) and the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee (2019). OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital consulted with Columbus Public Health and Franklin County Public Health in developing implementation strategies that address mental health and addiction, income/poverty and maternal and infant health, in alignment with the Ohio 2017-2019 State Health Improvement Plan (Ohio Department of Health, n.d., a). E. Process of Developing the Implementation Strategy to Address Significant Health Needs Identified in the CHNA Report The joint implementation strategy was developed by OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital based on existing programs and initiatives that appropriately address the three priority health needs in Franklin County, namely: mental health and addiction, income/poverty, and maternal and infant health. All healthcare services and programs that are provided by Riverside Methodist Hospital, Grant Medical Center, Doctors Hospital, Dublin Methodist Hospital, and Rehabilitation Hospital are integrated and complementary. Each hospital has allocated an operating budget to enable the successful implementation of these programs from Fiscal Year 2020 to Fiscal Year 2022. These programs are accessible to all residents of Franklin County regardless of ability to pay. The healthcare services, programs and initiatives that Riverside Methodist Hospital, Grant Medical Center, Doctors Hospital, Dublin Methodist Hospital and Rehabilitation Hospital will implement during Fiscal Year 2020-2022 are summarized separately per hospital for each priority health need. OhioHealth Riverside Methodist Hospital Implementation Strategy (Actions, Anticipated Impact, Resources and Collaborative Partners) The actions to be taken, anticipated impact, resources to be committed and collaborative partners by OhioHealth Riverside Methodist Hospital, as part of the Joint Implementation Strategy with Grant Medical Center, Doctors Hospital, Dublin Methodist Hospital and Rehabilitation Hospital are shown in Tables 1 to 6. Riverside Methodist Hospital will address the three priority health needs in Franklin County, namely: mental health and addiction, income/poverty, and maternal and infant health. Implementation Strategy to Meet Community Health Needs

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). A total 21.8 percent (two out of 10 residents) of Franklin County population were depressed. Suicide deaths were 12.3 per 100,000 population. Hospitalizations resulting from assault or alleged abuse were 87.2 per 100,000 population. Hospitalizations resulting from attempted suicide were 4.8 per 100,000 population. Psychiatric admissions were 35.7 per 1,000 population (three out of every 100 admissions). Homicides were eight per 100,000 population. There were 11,224 domestic violence incidents, 6,781 domestic violence victims, of whom 43.3 percent presented with injuries. There were 13,580 child abuse cases, broken down as follows: (a) 42 percent had physical abuse, (b) 19 percent were neglected, (c) 9 percent were sexually abused, (d) 1 percent had emotional maltreatment, (e) 10 percent had multiple allegations of abuse or neglect, and (e) 19 percent belonged to families in need of services. A total 13.7 per 1,000 population (one in every 100 children) aged 18 and under were in foster care. There were 1,635 reports of abuse, neglect, and exploitation of persons aged 60 and older in homes and apartments. There were 11,846 complaints about abuse, neglect, and exploitation in long-term care facilities. In Franklin County, there is one social worker for every 339 residents. There is one chemical counselor for every 1,137 residents. There is one psychologist for every 2,379 residents. Emergency Department (ED) visits related to mental health were 165.7 per 1,000 population (approximately two out of every 10 ED visits). Goal To improve mental health and reduce addiction among Franklin County youth and adults Objective To implement community-based programs and services that address mental health and addiction among Franklin County youth and adults OhioHealth Riverside Methodist Hospital’s Intended Actions to Address the Health Need a) Continue assessing depression and anxiety in patients (aged 12 and older) of OhioHealth Physician Group’s

primary care practices using the nine-item Patient Health Questionnaire (PHQ-9). Patients with mild to moderately severe depression and anxiety (PHQ-9 score of 10 or higher) will have access to brief interventions that will be provided by social workers or counselors with physician referral. Psychiatrists or psychiatric nurse practitioners will review the social worker or counselor’s patient caseload and provide recommendations to the primary care physician related to medications. OhioHealth’s Behavioral Health Integration program enables assessment of depression and anxiety, provision of brief interventions to patients, and collaboration among primary care physicians, psychiatrists or psychiatric nurse practitioners related to the need for prescribing medications. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 2.)

b) Continue assessing suicide risk for hospitalized patients aged 12 or older, using the S.A.D. P.E.R.S.O.N.S. scale for suicide risk. S.A.D. P.E.R.S.O.N.S. stands for Sex, Age, Depression, Prior History, Ethanol abuse, Rational Thinking Loss, Support system loss, Organized plan, No significant other, Sickness (Warden et al., 2014). If screened positive, the patient will be referred to appropriate follow-up care. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 3.)

c) Continue offering medication-assisted treatment (MAT) to pregnant women through OB/GYN Community Care based at OhioHealth Riverside Methodist Hospital. Patients from OB/GYN Community Care are referred to OhioHealth Riverside Family Practice Center to enable continuity of MAT for mother and serve as a medical home for both mother and baby.

d) Continue offering medication-assisted treatment (MAT) and consultation to pregnant women with opioid use

disorder at OhioHealth Maternal Fetal Medicine Physicians to improve the care and outcomes of mothers and babies who are affected by opioid use disorder. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

e) Continue addressing mental health and addiction and social determinants of health needs of patients with mental

health and/or addiction diagnoses. Patients will be referred to community agencies that provide the following: (i) inpatient and outpatient mental and behavioral health treatment and counseling services and/or (ii) assistance with transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

f) Continue offering patient-centered women’s health services through OhioHealth Wellness on Wheels Women’s Health to CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

g) Continue implementing the OhioHealth Grant Medical Center Addiction Medicine Fellowship program. The program aims to adequately train physician fellows to develop clinical competencies, adhere to ethical principles, and improve knowledge and skills in addiction medicine. The program will accept patient referrals from all OhioHealth hospitals in Franklin County, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital. The Addiction Medicine Fellowship program will also collaborate with Nationwide Children’s Hospital, Maryhaven Addiction Stabilization Center, Amethyst Inc., Alvis, CompDrug, and the Ohio Department of Rehabilitation and Correction to ensure continuity of care.

h) Continue implementing OhioHealth trauma programs, namely: Grant Medical Center Trauma Level I and Riverside Methodist Hospital Trauma Level II programs, that include but are not limited to: (i) screening, brief intervention and referral to treatment (SBIRT), (ii) Trauma Recovery Center, (iii) speakerships or health education sessions, and (iv) student preceptorships focused on mental health and addiction.

i) Continue offering the OhioHealth Behavioral Health Family Support Group at The Dempsey Family Education and Resource Center at OhioHealth. The support group is offered for adults every Tuesday (up to 52 times per fiscal year) and is co-facilitated by licensed mental healthcare providers from the OhioHealth Behavioral Health department and Mental Health America of Franklin County. On the last Tuesday of the month, the Behavioral Health Family Support Group features a guest speaker and is open to all age groups. The support group focuses on issues faced by a family member or care provider who provides emotional, financial or practical support to a person with a mental health condition. Examples of topics discussed include but are not limited to: supporting recovery, maintaining healthy relationships, establishing boundaries, mental health conditions and recommended treatments, area resources, self-care and resiliency, the grief process, and mindfulness.

j) Continue offering at least one continuing education session per fiscal year to nurses through the OhioHealth Faith Community Nursing program that will focus on understanding depression and anxiety.

k) Continue offering the mental health and addiction outreach programs through the OhioHealth Faith Community Relations. The outreach programs will include but are not limited to: (i) Mental Health First Aid and (ii) Congregational Care and Hospital Visitation Series.

l) Continue distributing free Naloxone kits and education on administering Naloxone at the OhioHealth Grant

Medical Center Emergency Department, OhioHealth Westerville Emergency Care Center, and OhioHealth Pickerington Emergency Care Center.

m) Continue serving as speakers or educators during the free Naloxone trainings, “Be the One to Save a Life,” for

residents and representatives of community organizations in Franklin County. The training will be focused on how to use the Naloxone kit to stop deadly overdose from opiates or heroin. The outreach program is made possible through a collaboration among Franklin County Public Health, City of Columbus, Columbus Public Health, Maryhaven Addiction Stabilization Center, and the Central Ohio Hospital Council (Franklin County Public Health, 2019).

n) Continue partnering with the Franklin County Opiate Task Force by (a) serving as co-chair of the Resource Development Committee, (b) participating in the Risk Reduction Subcommittee, (c) participating in the Prevention and Community Education Subcommittee, (d) participating in the Recovery Subcommittee and (d) cash contributions to the “Don’t Live in Denial, Ohio” campaign.

Anticipated Impact of These Actions a) Per fiscal year, at least 100 patients per each of the 27 clinics will be assessed for risk for depression and

anxiety using the PHQ-9. With physician referral, patients who score 10 or above can be referred to OhioHealth social workers or counselors who will provide brief interventions for patients with mild to moderately severe depression and anxiety (scores of 10 or above). Patients eligible for this program typically have PHQ-9 scores of 10 to 19. When appropriate, patients who need ongoing counseling or more in-depth services will be referred to community-based agencies that provide mental, behavioral and substance abuse treatments. A psychiatrist or psychiatric nurse practitioner will review the caseload every week and make medication-related recommendations to the primary care provider if appropriate.

b) Per fiscal year, suicide risk assessment of at least 90 percent of all inpatient admissions. Brief intervention and

community referral of patients who screened positive for suicidal ideation will be provided by social workers or counselors when appropriate.

c) Per fiscal year, up to 25 OB/GYN patients from Riverside Methodist Hospital Community Care clinic will be

provided medication-assisted treatment (MAT) and referred to OhioHealth Graduate Medical Education family practice clinics for continuity of care.

d) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-

assisted treatment (MAT), prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

e) Per fiscal year, at least 100 patients with mental health and/or addiction diagnoses will be referred to mental and

behavioral health agencies for treatment and/or counseling services. Per fiscal year, at least 100 patients will be referred to HandsOn Central Ohio or to community agencies that address transportation, housing, utilities, prescription medications, employment and other social determinants of health needs.

f) Per fiscal year, at least 25 patients from CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction

Stabilization Center will be provided patient-centered and comprehensive women’s health services by the OhioHealth Wellness on Wheels Women’s Health mobile clinic.

g) Per fiscal year, at least 25 patients with mental health and addiction diagnoses will be served by the OhioHealth

Grant Medical Center Addiction Medicine Fellowship program. Per fiscal year, at least 10 patients from OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital will be referred to the Grant Medical Center Addiction Medicine Fellowship program for follow-up care. Patients will be referred to community-based organizations that help with assistance for transportation, food, housing, utility bills, and other social determinants of health needs.

h) Per fiscal year, OhioHealth Grant Medical Center and OhioHealth Riverside Methodist Hospital Trauma

programs will report the following outcomes for patients with mental health and addiction issues: (i) at least 100 patients will be served by the Grant Medical Center Trauma Recovery Center, (ii) at least 100 patients will be administered screening, brief intervention and referral to treatment (SBIRT), (iii) at least five speakerships or health education sessions, and (iv) at least two students will be precepted on mental health and addiction assessment and counseling.

i) Per fiscal year, up to 100 persons will attend the OhioHealth Behavioral Health Family Support Group at The Dempsey Family Education and Resource Center. Of the participants who attend three or more times and complete the annual anonymous survey, the following impacts will be reported: (i) at least 75 percent will self-report that they learned various techniques to maintain good mental and behavioral health through new coping strategies, (ii) at least 75 percent will agree that they feel listened to by the facilitators and/or other members, (iii) at least 75 percent will report good customer service experience, and (iv) at least 75 percent will recommend the group to other people in search of a family support group.

At least 90 percent will meet the learning objectives of the continuing education session. Nurse continuing education hours will be provided through OhioHealth Learning.

k) Per fiscal year, at least 40 people from at least five congregations will be served by OhioHealth Faith Community

Relations outreach programs. l) Per fiscal year, at least 50 patients and families will be given Naloxone kits and corresponding education on

Naloxone administration. m) Per fiscal year, OhioHealth will participate in up to five community-based education events on Naloxone

administration in Franklin County. n) Per fiscal year, OhioHealth in partnership with the Franklin County Opiate Task Force will provide

accomplishments related to at least one of the following goals: (a) prevent opiate abuse and addiction, (b) reduce the number of opiate-related deaths, (c) expand access for treatment, and (d) improve the safety of the Franklin County communities.

Committed OhioHealth Riverside Methodist Hospital Resources OhioHealth Riverside Methodist Hospital clinical and administrative staff committed to these mental health and addiction programs will include: physicians, nurses, social workers, case managers, community health workers and other administrative staff. A total of 205,186 residents of Franklin County, which represents 17 percent of the population, has income below the federal poverty level. Among families, 12.5 percent have income below the federal poverty level. Among residents under 18 years of age, 24.5 percent have income below the federal poverty level. Among schoolchildren, 53.6 percent are eligible for free or reduced lunch. The top 10 Franklin County ZIP codes with the lowest median household income are 43210 (The Ohio State University area), 43211 (Linden), 43203 (East Side), 43222 (West Side), 43201 (Clintonville/near North), 43223 (West Side), 43205 (East Side), 43227 (far East Side), 43224 (Linden), 43219 (Agler/Cassady). More than 40 percent of Franklin County residents living in 43201 (Clintonville/near North), 43211 (Linden) and 43203 (East Side) have household income below the federal poverty level. Among households with children, the ZIP codes with the lowest household income were 43201 (Clintonville/near North), 43205 (East Side), 43203 (East Side), 43211 (Linden) and 43222 (West Side). Among the Franklin County residents who live below the federal poverty level, 47 percent were white and 40 percent were black or African American. Housing and homelessness. A total of 1,229 persons use emergency shelters, of whom 32.4 percent were part of families. A total of 25 percent of Franklin County residents have a high school diploma. A total of 25 percent of Franklin County residents have a high school diploma. A total of 9.7 percent of Franklin County residents aged 25 and older did not have a high school diploma or General Education Diploma (GED). OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital provide internships to 11th and 12th grade students with disabilities so they can develop marketable, practical work skills and develop social skills to build mutual respect with peers and supervisors.

d) Continue partnering with Junior Achievement of Central Ohio®, volunteer-led, K-12 programs that enable children

and youth to understand managing finances, promote a sense of entrepreneurship, develop skills for being successful in life and career, and learn the value of contributing towards the progress of their communities (Junior Achievement of Central Ohio, n.d.). Examples of Junior Achievement of Central Ohio® programs include: (i) JA BE Entrepreneural®, (ii) JA BizTown®, (iii) JA Career Success®, (iv) JA Company Program® Blended Model, (v) JA Economics for Success®, (vi) JA Economics®, (vii) JA Exploring Economics®, and (viii) JA Finance Park® (Junior Achievement of Central Ohio, n.d.).

e) Continue offering the “Food is Health” program to patients of OhioHealth Riverside Family Practice Center who

have diabetes and are identified as food insecure. The “Food is Health” program is offered three half-days a week at the center. Patients may attend every week or every other week. The program consists of a brief health education session and fresh produce pick-up. The education focuses on simple nutrition topics, including eating healthy, meal planning, and simple healthy recipes. Medical residents facilitate the education session with a group of four patients. After the education session, patients pick up fresh produce and healthy shelf-stable items at the center’s onsite food pantry with the help of trained medical students. A one- week food supply is provided to every member of the household.

f) Continue offering the services of the social worker and community health workers from OhioHealth Wellness on

Wheels Women’s Health and Primary Care mobile clinics so patients with low incomes or stricken by poverty are empowered to make their medical appointments, fill their medication prescriptions, and obtain follow-up care as needed. The social worker and community health workers collaborate with the multidisciplinary healthcare team in addressing patients’ transportation needs, food insecurity, job search, access to health insurance, and other social determinants of health needs.

g) Continue partnering with various chambers of commerce in Franklin County as it relates to workforce

development, including but not limited to: (i) Clintonville Area Chamber of Commerce, (ii) Columbus Chamber of Commerce, (iii) Dublin Chamber of Commerce, (iv) Gahanna Chamber of Commerce, (v) Greater Powell Chamber of Commerce, (vi) Grove City Chamber of Commerce, (vii) Hilliard Chamber of Commerce, (viii) New Albany Chamber of Commerce, (ix) Tri-Village Chamber Partnership (Upper Arlington, Marble Cliff, Grandview), (x) Westerville Chamber of Commerce, and (xi) Worthington Chamber of Commerce.

i) Continue offering the OhioHealth Charity Care Policy and medical financial assistance to patients with limited or no resources and inadequate medical insurance coverage. OhioHealth offers high-quality care to everyone, regardless of ability to pay. The OhioHealth Charity Care Policy includes: (i) substantial charity care guidelines that provide free care for individuals and families who earn less than 200 percent of the federal poverty level, (ii) sliding scale fees to provide substantially discounted care for individuals and families who are between 200 and 400 percent of the federal poverty level, and (iii) hardship policy for those patients who could not otherwise qualify for charity care but have unique circumstances. OhioHealth offers interest-free loans for up to one year to assist patients. OhioHealth also has an uninsured discount policy for individuals without health insurance who do not qualify for charity care.

Anticipated Impact of These Actions a) Per fiscal year, at least 10 persons from Franklin County will participate in the OhioHealth HR department’s

employment outreach at the Reeb Avenue Center and Godman Guild. Per fiscal year, at least one participant from the employment outreach at either location will be hired as an OhioHealth associate.

b) Per fiscal year, HandsOn Central Ohio will serve at least 1,000 telephone calls from OhioHealth associates and

their families through the OhioHealth LinkLine. All callers will be referred to community resources or OhioHealth programs.

c) Per fiscal year, at least 50 students from Project SEARCH will obtain internships at OhioHealth Riverside

Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital. At least one student who completed the Project SEARCH internship rotations will be hired by OhioHealth.

d) Per fiscal year, the Junior Achievement® of Central Ohio will serve at least 5,000 students, conduct at least 300

classes, and conduct activities in at least 60 schools.

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Infant mortality. In 2017, the infant mortality rate in Franklin County was 8.2 per 1,000 live births. Franklin County failed to meet the Healthy People 2020 infant mortality rate target of 6 per 1,000 live births. In 2017, 155 infants died before age one. The infant mortality rate among blacks was 14.6 per 1,000 live births, which was 2.7 times the infant mortality rate among whites (5.5 per 1,000 live births). It is noteworthy that among the 155 infants who died before age one, 116 babies (75 percent) died before reaching 28 days old, and 39 infants (25 percent) died between 28 days and one year old. The top eight ZIP codes in Franklin County with the highest infant deaths include the following: (a) 43224 (Linden), (b) 43211 (Linden), (c) 43219 (Agler/Cassady), (d) 43223 (West Side), (e) 43206 (East Side), (f) 43207 (far South), (g) 43232 (far East Side), and (h) 43068 (far East Side). Preterm birth and low birth weight. In Franklin County, preterm birth (babies born before 37 weeks gestation) was 10.7 percent. Franklin County failed to meet the Healthy People 2020 target (9.4 percent) for preterm birth. Low birth weight (babies born less than 2,500 grams) was 7.4 percent. Franklin County met the Healthy People 2020 target weight (7.8 percent) for low birth weight. Preconception, pregnancy health and unhealthy behaviors. In Franklin County, the health status of women of childbearing age is as follows: (a) 4.7 percent had either Type 1 or Type 2 diabetes, (b) 4.9 percent had hypertension, (c) 48.5 percent were overweight or obese, (d) 11 percent were current smokers, (e) 5 percent smoked cigarettes during the third trimester, (f) 7.4 percent drank alcohol during the third trimester, (g) 2.1 percent were heavy drinkers before becoming pregnant, (h) 49.9 percent had folic acid deficiency, (i) 24.8 percent had unintended pregnancies, (j) 12 percent of women aged 18 to 44 had no health insurance, (k) 10.9 percent have not had a health check-up in the past year, (l) 13.1 percent have not had a Pap smear in the past three years, and (m) a total of 12.3 out of 1,000 live births had neonatal abstinence syndrome (NAS), which is characterized by withdrawal symptoms from drug exposure in the mother's womb. A total of 1.9 percent of births in Franklin County occurred without prenatal care. Goal To reduce infant mortality rate, percent of preterm birth and percent of low birth weight among babies born in Franklin County Objectives (a) To improve birth outcomes among women of childbearing age by providing high-quality prenatal care and postpartum care, and health education before and during pregnancy and after delivery of her baby; (b) to provide medication-assisted treatment (MAT) to pregnant women with opiate use disorder; and (c) to provide comprehensive women's healthcare services to women of childbearing age who are uninsured or underinsured.

Actions OhioHealth Riverside Methodist Hospital Intends to Take to Address the Health Need a) Continue implementing standardized screening and evidence-based women's health services at OhioHealth

Physician Group clinics offering obstetrics and gynecological services and hospital-based labor and delivery and postpartum units. Women's health services will include but are not limited to: (i) progesterone treatment, including screening for women who are at high risk for preterm birth, (ii) provider counseling with patients about preconception health and reproductive life plans, (iii) comprehensive contraceptive options, (iv) increase breastfeeding support, and (e) referral to home visiting programs in Franklin County on an as-needed basis. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 6).

b) Continue offering medication-assisted treatment (MAT) to pregnant women through OB/GYN Community Care at

OhioHealth Riverside Methodist Hospital and OhioHealth Family Medicine Grant. Patients from OB/GYN Community Care are referred to OhioHealth Riverside Family Practice Center to enable continuity of MAT for mother and serve as a medical home for both mother and baby.

c) Continue offering medication-assisted treatment (MAT) and consultation to pregnant women with opioid use disorder at OhioHealth Maternal Fetal Medicine Physicians to improve the care and outcomes of mothers and babies who are affected by opioid use disorder. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

d) Continue providing patient-centered women’s health services (obstetrics and gynecology) through the

OhioHealth Wellness on Wheels Women’s Health mobile clinic, which serves predominantly uninsured or underinsured women in Franklin County. The OhioHealth Wellness on Wheels Women’s Health mobile clinic also provides women's health services at CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

e) Continue collaborating with the Ohio Better Birth Outcomes (OBBO) collaborative, which aims to reduce the infant mortality rate in Franklin County by improving healthcare delivery for women and their families through evidence-based quality improvement projects. OBBO focuses on three initiatives: (i) improving reproductive health, (ii) increasing access to prenatal care, and (iii) improving quality of clinical care to reduce premature birth (Ohio Wellness Coalition, 2010-2019).

f) Continue participating in the Ohio Hospital Association/Ohio Department of Health “Ohio First Steps for Healthy Babies” program, which encourages hospitals in Ohio to promote, protect and support breastfeeding. Hospitals earn an award for every two steps achieved based on the “Ten Steps to Successful Breastfeeding” developed by the World Health Organization and Baby-Friendly USA (Ohio Department of Health, n.d., b).

g) Continue operating the OhioHealth Mothers’ Milk Bank, which provides pasteurized donor breast milk to infants who are preterm or have life-threatening medical conditions. The OhioHealth Mothers’ Milk Bank is a member of the Human Milk Banking Association of North America (HMBANA). The OhioHealth Mothers’ Milk Bank sends donated milk to various neonatal intensive care units (NICUs) and mother/baby (postpartum) units in Franklin County hospitals, namely: (i) OhioHealth Riverside Methodist Hospital, (ii) OhioHealth Grant Medical Center, (iii) OhioHealth Doctors Hospital, (iv) OhioHealth Dublin Methodist Hospital, (v) The Ohio State University Wexner Medical Center, (vi) Mount Carmel East, Mount Carmel West, and (vii) Mount Carmel St. Ann’s.

c) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-assisted treatment (MAT), prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

d) Per fiscal year, up to 500 women served by the OhioHealth Wellness on Wheels Women's Health mobile clinic. Birth outcomes and pregnancy health behaviors include but are not limited to: infant mortality rate, preterm birth and low birth weight will meet the Healthy People 2020 targets.

e) Per fiscal year, active participation and engagement in the Ohio Better Birth Outcomes projects related to improving reproductive health, access to care, and quality of clinical care provided to women and their babies.

f) Per fiscal year, OhioHealth Riverside Methodist Hospital labor and delivery and postpartum units will administer

the “Ohio First Steps for Healthy Babies” program through the following: (i) at least 10 persons attending “Childbirth Education” classes, (ii) at least 25 persons attending “Breastfeeding Basics” classes, and (iii) provide at least 10,000 inpatient lactation encounters.

g) Per fiscal year, the OhioHealth Mothers’ Milk Bank will distribute at least 20,000 ounces of pasteurized donor breast milk to hospitals in Franklin County to provide important first food for babies who are either premature or with life-threatening conditions.

Grant Medical Center will address the three priority health needs in Franklin County, namely: mental health and addiction, income/poverty, and maternal and infant health. In alignment with the Ohio 2017-2019 State Health Improvement Plan, Grant Medical Center will address mental health and addiction and maternal and infant health

Implementation Strategy to Meet Community Health Needs

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). A total 21.8 percent (two out of 10 residents) of Franklin County population were depressed. Suicide deaths were 12.3 per 100,000 population. Hospitalizations resulting from assault or alleged abuse were 87.2 per 100,000 population. Hospitalizations resulting from attempted suicide were 4.8 per 100,000 population. Psychiatric admissions were 35.7 per 1,000 population (three out of every 100 admissions). Homicides were eight per 100,000 population. There were 11,224 domestic violence incidents, 6,781 domestic violence victims, of whom 43.3 percent presented with injuries. There were 13,580 child abuse cases, broken down as follows: (a) 42 percent had physical abuse, (b) 19 percent were neglected, (c) 9 percent were sexually abused, (d) 1 percent had emotional maltreatment, (e) 10 percent had multiple allegations of abuse or neglect, and (e) 19 percent belonged to families in need of services. A total 13.7 per 1,000 population (one in every 100 children) aged 18 and under were in foster care. There were 1,635 reports of abuse, neglect, and exploitation of persons aged 60 and older in homes and apartments. There were 11,846 complaints about abuse, neglect, and exploitation in long-term care facilities. In Franklin County, there is one social worker for every 339 residents. There is one chemical counselor for every 1,137 residents. There is one psychologist for every 2,379 residents. Emergency Department (ED) visits related to mental health were 165.7 per 1,000 population (approximately two out of every 10 ED visits). Goal To improve mental health and reduce addiction among Franklin County youth and adults Objective To implement community-based programs and services that address mental health and addiction among Franklin County youth and adults OhioHealth Grant Medical Center’s Intended Actions to Address the Health Need a) Continue assessing depression and anxiety in patients (aged 12 and older) of OhioHealth Physician Group’s

primary care practices using the nine-item Patient Health Questionnaire (PHQ-9). Patients with mild to moderately severe depression and anxiety (PHQ-9 score of 10 or higher) will have access to brief interventions that will be provided by social workers or counselors with physician referral. Psychiatrists or psychiatric nurse practitioners will review the social worker or counselor’s patient caseload and provide recommendations to the primary care physician related to medications. OhioHealth’s Behavioral Health Integration program enables assessment of depression and anxiety, provision of brief interventions to patients, and collaboration among primary care physicians, psychiatrists or psychiatric nurse practitioners related to the need for prescribing medications. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 8.)

b) Continue assessing suicide risk for hospitalized patients aged 12 or older, using the S.A.D. P.E.R.S.O.N.S. scale

for suicide risk. S.A.D. P.E.R.S.O.N.S. stands for Sex, Age, Depression, Prior History, Ethanol abuse, Rational Thinking Loss, Support system loss, Organized plan, No significant other, Sickness (Warden et al., 2014). If screened positive, the patient will be referred to appropriate follow-up care. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 9.)

c) Continue offering medication-assisted treatment (MAT) to pregnant women through OhioHealth Family Medicine Grant and serve as a medical home for mother and baby.

d) Continue offering medication-assisted treatment (MAT) and consultation to pregnant women with opioid use

disorder at OhioHealth Maternal Fetal Medicine Physicians to improve the care and outcomes of mothers and babies who are affected by opioid use disorder. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

e) Continue addressing mental health and addiction and social determinants of health needs of patients with mental

health and/or addiction diagnoses by referring patients to community agencies that provide the following: (i) inpatient and outpatient mental and behavioral health treatment and counseling services, and/or (ii) assistance with transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

f) Continue offering patient-centered women’s health services provided by the OhioHealth Wellness on Wheels

Women’s Health mobile clinic to CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

g) Continue implementing the OhioHealth Grant Medical Center Addiction Medicine Fellowship program, which

aims to adequately teach physician fellows to develop clinical competencies, adhere to ethical principles, and improve knowledge and skills in addiction medicine. The program will accept patient referrals from all OhioHealth hospitals in Franklin County, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital. The Addiction Medicine Fellowship program will also collaborate with Nationwide Children’s Hospital, Maryhaven Addiction Stabilization Center, Amethyst Inc., Alvis, CompDrug, and the Ohio Department of Rehabilitation and Correction to ensure continuity of care.

h) Continue implementing OhioHealth trauma programs, namely: Grant Medical Center Trauma Level I and

Riverside Methodist Hospital Trauma Level II programs, that include but are not limited to: (i) screening, brief intervention and referral to treatment (SBIRT), (ii) Trauma Recovery Center, (iii) speakerships or health education sessions, and (iv) student preceptorships focused on mental health and addiction.

i) Continue offering the OhioHealth Behavioral Health Family Support Group at The Dempsey Family Education

and Resource Center at OhioHealth. The support group is offered for adults every Tuesday (up to 52 times per fiscal year) and is co-facilitated by licensed mental healthcare providers from the OhioHealth Behavioral Health department and Mental Health America of Franklin County. On the last Tuesday of the month, the Behavioral Health Family Support Group features a guest speaker and is open to all age groups. The support group focuses on issues faced by a family member or care provider who provides emotional, financial or practical support to a person with a mental health condition. Examples of topics discussed include but are not limited to: supporting recovery, maintaining healthy relationships, establishing boundaries, mental health conditions and recommended treatments, area resources, self-care and resiliency, the grief process, and mindfulness.

j) Continue offering at least one continuing education session per fiscal year to nurses through the OhioHealth

Faith Community Nursing program that will focus on understanding depression and anxiety.

k) Continue offering the mental health and addiction outreach programs through the OhioHealth Faith Community Relations. The outreach programs will include but are not limited to: (i) Mental Health First Aid and (ii) Congregational Care and Hospital Visitation Series.

l) Continue distributing free Naloxone kits and education on administering Naloxone at the OhioHealth Grant

Medical Center Emergency Department, OhioHealth Westerville Emergency Care Center, and OhioHealth Pickerington Emergency Care Center.

m) Continue serving as speakers or educators during the free Naloxone trainings, “Be the One to Save a Life,” for

residents and representatives of community organizations in Franklin County. The training will be focused on how to use the Naloxone kit to stop deadly overdose from opiates or heroin. The outreach program is made possible through a collaboration among Franklin County Public Health, City of Columbus, Columbus Public Health, Maryhaven Addiction Stabilization Center, and the Central Ohio Hospital Council (Franklin County Public Health, 2019).

n) Continue partnering with the Franklin County Opiate Task Force by (a) serving as co-chair of the Resource Development Committee, (b) participating in the Risk Reduction Subcommittee, (c) participating in the Prevention and Community Education Subcommittee, (d) participating in the Recovery Subcommittee and (d) cash contributions to the “Don’t Live in Denial, Ohio” campaign.

Anticipated Impact of These Actions a) Per fiscal year, at least 100 patients per each of the 27 clinics will be assessed for risk for depression and

anxiety using the PHQ-9. With physician referral, patients who will score 10 or above can be referred to OhioHealth social workers or counselors who will provide brief interventions for patients with mild to moderately severe depression and anxiety (scores of 10 or above). Patients eligible for this program typically have PHQ-9 scores of 10 to 19. When appropriate, patients who need ongoing counseling or more in-depth services will be referred to community-based agencies that provide mental, behavioral and substance abuse treatments. A psychiatrist or psychiatric nurse practitioner will review the caseload every week and make medication-related recommendations to the primary care provider if appropriate.

b) Per fiscal year, suicide risk assessment of at least 90 percent of all inpatient admissions. Brief intervention and

community referral of patients who screened positive for suicidal ideation will be provided by social workers or counselors when appropriate.

c) Per fiscal year, up to 25 patients from OhioHealth Family Medicine Grant clinic will be provided medication-

assisted treatment (MAT) and continuing care for mother and baby. d) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-

assisted treatment, prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

e) Per fiscal year, at least 100 patients with mental health and/or addiction diagnoses will be referred to mental and

behavioral health agencies for treatment and/or counseling services. Per fiscal year, at least 100 patients will be referred to HandsOn Central Ohio or to community agencies that address transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

f) Per fiscal year, at least 25 patients from CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction

Stabilization Center will be provided patient-centered and comprehensive women’s health services by the OhioHealth Wellness on Wheels Women’s Health mobile clinic.

g) Per fiscal year, at least 25 patients with mental health and addiction diagnoses will be served by the OhioHealth

Grant Medical Center Addiction Medicine Fellowship program. Per fiscal year, at least 10 patients from OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital will be referred to the Grant Medical Center Addiction Medicine Fellowship program for follow-up care. Patients will be referred to community-based organizations that help with assistance for transportation, food, housing, utility bills, and other social determinants of health needs.

h) Per fiscal year, OhioHealth Grant Medical Center and OhioHealth Riverside Methodist Hospital Trauma

programs will report the following outcomes for patients with mental health and addiction issues: (i) at least 100 patients will be served by the Grant Medical Center Trauma Recovery Center, (ii) at least 100 patients will be administered screening, brief intervention and referral to treatment (SBIRT), (iii) at least five speakerships or health education sessions, and (iv) at least two students will be precepted on mental health and addiction assessment and counseling.

i) Per fiscal year, up to 100 persons will attend the OhioHealth Behavioral Health Family Support Group at The

Dempsey Family Education and Resource Center. Of the participants who attend three or more times and complete the annual anonymous survey, the following impacts will be reported: (a) at least 75 percent will self-report that they learned various techniques to maintain good mental and behavioral health through new coping strategies, (b) at least 75 percent will agree that they feel listened to by the facilitators and/or other members, (c) at least 75 percent will report good customer service experience, and (d) at least 75 percent will recommend the group to other people in search of a family support group.

j) Per fiscal year, at least 20 nurses will attend the continuing education session that is focused on mental health and addiction. At least 90 percent will meet the learning objectives of the continuing education session. Nurse continuing education hours will be provided through OhioHealth Learning.

k) Per fiscal year, at least 40 people from at least five congregations will be served by OhioHealth Faith Community

Relations outreach programs. l) Per fiscal year, at least 50 patients and families will be given Naloxone kits and corresponding education on

Naloxone administration.

m) Per fiscal year, OhioHealth will participate in up to five community-based education events in Franklin County on Naloxone administration.

n) Per fiscal year, OhioHealth in partnership with the Franklin County Opiate Task Force will provide

accomplishments related to at least one of the following goals: (a) prevent opiate abuse and addiction, (b) reduce the number of opiate-related deaths, (c) expand access for treatment, and (d) improve the safety of the Franklin County communities.

Committed OhioHealth Grant Medical Center’s Resources OhioHealth Grant Medical Center clinical and administrative staff committed to these mental health and addiction programs will include but are not limited to: physicians, nurses, social workers, case managers, community health workers and administrative staff. Clinical and administrative staff of OhioHealth Physician Group, Graduate Medical Education, OhioHealth Faith Community Relations and OhioHealth Community Health and Wellness will also be involved. A total of 205,186 residents of Franklin County, which represents 17 percent of the population, has income below the federal poverty level. Among families, 12.5 percent have income below the federal poverty level. Among residents under 18 years of age, 24.5 percent have income below the federal poverty level. Among schoolchildren, 53.6 percent are eligible for free or reduced lunch. The top 10 Franklin County ZIP codes with the lowest median household income are 43210 (The Ohio State University area), 43211 (Linden), 43203 (East Side), 43222 (West Side), 43201 (Clintonville/near North), 43223 (West Side), 43205 (East Side), 43227 (far East Side), 43224 (Linden), 43219 (Agler/Cassady). More than 40 percent of Franklin County residents living in 43201 (Clintonville/near North), 43211 (Linden) and 43203 (East Side) have household income below the federal poverty level. Among households with children, the ZIP codes with the lowest household income were 43201 (Clintonville/near North), 43205 (East Side), 43203 (East Side), 43211 (Linden) and 43222 (West Side). Among the Franklin County residents who live below the federal poverty level, 47 percent were white and 40 percent were black or African American. Housing and homelessness. A total of 1,229 persons use emergency shelters, of whom 32.4 percent were part of families. Among Franklin County residents who use emergency shelters, 76 percent were black or African American, 22 percent were white, 3 percent were Hispanic and 2 percent were other races. A total of 17.2 percent of households pay housing costs that were greater than or equal to 50 percent of their income. A total of 31.9 percent of households pay housing costs that were greater than or equal to 30 percent of their income. The eviction rate in Columbus is 4.6 for every 100 renter homes. Food insecurity. A total of 17.4 percent of Franklin County residents experience food insecurity, which means they lack access to adequate food and have limited availability of nutritious foods. A total of 14.6 percent of households residing in Franklin County rely on food stamps. Among those households with children, 22 percent depend on food stamp assistance. A total of 40 percent of households residing in ZIP codes 43203 (East Side), 43211 (Linden) and 43222 (West Side) receive food stamps. Education. A total of 65.4 percent of Franklin County residents had either one of the following: (a) college courses, (b) an associate’s degree, (c) a bachelor’s degree and (d) graduate/professional degree. A total of 25 percent of Franklin County residents have a high school diploma. A total of 25 percent of Franklin County residents have a high school diploma. A total of 9.7 percent of Franklin County residents aged 25 and older did not have a high school diploma or General Education Diploma (GED). In Franklin County, the school districts that have the lowest high school graduation rates are Columbus City Schools, Whitehall City Schools, Groveport Madison Local Schools, South-Western City Schools, and Westerville City Schools. The top five ZIP codes with the highest percent of residents who do not have a high school diploma include 43222, 43223, 43211, 43207 and 43209. Based on Ohio Department of Education assessments, a total of 73.4 percent of students were ready for kindergarten and 91.4 percent of third graders demonstrated reading proficiency. Unemployment. A total of 30.3 percent of Franklin County residents were not in the labor force. The annual average unemployment rate was 4 percent. Objective To address income/poverty in Franklin County through community partnerships, access to quality healthcare regardless of ability to pay, and referral and linkage to social service agencies Actions OhioHealth Grant Medical Center Intends to Take to Address the Health Need a) As part of the OhioHealth outreach to promote employment in the healthcare field, OhioHealth’s HR department

will continue to collaborate with Godman Guild and Reeb Avenue Center in providing Franklin County residents with: (i) learning opportunities about the patient transportation and patient support assistant (PSA) jobs at OhioHealth, (ii) participate in one to two weeks of hands-on training and shadowing of OhioHealth employees who are currently on these jobs, and (iii) interview training.

b) As part of OhioHealth HR’s partnership with HandsOn Central Ohio, OhioHealth will continue to offer associates

and their family members access to “LinkLine,” a dedicated, confidential toll-free telephone line offering assistance with social services such as but not limited to: low-income housing, rent payment assistance, food assistance, and utility assistance.

c) Continue partnering with Project SEARCH and Columbus City Schools Community Training Program;

OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital provide internships to 11th and 12th grade students with disabilities so they can develop marketable, practical work skills and develop social skills to build mutual respect with peers and supervisors.

The social worker and community health workers collaborate with the multidisciplinary healthcare team in addressing patients’ transportation needs, food insecurity, job search, access to health insurance, and other social determinants of health needs.

no resources and inadequate medical insurance coverage. OhioHealth offers high-quality care to everyone, regardless of ability to pay. The OhioHealth Charity Care Policy includes: (i) substantial charity care guidelines that provide free care for individuals and families who earn less than 200 percent of the federal poverty level, (ii) sliding scale fees to provide substantially discounted care for individuals and families who are between 200 and 400 percent of the federal poverty level, and (iii) hardship policy for those patients who could not otherwise qualify for charity care but have unique circumstances. OhioHealth offers interest-free loans for up to one year to assist patients. Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Infant mortality. In 2017, the infant mortality rate in Franklin County was 8.2 per 1,000 live births. Franklin County failed to meet the Healthy People 2020 infant mortality rate target of 6 per 1,000 live births. In 2017, 155 infants died before age one. The infant mortality rate among blacks was 14.6 per 1,000 live births, which was 2.7 times the infant mortality rate among whites (5.5 per 1,000 live births). It is noteworthy that among the 155 infants who died before age one, 116 babies (75 percent) died before reaching 28 days old, and 39 infants (25 percent) died between 28 days and one year old. The top eight ZIP codes in Franklin County with the highest infant deaths include the following: (a) 43224 (Linden), (b) 43211 (Linden), (c) 43219 (Agler/Cassady), (d) 43223 (West Side), (e) 43206 (East Side), (f) 43207 (far South), (g) 43232 (far East Side), and (h) 43068 (far East Side). Preterm birth and low birth weight. In Franklin County, preterm birth (babies born before 37 weeks gestation) was 10.7 percent. Franklin County failed to meet the Healthy People 2020 target (9.4 percent) for preterm birth. Low birth weight (babies born less than 2,500 grams) was 7.4 percent. Franklin County met the Healthy People 2020 target weight (7.8 percent) for low birth weight. Preconception, pregnancy health and unhealthy behaviors. In Franklin County, the health status of women of childbearing age is as follows: (a) 4.7 percent had either Type 1 or Type 2 diabetes, (b) 4.9 percent had hypertension, (c) 48.5 percent were overweight or obese, (d) 11 percent were current smokers, (e) 5 percent smoked cigarettes during the third trimester, (f) 7.4 percent drank alcohol during the third trimester, (g) 2.1 percent were heavy drinkers before becoming pregnant, (h) 49.9 percent had folic acid deficiency, (i) 24.8 percent had unintended pregnancies, (j) 12 percent of women aged 18 to 44 had no health insurance, (k) 10.9 percent have not had a health check-up in the past year, (l) 13.1 percent have not had a Pap smear in the past three years, and (m) a total of 12.3 out of 1,000 live births had neonatal abstinence syndrome (NAS), which is characterized by withdrawal symptoms from drug exposure in the mother's womb. A total of 1.9 percent of births in Franklin County occurred without prenatal care. Goal To reduce infant mortality rate, percent of preterm birth and percent of low birth weight among babies born in Franklin County Objectives (a) To improve birth outcomes among women of childbearing age by providing high-quality prenatal care and postpartum care, and health education before and during pregnancy and after delivery of her baby; (b) to provide medication-assisted treatment (MAT) to pregnant women with opiate use disorder; and (c) to provide comprehensive women's healthcare services to women of childbearing age who are uninsured or underinsured.

Physician Group clinics offering obstetrics and gynecological services and hospital-based labor and delivery and postpartum units. Women's health services will include but are not limited to: (i) progesterone treatment, including screening for women who are at high risk for preterm birth, (ii) provider counseling with patients about preconception health and reproductive life plans, (iii) comprehensive contraceptive options, (iv) increase breastfeeding support, and (e) referral to home visiting programs in Franklin County on an as-needed basis. b) Continue offering medication-assisted treatment (MAT) to pregnant women through OhioHealth Family Medicine Grant and serve as a medical home for mother and baby.

c) Continue offering medication-assisted treatment (MAT) and consultation to pregnant women with opioid use disorder at OhioHealth Maternal Fetal Medicine Physicians to improve the care and outcomes of mothers and babies who are affected by opioid use disorder. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

d) Continue providing patient-centered women’s health services (obstetrics and gynecology) through the

OhioHealth Wellness on Wheels Women’s Health mobile clinic, which serves predominantly uninsured or underinsured women in Franklin County. The OhioHealth Wellness on Wheels Women’s Health mobile clinic also provides women's health services at CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

e) Continue collaborating with the Ohio Better Birth Outcomes (OBBO) collaborative, which aims to reduce the infant mortality rate in Franklin County by improving healthcare delivery for women and their families through evidence-based quality improvement projects. OBBO focuses on three initiatives: (i) improving reproductive health, (ii) increasing access to prenatal care, and (iii) improving quality of clinical care to reduce premature birth (Ohio Wellness Coalition, 2010-2019).

f) Continue participating in the Ohio Hospital Association/Ohio Department of Health “Ohio First Steps for Healthy Babies” program, which encourages hospitals in Ohio to promote, protect and support breastfeeding. Hospitals earn an award for every two steps achieved based on the “Ten Steps to Successful Breastfeeding” developed by the World Health Organization and Baby-Friendly USA (Ohio Department of Health, n.d., b).

g) Continue operating the OhioHealth Mothers’ Milk Bank, which provides pasteurized donor breast milk to infants who are preterm or have life-threatening medical conditions. The OhioHealth Mothers’ Milk Bank is a member of the Human Milk Banking Association of North America (HMBANA). The OhioHealth Mothers’ Milk Bank sends donated milk to various neonatal intensive care units (NICUs) and mother/baby (postpartum) units in Franklin County hospitals, namely: (i) OhioHealth Riverside Methodist Hospital, (ii) OhioHealth Grant Medical Center, (iii) OhioHealth Doctors Hospital, (iv) OhioHealth Dublin Methodist Hospital, (v) The Ohio State University Wexner Medical Center, (vi) Mount Carmel East, Mount Carmel West, and (vii) Mount Carmel St. Ann’s.

h) Continue offering OhioHealth Grant Medical Center’s CenteringPregnancy program, which enables pregnant women to receive their prenatal care in a group setting and participate in a facilitated discussion and interactive activities on health topics that are practical and relevant to the group’s needs. Grant Medical Center’s CenteringPregnancy program is accredited by the Centering® Healthcare Institute (Centering Healthcare Institute, 2009-2019). The women in a group have similar due dates, which enables opportunities for bonding and support within the group. Pregnant women and their partners are provided in-depth knowledge of pregnancy, fetal development, breastfeeding, labor and delivery, newborn care, family planning and other related topics of interest to the group. CenteringPregnancy is staffed by a nurse coordinator and family physician and facilitator.

at least one year after delivery through OhioHealth Family Medicine Grant, a primary care clinic under Graduate Medical Education that will also provide continuity of care for the mother and her baby.

c) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-assisted treatment (MAT), prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

d) Per fiscal year, up to 500 women served by the OhioHealth Wellness on Wheels Women's Health mobile clinic.

Birth outcomes and pregnancy health behaviors include but are not limited to: infant mortality rate, preterm birth and low birth weight will meet the Healthy People 2020 targets.

e) Per fiscal year, active participation and engagement in the Ohio Better Birth Outcomes (OBBO) projects related to improving reproductive health, access to care, and quality of clinical care provided to women and their babies.

f) Per fiscal year, OhioHealth Grant Medical Center labor and delivery and postpartum units will administer the “Ohio First Steps for Healthy Babies” program through the following: (i) at least 10 persons attending “Childbirth Education” classes, (ii) at least 25 persons attending “Breastfeeding Basics” classes, and (iii) provide at least 1,500 inpatient lactation encounters.

Doctors Hospital will address the three priority health needs in Franklin County, namely: mental health and addiction, income/poverty, and maternal and infant health. Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). A total 21.8 percent (two out of 10 residents) of Franklin County population were depressed. Suicide deaths were 12.3 per 100,000 population. Hospitalizations resulting from assault or alleged abuse were 87.2 per 100,000 population. Hospitalizations resulting from attempted suicide were 4.8 per 100,000 population. Psychiatric admissions were 35.7 per 1,000 population (three out of every 100 admissions). Homicides were eight per 100,000 population. There were 11,224 domestic violence incidents, 6,781 domestic violence victims, of whom 43.3 percent presented with injuries. There were 13,580 child abuse cases, broken down as follows: (a) 42 percent had physical abuse, (b) 19 percent were neglected, (c) 9 percent were sexually abused, (d) 1 percent had emotional maltreatment, (e) 10 percent had multiple allegations of abuse or neglect, and (e) 19 percent belonged to families in need of services. A total 13.7 per 1,000 population (one in every 100 children) aged 18 and under were in foster care. There were 1,635 reports of abuse, neglect, and exploitation of persons aged 60 and older in homes and apartments. There were 11,846 complaints about abuse, neglect, and exploitation in long-term care facilities. In Franklin County, there is one social worker for every 339 residents. There is one chemical counselor for every 1,137 residents. There is one psychologist for every 2,379 residents. Emergency Department (ED) visits related to mental health were 165.7 per 1,000 population (approximately two out of every 10 ED visits). Goal To improve mental health and reduce addiction among Franklin County youth and adults Objective To implement community-based programs and services that address mental health and addiction among Franklin County youth and adults OhioHealth Doctors Hospital’s Intended Actions to Address the Health Need a) Continue assessing depression and anxiety in patients (aged 12 and older) of OhioHealth Physician Group’s

primary care practices using the nine-item Patient Health Questionnaire (PHQ-9). Patients with mild to moderately severe depression and anxiety (PHQ-9 score of 10 or higher) will have access to brief interventions that will be provided by social workers or counselors with physician referral. Psychiatrists or psychiatric nurse practitioners will review the social worker or counselor’s patient caseload and provide recommendations to the primary care physician related to medications. OhioHealth’s Behavioral Health Integration program enables assessment of depression and anxiety, provision of brief interventions to patients, and collaboration among primary care physicians, psychiatrists or psychiatric nurse practitioners related to the need for prescribing medications. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 14.)

b) Continue assessing suicide risk for hospitalized patients aged 12 or older, using the S.A.D. P.E.R.S.O.N.S. scale

for suicide risk. S.A.D. c) Continue offering medication-assisted treatment (MAT) and consultation to pregnant women with opioid use

disorder at OhioHealth Maternal Fetal Medicine Physicians to improve the care and outcomes of mothers and babies who are affected by opioid use disorder. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

d) Continue addressing mental health and addiction and social determinants of health needs of patients with mental

health and/or addiction diagnoses by referring patients to community agencies that provides the following: (i) inpatient and outpatient mental and behavioral health treatment and counseling services and/or (ii) assistance with transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

e) Continue offering patient-centered women’s health services provided by the OhioHealth Wellness on Wheels

Women’s Health to CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

f) Continue implementing the OhioHealth Grant Medical Center Addiction Medicine Fellowship program. The

program aims to adequately train physician fellows to develop clinical competencies, adhere to ethical principles, and improve knowledge and skills in addiction medicine. The program will accept patient referrals from all OhioHealth hospitals in Franklin County, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital. The Addiction Medicine Fellowship program will also collaborate with Nationwide Children’s Hospital, Maryhaven Addiction Stabilization Center, Amethyst Inc., Alvis, CompDrug and the Ohio Department of Rehabilitation and Correction to ensure continuity of care.

g) Continue offering the OhioHealth Behavioral Health Family Support Group at The Dempsey Family Education

and Resource Center at OhioHealth. The support group is offered for adults every Tuesday (up to 52 times per fiscal year) and is co-facilitated by licensed mental healthcare providers from the OhioHealth Behavioral Health department and Mental Health America of Franklin County. On the last Tuesday of the month, the Behavioral Health Family Support Group features a guest speaker and is open to all age groups. The support group focuses on issues faced by a family member or care provider who provides emotional, financial or practical support to a person with a mental health condition. Examples of topics discussed include but are not limited to: supporting recovery, maintaining healthy relationships, establishing boundaries, mental health conditions and recommended treatments, area resources, self-care and resiliency, the grief process, and mindfulness.

h) Continue offering at least one continuing education session per fiscal year to nurses through the OhioHealth

Faith Community Nursing program that will focus on understanding depression and anxiety.

i) Continue offering the mental health and addiction outreach programs through the OhioHealth Faith Community Relations. The outreach programs will include but are not limited to: (i) Mental Health First Aid and (ii) Congregational Care and Hospital Visitation Series.

j) Continue distributing free Naloxone kits and education on administering Naloxone at the OhioHealth Grant

Medical Center Emergency Department, OhioHealth Westerville Emergency Care Center, and OhioHealth Pickerington Emergency Care Center.

k) Continue serving as speakers or educators during the free Naloxone trainings, “Be the One to Save a Life,” for

residents and representatives of community organizations in Franklin County. The training will be focused on how to use the Naloxone kit to stop deadly overdose from opiates or heroin. The outreach program is made possible through a collaboration among Franklin County Public Health, City of Columbus, Columbus Public Health, Maryhaven Addiction Stabilization Center, and the Central Ohio Hospital Council (Franklin County Public Health, 2019).

l) Continue partnering with the Franklin County Opiate Task Force by (a) serving as co-chair of the Resource

Development Committee, (b) participating in the Risk Reduction Subcommittee, (c) participating in the Prevention and Community Education Subcommittee, (d) participating in the Recovery Subcommittee and (d) cash contributions to the “Don’t Live in Denial, Ohio” campaign.

Anticipated Impact of These Actions a) Per fiscal year, at least 100 patients per each of the 27 clinics will be assessed for risk for depression and

anxiety using the PHQ-9. With physician referral, patients who will score 10 or above can be referred to OhioHealth social workers or counselors who will provide brief interventions for patients with mild to moderately severe depression and anxiety (scores of 10 or above). Patients eligible for this program typically have PHQ-9 scores of 10 to 19. When appropriate, patients who need ongoing counseling or more in-depth services will be referred to community-based agencies that provide mental, behavioral and substance abuse treatments. A psychiatrist or psychiatric nurse practitioner will review the caseload every week and make medication-related recommendations to the primary care provider if appropriate.

b) Per fiscal year, suicide risk assessment of at least 90 percent of all inpatient admissions. Brief intervention and

community referral of patients who screened positive for suicidal ideation will be provided by social workers or counselors when appropriate.

c) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-

assisted treatment, prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

d) Per fiscal year, at least 100 patients with mental health and/or addiction diagnoses will be referred to mental and

behavioral health agencies for treatment and/or counseling services. Per fiscal year, at least 100 patients will be referred to HandsOn Central Ohio or to community agencies that address transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

e) Per fiscal year, at least 25 patients from CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction

Stabilization Center will be provided patient-centered and comprehensive women’s health services by the OhioHealth Wellness on Wheels Women’s Health mobile clinic.

f) Per fiscal year, at least 25 patients with mental health and addiction diagnoses will be served by the OhioHealth

Grant Medical Center Addiction Medicine Fellowship program. Per fiscal year, at least 10 patients from OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital will be referred to the Grant Medical Center Addiction Medicine Fellowship program for follow-up care. Patients will be referred to community-based organizations that help with assistance for transportation, food, housing, utility bills, and other social determinants of health needs.

g) Per fiscal year, up to 100 persons will attend the OhioHealth Behavioral Health Family Support Group at The

Dempsey Family Education and Resource Center. Of the participants who attend three or more times and complete the annual anonymous survey, the following impacts will be reported: (a) at least 75 percent will self-report that they learned various techniques to maintain good mental and behavioral health through new coping strategies, (b) at least 75 percent will agree that they feel listened to by the facilitators and/or other members, (c) at least 75 percent will report good customer service experience, and (d) at least 75 percent will recommend the group to other people in search of a family support group.

h) Per fiscal year, at least 20 nurses will attend the continuing education session that is focused on mental health

and addiction. At least 90 percent will meet the learning objectives of the continuing education session. Nurse continuing education hours will be provided through OhioHealth Learning.

i) Per fiscal year, at least 40 people from at least five congregations will be served by OhioHealth Faith Community

Relations outreach programs. j) Per fiscal year, at least 50 patients and families will be given Naloxone kits and corresponding education on

Naloxone administration.

k) Per fiscal year, OhioHealth will participate in up to five community-based education events in Franklin County on Naloxone administration.

l) Per fiscal year, OhioHealth in partnership with the Franklin County Opiate Task Force will provide

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Income. A total of 205,186 residents of Franklin County, which represents 17 percent of the population, has income below the federal poverty level. Among families, 12.5 percent have income below the federal poverty level. Among residents under 18 years of age, 24.5 percent have income below the federal poverty level. Among schoolchildren, 53.6 percent are eligible for free or reduced lunch. The top 10 Franklin County ZIP codes with the lowest median household income are 43210 (The Ohio State University area), 43211 (Linden), 43203 (East Side), 43222 (West Side), 43201 (Clintonville/near North), 43223 (West Side), 43205 (East Side), 43227 (far East Side), 43224 (Linden), 43219 (Agler/Cassady). More than 40 percent of Franklin County residents living in 43201 (Clintonville/near North), 43211 (Linden) and 43203 (East Side) have household income below the federal poverty level. Among households with children, the ZIP codes with the lowest household income were 43201 (Clintonville/near North), 43205 (East Side), 43203 (East Side), 43211 (Linden) and 43222 (West Side). Among the Franklin County residents who live below the federal poverty level, 47 percent were white and 40 percent were black or African American. Housing and homelessness. A total of 1,229 persons use emergency shelters, of whom 32.4 percent were part of families. Among Franklin County residents who use emergency shelters, 76 percent were black or African American, 22 percent were white, 3 percent were Hispanic and 2 percent were other races. A total of 17.2 percent of households pay housing costs that were greater than or equal to 50 percent of their income. Food insecurity. A total of 17.4 percent of Franklin County residents experience food insecurity, which means they lack access to adequate food and have limited availability of nutritious foods. A total of 14.6 percent of households residing in Franklin County rely on food stamps. Among those households with children, 22 percent depend on food stamp assistance. A total of 40 percent of households residing in ZIP codes 43203 (East Side), 43211 (Linden) and 43222 (West Side) receive food stamps. Education. A total of 65.4 percent of Franklin County residents had either one of the following: (a) college courses, (b) an associate’s degree, (c) a bachelor’s degree and (d) graduate/professional degree. A total of 25 percent of Franklin County residents have a high school diploma. A total of 25 percent of Franklin County residents have a high school diploma. A total of 9.7 percent of Franklin County residents aged 25 and older did not have a high school diploma or General Education Diploma (GED). In Franklin County, the school districts that have the lowest high school graduation rates are Columbus City Schools, Whitehall City Schools, Groveport Madison Local Schools, South-Western City Schools, and Westerville City Schools. The top five ZIP codes with the highest percent of residents who do not have a high school diploma include 43222, 43223, 43211, 43207 and 43209. Based on Ohio Department of Education assessments, a total of 73.4 percent of students were ready for kindergarten and 91.4 percent of third graders demonstrated reading proficiency. Unemployment. Objective To address income/poverty in Franklin County through community partnerships, access to quality healthcare regardless of ability to pay, and referral and linkage to social service agencies Actions OhioHealth Doctors Hospital Intends to Take to Address the Health Need a) As part of the OhioHealth outreach to promote employment in the healthcare field, OhioHealth’s HR department

will continue to collaborate with Godman Guild and Reeb Avenue Center in providing Franklin County residents with: (i) learning opportunities about the patient transportation and patient support assistant (PSA) jobs at OhioHealth, (ii) participate in one to two weeks of hands-on training and shadowing of OhioHealth employees who are currently on these jobs, and (iii) interview training.

b) As part of OhioHealth HR’s partnership with HandsOn Central Ohio, OhioHealth will continue to offer associates

and their family members access to “LinkLine,” a dedicated, confidential toll-free telephone line offering assistance with social services such as but not limited to: low-income housing, rent payment assistance, food assistance, and utility assistance.

c) Continue partnering with Project SEARCH and Columbus City Schools Community Training Program;

OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital provide internships to 11th and 12th grade students with disabilities so they can develop marketable, practical work skills and develop social skills to build mutual respect with peers and supervisors.

d) Continue partnering with Junior Achievement of Central Ohio®, a volunteer-led, K-12 programs that enable

children and youth to understand managing finances, promote a sense of entrepreneurship, develop skills for being successful in life and career, and learn the value of contributing towards the progress of their communities (Junior Achievement of Central Ohio, n.d.). Examples of the Junior Achievement of Central Ohio® programs include: (i) JA BE Entrepreneural®, (ii) JA BizTown®, (iii) JA Career Success®, (iv) JA Company Program® Blended Model, (v) JA Economics for Success®, (vi) JA Economics®, (vii) JA Exploring Economics®, and (viii) JA Finance Park® (Junior Achievement of Central Ohio, n.d.).

Wheels Women’s Health and Primary Care mobile clinics so patients with low incomes or stricken by poverty are empowered to make their medical appointments, fill their medication prescriptions, and obtain follow-up care as needed. The social worker and community health workers collaborate with the multidisciplinary healthcare team in addressing patients’ transportation needs, food insecurity, job search, access to health insurance, and other social determinants of health needs.

f) Continue partnering with various chambers of commerce in Franklin County as it relates to workforce

g) Partner with the Columbus Women's Commission, which focuses on pay equity, affordable housing, health and

workforce development (City of Columbus, 2019). h) Continue offering the OhioHealth Charity Care Policy and medical financial assistance to patients with limited or

no resources and inadequate medical insurance coverage. OhioHealth offers high-quality care to everyone, regardless of ability to pay. The OhioHealth Charity Care Policy includes: (i) substantial charity care guidelines that provide free care for individuals and families who earn less than 200 percent of the federal poverty level, (ii) sliding scale fees to provide substantially discounted care for individuals and families who are between 200 and 400 percent of the federal poverty level, and (iii) hardship policy for those patients who could not otherwise qualify for charity care but have unique circumstances. OhioHealth offers interest-free loans for up to one year to assist patients. OhioHealth also has an uninsured discount policy for individuals without health insurance who do not qualify for charity care.

and their families through the OhioHealth LinkLine. All callers will be referred to community resources or OhioHealth programs.

c) Per fiscal year, at least 50 students from Project SEARCH will obtain internships at OhioHealth Riverside

Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital. At least one student who completed the Project SEARCH internship rotations will be hired by OhioHealth.

d) Per fiscal year, the Junior Achievement® of Central Ohio will serve at least 5,000 students, conduct at least 300

classes, and conduct activities in at least 60 schools. e) Per fiscal year, at least 50 patients will be assisted by either the social worker or community health workers from

OhioHealth Wellness on Wheels Women’s Health and Primary Care mobile clinics related to income and poverty-related social determinants of health needs.

f) Per fiscal year, at least one project addressing workforce development by at least one chamber of commerce

partner in Franklin County will be supported through active involvement and in-kind support of OhioHealth. g) Per fiscal year, at least 10 additional employers will sign the "Columbus Commitment: Achieving Pay Equity,"

which fosters equal pay for equal work, specifically for African American and Latina women. Per fiscal year, the Columbus Women's Commission will organize at least one conference, workshop or summit that is focused on gender pay equity, affordable housing, health and workforce development to benefit women and their families.

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Infant mortality. In 2017, the infant mortality rate in Franklin County was 8.2 per 1,000 live births. Franklin County failed to meet the Healthy People 2020 infant mortality rate target of 6 per 1,000 live births. In 2017, 155 infants died before age one. The infant mortality rate among blacks was 14.6 per 1,000 live births, which was 2.7 times the infant mortality rate among whites (5.5 per 1,000 live births). It is noteworthy that among the 155 infants who died before age one, 116 babies (75 percent) died before reaching 28 days old, and 39 infants (25 percent) died between 28 days and one year old. The top eight ZIP codes in Franklin County with the highest infant deaths include the following: (a) 43224 (Linden), (b) 43211 (Linden), (c) 43219 (Agler/Cassady), (d) 43223 (West Side), (e) 43206 (East Side), (f) 43207 (far South), (g) 43232 (far East Side), and (h) 43068 (far East Side). Preterm birth and low birth weight. In Franklin County, preterm birth (babies born before 37 weeks gestation) was 10.7 percent. Franklin County failed to meet the Healthy People 2020 target (9.4 percent) for preterm birth. Low birth weight (babies born less than 2,500 grams) was 7.4 percent. Franklin County met the Healthy People 2020 target weight (7.8 percent) for low birth weight. Preconception, pregnancy health and unhealthy behaviors. In Franklin County, the health status of women of childbearing age is as follows: (a) 4.7 percent had either Type 1 or Type 2 diabetes, (b) 4.9 percent had hypertension, (c) 48.5 percent were overweight or obese, (d) 11 percent were current smokers, (e) 5 percent smoked cigarettes during the third trimester, (f) 7.4 percent drank alcohol during the third trimester, (g) 2.1 percent were heavy drinkers before becoming pregnant, (h) 49.9 percent had folic acid deficiency, (i) 24.8 percent had unintended pregnancies, (j) 12 percent of women aged 18 to 44 had no health insurance, (k) 10.9 percent have not had a health check-up in the past year, (l) 13.1 percent have not had a Pap smear in the past three years, and (m) a total of 12.3 out of 1,000 live births had neonatal abstinence syndrome (NAS), which is characterized by withdrawal symptoms from drug exposure in the mother's womb. A total of 1.9 percent of births in Franklin County occurred without prenatal care. Goal To reduce infant mortality rate, percent of preterm birth and percent of low birth weight among babies born in Franklin County Objectives (a) To improve birth outcomes among women of childbearing age by providing high-quality prenatal care and postpartum care, and health education before and during pregnancy and after delivery of her baby; (b) to provide medication-assisted treatment (MAT) to pregnant women with opiate use disorder; and (c) to provide comprehensive women's healthcare services to women of childbearing age who are uninsured or underinsured.

Physician Group clinics offering obstetrics and gynecological services and hospital-based labor and delivery and postpartum units. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

d) Continue providing patient-centered women’s health services (obstetrics and gynecology) through the

OhioHealth Wellness on Wheels Women’s Health mobile clinic, which serves predominantly uninsured or underinsured women in Franklin County. The OhioHealth Wellness on Wheels Women’s Health mobile clinic also provides women's health services at CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

e) Continue collaborating with the Ohio Better Birth Outcomes (OBBO) collaborative, which aims to reduce the infant mortality rate in Franklin County by improving healthcare delivery for women and their families through evidence-based quality improvement projects. OBBO focuses on three initiatives: (i) improving reproductive health, (ii) increasing access to prenatal care, and (iii) improving quality of clinical care to reduce premature birth (Ohio Wellness Coalition, 2010-2019).

f) Continue participating in the Ohio Hospital Association/Ohio Department of Health “Ohio First Steps for Healthy Babies” program, which encourages hospitals in Ohio to promote, protect and support breastfeeding. Hospitals earn an award for every two steps achieved based on the “Ten Steps to Successful Breastfeeding” developed by the World Health Organization and Baby-Friendly USA (Ohio Department of Health, n.d., b).

g) Continue operating the OhioHealth Mothers’ Milk Bank, which provides pasteurized donor breast milk to infants who are preterm or have life-threatening medical conditions. The OhioHealth Mothers’ Milk Bank is a member of the Human Milk Banking Association of North America (HMBANA). c) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-assisted treatment, prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

d) Per fiscal year, up to 500 women served by the OhioHealth Wellness on Wheels Women's Health mobile clinic.

Birth outcomes and pregnancy health behaviors include but are not limited to: infant mortality rate, preterm birth and low birth weight will meet the Healthy People 2020 targets.

e) Per fiscal year, active participation and engagement in the Ohio Better Birth Outcomes (OBBO) projects related to improving reproductive health, access to care, and quality of clinical care provided to women and her baby.

f) Per fiscal year, the following outcomes will be reported by OhioHealth Doctors Hospital: (a) up to 1,000 health education interactions with patients on the importance of breastfeeding to the overall health of mothers and babies, (b) up to 800 teaching interactions on proper breastfeeding techniques, (c) up to 20 interactions related to coordination of breastfeeding pumps rental, (d) at least 200 breastfeeding assistance provided through the telephone, (e) at least 25 patients served during breastfeeding classes, (f) at least 10 patients served through outpatient lactation consults, (g) at least 50 hours of online education on breastfeeding education, (h) at least 10 healthcare nurses and lactation consultants educated on the importance of breastfeeding, and (i) at least 75 percent breastfeeding rate.

g) Per fiscal year, the OhioHealth Mothers’ Milk Bank will distribute at least 20,000 ounces of pasteurized donor breast milk to hospitals in Franklin County to provide important first food for babies who are either premature or with life-threatening conditions.

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). A total 21.8 percent (two out of 10 residents) of Franklin County population were depressed. Suicide deaths were 12.3 per 100,000 population. Hospitalizations resulting from assault or alleged abuse were 87.2 per 100,000 population. Hospitalizations resulting from attempted suicide were 4.8 per 100,000 population. Psychiatric admissions were 35.7 per 1,000 population (three out of every 100 admissions). Homicides were eight per 100,000 population. There were 11,224 domestic violence incidents, 6,781 domestic violence victims, of whom 43.3 percent presented with injuries. There were 13,580 child abuse cases, broken down as follows: (a) 42 percent had physical abuse, (b) 19 percent were neglected, (c) 9 percent were sexually abused, (d) 1 percent had emotional maltreatment, (e) 10 percent had multiple allegations of abuse or neglect, and (e) 19 percent belonged to families in need of services. A total 13.7 per 1,000 population (one in every 100 children) aged 18 and under were in foster care. There were 1,635 reports of abuse, neglect, and exploitation of persons aged 60 and older in homes and apartments. There were 11,846 complaints about abuse, neglect, and exploitation in long-term care facilities. In Franklin County, there is one social worker for every 339 residents. There is one chemical counselor for every 1,137 residents. There is one psychologist for every 2,379 residents. Emergency Department (ED) visits related to mental health were 165.7 per 1,000 population (approximately two out of every 10 ED visits). Goal To improve mental health and reduce addiction among Franklin County youth and adults Objective To implement community-based programs and services that address mental health and addiction among Franklin County youth and adults OhioHealth Dublin Methodist Hospital’s Intended Actions to Address the Health Need a) Continue assessing depression and anxiety in patients (aged 12 and older) of OhioHealth Physician Group’s

primary care practices using the nine-item Patient Health Questionnaire (PHQ-9). Patients with mild to moderately severe depression and anxiety (PHQ-9 score of 10 or higher) will have access to brief interventions that will be provided by social workers or counselors with physician referral. Psychiatrists or psychiatric nurse practitioners will review the social worker or counselor’s patient caseload and provide recommendations to the primary care physician related to medications. OhioHealth’s Behavioral Health Integration program enables assessment of depression and anxiety, provision of brief interventions to patients, and collaboration among primary care physicians, psychiatrists or psychiatric nurse practitioners related to the need for prescribing medications. (This strategy aligns with the Ohio 2017-2019 State Health Improvement Plan. See Table 20.)

b) Continue assessing suicide risk for hospitalized patients aged 12 or older, using the S.A.D. P.E.R.S.O.N.S. scale

for suicide risk. S.A.D. P.E.R.S.O.N.S. stands for Sex, Age, Depression, Prior History, Ethanol abuse, Rational Thinking Loss, Support system loss, Organized plan, No significant other, Sickness (Warden et al., 2014). e) Continue offering patient-centered women’s health services provided by the OhioHealth Wellness on Wheels

Women’s Health mobile clinic to CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

f) Continue implementing the OhioHealth Grant Medical Center Addiction Medicine Fellowship program. The

program aims to adequately train physician fellows to develop clinical competencies, adhere to ethical principles, and improve knowledge and skills in addiction medicine. The program will accept patient referrals from all OhioHealth hospitals in Franklin County, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital. The Addiction Medicine Fellowship program will also collaborate with Nationwide Children’s Hospital, Maryhaven Addiction Stabilization Center, Amethyst Inc., Alvis, CompDrug and the Ohio Department of Rehabilitation and Correction to ensure continuity of care.

g) Continue offering the OhioHealth Behavioral Health Family Support Group at The Dempsey Family Education

and Resource Center at OhioHealth. The support group is offered for adults every Tuesday (up to 52 times per fiscal year) and is co-facilitated by licensed mental healthcare providers from the OhioHealth Behavioral Health department and Mental Health America of Franklin County. On the last Tuesday of the month, the Behavioral Health Family Support Group features a guest speaker and is open to all age groups. The support group focuses on issues faced by a family member or care provider who provides emotional, financial or practical support to a person with a mental health condition. Examples of topics discussed include but are not limited to: supporting recovery, maintaining healthy relationships, establishing boundaries, mental health conditions and recommended treatments, area resources, self-care and resiliency, the grief process, and mindfulness.

h) Continue offering at least one continuing education session per fiscal year to nurses through the OhioHealth

Faith Community Nursing program focused on understanding depression and anxiety. i) Continue offering the mental health and addiction outreach programs through the OhioHealth Faith Community

Relations. The outreach programs will include but are not limited to: (i) Mental Health First Aid and (ii) Congregational Care and Hospital Visitation Series.

j) Continue distributing free Naloxone kits and education on administering Naloxone at the OhioHealth Grant

Medical Center Emergency Department, OhioHealth Westerville Emergency Care Center, and OhioHealth Pickerington Emergency Care Center.

k) Continue serving as speakers or educators during the free Naloxone trainings, “Be the One to Save a Life,” for residents and representatives of community organizations in Franklin County. The training will be focused on how to use the Naloxone kit to stop deadly overdose from opiates or heroin. The outreach program is made possible through a collaboration among Franklin County Public Health, City of Columbus, Columbus Public Health, Maryhaven Addiction Stabilization Center and the Central Ohio Hospital Council (Franklin County Public Health, 2019).

l) Continue partnering with the Franklin County Opiate Task Force by (a) serving as co-chair of the Resource

Development Committee, (b) participating in the Risk Reduction Subcommittee, (c) participating in the Prevention and Community Education Subcommittee, (d) participating in the Recovery Subcommittee and (d) cash contributions to the “Don’t Live in Denial, Ohio” campaign.

Anticipated Impact of These Actions a) Per fiscal year, at least 100 patients per each of the 27 clinics will be assessed for risk for depression and

anxiety using the PHQ-9. With physician referral, patients who score 10 or above can be referred to OhioHealth social workers or counselors who will provide brief interventions for patients with mild to moderately severe depression and anxiety (scores of 10 or above). Patients eligible for this program typically have PHQ-9 scores of 10 to 19. When appropriate, patients who need ongoing counseling or more in-depth services will be referred to community-based agencies that provide mental, behavioral and substance abuse treatments. A psychiatrist or psychiatric nurse practitioner will review the caseload every week and make medication-related recommendations to the primary care provider if appropriate.

b) Per fiscal year, suicide risk assessment of at least 90 percent of all inpatient admissions. Brief intervention and

community referral of patients who screened positive for suicidal ideation will be provided by social workers or counselors when appropriate.

c) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-

assisted treatment, prenatal care and consultation services by OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

d) Per fiscal year, at least 100 patients with mental health and/or addiction diagnoses will be referred to mental and behavioral health agencies for treatment and/or counseling services. Per fiscal year, at least 100 patients will be referred to HandsOn Central Ohio or to community agencies that address transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

e) Per fiscal year, at least 25 patients from CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction

Stabilization Center will be provided patient-centered and comprehensive women’s health services by the OhioHealth Wellness on Wheels Women’s Health mobile clinic.

f) Per fiscal year, at least 25 patients with mental health and addiction diagnoses will be served by the OhioHealth

Grant Medical Center Addiction Medicine Fellowship program. Per fiscal year, at least 10 patients from OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital will be referred to the Grant Medical Center Addiction Medicine Fellowship program for follow-up care. Patients will be referred to community-based organizations that helps with assistance for transportation, food, housing, utility bills, and other social determinants of health needs.

g) Per fiscal year, up to 100 persons will attend the OhioHealth Behavioral Health Family Support Group at The

Dempsey Family Education and Resource Center. Of the participants who attend three or more times and complete the annual anonymous survey, the following impacts will be reported: (i) at least 75 percent will self-report that they learned various techniques to maintain good mental and behavioral health through new coping strategies, (ii) at least 75 percent will agree that they feel listened to by the facilitators and/or other members, (iii) at least 75 percent will report good customer service experience, and (iv) at least 75 percent will recommend the group to other people in search of a family support group.

h) Per fiscal year, at least 20 nurses will attend the continuing education session that is focused on mental health

and addiction. At least 90 percent will meet the learning objectives of the continuing education session. Nurse continuing education hours will be provided through OhioHealth Learning.

i) Per fiscal year, at least 40 people from at least five congregations will be served by OhioHealth Faith Community

Relations outreach programs. j) Per fiscal year, at least 50 patients and families will be given Naloxone kits and corresponding education on

Naloxone administration. k) Per fiscal year, OhioHealth will participate in up to five community-based education events in Franklin County on

Naloxone administration. l) Per fiscal year, OhioHealth in partnership with the Franklin County Opiate Task Force will provide

accomplishments related to at least one of the following goals: (a) prevent opiate abuse and addiction, (b) reduce the number of opiate-related deaths, (c) expand access for treatment, and (d) improve the safety of the Franklin County communities.

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Income. A total of 205,186 residents of Franklin County, which represents 17 percent of the population, has income below the federal poverty level. Among families, 12.5 percent have income below the federal poverty level. Among residents under 18 years of age, 24.5 percent have income below the federal poverty level. Among schoolchildren, 53.6 percent are eligible for free or reduced lunch. The top 10 Franklin County ZIP codes with the lowest median household income are 43210 (The Ohio State University area), 43211 (Linden), 43203 (East Side), 43222 (West Side), 43201 (Clintonville/near North), 43223 (West Side), 43205 (East Side), 43227 (far East Side), 43224 (Linden), 43219 (Agler/Cassady). More than 40 percent of Franklin County residents living in 43201 (Clintonville/near North), 43211 (Linden) and 43203 (East Side) have household income below the federal poverty level. Among households with children, the ZIP codes with the lowest household income were 43201 (Clintonville/near North), 43205 (East Side), 43203 (East Side), 43211 (Linden) and 43222 (West Side). Among the Franklin County residents who live below the federal poverty level, 47 percent were white and 40 percent were black or African American. Housing and homelessness. A total of 1,229 persons use emergency shelters, of whom 32.4 percent were part of families. Among Franklin County residents who use emergency shelters, 76 percent were black or African American, 22 percent were white, 3 percent were Hispanic and 2 percent were other races. A total of 17.2 percent of households pay housing costs that were greater than or equal to 50 percent of their income. A total of 31.9 percent of households pay housing costs that were greater than or equal to 30 percent of their income. The eviction rate in Columbus is 4.6 for every 100 renter homes. Food insecurity. A total of 17.4 percent of Franklin County residents experience food insecurity, which means they lack access to adequate food and have limited availability of nutritious foods. A total of 14.6 percent of households residing in Franklin County rely on food stamps. Among those households with children, 22 percent depend on food stamp assistance. A total of 40 percent of households residing in ZIP codes 43203 (East Side), 43211 (Linden) and 43222 (West Side) receive food stamps. Education. A total of 65.4 percent of Franklin County residents had either one of the following: (a) college courses, (b) an associate’s degree, (c) a bachelor’s degree and (d) graduate/professional degree. A total of 25 percent of Franklin County residents have a high school diploma. A total of 25 percent of Franklin County residents have a high school diploma. A total of 9.7 percent of Franklin County residents aged 25 and older did not have a high school diploma or General Education Diploma (GED). In Franklin County, the school districts that have the lowest high school graduation rates are Columbus City Schools, Whitehall City Schools, Groveport Madison Local Schools, South-Western City Schools, and Westerville City Schools. will continue to collaborate with Godman Guild and Reeb Avenue Center in providing Franklin County residents with: (i) learning opportunities about the patient transportation and patient support assistant (PSA) jobs at OhioHealth, (ii) participate in one to two weeks of hands-on training and shadowing of OhioHealth employees who are currently on these jobs, and (iii) interview training.

b) As part of OhioHealth HR’s partnership with HandsOn Central Ohio, OhioHealth will continue to offer associates

and their family members access to “LinkLine,” a dedicated, confidential toll-free telephone line offering assistance with social services such as but not limited to: low-income housing, rent payment assistance, food assistance, and utility assistance.

c) Continue partnering with Project SEARCH and Columbus City Schools Community Training Program;

OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital provide internships to 11th and 12th grade students with disabilities so they can develop marketable, practical work skills and develop social skills to build mutual respect with peers and supervisors.

d) Continue partnering with Junior Achievement of Central Ohio®, volunteer-led, K-12 programs that enable children

and youth to understand managing finances, promote a sense of entrepreneurship, develop skills for being successful in life and career, and learn the value of contributing towards the progress of their communities (Junior Achievement of Central Ohio, n.d.). Wheels Women’s Health and Primary Care mobile clinics so patients with low incomes or stricken by poverty are empowered to make their medical appointments, fill their medication prescriptions, and obtain follow-up care as needed. The social worker and community health workers collaborate with the multidisciplinary healthcare team in addressing patients’ transportation needs, food insecurity, job search, access to health insurance, and other social determinants of health needs.

f) Continue partnering with various chambers of commerce in Franklin County as it relates to workforce

development, including but not limited to: (i) Clintonville Area Chamber of Commerce, (ii) Columbus Chamber of Commerce, (iii) Dublin Chamber of Commerce, (iv) Gahanna Chamber of Commerce, (v) Greater Powell Chamber of Commerce, (vi) Grove City Chamber of Commerce, (vii) Hilliard Chamber of Commerce, (viii) New Albany Chamber of Commerce, (ix) Tri-Village Chamber Partnership (Upper Arlington, Marble Cliff, Grandview), (x) Westerville Chamber of Commerce and (xi) Worthington Chamber of Commerce.

g) Partner with the Columbus Women's Commission, which focuses on pay equity, affordable housing, health and

workforce development (City of Columbus, 2019). h) Continue offering the OhioHealth Charity Care Policy and medical financial assistance to patients with limited or

no resources and inadequate medical insurance coverage. OhioHealth offers high-quality care to everyone, regardless of ability to pay. The OhioHealth Charity Care Policy includes: (i) substantial charity care guidelines that provide free care for individuals and families who earn less than 200 percent of the federal poverty level, (ii) sliding scale fees to provide substantially discounted care for individuals and families who are between 200 and 400 percent of the federal poverty level, and (iii) hardship policy for those patients who could not otherwise qualify for charity care but have unique circumstances. OhioHealth offers interest-free loans for up to one year to assist patients. OhioHealth also has an uninsured discount policy for individuals without health insurance who do not qualify for charity care.

b) Per fiscal year, HandsOn Central Ohio will serve at least 1,000 telephone calls from OhioHealth associates and

their families through the OhioHealth LinkLine. All callers will be referred to community resources or OhioHealth programs.

c) Per fiscal year, at least 50 students from Project SEARCH will obtain internships at OhioHealth Riverside

Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital. At least one student who completed the Project SEARCH internship rotations will be hired by OhioHealth.

d) Per fiscal year, the Junior Achievement® of Central Ohio will serve at least 5,000 students, conduct at least 300

classes, and conduct activities in at least 60 schools. e) Per fiscal year, at least 50 patients will be assisted by either the social worker or community health workers from

OhioHealth Wellness on Wheels Women’s Health and Primary Care mobile clinics related to income and poverty-related social determinants of health needs.

f) Per fiscal year, at least one project addressing workforce development by at least one chamber of commerce

partner in Franklin County will be supported through active involvement and in-kind support of OhioHealth. g) Per fiscal year, at least 10 additional employers will sign the "Columbus Commitment: Achieving Pay Equity,"

which fosters equal pay for equal work, specifically for African American and Latina women. Per fiscal year, the Columbus Women's Commission will organize at least one conference, workshop or summit that is focused on gender pay equity, affordable housing, health, and workforce development to benefit women and their families.

h) Per fiscal year, the number and percent of patients and their demographic breakdown (age, gender, race,

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Infant mortality. In 2017, the infant mortality rate in Franklin County was 8.2 per 1,000 live births. Franklin County failed to meet the Healthy People 2020 infant mortality rate target of 6 per 1,000 live births. In 2017, 155 infants died before age one. The infant mortality rate among blacks was 14.6 per 1,000 live births, which was 2.7 times the infant mortality rate among whites (5.5 per 1,000 live births). It is noteworthy that among the 155 infants who died before age one, 116 babies (75 percent) died before reaching 28 days old, and 39 infants (25 percent) died between 28 days and one year old. The top eight ZIP codes in Franklin County with the highest infant deaths include the following: (a) 43224 (Linden), (b) 43211 (Linden), (c) 43219 (Agler/Cassady), (d) 43223 (West Side), (e) 43206 (East Side), (f) 43207 (far South), (g) 43232 (far East Side), and (h) 43068 (far East Side). Preterm birth and low birth weight. In Franklin County, preterm birth (babies born before 37 weeks gestation) was 10.7 percent. Franklin County failed to meet the Healthy People 2020 target (9.4 percent) for preterm birth. Low birth weight (babies born less than 2,500 grams) was 7.4 percent. Franklin County met the Healthy People 2020 target weight (7.8 percent) for low birth weight. Preconception, pregnancy health and unhealthy behaviors. In Franklin County, the health status of women of childbearing age is as follows: (a) 4.7 percent had either Type 1 or Type 2 diabetes, (b) 4.9 percent had hypertension, (c) 48.5 percent were overweight or obese, (d) 11 percent were current smokers, (e) 5 percent smoked cigarettes during the third trimester, (f) 7.4 percent drank alcohol during the third trimester, (g) 2.1 percent were heavy drinkers before becoming pregnant, (h) 49.9 percent had folic acid deficiency, (i) 24.8 percent had unintended pregnancies, (j) 12 percent of women aged 18 to 44 had no health insurance, (k) 10.9 percent have not had a health check-up in the past year, (l) 13.1 percent have not had a Pap smear in the past three years, and (m) a total of 12.3 out of 1,000 live births had neonatal abstinence syndrome (NAS), which is characterized by withdrawal symptoms from drug exposure in the mother's womb. A total of 1.9 percent of births in Franklin County occurred without prenatal care. Physician Group clinics offering obstetrics and gynecological services and hospital-based labor and delivery and postpartum units. Women's health services will include but are not limited to: (i) progesterone treatment, including screening for women who are at high risk for preterm birth, (ii) provider counseling with patients about preconception health and reproductive life plans, (iii) comprehensive contraceptive options, (iv) increase breastfeeding support, and (e) referral to home visiting programs in Franklin County on an as-needed basis. b) Continue offering medication-assisted treatment (MAT) and consultation to pregnant women with opioid use disorder at OhioHealth Maternal Fetal Medicine Physicians to improve the care and outcomes of mothers and babies who are affected by opioid use disorder. OhioHealth Maternal Fetal Medicine Physicians will actively collaborate with the Ohio Perinatal Quality Collaborative (OPQC) by submitting clinical data and evidence-based practice and findings as part of the Maternal Opiate Medical Supports Plus (MOMS+) program.

c) Continue providing patient-centered women’s health services (obstetrics and gynecology) through the

OhioHealth Wellness on Wheels Women’s Health mobile clinic, which serves predominantly uninsured or underinsured women in Franklin County. The OhioHealth Wellness on Wheels Women’s Health mobile clinic also provides women's health services at CATCH Court, Amethyst Inc., CompDrug and Maryhaven Addiction Stabilization Center to serve women with substance abuse issues or who have been sexually trafficked.

d) Continue collaborating with the Ohio Better Birth Outcomes (OBBO) collaborative, which aims to reduce the infant mortality rate in Franklin County by improving healthcare delivery for women and their families through evidence-based quality improvement projects. OBBO focuses on three initiatives: (i) improving reproductive health, (ii) increasing access to prenatal care, and (iii) improving quality of clinical care to reduce premature birth (Ohio Wellness Coalition, 2010-2019).

e) Continue participating in the Ohio Hospital Association/Ohio Department of Health “Ohio First Steps for Healthy Babies” program, which encourages hospitals in Ohio to promote, protect and support breastfeeding. Hospitals earn an award for every two steps achieved based on the “Ten Steps to Successful Breastfeeding” developed by the World Health Organization and Baby-Friendly USA (Ohio Department of Health, n.d., b).

f) Continue operating the OhioHealth Mothers’ Milk Bank, which provides pasteurized donor breast milk to infants who are preterm or have life-threatening medical conditions. The OhioHealth Mothers’ Milk Bank is a member of the Human Milk Banking Association of North America (HMBANA). The OhioHealth Mothers’ Milk Bank sends donated milk to various neonatal intensive care units (NICUs) and mother/baby (postpartum) units in Franklin County hospitals, namely: (i) OhioHealth Riverside Methodist Hospital, (ii) OhioHealth Grant Medical Center, (iii) OhioHealth Doctors Hospital, (iv) OhioHealth Dublin Methodist Hospital, (v) The Ohio State University Wexner Medical Center, (vi) Mount Carmel East, Mount Carmel West, and (vii) Mount Carmel St. Ann’s.

Anticipated Impact of These Actions a) Per fiscal year, the total number of patients served by each OhioHealth hospital labor and delivery and

postpartum units, including percent demographics breakdown (age, gender, race, ethnicity), will be reported for each OhioHealth Hospital, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital. Per fiscal year, data on percent preterm birth and low birth weight babies born per hospital will be reported.

b) Per fiscal year, at least 25 pregnant patients suffering from opioid use disorder will be provided medication-

assisted treatment (MAT), prenatal care and consultation services by the OhioHealth Maternal Fetal Medicine Physicians to improve care of affected mothers and babies. Per fiscal year, OhioHealth Maternal Fetal Medicine Physicians will report evidence-based findings from quality improvements through the Maternal Opiate Medical Supports Plus (MOMS+) program, in collaboration with the Ohio Perinatal Quality Collaborative (OPQC).

OhioHealth Rehabilitation Hospital Implementation Strategy (Actions, Anticipated Impact, Resources and Collaborative Partners) The actions to be taken, anticipated impact, resources to be committed and collaborative partners by OhioHealth Rehabilitation Hospital, as part of the Joint Implementation Strategy with Riverside Methodist Hospital, Grant Medical Center, Doctors Hospital and Dublin Methodist Hospital are shown in Tables 25 to 28. OhioHealth Rehabilitation Hospital will address two of the three priority health needs in Franklin County, namely: mental health and addiction and income/poverty. The Rehabilitation Hospital will not address maternal and infant health, which is the third priority health need in Franklin County. The Rehabilitation Hospital is a specialty hospital and does not provide healthcare or outreach services that address maternal and infant health. In addition, the Rehabilitation Hospital does not have the expertise to address this need and that OhioHealth Riverside Methodist, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital have. In alignment with the Ohio 2017-2019 State Health Improvement Plan, the Rehabilitation Hospital will address mental health and addiction.

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). A total 21.8 percent (two out of 10 residents) of Franklin County population were depressed. Suicide deaths were 12.3 per 100,000 population. Hospitalizations resulting from assault or alleged abuse were 87.2 per 100,000 population. Hospitalizations resulting from attempted suicide were 4.8 per 100,000 population. Psychiatric admissions were 35.7 per 1,000 population (three out of every 100 admissions). Homicides were eight per 100,000 population. There were 11,224 domestic violence incidents, 6,781 domestic violence victims, of whom 43.3 percent presented with injuries. There were 13,580 child abuse cases, broken down as follows: (a) 42 percent had physical abuse, (b) 19 percent were neglected, (c) 9 percent were sexually abused, (d) 1 percent had emotional maltreatment, (e) 10 percent had multiple allegations of abuse or neglect, and (e) 19 percent belonged to families in need of services. A total 13.7 per 1,000 population (one in every 100 children) aged 18 and under were in foster care. There were 1,635 reports of abuse, neglect, and exploitation of persons aged 60 and older in homes and apartments. There were 11,846 complaints about abuse, neglect, and exploitation in long-term care facilities. In Franklin County, there is one social worker for every 339 residents. There is one chemical counselor for every 1,137 residents. There is one psychologist for every 2,379 residents. Emergency Department (ED) visits related to mental health were 165.7 per 1,000 population (approximately two out of every 10 ED visits). Goal To improve mental health and reduce addiction among Franklin County youth and adults Objective To implement community-based programs and services that address mental health and addiction among Franklin County youth and adults OhioHealth Rehabilitation Hospital’s Intended Actions to Address the Health Need a) Continue assessing depression and anxiety in patients using the nine-item Patient Health Questionnaire

The Addiction Medicine Fellowship program will also collaborate with Nationwide Children’s Hospital, Maryhaven Addiction Stabilization Center, Amethyst Inc., Alvis, CompDrug and the Ohio Department of Rehabilitation and Correction to ensure continuity of care.

e) Continue offering the OhioHealth Behavioral Health Family Support Group at The Dempsey Family Education

and Resource Center at OhioHealth. The support group is offered for adults every Tuesday (up to 52 times per fiscal year) and is co-facilitated by licensed mental healthcare providers from the OhioHealth Behavioral Health department and Mental Health America of Franklin County. On the last Tuesday of the month, the Behavioral Health Family Support Group features a guest speaker and is open to all age groups. The support group focuses on issues faced by a family member or care provider who provides emotional, financial or practical support to a person with a mental health condition. Examples of topics discussed include but are not limited to: supporting recovery, maintaining healthy relationships, establishing boundaries, mental health conditions and recommended treatments, area resources, self-care and resiliency, the grief process, and mindfulness.

f) Continue offering at least one continuing education session per fiscal year to nurses through the OhioHealth

Faith Community Nursing program that will focus on understanding depression and anxiety. g) Continue offering the mental health and addiction outreach programs through the OhioHealth Faith Community

Relations. The outreach programs will include but are not limited to: (i) Mental Health First Aid and (ii) Congregational Care and Hospital Visitation Series.

h) Continue distributing free Naloxone kits and education on administering Naloxone at the OhioHealth Grant

Medical Center Emergency Department, OhioHealth Westerville Emergency Care Center, and OhioHealth Pickerington Emergency Care Center.

i) Continue serving as speakers or educators during the free Naloxone trainings, “Be the One to Save a Life,” for

residents and representatives of community organizations in Franklin County. The training will be focused on how to use the Naloxone kit to stop deadly overdose from opiates or heroin. The outreach program is made possible through a collaboration among Franklin County Public Health, City of Columbus, Columbus Public Health, Maryhaven Addiction Stabilization Center and the Central Ohio Hospital Council (Franklin County Public Health, 2019).

j) Continue partnering with the Franklin County Opiate Task Force by (a) serving as co-chair of the Resource

Development Committee, (b) participating in the Risk Reduction Subcommittee, (c) participating in the Prevention and Community Education Subcommittee, (d) participating in the Recovery Subcommittee and (d) cash contributions to the “Don’t Live in Denial, Ohio” campaign.

Anticipated Impact of These Actions a) Per fiscal year, at least 100 patients will be screened for anxiety and depression. When appropriate, patients who

need ongoing counseling or more in-depth services will be referred to community-based agencies that provide mental, behavioral and substance abuse treatments. A psychiatrist or psychiatric nurse practitioner will review the caseload every week and make medication-related recommendations to the primary care provider if appropriate.

b) Per fiscal year, suicide risk assessment of at least 90 percent of all inpatient admissions. Brief intervention and

community referral of patients who screened positive for suicidal ideation will be provided by social workers or counselors when appropriate.

c) Per fiscal year, at least 100 patients with mental health and/or addiction diagnoses will be referred to mental and

behavioral health agencies for treatment and/or counseling services. Per fiscal year, at least 100 patients will be referred to HandsOn Central Ohio or to community agencies that address transportation, housing, utilities, prescription medications, employment, and other social determinants of health needs.

d) Per fiscal year, at least 25 patients with mental health and addiction diagnoses will be served by the OhioHealth

Grant Medical Center Addiction Medicine Fellowship program. Per fiscal year, at least 10 patients from OhioHealth Riverside Methodist Hospital, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital will be referred to the Grant Medical Center Addiction Medicine

Fellowship program for follow-up care. Patients will be referred to community-based organizations that help with assistance for transportation, food, housing, utility bills, and other social determinants of health needs.

e) Per fiscal year, up to 100 persons will attend the OhioHealth Behavioral Health Family Support Group at The

Dempsey Family Education and Resource Center. Of the participants who attend three or more times and complete the annual anonymous survey, the following impacts will be reported: (i) at least 75 percent will self-report that they learned various techniques to maintain good mental and behavioral health through new coping strategies, (ii) at least 75 percent will agree that they feel listened to by the facilitators and/or other members, (iii) at least 75 percent will report good customer service experience, and (iv) at least 75 percent will recommend the group to other people in search of a family support group.

f) Per fiscal year, at least 20 nurses will attend the continuing education session that is focused on mental health

and addiction. At least 90 percent will meet the learning objectives of the continuing education session. Nurse continuing education hours will be provided through OhioHealth Learning.

g) Per fiscal year, at least 40 people from at least five congregations will be served by OhioHealth Faith Community

Relations outreach programs. h) Per fiscal year, at least 50 patients and families will be given Naloxone kits and corresponding education on

Naloxone administration. i) Per fiscal year, OhioHealth will participate in up to five community-based education events in Franklin County on

Naloxone administration. j) Per fiscal year, OhioHealth in partnership with the Franklin County Opiate Task Force will provide

accomplishments related to at least one of the following goals: (a) prevent opiate abuse and addiction, (b) reduce the number of opiate-related deaths, (c) expand access for treatment, and (d) improve the safety of the Franklin County communities.

Implementation Strategy to Meet Community Health Needs

Brief Description of Need Data described here were obtained from the Franklin County HealthMap 2019 (Franklin County Community Health Needs Assessment Steering Committee, 2019). Income. A total of 205,186 residents of Franklin County, which represents 17 percent of the population, has income below the federal poverty level. Among families, 12.5 percent have income below the federal poverty level. Among residents under 18 years of age, 24.5 percent have income below the federal poverty level. Among schoolchildren, 53.6 percent are eligible for free or reduced lunch. The top 10 Franklin County ZIP codes with the lowest median household income are 43210 (The Ohio State University area), 43211 (Linden), 43203 (East Side), 43222 (West Side), 43201 (Clintonville/near North), 43223 (West Side), 43205 (East Side), 43227 (far East Side), 43224 (Linden), 43219 (Agler/Cassady). More than 40 percent of Franklin County residents living in 43201 (Clintonville/near North), 43211 (Linden) and 43203 (East Side) have household income below the federal poverty level. Among households with children, the ZIP codes with the lowest household income were 43201 (Clintonville/near North), 43205 (East Side), 43203 (East Side), 43211 (Linden) and 43222 (West Side). Among the Franklin County residents who live below the federal poverty level, 47 percent were white and 40 percent were black or African American. Housing and homelessness. A total of 1,229 persons use emergency shelters, of whom 32.4 percent were part of families. Among Franklin County residents who use emergency shelters, 76 percent were black or African American, 22 percent were white, 3 percent were Hispanic and 2 percent were other races. A total of 17.2 percent of households pay housing costs that were greater than or equal to 50 percent of their income. A total of 31.9 percent of households pay housing costs that were greater than or equal to 30 percent of their income. The eviction rate in Columbus is 4.6 for every 100 renter homes. Food insecurity. A total of 17.4 percent of Franklin County residents experience food insecurity, which means they lack access to adequate food and have limited availability of nutritious foods. A total of 14.6 percent of households residing in Franklin County rely on food stamps. Among those households with children, 22 percent depend on food stamp assistance. A total of 40 percent of households residing in ZIP codes 43203 (East Side), 43211 (Linden) and 43222 (West Side) receive food stamps. Education. A total of 65.4 percent of Franklin County residents had either one of the following: (a) college courses, (b) an associate’s degree, (c) a bachelor’s degree and (d) graduate/professional degree. A total of 25 percent of Franklin County residents have a high school diploma. A total of 9.7 percent of Franklin County residents aged 25 and older did not have a high school diploma or General Education Diploma (GED). In Franklin County, the school districts that have the lowest high school graduation rates are Columbus City Schools, Whitehall City Schools, Groveport Madison Local Schools, South-Western City Schools, and Westerville City Schools. The top five ZIP codes with the highest percent of residents who do not have a high school diploma include 43222, 43223, 43211, 43207 and 43209. Based on Ohio Department of Education assessments, a total of 73.4 percent of students were ready for kindergarten and 91.4 percent of third graders demonstrated reading proficiency. Unemployment. A total of 30.3 percent of Franklin County residents were not in the labor force. The annual average unemployment rate was 4 percent. Implementation Strategy to Meet Community Health Needs

Objective To address income/poverty in Franklin County through community partnerships, access to quality healthcare regardless of ability to pay, and referral and linkage to social service agencies Actions OhioHealth Rehabilitation Hospital Intends to Take to Address the Health Need a) As part of the OhioHealth outreach to promote employment in the healthcare field, OhioHealth’s HR department

will continue to collaborate with Godman Guild and Reeb Avenue Center in providing Franklin County residents with: (i) learning opportunities about the patient transportation and patient support assistant (PSA) jobs at OhioHealth, (ii) participate in one to two weeks of hands-on training and shadowing of OhioHealth employees who are currently on these jobs, and (iii) interview training.

b) As part of OhioHealth HR’s partnership with HandsOn Central Ohio, OhioHealth will continue to offer associates

and their family members access to “LinkLine,” a dedicated, confidential toll-free telephone line offering assistance with social services such as but not limited to: low-income housing, rent payment assistance, food assistance, and utility assistance.

c) Continue partnering with Project SEARCH and Columbus City Schools Community Training Program;

OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital provide internships to 11th and 12th grade students with disabilities so they can develop marketable, practical work skills and develop social skills to build mutual respect with peers and supervisors.

d) Continue partnering with Junior Achievement of Central Ohio®, volunteer-led, K-12 programs that enable children

and youth to understand managing finances, promote a sense of entrepreneurship, develop skills for being successful in life and career, and learn the value of contributing towards the progress of their communities (Junior Achievement of Central Ohio, n.d.). Examples of Junior Achievement of Central Ohio® programs include: (i) JA BE Entrepreneural®, (ii) JA BizTown®, (iii) JA Career Success®, (iv) JA Company Program® Blended Model, (v) JA Economics for Success®, (vi) JA Economics®, (vii) JA Exploring Economics®, and (viii) JA Finance Park® (Junior Achievement of Central Ohio, n.d.).

e) Continue partnering with various chambers of commerce in Franklin County as it relates to workforce

development, including but not limited to: (i) Clintonville Area Chamber of Commerce, (ii) Columbus Chamber of Commerce, (iii) Dublin Chamber of Commerce, (iv) Gahanna Chamber of Commerce, (v) Greater Powell Chamber of Commerce, (vi) Grove City Chamber of Commerce, (vii) Hilliard Chamber of Commerce, (viii) New Albany Chamber of Commerce, (ix) Tri-Village Chamber Partnership (Upper Arlington, Marble Cliff, Grandview), (x) Westerville Chamber of Commerce and (xi) Worthington Chamber of Commerce.

f) Partner with the Columbus Women's Commission, which focuses on pay equity, affordable housing, health and

workforce development (City of Columbus, 2019). g) Continue offering the OhioHealth Charity Care Policy and medical financial assistance to patients with limited or

no resources and inadequate medical insurance coverage. OhioHealth offers high-quality care to everyone, regardless of ability to pay. The OhioHealth Charity Care Policy includes: (i) substantial charity care guidelines that provide free care for individuals and families who earn less than 200 percent of the federal poverty level, (ii) sliding scale fees to provide substantially discounted care for individuals and families who are between 200 and 400 percent of the federal poverty level, and (iii) hardship policy for those patients who could not otherwise qualify for charity care but have unique circumstances. OhioHealth offers interest-free loans for up to one year to assist patients. OhioHealth also has an uninsured discount policy for individuals without health insurance who do not qualify for charity care.

Anticipated Impact of These Actions a) Per fiscal year, at least 10 persons from Franklin County will participate in the OhioHealth HR department’s

employment outreach at the Reeb Avenue Center and Godman Guild. Per fiscal year, at least one participant from the employment outreach at either location will be hired as an OhioHealth associate.

b) Per fiscal year, the HandsOn Central Ohio will serve at least 1,000 telephone calls from OhioHealth associates

and their families through the OhioHealth LinkLine. All callers will be referred to community resources or OhioHealth programs.

c) Per fiscal year, at least 50 students from Project SEARCH will obtain internships at OhioHealth Riverside

Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital and OhioHealth Dublin Methodist Hospital. At least one student who completed the Project SEARCH internship rotations will be hired by OhioHealth.

d) Per fiscal year, the Junior Achievement® of Central Ohio will serve at least 5,000 students, conduct at least 300

classes, and conduct activities in at least 60 schools. e) Per fiscal year, at least one project addressing workforce development by at least one chamber of commerce

partner in Franklin County will be supported through active involvement and in-kind support of OhioHealth. f) Per fiscal year, at least 10 additional employers will sign the "Columbus Commitment: Achieving Pay Equity,"

which fosters equal pay for equal work, specifically for African American and Latina women. Per fiscal year, the Columbus Women's Commission will organize at least one conference, workshop or summit that is focused on gender pay equity, affordable housing, health, and workforce development to benefit women and their families.

g) Per fiscal year, the number and percent of patients and their demographic breakdown (age, gender, race,

ethnicity) who will be eligible for OhioHealth Charity Care guidelines will be reported for each OhioHealth hospital in Franklin County, namely: OhioHealth Riverside Methodist Hospital, OhioHealth Grant Medical Center, OhioHealth Doctors Hospital, OhioHealth Dublin Methodist Hospital and OhioHealth Rehabilitation Hospital. Per fiscal year, the number of patients and their demographic breakdown (age, gender, race, ethnicity) who will be eligible for OhioHealth Charity Care guidelines will be reported for all OhioHealth Physician Group clinics in Franklin County.

Committed Rehabilitation Hospital Resources (a) OhioHealth Rehabilitation Hospital clinical and administrative staff; (b) OhioHealth HR administrative staff; (c) clinical and administrative staff of OhioHealth Physician Group, OhioHealth Riverside Family Practice and Graduate Medical Education, including but not limited to: physicians, nurses, pharmacists, certified diabetes educator, social worker, community health worker; (d) clinical and administrative staff of OhioHealth Community Health and Wellness and other system support departments; (e) accounting and administrative staff of OhioHealth Patient Financial Services, Revenue Cycle and Finance. The Rehabilitation Hospital will also allocate budget for supplies and miscellaneous expenses, education materials and space as needed.
